Short answer: choose the messaging service that matches your province or territory, professional rules, data flows and clinical workflow—not the one with the strongest “secure” or “compliant” label. For a Canadian therapy practice, assess encryption alongside identity verification, staff access, data location and subprocessors, retention, breach response, client accessibility, consent and recordkeeping. Ordinary texting may be acceptable for a narrowly defined, low-detail purpose in some circumstances, but it is a poor default for sensitive therapy information unless your legal, professional and clinical risk assessment supports it.

Start with the jurisdiction and the kind of practice you run

There is no single Canadian rule that names one approved therapy-messaging app. The requirements depend on where your practice operates, your professional designation, whether you work for a public or private organization, and where information travels.

Federal and provincial privacy laws

The federal Personal Information Protection and Electronic Documents Act (PIPEDA) generally applies to private-sector organizations collecting, using or disclosing personal information in commercial activity. Alberta, British Columbia and Quebec have substantially similar private-sector privacy laws for in-province activity. Ontario, New Brunswick, Nova Scotia and Newfoundland and Labrador have substantially similar legislation for personal health information. PIPEDA can still apply to commercial personal information that crosses provincial or national borders. A practice may therefore have obligations under more than one regime.

Before selecting a service, document:

  • the province or territory where the practice and each practitioner are located;
  • the professional designation and regulator (for example, a physician, psychologist, social worker, counsellor or psychotherapist);
  • whether the practice is private, employed by a health organization, or contracted by a public body;
  • where clients, staff, servers and support personnel are located; and
  • whether messages or backups cross provincial or national borders.

Use that map to confirm the applicable privacy statute, regulator standards, contractual requirements and any employer policy. Do not treat a national marketing claim as proof that a service satisfies every provincial rule.

What PIPEDA actually requires

PIPEDA sets out 10 fair information principles, including accountability, identifying purposes, consent, limiting collection and retention, safeguards, openness and individual access. Its safeguards principle requires protection appropriate to the sensitivity of the information. The Office of the Privacy Commissioner of Canada states: “PIPEDA does not specify particular security safeguards that must be used.” It also expects organizations to keep protections current as technology and risks evolve.

That means the defensible decision is a documented risk assessment: what information will be sent, to whom, through which systems, with what controls and for how long. An app can support that assessment, but an app cannot replace it.

Profession-specific rules are separate

Professional regulators can impose additional requirements. For example, the College of Physicians and Surgeons of Ontario (CPSO) policy says physicians must use encrypted electronic communication for personal health information sent to other providers, except in emergencies or circumstances requiring unencrypted communication. For communication with patients, physicians must use encryption where possible. If encryption is unavailable and unencrypted communication is considered, the policy directs physicians to consider sensitivity, volume and frequency, purpose, patient expectations, alternatives and urgency, and to obtain express documented consent for that unencrypted communication.

This is an Ontario physician example, not a universal rule for counsellors, psychotherapists or every other therapist. Check the standard that applies to your own designation.

Why ordinary texting is a weak default for therapy

Therapy messages can reveal diagnoses, symptoms, trauma history, medication information, risk assessments, appointment details and other highly sensitive facts. The risk is not limited to interception while a message is in transit. A wrong-number send, a shared family phone, a staff member using a personal account, an unlocked notification preview or a retained cloud backup can disclose information just as readily.

Consumer texting also makes it difficult to prove who accessed an account, apply role-based permissions, preserve a complete clinical record, delete duplicate copies, or explain exactly when the practice checks messages. A client may reasonably assume that a text is monitored immediately, while a clinician may check it only twice a day. That mismatch creates both safety and privacy problems.

Texting is not automatically unlawful in every situation. A carefully limited message—such as “Please call the office”—may disclose little. But the practice should define permitted content, obtain and document the client’s preference or consent where appropriate, explain the risks and response times, and provide a safer alternative for substantive clinical communication.

What to compare in a secure messaging service

Evaluate each candidate using current vendor documentation, the service agreement and a privacy/security questionnaire. Record the answer rather than inferring it from a badge, sales page or the word “encrypted.”

Decision area Questions to verify Why it matters in therapy
Message and attachment protection Are messages and attachments encrypted in transit and at rest? How does a client receive and open a message? Can files be password-protected? Clinical notes, forms and reports need protection beyond a normal mail or text notification. Ontario guidance advises encrypting or password-protecting attachments and sending passwords through a separate channel when appropriate.
Identity and recipient controls How is a client’s identity verified? Can staff confirm the current phone number or email before sending? Is there a warning before an external recipient is selected? Misaddressed messages are a common, preventable disclosure. The workflow should make the correct recipient easy to verify.
Account and staff access Does the service support strong authentication, individual accounts, role-based permissions, session timeouts and administrator audit logs? Can personal accounts be blocked? Need-to-know access limits exposure when staff roles change or an account is compromised.
Content minimization Can notifications avoid showing sensitive text? Can staff use neutral subject lines and send only the minimum necessary detail? A lock-screen preview or email subject can reveal therapy information even when the underlying app is protected.
Retention and records How long do messages remain on the service, devices and backups? Can relevant communications be exported to the clinical record? Can duplicate copies be securely removed? Retention must be deliberate, consistent with the record-retention rule that applies to your practice, and manageable when a client requests access.
Vendor and data handling What data does the provider handle? Where is it stored? Which subprocessors and support personnel can access it? What contractual privacy, confidentiality and breach commitments apply? Cross-border storage or support may change the legal analysis and should never be assumed from a product’s branding.
Client access and accessibility Can clients use the service without a difficult installation or account process? Does it work with assistive technologies and on common devices? Can clients state communication preferences? A technically strong channel fails if clients cannot reliably access it or do not understand its limitations.
Monitoring and urgent contact Can the practice publish office hours, expected response times and an “unmonitored” notice? Can a client reach crisis or emergency help another way? Messaging is asynchronous; it must not be presented as real-time crisis support.
Exit and continuity Can the practice export records in a usable format, delete an account, retrieve data after termination and transfer operations if the provider changes? You need continuity if the service closes, changes terms or no longer meets your requirements.

Encryption: ask what is actually protected

“Encrypted” is incomplete information. Ask whether encryption covers transport, stored data, attachments, backups and notifications. Find out whether the provider can decrypt content for support or administration, and who controls the keys. End-to-end encryption may reduce provider access, but it can complicate account recovery, organizational retention and clinical-record export. A service with strong transport and storage encryption but broad administrator access may present a different risk from one with end-to-end encryption and limited recovery options.

Ask for technical and contractual answers in plain language:

Build a written messaging policy before rollout

The Information and Privacy Commissioner of Ontario’s A Privacy Management Handbook for Small Health Care Organizations says: “If you decide to use email and other digital messaging apps, you should address this in your privacy policy.” A practical policy turns a tool into a controlled clinical process.

Define permitted and prohibited content

State which matters may be handled through the channel, such as appointment logistics, forms, billing questions or a brief non-urgent follow-up. List what must use a different process: crisis communication, detailed diagnostic information, highly sensitive documents, changes requiring immediate clinical assessment, or anything that cannot wait for the stated response window. If your service supports attachments, specify file types and whether password protection is required.

Explain consent and client choice

Tell clients what information may be sent, the principal risks, who may access it, whether data may be stored outside the province or Canada, and how to change their preference. Offer a reasonable alternative where feasible. Record the client’s choice in the clinical record, including any limits the client sets. Consent should be understandable and specific; a generic acceptance of office terms is not a substitute for explaining the messaging channel.

Set monitoring and response expectations

Publish the hours during which messages are reviewed, the usual response window and holidays or closures. Make clear that message notifications are not proof that a clinician has read a message. Add a prominent instruction for emergencies and crisis situations, directing the person to emergency services or an appropriate crisis resource rather than waiting for a reply.

Control accounts and devices

Require approved professional accounts, individual logins, strong authentication and current devices. Prohibit forwarding to personal accounts unless specifically authorized and protected. Limit access by role, review access when a staff member changes duties, and remove access promptly at departure. Enable automatic locking and keep operating systems and the messaging client current.

Minimize copies and preserve the record

Use neutral notification text and avoid unnecessary health details in message bodies. Decide which communications belong in the clinical record, who adds them, and when. Keep only the copies required for care, legal obligations and the applicable retention schedule; securely remove redundant device or server copies when they are no longer needed and the service permits it. Document the process for responding to a client’s access request.

A step-by-step selection and implementation process

  1. Map the practice. List jurisdictions, professional designations, locations of clients and staff, practice setting, systems already used for scheduling and records, and every place a message could be stored or backed up.
  2. Classify the information. Separate logistics from clinical content, attachments, assessment results and crisis-related information. Decide which categories are never sent through asynchronous messaging.
  3. Write mandatory controls. Turn the policy into requirements: individual accounts, strong authentication, recipient verification, attachment protection, audit logs, export, deletion, breach notification and client accessibility.
  4. Question vendors in writing. Request current security documentation, privacy terms, subprocessor list, storage locations, support-access model, incident process, retention/deletion schedule, accessibility information, export format and termination procedure. Have the contract reviewed by the person responsible for privacy or legal advice where appropriate.
  5. Run a controlled pilot. Test invitations, identity verification, wrong-recipient prevention, notifications, attachments, account recovery, staff permissions, audit logs, record export and deletion using non-client test data.
  6. Document the decision. Record the risks considered, safeguards selected, unresolved limitations, approval authority and review date. Keep vendor answers with the decision, not only in a salesperson’s email.
  7. Train and launch narrowly. Start with the least sensitive permitted use. Train staff on recipient checks, neutral notifications, urgent-contact instructions, incident escalation and record entry. Give clients plain-language instructions and an alternative contact method.
  8. Review regularly. Recheck vendor terms, subprocessors, storage, software updates, regulator guidance, incident history, client complaints and workflow changes. Reassess after a breach, a new jurisdiction, a new service integration or a change in professional role.

What to do when something goes wrong

Wrong recipient or exposed notification

Stop further disclosure, preserve relevant logs, notify the privacy lead, document what was sent and to whom, and follow the applicable breach-assessment and notification process. Ask the unintended recipient to delete the message without forwarding it, but do not rely on that request as the only control. Review why the recipient check or notification setting failed.

Lost device or compromised account

Disable the account or session, reset credentials, revoke tokens, and use remote-device controls where available. Determine which messages and attachments were accessible, whether backups exist, and whether other accounts share credentials. Preserve evidence for the incident assessment while limiting further access.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Service outage

Use the documented alternate contact method, tell clients how response times are affected, and avoid moving sensitive conversations to an unapproved personal channel. After restoration, reconcile messages with the clinical record and investigate any missing or duplicated content.

Ontario-specific resources and limits

An Ontario practice may investigate Ontario Health’s virtual-care verification and procurement resources where the relevant program applies. Those resources can inform an Ontario purchasing decision, but they do not establish suitability for another province or territory, every professional designation or every private practice. Verify the current program requirements before relying on them.

Likewise, a physician in Ontario should read the current CPSO policy directly. A counsellor or psychotherapist should consult the standard of the regulator that governs that designation. Privacy statutes and professional rules can change; assign someone to monitor updates rather than treating the launch checklist as permanent.

How to make the final decision

Choose the service that meets all non-negotiable controls and creates the fewest unsafe workarounds. If two options remain, compare them across five practical dimensions:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Security controls: encryption scope, authentication, permissions, auditability and incident response.
  • Privacy and data terms: purposes, storage locations, subprocessors, support access, retention and deletion.
  • Client experience: accessibility, device support, account recovery, language needs and ease of switching channels.
  • Clinical workflow: monitoring, urgent-contact boundaries, record export, scheduling integration and staff workload.
  • Total cost and continuity: subscription and implementation costs, training, administrative time, export capability and the cost of changing providers later.

Do not rank named products without a current, like-for-like review of their contracts and technical behavior. A smaller service that your team can administer correctly is safer than a feature-rich platform that encourages staff to bypass policy. The appropriate app is the one your practice can govern, your clients can use and your regulator-facing records can explain.
