Choose glass ionomer cement (GIC) by matching its formulation and evidence to the restoration—not by choosing the product with the strongest marketing claim. First identify whether you need a conventional restorative GIC, high-viscosity GIC (HVGIC), resin-modified GIC (RMGIC), luting cement, liner/base, or sealant. Then check the tooth and cavity, expected loading, caries risk, moisture control, curing method, coating requirements, and delivery equipment in the current instructions for use (IFU).

For selected Class I and II posterior permanent restorations, HVGIC can perform comparably to composite for fracture or retention through 36 months in one systematic review, while another review found very-low-certainty evidence favoring bulk-fill composite for some one- and two-year outcomes. Those findings are not a universal product ranking. A 2024 guideline specifically advises against GIC for the permanent restoration of Class III and IV defects. Use the decision process below, then verify the exact product and indication in its current IFU.

1. Start with the clinical indication

The word “glass ionomer” covers materials with different compositions, setting reactions, and intended roles. A restorative HVGIC is not automatically suitable as a luting cement, liner, base, or sealant. The first decision is therefore the job the material must perform.

Clinical need Material category to investigate Selection questions
Small or moderate restorative cavity Conventional restorative GIC or HVGIC Is the site suitable for the material’s strength, wear resistance, and moisture tolerance?
Selected cervical carious or non-carious lesion Restorative GIC or an indicated RMGIC Can you obtain enough isolation, and does the formulation require light curing?
Selected posterior permanent Class I or II restoration HVGIC or resin-based restorative material, chosen from the evidence and loading demands What are the occlusal forces, cavity dimensions, and expected service period?
Atraumatic restorative treatment (ART) Field-tested HVGIC intended for ART or suitable restorative use Does the material have relevant field evidence and a workable hand-mixing or capsule protocol?
Cementing an indirect restoration Luting GIC or RMGIC, not a restorative HVGIC by default Are film thickness, retention, working time, and curing mode appropriate?
Liner, base, or sealant A formulation specifically labeled for that role Does the IFU define the indication, thickness, placement, and compatibility requirements?

The American Dental Association explains the acid–base chemistry and categories of direct restorative materials in its overview of materials for direct restorations. Use the product’s labeled indication rather than assuming that all GICs behave alike.

2. Understand the material classes

Conventional GIC

Conventional GIC forms through an acid–base reaction between fluoro-aluminosilicate glass powder and an aqueous polyacid system. It chemically bonds to tooth structure and releases fluoride, but its setting reaction is sensitive to water gain and water loss during the early phase. The exact powder, liquid, particle size, polyacid, and powder-to-liquid ratio determine handling and mechanical behavior.

High-viscosity GIC

HVGIC is a conventional formulation with a higher powder-to-liquid ratio. The denser mix is intended to improve handling and mechanical properties for selected restorative uses. Examples named in a 2024 clinical review include GC Fuji IX GP, GC Equia Forte Fil, 3M Ketac Molar Easymix, 3M Ketac Universal, Dentsply Sirona Chemfil Rock, and SDI Riva Self Cure. This is an illustrative list, not a current availability check or performance ranking; confirm the present formula, indication, and regional supply before purchase.

Resin-modified GIC

RMGIC adds resin components to the glass-ionomer system. Depending on the product, it may be light-cured, self-cured, or dual-cured. The ADA notes shorter setting time and improvements in selected strength and polishability compared with earlier GIC formulations. A 2024 review describes RMGIC use in cervical carious and non-carious lesions, including situations in which ideal isolation is difficult. Those are category-level observations; an individual RMGIC still has to be used within its own IFU.

Role-specific formulations

Restorative GIC, luting cement, liner/base, and sealant formulations differ in viscosity, film thickness, working time, setting behavior, fluoride release, and intended thickness. A luting cement may be too fluid or too weak in bulk for a restoration; a restorative material may be unsuitable for seating an indirect crown. Keep the role label visible when comparing products.

3. Match the cement to the tooth, cavity, and patient

Tooth and cavity location

Determine whether the defect is anterior or posterior, occlusal or proximal, cervical, root-surface, or an ART cavity. Record the cavity class, depth, width, remaining enamel and dentin, and whether margins are on enamel, dentin, cementum, or a mixed substrate. Location affects isolation, bonding, finishing, and functional loading.

Size and occlusal loading

Small, well-contained lesions place different demands on a cement than broad proximal boxes, cusp-replacing restorations, or areas in heavy occlusion. Examine parafunction, opposing contacts, bruxism risk, and whether the restoration is expected to serve as a definitive long-term restoration or an interim step. If the defect exceeds the product’s labeled restorative indication or receives stresses beyond the evidence base, do not infer suitability from a high compressive-strength number alone.

Caries risk and fluoride strategy

HVGIC may be a valuable option for selected patients at high caries risk, especially when chemical adhesion, fluoride release, simplified placement, or easier moisture management is clinically useful. Caries risk does not override cavity size, loading, or the product’s indication. Combine the material choice with preventive care, diet counseling, fluoride exposure, and recall planning.

Moisture control

GIC tolerates some clinical moisture challenges better than hydrophobic resin procedures, but it is not moisture-proof. Saliva contamination, blood, desiccation, and uncontrolled crevicular fluid can compromise adaptation and the early set. Decide whether rubber dam, cotton-roll isolation, retraction, suction, or a shorter-setting RMGIC is needed before opening the capsule or mixing powder and liquid.

Anterior boundaries

An evidence-based 2024 guideline states: “Glass-ionomer cements should not be used for the permanent restoration of Class III and IV defects.” The recommendation applies to permanent anterior Class III and IV restorations as stated; do not extend it to every anterior or cervical situation without supporting evidence. Read the complete guideline at Direct Composite Restorations on Permanent Teeth in the Anterior and Posterior Region – Part 1.

4. Use evidence without overreading laboratory numbers

Laboratory screening thresholds

A 2021 Journal of Dentistry consensus study had 21 experts evaluate 18 conventional GIC brands. It treated compressive strength, microhardness, acid erosion, and fluoride release as primary properties for restorative thresholds; contrast ratio and translucency were secondary. Eleven of the 18 tested brands met the study’s laboratory thresholds for consideration as long-term restorative materials. The authors explicitly said that clinical-trial results must also inform selection. The consensus record is available at PubMed.

These thresholds are a screening tool, not a clinical endorsement. Laboratory specimens use controlled dimensions, curing or storage conditions, and loading methods that may not reproduce saliva, occlusal fatigue, operator variability, cavity geometry, or patient behavior.

HVGIC versus composite in posterior teeth

A 2023 systematic review included ten studies, with eight contributing to meta-analyses. Across the included studies, 849 HVGIC and 800 composite restorations were followed. For Class I posterior permanent restorations at 36 months, the fracture or retention risk difference was −0.00 (95% confidence interval −0.03 to 0.03; p = 0.98). The evidence certainty was moderate, and risk of bias was uncertain in the included studies. The result supports comparable performance for that analyzed outcome and period—not equivalence for every cavity, product, or patient. See the review at PubMed.

A separate review with different findings

A 2024 systematic review included five randomized controlled trials and one retrospective study. Its pooled evidence was rated very low certainty. Bulk-fill resin materials had more favorable retention at one and two years and marginal adaptation, while coated HVGIC systems showed acceptable clinical behavior. Because this review used a different evidence set, follow-up, and outcome mix, it should be read alongside—not collapsed into—the 36-month review. Its record is at PubMed.

Evidence source Population or comparison Result How to use it
2021 consensus study 21 experts; 18 conventional GIC brands 11 met specified laboratory thresholds Screen candidates, then verify clinical trials and indication
2023 systematic review 849 HVGIC and 800 composite restorations; selected posterior permanent restorations Class I fracture/retention risk difference at 36 months: −0.00 (95% CI −0.03 to 0.03; p = 0.98); moderate certainty Supports comparability for the analyzed outcome and follow-up, not a universal winner
2024 systematic review Five randomized trials and one retrospective study; Class I and II permanent-tooth cavities Very-low-certainty evidence favored bulk-fill materials for some one- and two-year outcomes; coated HVGIC behavior was acceptable Keep the low certainty, coating condition, and short follow-up attached to the claim

Do not rely on a product name preserved in an old comparison chart. The ADA’s 2008 product comparison can illustrate that accessory needs vary, but its product and cost details are dated: Glass Ionomer-Containing Restorative Features According to the Manufacturer.

Capsule, hand-mix, and accessory requirements

Capsule delivery can require a compatible triturator and capsule applicator. Hand-mix products require accurate proportioning, a mixing pad or slab, and timing discipline. A capsule may improve dose consistency but adds equipment, maintenance, and compatibility checks. Confirm whether the package includes the applicator, whether the triturator speed and cycle are specified, and whether the capsule can be extruded directly into the cavity.

Coatings and surface protection

Some HVGIC systems depend on a resin-based coating or protective varnish after placement. Treat the coating as part of the restorative system: verify its curing requirements, contamination sensitivity, recoat instructions, and finishing sequence. A coated HVGIC result cannot be assumed to represent an uncoated formulation.

6. A practical step-by-step selection workflow

  1. Define the restoration. Record permanent or primary tooth, cavity class, surface, dimensions, occlusal contacts, and whether the treatment is definitive, interim, or ART.
  2. Assign the material role. Eliminate products labeled only as luting cement, liner/base, or sealant when a bulk restorative material is required.
  3. Screen the patient factors. Assess caries activity, saliva control, gag reflex, cooperation, parafunction, oral hygiene, recall reliability, and esthetic expectations.
  4. Choose the class. Compare conventional HVGIC and RMGIC only when both are indicated. Note curing mode, early moisture sensitivity, polishability, and whether the resin component changes isolation or repair requirements.
  5. Check the evidence for the exact indication. Look for clinical trials in the same tooth type, cavity class, restoration size, follow-up period, and coating protocol. Do not substitute a laboratory threshold for clinical evidence.
  6. Verify the IFU and accessories. Confirm mixing or trituration, applicator, conditioner, coating, curing light, matrix, finishing instruments, and storage requirements.
  7. Plan placement and protection. Decide isolation, matrix and wedge, conditioning, insertion timing, contouring, initial protection, and final finishing before mixing.
  8. Document the choice. Record product name, formulation, lot, indication, coating, curing method, and follow-up plan so repairs or failures can be traced to the exact system.

7. Decision paths for common situations

Posterior Class I or II permanent restoration

For a contained cavity with manageable loading and acceptable isolation, an indicated HVGIC can be considered after reviewing the posterior evidence and the product’s IFU. For a broad proximal box, cusp involvement, heavy occlusion, or uncertain long-term retention, compare the material with a resin-based alternative using cavity-specific clinical evidence. If the HVGIC system requires a coating, include that step in the feasibility assessment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

High-caries-risk patient

Consider whether fluoride release, chemical adhesion, and a simpler moisture-control protocol add practical value. Select a restorative formulation with evidence for the cavity and follow the prevention and recall plan; high caries risk alone does not justify using GIC in an unsuitable high-load defect.

ART or field conditions

Prioritize an HVGIC with relevant field-testing, workable hand-mix or capsule handling, sufficient working time, and packaging that can be used where rotary equipment and ideal isolation are unavailable. Follow the ART protocol and protect the early set as directed.

Cervical carious or non-carious lesion

An indicated restorative GIC or RMGIC may be appropriate when margins are on dentin or cementum and isolation is difficult. Check whether the product is light- or self-cured, whether a conditioner is required, and how the manufacturer directs finishing and surface protection.

Permanent anterior Class III or IV defect

Do not select GIC as the permanent restoration where the cited 2024 guideline advises against it. Use a material and technique supported for the specific anterior indication, and do not broaden the recommendation beyond Class III and IV without evidence.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

8. Placement and handling factors that change outcomes

Conditioning and bonding

Some GIC systems use a polyacrylic-acid conditioner; others specify a different pretreatment or none. Apply only the product’s stated conditioner, for the stated time, then rinse or dry exactly as directed. Substituting an adhesive or conditioner from another system can alter wetting, bond development, or setting.

Mixing and timing

Use the specified powder-to-liquid ratio and mixing interval. With hand mixing, incorporate powder in the manufacturer’s recommended increments and start the mix timer before dispensing. With capsules, use the specified triturator cycle and place immediately within the working time. A material that has begun to lose gloss or develop stringiness may no longer adapt predictably.

Early moisture protection

Protect the restoration from saliva and blood during the early set, but do not desiccate the surface. Apply the specified varnish, resin coat, or isolation method. If the surface is contaminated or visibly dried outside the IFU limits, follow the manufacturer’s correction protocol rather than polishing over the problem.

Finishing and occlusion

Respect the product’s finishing window and instrument recommendations. Remove excess, establish contacts and anatomy, and check occlusion after the material reaches the stated set. Avoid leaving a high contact that subjects a developing restoration to unnecessary stress.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

9. Common selection mistakes and their fixes

  • Mistake: treating every GIC as interchangeable. Fix: classify restorative, HVGIC, RMGIC, luting, liner/base, or sealant before comparing products.
  • Mistake: choosing from compressive strength alone. Fix: review microhardness, acid erosion, fluoride release, translucency, clinical trials, cavity class, and loading together.
  • Mistake: ignoring the coating. Fix: evaluate the complete coated or uncoated system represented by the evidence and IFU.
  • Mistake: assuming “moisture tolerant” means contamination proof. Fix: plan isolation and early-set protection; saliva and blood still threaten adaptation.
  • Mistake: using an old product chart for today’s purchase. Fix: verify current formula, packaging, accessories, indication, and regional availability with the manufacturer.
  • Mistake: extending evidence beyond its follow-up. Fix: state whether the supporting study followed restorations for one, two, or 36 months and keep its certainty rating visible.
  • Mistake: selecting a restorative HVGIC for another role. Fix: use a product specifically labeled for luting, lining, basing, or sealing when that is the clinical task.

10. A concise chairside checklist

  • Tooth type and permanence recorded.
  • Cavity class, dimensions, margins, and occlusal loading assessed.
  • Restorative, HVGIC, RMGIC, luting, liner/base, or sealant role identified.
  • Patient caries risk, cooperation, parafunction, and esthetic needs considered.
  • Isolation method and early-set moisture protection planned.
  • Current IFU reviewed for conditioner, mixing, curing, coating, finishing, and contraindications.
  • Clinical evidence matches the indication, not merely a laboratory ranking.
  • Capsule applicator, triturator, matrix, curing light, and coating are compatible and available.
  • Lot, product, coating, and follow-up plan documented.

The safest choice is the formulation whose labeled role, handling system, clinical evidence, and patient-specific demands all align. When those elements conflict, change the material or treatment plan rather than forcing a generic “glass ionomer” label to cover an unsuitable indication.
