Choose a commercially prepared, FDA-cleared biological indicator (BI) containing Geobacillus stearothermophilus spores and labeled for your exact steam sterilizer and cycle parameters. Before buying, confirm the carrier or self-contained format, any process-challenge device requirement, incubation temperature and time, readout method, controls, and compatibility with your incubator or reader. Use the BI with mechanical records and chemical indicators; neither chemical indicators nor a Bowie-Dick test can replace a BI.

What a steam-sterilization biological indicator does

A BI is a standardized population of highly resistant bacterial spores used to challenge a sterilization process. After the cycle, the BI is incubated or processed in an automated reader. Growth indicates that the challenge was not reliably inactivated; a validated negative result supports that the tested cycle achieved the required lethality for the indicator configuration.

For healthcare steam sterilization, the appropriate organism is Geobacillus stearothermophilus. CDC guidance describes a typical steam-monitoring population of 105 spores. The organism is selected because its resistance to moist heat provides a meaningful challenge for steam processes. CDC reports D121°C values of about 1–2 minutes for G. stearothermophilus used in steam monitoring. A D-value is a resistance characterization, not a recommendation to set a sterilizer cycle to that time.

CDC describes an ideal biological monitor as easy to use, inexpensive, protected from outside contamination, capable of providing a positive result quickly enough for corrective action, and positive only when sterilization parameters are inadequate to kill the microbial challenge. In practice, no single feature decides the purchase: the BI must fit the sterilizer, cycle, challenge configuration, reading system, and facility procedure as a complete system.

Start with the sterilizer and cycle

Do not select a BI merely because its package says “for steam.” First identify the equipment and the cycle you actually run. The BI labeling must cover that sterilizer type and the cycle’s temperature, exposure time, pressure or saturated-steam conditions, and air-removal method. Follow the sterilizer instructions and the BI instructions for use (IFU) together; a BI authorized for one configuration is not automatically authorized for another.

What to identify Why it matters for BI selection What to confirm in the BI IFU
Sterilizer type Steam sterilizers differ in chamber design, controls, and air-removal method. Explicit labeling for the equipment category in your department.
Cycle type Gravity-displacement and dynamic-air-removal cycles challenge air removal and steam contact differently. Permitted cycle family and the temperature/exposure combinations listed by the manufacturer.
Load configuration Wrapped sets, porous loads, lumened devices, and implant loads can require different challenge arrangements. Whether the BI is used alone or inside a process-challenge device (PCD) or test pack, and where it is placed.
Routine versus special cycles A cycle used for a particular device may have parameters outside your routine program. That exact cycle is within the BI’s claimed intended use; do not extrapolate from a similar-sounding cycle.

If your facility has more than one sterilizer or more than one cycle family, map each combination separately. A single BI product may be suitable for several combinations, but that must be demonstrated by its current labeling rather than assumed.

Choose the biological indicator configuration

Organism and stated population

Look for G. stearothermophilus and the manufacturer’s stated spore population. CDC’s commonly cited steam-monitoring population is 105; the product label controls the actual population, resistance characteristics, and acceptance criteria for that device. Record the lot information required by your quality system so an unexpected result can be traced to a specific production lot.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Carrier or self-contained design

A BI may use a separate spore carrier that is transferred to growth medium after processing, or a self-contained design in which the carrier and medium are integrated. The choice affects handling, contamination risk, incubation equipment, and the time required to obtain a result. Select only a format for which your staff can follow the complete handling and reading procedure without improvisation.

Process-challenge device and test pack

Some products are intended to be placed in a specified PCD or test pack that presents a defined challenge to steam penetration and air removal. Others are labeled for direct placement. Use the PCD, pack, or holder named in the IFU, and place it where the instructions identify the most difficult location in the load or chamber. Substituting a homemade pack or moving the BI to a convenient location changes the challenge and can invalidate the result.

Controls

The procedure should identify the positive control and any process or negative control required by the product instructions. A control that fails to grow when it should, or a control that shows unexpected growth, means the run cannot be interpreted as a simple pass or fail. Document control results with the processed BI and investigate according to the IFU and facility policy.

Decide how the BI will be incubated and read

Readout speed is a workflow choice, not a reason to ignore compatibility. Conventional systems rely on visible growth after an incubation period. Rapid-read systems detect a validated metabolic or fluorescent signal sooner, but they require the specified reader or incubator and the exact BI model approved for it.

Readout approach Advantages Requirements and limitations
Conventional growth readout Uses a straightforward incubation-and-observation process and may fit an existing laboratory workflow. Requires the temperature, incubation duration, medium, and interpretation rules in the BI IFU. Results are not interchangeable with a rapid system’s timing.
Rapid readout Can provide an earlier indication and reduce the time that loads remain on hold. Requires a compatible reader or incubator, approved BI chemistry, staff training, and adherence to the manufacturer’s validated read time. “Rapid” does not mean immediate or universally compatible.

CDC describes incubation at 55–60°C for G. stearothermophilus and notes rapid-read systems. Treat that range as general guidance, not permission to use any incubator or shorten any product’s read time. The selected BI’s IFU specifies the actual temperature, duration, reader settings, and result interpretation. Verify that your equipment can maintain those conditions and that service, calibration, and alarm procedures are documented.

Check regulatory status and standards

United States regulatory check

For US healthcare use, verify that the BI is FDA-cleared for its stated intended use. FDA describes healthcare-sterilizer-monitoring BIs as class II medical devices requiring a 510(k) premarket notification. Confirm the clearance applies to the steam application and configuration you plan to use, rather than relying on a distributor’s generic description.

Moist-heat standard

AAMI lists ANSI/AAMI/ISO 11138-3:2017/(R)2024 as the standard for biological indicators for moist heat, including steam. The catalog designation identifies the applicable edition; the complete standard and the product labeling contain the acceptance criteria and performance details. Ask your quality or infection-prevention program how the current standard, local regulations, accreditation requirements, and facility policy are applied together.

Documentation to retain

  • Current BI IFU and revision date.
  • Regulatory clearance information and intended-use statement.
  • Spore organism, stated population, carrier format, and PCD instructions.
  • Incubator or reader compatibility, temperature, time, controls, and interpretation rules.
  • Lot number, expiration date, storage conditions, and receiving records.

Use the BI as one part of a three-part monitoring system

Mechanical, chemical, and biological monitoring answer different questions. Mechanical monitoring records the cycle’s time, temperature, and pressure. A chemical indicator reacts to one or more process parameters and helps show that an item or pack was exposed to the process. A BI challenges the process with resistant spores and directly evaluates microbial killing. The three information sources should be reviewed together under the applicable procedure.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Monitor Question it answers What it cannot establish by itself
Mechanical records Did the sterilizer display the programmed time, temperature, and pressure? That steam reached every location or that resistant spores were killed.
Chemical indicator Did the indicator experience the parameter(s) to which it responds? That the item is sterile or that a BI challenge would be inactivated.
Biological indicator Was the spore challenge inactivated under the tested conditions? That every item in a different location or load was processed correctly.
Bowie-Dick test In an empty dynamic-air-removal sterilizer, did the air-removal and steam-penetration test perform as intended? That a BI in a load was killed; it is not a replacement for BI monitoring.

Use a Bowie-Dick test only for the empty dynamic-air-removal sterilizer situation described by its instructions. It addresses air removal, whereas the BI addresses inactivation of a defined microbial challenge.

Apply the monitoring frequency required for your setting

CDC recommends BI monitoring at least weekly for steam sterilizers. CDC also recommends using a BI with every load that contains implantable items and, when possible, quarantining that load until the BI result is negative. Apply the current CDC recommendation together with applicable regulations, accreditation requirements, and your facility’s written policy; a local rule may be more frequent.

Define who places the BI, who releases a load, who reviews controls, and where the record is stored. A usable record links the BI to the sterilizer identification, cycle, load contents, date and time, operator, mechanical printout or electronic record, chemical-indicator result, BI lot, incubation or reader result, and any corrective action.

Step-by-step selection process

  1. Inventory every steam process. List each sterilizer, cycle family, temperature and exposure combinations, load type, and whether the cycle uses gravity displacement or dynamic air removal.
  2. Define the challenge location. Follow the sterilizer and BI procedures to identify the location or PCD that represents the most difficult routine challenge. Do not choose a location solely because it is easy to retrieve.
  3. Screen candidate BIs by organism and intended use. Require G. stearothermophilus and labeling that explicitly covers your sterilizer and cycle parameters.
  4. Compare the physical configuration. Check the stated population, carrier or self-contained design, PCD or test-pack requirement, controls, storage, shelf life, and handling steps.
  5. Match the readout system. Decide whether your workflow needs conventional or rapid readout. Confirm that the incubator or reader is approved for that BI model and can maintain the specified conditions.
  6. Verify regulatory and standards documentation. For US healthcare use, confirm FDA 510(k) clearance and retain the current IFU. Check how ANSI/AAMI/ISO 11138-3:2017/(R)2024 and local policy apply to your program.
  7. Run a staff workflow check. Have the people who will process, incubate, read, and document the BI perform the procedure using the actual equipment. Correct ambiguous steps before routine purchasing.
  8. Approve the complete system. Quality, infection prevention, sterile processing, and biomedical or facilities staff should sign off on the BI, PCD, reader, placement, controls, records, and response to an abnormal result.

How to process and interpret a routine BI

  1. Check the BI package for damage, expiration, correct storage, and the lot number required by your record.
  2. Prepare the BI or PCD exactly as the IFU describes. Do not open a self-contained unit or alter a test pack before exposure unless instructed.
  3. Place it in the specified challenge location with the load arranged as required by the sterilizer procedure.
  4. Run the complete cycle. Record the mechanical parameters and apply the required chemical indicators.
  5. After the cycle, handle the BI using the specified cooling, activation, transport, and contamination-control steps.
  6. Incubate or load it into the approved reader at the stated temperature and for the stated time. Use the required control(s).
  7. Interpret the result only at the authorized read point. Record positive, negative, invalid, or control-failure status and the associated load decision.
  8. Release, hold, or reprocess the load according to the written procedure. For implant loads, follow the quarantine rule while awaiting the BI result whenever possible.

A negative BI does not erase a failed mechanical record, a failed chemical indicator, a leaking package, wet load, or other process deviation. Investigate conflicting evidence rather than selecting the result that is most convenient.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What to do when a steam BI is positive

A positive result is a process signal that requires a defined investigation, not an automatic conclusion about every load ever processed. CDC’s suggested steam-positive protocol provides this sequence:

  1. Take the sterilizer out of service and notify the appropriate supervisor and infection-control contact.
  2. Repeat BI testing in three consecutive cycles. Verify that the BI was placed, handled, incubated, and interpreted correctly and that the controls behaved as expected.
  3. Review cycle settings and records. Check time, temperature, pressure, air-removal phase, loading, packaging, and any alarms or deviations.
  4. Ask maintenance or biomedical engineering to evaluate the equipment and steam supply. The investigation should include the utilities and hardware that can prevent adequate saturated-steam contact.
  5. Escalate persistent positives. If additional tests remain positive, CDC suggests considering a BI from a different manufacturer as part of the investigation, while following the facility protocol and applicable rules.
  6. Treat implicated items as nonsterile when required. Recall and reprocess affected items as directed by the facility’s infection-prevention and recall procedure. Do not release them solely because a chemical indicator changed color.

Document the initial positive, repeat results, control results, equipment findings, affected loads, notifications, and final disposition. Your written policy should define who authorizes return to service and how patients or procedure areas are evaluated if an item may have been used.

Compare products on the factors that change real-world performance

Comparison factor Questions for the purchasing team Why it affects the decision
Clearance and compatibility Is the product FDA-cleared for healthcare steam monitoring, and does its intended use name our sterilizer and cycles? A generic steam claim may not cover the actual cycle or configuration.
Spore challenge Is the organism G. stearothermophilus? What population and resistance information are stated? The challenge must be appropriate for moist heat and traceable to the product labeling.
Carrier and PCD Is it self-contained or a separate carrier? Is a specified PCD or test pack mandatory? Handling, placement, contamination risk, and challenge severity depend on the configuration.
Readout What incubation temperature and time apply? Is a dedicated rapid reader required? Readout speed and equipment determine how long loads remain on hold and what capital equipment is needed.
Controls and interpretation Which controls are required, and what constitutes a positive, negative, or invalid result? Staff must be able to make an unambiguous release decision.
Supply and storage What package quantities, expiration dating, storage conditions, and lot documentation are provided? A low unit price can be offset by waste, expired stock, or difficult inventory control.
Total workflow cost What are the costs of the BI, PCDs, incubator or reader, calibration, labor, training, and held or reprocessed loads? The purchase decision should reflect the complete monitoring system, not only the box price.

Current package quantities, shelf life, storage limits, and pricing are product-specific and were not established here. Obtain them from the current manufacturer labeling and an authorized supplier before approval.

Common selection mistakes and their fixes

  • Choosing by the word “steam” alone: Require explicit cycle and sterilizer compatibility in the IFU.
  • Using an incompatible incubator: Match the reader, temperature, time, and BI model as a validated combination.
  • Placing the BI wherever space is available: Follow the specified challenge location or PCD procedure.
  • Treating a chemical indicator as proof of sterility: Use chemical, mechanical, and biological evidence for their distinct purposes.
  • Using a Bowie-Dick result as a BI result: Keep the empty-chamber air-removal test separate from microbial challenge monitoring.
  • Ignoring controls or lot records: A control failure or untraceable lot makes the result unsuitable for a confident release decision.
  • Releasing an implant load before its required BI result: Quarantine when possible and follow the facility’s implant-load policy.
  • Continuing production after a positive BI: Remove the sterilizer from service and begin the documented investigation before routine use resumes.

Examples of matching the choice to the situation

Multiple cycles in a hospital sterile-processing department

List each cycle family and identify whether the existing BI labeling covers all of them. If one cycle falls outside the label, either select a BI with the broader documented intended use or manage that cycle with a separately approved indicator and procedure. Do not create a compatibility claim from similar temperature numbers.

A dental or outpatient facility with limited equipment

A self-contained BI may simplify handling when there is no laboratory workflow for transferring a carrier to culture medium. The facility still must provide the incubator or reader required by the IFU, train staff, document controls, and meet the applicable monitoring frequency. A smaller operation does not eliminate the need to match the BI to its exact sterilizer cycle.

Implantable items that cannot wait for routine release

Use the BI with every implant load as recommended by CDC and plan the hold or quarantine step before the cycle starts. If the workflow cannot accommodate the BI result, escalate that operational constraint to infection prevention and sterile-processing leadership rather than silently releasing the load without the required evidence.

Final purchasing checklist

  • Organism: G. stearothermophilus spores intended for steam.
  • Population: Stated by the manufacturer; CDC describes 105 as a typical steam-monitoring population.
  • Cycle fit: Sterilizer type, cycle family, temperature, exposure, and air-removal method explicitly covered.
  • Configuration: Carrier or self-contained design, PCD or test-pack requirement, and placement instructions understood.
  • Readout: Conventional or rapid method, compatible incubator or reader, specified 55–60°C guidance where applicable, and product-specific time.
  • Regulatory status: FDA clearance and intended-use documentation for US healthcare use.
  • Standards: Program alignment with ANSI/AAMI/ISO 11138-3:2017/(R)2024 and local requirements.
  • Workflow: Controls, training, records, storage, lot traceability, and result-release authority assigned.
  • Monitoring plan: At least weekly BI monitoring, every implant load, and quarantine rules defined.
  • Escalation: Positive-result investigation, maintenance contact, recall/reprocessing pathway, and return-to-service approval written down.

The right BI is not simply the fastest reader or the lowest unit price. It is the cleared, labeled, and operationally compatible system that challenges your actual steam cycle, produces an interpretable result under controlled incubation, and fits a monitoring program capable of responding when that result is positive.
