You cannot confirm an initial access broker (IAB) has access to your organization with a single internet scan. A scan can identify reachable systems and weaknesses that could enable access; it cannot prove that a criminal has obtained, retained, or advertised a foothold. Check exposure by combining an authorized external asset review with vulnerability and configuration checks, credential monitoring, and internal identity, endpoint, and network evidence. Escalate suspicious activity as a potential incident rather than treating it as a routine patching task.

What it means to be exposed to an initial access broker

An initial access broker is a criminal intermediary that obtains or arranges unauthorized access to an organization’s systems and sells or transfers that access to other criminals. A public-facing service, an unpatched vulnerability, or a compromised credential may create an opportunity for initial access, but none of those findings alone establishes that a broker has accessed or listed your organization.

That distinction matters when deciding what to do next. A reachable service that has an approved business purpose is a risk to assess and secure, not proof of compromise. A leaked password may be expired or never used. Suspicious authentication activity or an unknown account, on the other hand, calls for an incident investigation—not just a vulnerability ticket. CISA’s August 18, 2026 Medusa advisory update identifies brokered access among the actors’ initial access routes; it does not say that every exposed organization is targeted or compromised. Read the CISA, FBI, and HHS Medusa advisory update.

There is no general statistic in the cited guidance that establishes what share of organizations are currently exposed to IABs. A figure about ransomware victims, a broker listing, or vulnerability prevalence would not answer that question. For example, the advisory reports more than 500 Medusa victims as of April 2026 across multiple critical-infrastructure sectors; that is not a measure of all IAB activity or the likelihood that a particular organization is exposed.

Use a layered check, not a single scan

Assess the question through several evidence sources. External discovery shows what appears reachable from outside; configuration and vulnerability reviews help determine whether that exposure is necessary and secure; identity and endpoint telemetry can show whether someone may already be using an access path. No single view can prove that an IAB does or does not hold access.

  • External asset coverage: Which public addresses, domains, cloud assets, and remote access services can be reached?
  • Service and vulnerability evidence: What is exposed, who owns it, what purpose does it serve, and is it patched and appropriately restricted?
  • Credential and identity evidence: Are organizational credentials exposed, and do sign-in logs show unusual access or privilege changes?
  • Endpoint and network evidence: Are there signs of unauthorized access, unfamiliar tools, or persistence that merit investigation?

Compare any checking method against its asset coverage, evidence type, data freshness, ability to validate ownership, and path to remediation or incident escalation. A product view with limited operating-system or cloud coverage should not be treated as a complete inventory.

Think of the review as a chain of questions rather than a pass/fail test: what is exposed, is that exposure intended, is it vulnerable or poorly controlled, and is there evidence it has been used? A “yes” to the first question establishes an exposure to assess. Answers to the later questions determine whether to remediate a weakness, investigate potential unauthorized access, or do both.

Step 1: Define scope and authorization

Before scanning, establish which assets belong to the organization and which systems you are authorized to assess. Assemble known public IP ranges, domains, cloud environments, remote access gateways, and assets hosted by third parties. Include relevant subsidiaries or environments only when they are within the agreed scope. Record an owner or responsible team for each known asset where possible.

Use the organization’s asset inventory, firewall and cloud configuration, and service records to establish the expected public footprint. External discovery can reveal stale, forgotten, or misattributed assets, so validate a finding with an administrator or asset owner before changing or disrupting a system. Confirm that the scan is authorized and within scope; an address that appears to be associated with your organization is not, by itself, proof of ownership or permission to test it.

At this stage, separate known and approved services from items that are unexplained. That distinction makes later review actionable: a service with a named owner and business purpose can be checked against its intended configuration, while an unknown service needs ownership and purpose established before it is accepted as legitimate.

Build the inventory from more than one record where possible. Public IP ranges and domains provide a starting point, but cloud environments, remote access gateways, third-party-hosted assets, and forgotten systems can be missed if the review relies on a single list. Reconcile discovered systems with the teams responsible for them; do not assume that an unfamiliar address is either harmless or owned by the organization until that is validated.

Step 2: Find services reachable from the internet

Use an authorized external scan alongside internal records to identify services that answer from the public internet. Reconcile each result against approved business needs and the organization’s firewall and cloud configuration. The aim is to find the actual public footprint—not merely the assets an inventory says should exist.

CISA recommends regular vulnerability scanning, especially for internet-facing devices, and advises disabling unnecessary operating-system applications and network protocols on internet-facing assets. Its #StopRansomware Guide also explains why exposed remote services deserve attention: “Threat actors often gain initial access to a network through exposed and poorly secured remote services, and later traverse the network using the native Windows RDP client.”

For each result, record what was observed and when, then compare it with the expected service and its owner. A reachable port or service is useful evidence of exposure, but it does not by itself tell you whether the system is vulnerable, whether access is appropriately restricted, or whether anyone has used it. Those questions require the next layer of review.

If your organization uses Microsoft Defender for Endpoint

Microsoft documents internet-facing device tags and detections in the Defender for Endpoint portal. The feature’s detection is based on external scans and incoming connections, and the page says that only onboarded Windows devices are identified. In the portal, a detection can show the public IP address and port associated with it, and advanced hunting queries are available. See Microsoft’s device investigation documentation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use that information as one view of exposure, not as a complete public-asset inventory. The documented feature does not establish coverage of all operating systems, cloud services, or assets outside the product’s coverage. Reconcile detections with other asset records and confirm ownership before taking action.

Step 3: Review every confirmed public service

For each service confirmed as reachable, document its owner, business purpose, software or firmware version, patch state, and access controls. Then decide whether public access is necessary. If the service has no business need to be internet-facing, remove or restrict that exposure. If it is required, verify that it is maintained and protected, and prioritize known exploited or high-impact vulnerabilities on systems reachable from the internet.

Pay particular attention to remote access. CISA advises against exposing Remote Desktop Protocol (RDP) and other remote services directly to the web. Where remote access is necessary, its guidance includes limiting use, auditing systems, closing unused RDP ports, enforcing multifactor authentication (MFA), and logging connection attempts. These measures reduce unnecessary access paths and improve the evidence available if an account or service is misused. Consult the CISA ransomware guide for its remote-access, scanning, and credential recommendations.

The August 18, 2026 CISA, FBI, and HHS Medusa update describes initial access through brokers, phishing, and exploitation of newly disclosed, unpatched internet-facing vulnerabilities. Its mitigations include patching, network segmentation, and filtering untrusted sources from remote services. Those are relevant defensive priorities, not evidence that every organization with a public service is being targeted. See the advisory for the stated vectors and mitigations.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use the service review to distinguish removal from hardening. If a service is not needed publicly, remove the exposure rather than preserving it without a business reason. If it is needed, verify its owner, version and patch state, and access controls; then limit remote access to its actual requirement, close unused RDP ports, and log attempts. CISA’s guidance also calls for auditing systems and applying MFA for remote access. These measures address risk, but they do not establish whether a broker already obtained access.

Prioritization matters when several issues are found. Start with high-impact or known exploited vulnerabilities on systems exposed to the internet, and review newly disclosed vulnerabilities affecting those systems promptly. The Medusa advisory’s discussion of exploitation of newly disclosed, unpatched internet-facing vulnerabilities supports treating patching as a practical priority. Pair patching with segmentation and filtering untrusted sources from remote services, rather than relying on a patch alone to answer whether suspicious access has occurred.

Step 4: Check credentials and authentication activity

Network exposure is only part of the assessment. Use approved organizational processes to check for exposed credentials, and examine authentication logs for activity that does not fit expected use. CISA recommends phishing-resistant MFA, particularly for email, VPN, and critical-system accounts, and suggests considering credential monitoring services. MFA should be considered alongside—not instead of—reviewing access records and protecting internet-facing systems.

Look for patterns such as unusual VPN or remote-access activity, unfamiliar locations or devices, repeated authentication failures followed by a success, new privileged accounts, or unexpected changes to privileges. A single unusual event may have a legitimate explanation; investigate it against the account’s owner, normal access patterns, and related logs rather than treating it automatically as proof of a broker’s presence.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A credential reported in an exposure source does not establish that it is still valid or was used. Verify its status through internal identity systems. When warranted, reset or revoke it, inspect active sessions, and review authentication records for related activity. If those records show suspicious use, investigate the activity rather than closing the matter after the credential change.

Credential checks and authentication-log reviews answer different questions. An exposure alert identifies information that may create risk; internal identity systems help confirm whether the credential remains valid, while sign-in records and session review help determine whether it may have been used. If a credential is reset or revoked, do not treat that action as the end of the review when there are signs of suspicious access. Check for related sessions and activity, and route suspicious findings into incident response.

Prioritize MFA for the accounts CISA identifies in its guidance: email, VPN, and critical-system accounts. In particular, CISA recommends phishing-resistant MFA. MFA does not replace patching or exposure reduction, and a clean MFA configuration does not establish that no unauthorized access occurred. It is one control within a broader review of credentials, remote services, and authentication evidence.

Step 5: Decide whether the finding is exposure or a suspected incident

Use the evidence to route the issue appropriately. An exposed but expected service, an unpatched system, or a credential exposure alert requires remediation and validation. Suspicious logins, unknown accounts, unexpected remote tools, or other indications of unauthorized access should trigger the organization’s incident response process.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When findings indicate a security weakness

  • Confirm the affected asset and its owner.
  • Remove public exposure if it is not needed, or restrict the service to the business requirement.
  • Patch internet-facing systems and address high-impact or known exploited vulnerabilities first.
  • Review remote-access controls, MFA coverage, and logging.
  • Recheck the asset after remediation to confirm the exposure has changed as intended.

Keep the result tied to the asset and the action taken. If a service was meant to be public, document its purpose and the controls reviewed; if it was not needed, validate that the external exposure has been removed. This closes the remediation loop without overstating what the result proves about past access.

When activity suggests unauthorized access

Follow the organization’s incident response process rather than limiting the work to routine remediation. CISA’s guide recommends identifying affected systems and accounts, containing relevant access paths—such as a VPN or public-facing asset—as appropriate, and examining system and authentication logs. It also advises threat hunting and extended analysis for persistence mechanisms. The containment decision should fit the suspected access path and affected systems; do not assume that patching a public service alone resolves an incident.

Start by identifying the systems and accounts implicated by the evidence, then consider which access paths need containment. CISA names VPNs and public-facing assets as examples of access paths to contain as appropriate. Examine relevant system and authentication logs, and extend the investigation with threat hunting for persistence mechanisms. The purpose is to determine the scope of potential access and whether it remains present—not merely to remove the first visible weakness.

A current Microsoft Defender XDR tenant may provide threat analytics that relates threat reports to organizational incidents, assets, vulnerability exposure, and recommended actions. Availability depends on eligible Defender licensing and roles. Microsoft cautions that recommended actions do not guarantee complete resilience, so threat analytics is an investigation aid—not a detector of broker listings or a substitute for incident response. See Microsoft’s threat analytics documentation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use threat analytics, where available, as a way to relate a threat report to the organization’s incidents, assets, and vulnerability exposure. Check that the tenant has eligible licensing and that users have the required roles. Treat recommended actions as guidance, not a guarantee of resilience or proof that an organization is absent from a broker’s listings. If there is evidence of unauthorized access, the incident response process remains the appropriate route.

How to judge whether a checking approach is useful

Tools and processes answer different questions. An external scan can help identify reachable services, while internal identity logs can show authentication activity. Credential monitoring may identify exposed account information, but does not establish that the credential is still valid or that an attacker used it. Choose methods based on the evidence needed and verify the boundaries of what each one covers.

What to compare Questions to ask Why it matters
Asset coverage Does it include the organization’s public IPs, domains, cloud services, subsidiaries, vendors, and relevant operating systems? Uncovered assets can leave gaps in an otherwise reassuring result.
Evidence type Does it show external reachability, vulnerability or version data, configuration, exposed credentials, or internal authentication activity? Different evidence types answer different parts of the exposure question.
Freshness When was the information last observed, and how often does it update? A result reflects what was observed at a point in time; it may not describe a changed asset or credential.
Validation Can an administrator confirm that a finding belongs to an owned asset and is accurate? Discovery can include stale, forgotten, or misattributed assets that need validation.
Response path Does the process support remediation, reassessment, and incident escalation? Identifying a weakness is not the same as fixing it or investigating evidence of access.

Combine an external view with internal identity, endpoint, and network evidence. Neither a clean perimeter result nor a credential alert alone can establish whether an IAB has access.

These approaches are complementary, not interchangeable. An external scan can reveal reachability, and vulnerability or configuration review can help assess what that exposure means. Credential monitoring identifies possible exposure of account information, while internal authentication records show activity against the organization’s systems. Endpoint and network evidence can contribute to investigating whether access has occurred. Ask what evidence a result actually provides before using it to make a conclusion.

Check freshness and coverage before interpreting a clean result. Ask when the information was last observed and how often it updates; then check which public IPs, domains, cloud services, operating systems, subsidiaries, and vendor-hosted assets are included. Validate that results belong to the organization with an administrator. A result that omits assets or has not been refreshed cannot establish that the omitted or changed systems are safe.

Common misreadings to avoid

  • “A port is open, so a broker has access.” An open service is a reachability finding. Confirm ownership, purpose, version, patch state, and controls before drawing conclusions.
  • “A scan found nothing, so we are safe.” A scan only speaks to its scope and observation. It does not rule out credentials, assets it did not cover, or an existing foothold.
  • “A leaked password proves an account was used.” The credential may not be valid or used. Verify it internally and inspect sessions and authentication records.
  • “A security product’s view is the whole internet footprint.” Check which assets, operating systems, and services are actually covered, then reconcile with other records.
  • “Patching alone closes an incident.” If evidence suggests unauthorized access, contain and investigate through incident response, including relevant logs and possible persistence.

What this check can—and cannot—tell you

A combined review can identify reachable services, vulnerabilities, misconfigurations, potential credential exposure, and signs of unusual authentication or other activity. These findings help decide whether to reduce exposure, improve controls, or investigate possible unauthorized access. They do not automatically establish that a criminal broker has obtained access or listed it for sale.

The distinction also applies to reassuring results. A scan covers only the assets and time period it observes; a product’s coverage may be limited to particular onboarded devices; credential monitoring does not confirm account use. Combine the evidence, validate asset ownership, and keep the conclusion proportional to what the evidence supports. If suspicious access indicators appear, move from exposure management to incident response.

Frequently Asked Questions

Can an internet scan tell me whether an initial access broker has my network?

No. An authorized external scan can identify services reachable from the internet, but a reachable service is a risk indicator—not proof that a broker obtained access or advertised it. Combine external findings with vulnerability and configuration checks, credential monitoring, and internal identity, endpoint, and network evidence.

What should I check first?

Define the authorized scope, then reconcile known public IP ranges, domains, cloud environments, remote access gateways, and third-party-hosted assets. Use an authorized external scan to find what answers from the internet, validate ownership, and prioritize unnecessary exposure and high-impact or known exploited vulnerabilities.

Does a leaked password mean someone used it?

Not necessarily. It may be expired or otherwise invalid, and its appearance in an exposure source does not prove it was used. Confirm its status through internal identity systems, reset or revoke it when warranted, inspect sessions and authentication records, and investigate suspicious activity.

What signs should trigger incident response?

Suspicious VPN or remote-access activity, unfamiliar locations or devices, repeated failures followed by a success, new privileged accounts, unexpected privilege changes, unknown accounts, or unexpected remote tools warrant investigation. Follow the organization’s incident response process, identify affected systems and accounts, examine relevant logs, and assess containment and threat hunting for persistence as appropriate.

Does Microsoft Defender for Endpoint provide a complete internet-facing asset inventory?

No. Microsoft’s documentation says its internet-facing device feature identifies onboarded Windows devices, based on external scans and incoming connections. It can show the public IP and port associated with a detection and offers advanced hunting queries, but it should be reconciled with other records and does not establish coverage of every operating system, cloud service, or asset.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Is there a reliable statistic for how many organizations are exposed to IABs?

The cited guidance does not provide a general statistic for the share of organizations currently exposed to IABs. CISA, FBI, and HHS reported more than 500 Medusa victims as of April 2026, but that figure concerns Medusa victims across multiple critical-infrastructure sectors—not all IAB activity or the probability that a particular organization is exposed.

Should I buy a scanner or other product to check for broker access?

A scanner, router, firewall, cable tester, or other generic product cannot determine whether a broker obtained or advertised access. The relevant checks depend on authorized asset inventories, scanning and configuration tools, identity telemetry, credential exposure monitoring, and incident response procedures. No specific product cost or partner program is established here; compare any tool by its coverage, evidence, freshness, validation, and response path.

The Bottom Line

Check exposure with an authorized, validated inventory of internet-facing assets, then review service necessity, patching, remote-access controls, credentials, and authentication activity. An exposed service or leaked credential is not proof of broker access. Suspicious logins, unknown accounts, or other evidence of unauthorized use should trigger incident response, including containment and investigation of relevant logs and possible persistence.
