Check an AI answer claim by claim, not link by link. Split the response into individual factual statements, open every cited source, find the exact passage that is supposed to support each statement, and compare the source’s wording, scope, date, evidence and caveats with the AI’s wording. If a claim cannot be traced, the citation is only topical, or an important qualification has disappeared, treat the claim as unverified until you revise it or confirm it independently.
This matters because fluent prose and a visible citation are not proof. Microsoft says an untraceable statement should be treated as unconfirmed, and the House of Commons Library advises treating every factual statement produced by AI as unverified until it has been thoroughly checked.
What it means for a source to support a claim
A source supports a claim only when the source’s own material establishes the proposition attached to it. A page can discuss the same subject, contain similar words, or appear in a search result without proving the statement in an AI answer.

- Direct support: the source explicitly states the same proposition, with the same subject, timeframe, units, population and conditions.
- Partial support: the source supports only part of a compound sentence or supports it with a limitation the answer omitted.
- Contradiction: the source reaches a different result, uses a narrower definition, or states an exception that changes the answer.
- Not addressed: the source is relevant to the general topic but does not establish the specific statement.
- Untraceable: the source cannot be found, the link does not work, or the cited material cannot be identified.
Two separate questions are easy to confuse. Citation recall asks how much of the answer’s generated content is actually supported by citations. Citation precision asks how often a citation supports the particular statement beside it. An answer can have many links and still perform poorly on either measure.
A claim-by-claim verification workflow
1. Break the answer into checkable claims
Do not verify a paragraph as one unit. Separate every factual proposition, including dates, figures, percentages, names, quotations, legal references, causal explanations and recommendations. Compound sentences often hide several independent claims.
For example, the sentence “The policy took effect in 2024, reduced costs by 20%, and applies to all small businesses” contains at least three claims. The effective date, the measured reduction and the scope of coverage may come from different evidence—or none at all. Put each one on its own line before checking citations.

2. Open the cited link itself
Follow the link rather than trusting a citation label, a footnote title or the AI’s description. Confirm that it loads, identifies the source named in the answer and concerns the same subject. A search-result snippet is not evidence, because it can be truncated, stale or detached from the page’s qualifications.
Government of British Columbia guidance warns that AI-provided links may be outdated, lead nowhere or point to unrelated material. A broken or mismatched link makes the attached claim unverified; do not infer what the missing page might have said.
3. Locate the supporting passage
Use the page’s search function for distinctive terms, then read the paragraph, table, footnote or section where the evidence appears. Record the page number, heading, table row or other location so another person can find it.
Ask: does this passage establish the exact proposition, or merely mention the topic? A study about a technology does not automatically support a claim about its safety, price or performance. A government page describing a rule does not necessarily prove that every organization follows it.

4. Compare wording, scope and qualifications
Place the AI sentence beside the source passage. Check the following details without silently broadening either one:
- Who or what was measured, regulated or described?
- What period, jurisdiction, edition or version does the evidence cover?
- Are figures expressed in the same units and based on the same denominator?
- Does the source report correlation, causation, an estimate, a forecast or an example?
- Are conditions, exclusions, confidence intervals, sample limits or definitions missing?
- Did the answer turn “may” or “under these conditions” into a universal statement?
Microsoft’s validation framework specifically asks whether an answer blended ideas, overstated certainty, filled gaps with assumptions or omitted dependencies and risks. A citation is not adequate when the source supports only a narrower, conditional version of the sentence.
5. Read around the passage
Read enough surrounding text to understand the method and context. Check introductions, definitions, methods, notes, appendices, tables and headings when they affect interpretation. A sentence in a summary may describe a selected subgroup; a footnote may reveal that a number is preliminary; a legal provision may have an exception in the next paragraph.
Write down what the source does not establish. That negative finding prevents a later rewrite from accidentally turning limited evidence into a broad conclusion.
6. Check authority and currency
Match the source to the significance of the claim. Prefer original documents, official data, relevant primary records, peer-reviewed work and authoritative analysis. A secondary article may help you find an original study, but it should not replace the original when the detail matters.
Confirm that the evidence is current enough. Names, officeholders, prices, software behavior, statistics, laws and technical specifications can change. The House of Commons Library and British Columbia guidance both identify dates, numbers, names, laws, quotations and citations as details that need deliberate checking.
7. Corroborate important or disputed claims
For consequential claims, find a genuinely independent source. Repeating the same wording across several pages is not independent confirmation if all of them copied one original report. Compare the underlying documents, definitions and dates.

Use appropriate professional or subject-matter review when a mistake could affect health, safety, legal rights, finances, security or major operational decisions. Independent confirmation increases confidence; it does not turn an uncertain source into certainty.

Keep a verification record for each claim. A simple ledger makes gaps visible and preserves the reasoning behind an edit.

If support is absent, revise the sentence to match the evidence, add a clear qualification, replace it with a supported claim or remove it. Microsoft recommends preserving this verification work and treating untraceable statements as unconfirmed.
Five tests for judging evidence quality
Use these tests after finding a passage. They help you compare two possible sources without treating the presence of a citation as a score.

Failure patterns that expose weak citations
Nonexistent or mismatched sources
The citation cannot be located, or the page title, author and subject do not match the answer. Treat the statement as unverified. Do not repair a fabricated reference by guessing a similar page.

Broken, stale or irrelevant links
A link may have moved, expired or redirected to a generic page. Check the publication’s official search, an archived authoritative record where appropriate, or a current version. If you cannot establish that the replacement is the same evidence, record the claim as unconfirmed.
Topical but non-supporting citations
An article can explain a subject without proving a particular number, outcome or recommendation. Read the relevant passage and mark the claim absent when the proposition is not there.

Overstatement
Watch for transformations such as “associated with” becoming “causes,” “can” becoming “will,” or a result in one group becoming a claim about everyone. Restore the source’s level of certainty and conditions.
Dropped caveats and omissions
A source’s limitation can be more important than its headline result. Check for exclusions, confidence ranges, competing explanations, safety warnings and exceptions. If leaving out the caveat changes the practical meaning, the original sentence is not fully supported.

Wrong or stale details
Verify names, dates, percentages, quotations, legal citations and version numbers character by character when they matter. A quotation that cannot be found in the cited document should not be presented as exact.
Unsupported synthesis
AI systems may combine several true passages into a conclusion that no source actually makes. Cite each premise separately, label your own inference as an inference and avoid implying that one citation proves the whole chain.
How much checking a claim requires
Use a risk-based approach, but never skip the basic traceability check.
- Low-impact background: confirm the source exists, find the passage and preserve ordinary qualifications.
- Time-sensitive or changeable details: check the date, version and jurisdiction immediately before publication or use.
- Numbers, rankings and quotations: verify the original table, denominator, wording and attribution.
- Legal, medical, financial, safety or security decisions: use current authoritative records, seek independent confirmation and obtain qualified professional advice where appropriate.
- Disputed or controversial claims: compare independent sources and explain meaningful disagreement rather than selecting the most confident wording.
Government of Canada guidance cautions that generated content may be inaccurate, out of date or incomplete and should be checked against trusted sources rather than treated as authoritative.
Using an AI assistant to help with verification
You can give an assistant the answer and the source text and ask it to align each claim with a passage. A useful instruction is:

For each factual claim in this answer, quote the exact supporting passage from the supplied source, identify the passage’s scope and caveats, and label the claim direct, partial, contradicted, absent or untraceable. Do not infer support from a citation label.Ask for a separate list of unsupported, overstated and inconsistent statements. Supply the actual document text or a precise excerpt when possible; asking an assistant to inspect a link it cannot access does not prove that the link supports anything.
This is an aid, not a verdict. Microsoft states, “Copilot can help you validate its output—but it can’t certify its own correctness.” If the assistant says a claim is supported, open the source and perform the comparison yourself, especially when the stakes are high.
An illustrative claim check
Suppose an AI answer says: “A 2025 study proves that the tool saves every organization 30% of its costs.” The citation points to a study reporting a 30% average reduction for a limited pilot group.

- Open the study and locate the result table and methods section.
- Check whether the 30% figure is an average, a range or a selected subgroup result, and identify the denominator and measurement period.
- Look for language such as “associated with,” “pilot,” “estimated” or “under these conditions.”
- Rewrite only what the evidence supports, for example: “In the study’s pilot group, the measured average reduction was 30% during the reported period.”
- Seek a separate study before making a recommendation about other organizations.
The example demonstrates why a citation can be real and still fail to support the sentence as written: the source may support a narrow result while the answer adds causation and universal scope.
What published guidance and evidence show
The House of Commons Library’s 2026 briefing says, “Users should treat every factual statement produced by AI as unverified until they have thoroughly checked it.” Microsoft’s guidance uses a similar rule: “If a statement can’t be traced back to a source, treat it as unconfirmed until verified.” OpenAI’s Help Center advises, “Use ChatGPT as a first draft, not a final source,” and recommends visiting links directly to verify important information, quotations, data, technical information and references to external documents.
An empirical paper, Evaluating Verifiability in Generative Search Engines (2023), evaluated Bing Chat, NeevaAI, Perplexity.ai and YouChat on a particular sample. Human evaluators found that 51.5% of generated sentences were fully supported by citations and that 74.5% of citations supported their associated sentence. Those figures describe the systems and sample studied; they are not a current rate for every AI product, model or topic. Their practical lesson is to check both recall—whether material statements have support—and precision—whether each citation supports the statement beside it.
A compact checklist before you rely on an AI answer
- Have you split the response into individual factual claims?
- Does every important claim have a source that actually exists?
- Did you open the source rather than trust the citation label or search snippet?
- Can you point to the exact supporting passage, table or record?
- Does the passage support the whole sentence, with the same scope, units, date and certainty?
- Did you read enough surrounding context to find exceptions and limitations?
- Is the source authoritative and current for this use?
- Have you independently confirmed consequential or disputed details?
- Did you record what you checked, when you checked it and what action you took?
- Have you removed, qualified or replaced every claim that remains unsupported?
When a source is inaccessible, do not infer support from the citation alone. Find an authoritative version if you can; otherwise mark the claim unverified and do not use it as established fact.
