Get emergency help now for trouble breathing, swelling of the face, lips, tongue or throat, fainting, a seizure, severe confusion, or any symptom that is rapidly worsening or could be life-threatening. Contact the prescribing clinician promptly for severe, persistent or worsening symptoms, repeated vomiting or diarrhea, inability to keep fluids down, sharply reduced urination, severe abdominal pain, a suspected dosing error, or unusual symptoms. For mild effects that are improving, follow the medicine’s Medication Guide and monitor them, but contact a pharmacist or prescriber if they persist or worsen.

The exact medicine, formulation, dose, other medicines and your medical conditions change the risk. This U.S. Food and Drug Administration (FDA)-based guide, current to October 13, 2026, cannot diagnose a symptom or make an individual triage decision.

Use this three-level check first

Do not wait for an online answer when a symptom is severe or progressing quickly. Use the table to choose the safest next step, then check the specific product’s Medication Guide and contact a health professional.

Situation What to do Examples
Possible emergency Call local emergency services or go to an emergency department now. Do not delay to report a side effect or search for more information. Difficulty breathing; swelling of the face, lips, tongue or throat; fainting; seizure; severe confusion; or another rapidly worsening, potentially life-threatening symptom.
Prompt clinician contact Call the prescriber, on-call service or pharmacist as soon as possible for medicine-specific instructions. Severe or persistent abdominal pain; repeated vomiting or diarrhea; inability to keep fluids down; markedly reduced urination; faintness; worsening symptoms; suspected dosing error; or a new, unusually severe reaction.
Mild and improving Follow the Medication Guide, record what happened and when, and monitor. Contact the prescriber or pharmacist if it does not keep improving or you are worried. Nausea, diarrhea, vomiting, constipation or abdominal discomfort that is mild, short-lived and still improving.

This is a practical decision framework, not a validated scoring system. A symptom’s severity, duration and trend matter more than whether it appears on a list of “common” reactions.

How to judge severity and trend

Ask four practical questions: Is the symptom intense enough to stop normal activities? Is it getting worse rather than easing? Can you drink, urinate and stay alert safely? Did it begin soon after a new dose, a dose increase or a possible measurement mistake? A “yes” answer should move you toward prompt professional advice. Breathing difficulty, collapse, seizure, severe confusion or rapidly worsening illness bypasses this checklist and requires emergency help.

There is no universal number of vomiting episodes or hours of pain that is safe for everyone. Age, kidney function, diabetes treatment, other medicines and the exact formulation change the risk. When in doubt, call a clinician or pharmacist rather than trying to calculate a personal threshold from a general article.

Get emergency help for these warning signs

Severe allergic reaction

Difficulty breathing or swelling involving the face, lips, tongue or throat can signal a severe allergic reaction. The FDA’s current WEGOVY prescribing information says to discontinue WEGOVY if a suspected hypersensitivity reaction occurs and promptly seek medical advice. FDA’s Zepbound approval announcement likewise says patients should stop Zepbound immediately and seek medical help if a severe allergic reaction is suspected. See the WEGOVY Prescribing Information and FDA’s Zepbound announcement.

Use emergency services for breathing trouble, throat swelling, collapse or rapidly worsening symptoms. Do not drive yourself if you are faint, confused or otherwise unsafe. Tell responders the medicine’s name, the dose and when it was taken if you can do so safely. Do not delay emergency care while searching for the package or trying to file a report.

Fainting, seizure or severe altered consciousness

Fainting, a seizure, severe confusion or inability to stay awake warrants emergency assessment. People using insulin or medicines that stimulate insulin release can have a higher risk of low blood sugar when taking a GLP-1 medicine; the WEGOVY label instructs at-risk patients to learn the symptoms of hypoglycemia. If the person cannot swallow safely, do not give food, drink or oral medicine; call emergency services.

A glucose reading may be useful information for responders when it is available without delaying help, but a device reading does not make an unsafe person safe to manage at home. Emergency clinicians need to know about all diabetes medicines, the last dose and any recent vomiting or inability to eat.

Any rapidly worsening or life-threatening symptom

The list above is not exhaustive. A rapidly worsening symptom, blue lips, severe weakness, chest pressure, or a person who cannot safely remain at home should be treated as an emergency even if the symptom is not named in the product label. If you are unsure whether a serious symptom can wait, seek urgent medical advice rather than waiting for the next routine appointment.

Call the prescriber promptly when symptoms are severe, persistent or worsening

Severe or persistent abdominal pain

Severe abdominal pain, especially with vomiting or pain that does not settle, needs clinical assessment. FDA labeling for semaglutide warns about pancreatitis and gallbladder disease and directs patients to discuss symptoms that could indicate either problem with a health care provider. Pain alone does not prove pancreatitis or gallstones, so do not try to diagnose yourself from an online symptom list. The relevant warnings are in the WEGOVY label and FDA’s cardiovascular indication announcement for WEGOVY at FDA.gov.

When calling, describe where the pain is, when it started, whether it is constant or comes in waves, its severity, and whether vomiting or other symptoms accompanied it. A clinician may need an examination or tests; the presence of a symptom alone cannot identify the cause or tell you which dose, if any, should be taken next.

Vomiting, diarrhea and dehydration symptoms

Contact a clinician if vomiting or diarrhea is repeated, you cannot keep fluids down, urination becomes markedly less frequent, you feel faint, or the problem is worsening. The WEGOVY label warns of acute kidney injury caused by volume depletion and says renal function should be monitored when adverse reactions can cause fluid loss. It does not set a universal number of episodes or a time limit that is safe for every patient.

Keep track of whether you are able to drink and urinate, but do not force fluids if you are unable to swallow safely or are repeatedly vomiting. Do not independently double, skip, restart or change a prescription dose to compensate for vomiting. Ask the prescriber or pharmacist what to do with the next dose, particularly if you have kidney disease, take diuretics, or use other medicines that affect blood pressure or fluid balance.

Diarrhea or vomiting that is mild and clearly improving may be monitored under the Medication Guide. It becomes a prompt-call issue when it interferes with hydration, causes faintness, markedly reduces urination, prevents essential medicines from being taken, or continues instead of improving.

Common effects that interfere with normal functioning

Nausea, diarrhea, vomiting, constipation and abdominal pain are listed among common WEGOVY reactions. “Common” does not mean harmless at every intensity. If an effect prevents drinking, eating enough to take essential medicines, sleeping, working safely or performing normal activities, contact the prescriber even if the symptom appears in the Medication Guide.

Similarly, a symptom that is new, unusually intense, or different from what happened after earlier doses deserves a call. The clinician can decide whether the medicine should be continued, adjusted or evaluated for another cause. Do not assume that a familiar side effect is safe simply because it happened before.

Check the exact medicine before deciding what is normal

Weight-loss medicines do not share one side-effect profile. Confirm the brand or generic name, formulation, strength, route and last dose. Read the current Medication Guide supplied with that product; labeling can change. Do not apply a warning written for one medicine to another without professional advice.

Write down whether the product is an approved prescription, an over-the-counter product, or a compounded preparation. A pen, vial and syringe can contain different concentrations or require different measuring steps. The same-looking volume is not necessarily the same dose. If any label, unit or device is confusing, pause and ask the dispensing pharmacist or prescriber rather than guessing.

GLP-1 medicines such as semaglutide and tirzepatide

For injectable GLP-1 medicines, review the product-specific warnings for allergic reactions, gastrointestinal effects, pancreatitis, gallbladder disease, kidney injury from dehydration and hypoglycemia risk when combined with insulin or an insulin secretagogue. In adult weight-reduction trials summarized in the 2025 WEGOVY label revision, severe gastrointestinal adverse reactions occurred in 4.1% of WEGOVY-treated participants versus 0.9% with placebo. Those are trial results for the studied product and population, not a personal probability or a threshold that determines when you should seek care.

The hypoglycemia warning is especially relevant when a person also uses insulin or a medicine that stimulates insulin release. Ask the prescriber in advance which symptoms should prompt a glucose check, a same-day call or emergency care. Do not change diabetes treatment on your own to manage a suspected side effect.

Over-the-counter alli (orlistat)

FDA added a kidney-injury and kidney-stone warning for alli (orlistat) in June 2026. Stop alli and consult a health care provider if you develop back or groin pain, painful urination, blood in the urine, swelling of the feet or legs, or less frequent urination. This instruction is specific to orlistat and should not be generalized to every weight-loss medicine. Read the FDA safety communication at FDA Approves Labeling Changes for OTC Weight Loss Drug alli.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

FDA’s review described 12 postmarketing alli-associated kidney-complication cases: eight acute kidney injuries, two acute kidney injuries with oxalate nephropathy, and two cases of hyperoxaluria with calcium oxalate stones. Eight people were hospitalized and five required dialysis. These reports may be incomplete and are not an incidence rate; they do not predict what will happen to an individual user. The numbers explain why the listed urinary and flank symptoms should not be ignored, but they do not establish that every similar symptom was caused by alli.

What to do if you may have taken too much

Prescription or pharmacy-dispensed product

Call the prescribing clinician, dispensing pharmacist or local poison control service for immediate, individualized instructions. Have the package available and state exactly what was taken, when and how it was measured. Do not take an extra dose later unless a clinician specifically tells you to.

A suspected error includes injecting twice, using the wrong pen strength, confusing milligrams with milliliters, drawing up the wrong volume, or taking a dose intended for another person. Even if you feel well, report the possible error promptly so a professional can explain what to watch for and when the next dose should occur.

Compounded semaglutide or tirzepatide

Tell the clinician that the product was compounded, where it came from, its concentration, the syringe or device used, the intended dose and the amount actually injected. FDA reports dosing mistakes, serious adverse events and some hospitalizations involving compounded GLP-1 products. Its safety communication explains that compounded products are not FDA-approved and do not receive FDA review for safety, effectiveness and quality before marketing. It also warns about fraudulent products and measurement errors; see FDA’s Concerns with Unapproved GLP-1 Drugs Used for Weight Loss.

As of May 31, 2026, FDA had received 990 adverse-event reports involving compounded semaglutide and more than 730 involving compounded tirzepatide. These are reports, not rates; they may be underreported, cannot establish that the product caused an individual event and should not be used to calculate personal risk. The important action is disclosure: a clinician needs the exact concentration and measuring device to interpret what happened.

How to prepare for a call or urgent visit

  1. Bring the product information. Keep the box, vial, pen or a clear photo available. Include the generic and brand name, concentration, formulation and expiration information if shown.
  2. Record the timing. Write down the dose, date and time of the last dose, when the symptom began, whether it is improving or worsening, and any doses that may have been missed or repeated.
  3. Describe the symptom precisely. Note location, severity, duration, vomiting or diarrhea frequency, fluid intake, urination, fever, rash, swelling, breathing changes, faintness and ability to stay awake.
  4. List other medicines and conditions. Mention insulin or medicines that increase insulin release, diabetes, kidney or gallbladder problems, allergies and every prescription, over-the-counter medicine and supplement.
  5. Disclose the source and any measuring problem. Say if the product was compounded, purchased online or from an unfamiliar source, or if a syringe, concentration or measuring step was confusing.

These details help a clinician distinguish an expected transient effect from dehydration, hypoglycemia, an allergic reaction, a medication interaction or a dosing error. They do not replace an examination or testing. If you go to urgent care or an emergency department, bring the container or a photograph rather than relying on memory.

What not to do while you are deciding

  • Do not wait for a scheduled appointment when breathing is difficult, the throat or face is swelling, you have fainted or seized, or symptoms are rapidly worsening.
  • Do not assume a symptom is safe because it is listed as common. Severity, persistence and inability to hydrate change the risk.
  • Do not stop, restart, double or change a prescription dose on your own based only on a general article. Obtain medicine-specific instructions, except where emergency labeling specifically directs immediate discontinuation for a suspected severe allergic reaction.
  • Do not take another person’s injection, use an unlabeled vial or guess a compounded concentration.
  • Do not use a glucose meter, smartwatch or symptom tracker as a substitute for emergency assessment. These tools may provide information for a clinician but cannot determine urgency by themselves.
  • Do not wait to contact a professional until you can prove the medicine caused the symptom. A suspected connection is enough to disclose; the clinician will assess other possible causes.

Medication Guides, follow-up and reporting

Keep the current Medication Guide with the medicine and review it whenever the dose, formulation or manufacturer changes. Ask the prescriber or pharmacist which symptoms should trigger a same-day call for your specific product and what to do if you vomit near dose time.

After you receive instructions, follow the stated plan for the next dose and know which changes require another call. If symptoms persist, worsen, or new warning signs appear, update the clinician rather than assuming the first advice still applies. A mild symptom that resolves can still be documented at the next visit, especially if it recurs after each dose.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Suspected adverse reactions can be reported to FDA MedWatch, but reporting does not replace medical evaluation. If symptoms are urgent, seek care first and report later when safe. FDA-approved labels and safety communications can change, so verify the current instructions for your medicine.

Questions readers commonly ask

Is this side effect normal?

A mild effect that is improving may be monitored according to the Medication Guide. Severe, persistent, worsening or function-limiting symptoms require a call to the prescriber, even when the effect is listed as common. “Normal” cannot be decided from the symptom name alone; the intensity, trend, hydration and exact product matter.

When should I call my doctor about a weight-loss injection?

Call promptly for severe abdominal pain, repeated vomiting or diarrhea, inability to keep fluids down, reduced urination, faintness, a new severe reaction or any suspected dosing error. Use emergency services for breathing trouble, facial or throat swelling, fainting, seizure or another rapidly worsening, life-threatening symptom.

Should I stop taking my weight-loss medication if I am vomiting?

Repeated vomiting or inability to keep fluids down warrants prompt professional advice because dehydration can injure the kidneys. Do not independently change the next dose; ask the prescriber or pharmacist for instructions tied to your exact product and schedule. A clinician may need to consider kidney function, other medicines and whether the vomiting reflects another urgent problem.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can stomach pain be pancreatitis?

Severe or persistent abdominal pain, particularly with vomiting, needs clinical assessment. Semaglutide labeling warns about pancreatitis and gallbladder disease, but symptoms alone cannot establish either diagnosis. Seek urgent help if the pain is rapidly worsening or accompanied by collapse, severe weakness or another emergency sign.

What if I took too much semaglutide?

Contact the prescriber, pharmacist or poison control service immediately with the product, concentration, amount, time and measuring device. If the product was compounded, disclose that fact and any uncertainty about the intended dose. Seek emergency care for severe or rapidly worsening symptoms. Do not take another dose to “correct” the mistake unless a professional gives that instruction.

Are all weight-loss medicines covered by the same warning?

No. Wegovy, Zepbound, alli and compounded products have different labels, formulations and instructions. For example, FDA’s urinary warning about back or groin pain, painful urination, blood in urine, leg or foot swelling and reduced urination is specific to alli. Confirm the exact medicine before applying any warning.

Should I report a suspected side effect?

You can report suspected adverse reactions to FDA MedWatch, but reporting is optional and does not replace treatment. First obtain emergency or clinician care when needed. Keep the product details and your symptom timeline so the report and the clinician’s assessment are as accurate as possible.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Bottom line

Use emergency services for breathing trouble, facial or throat swelling, fainting, seizure, severe confusion or any rapidly worsening, potentially life-threatening symptom. Contact the prescriber promptly for severe or persistent abdominal pain, repeated vomiting or diarrhea, inability to keep fluids down, markedly reduced urination, faintness, unusual reactions or a dosing error. Mild effects that are clearly improving can usually be monitored under the product’s Medication Guide, but the exact medicine and a person’s other treatments determine what is safe. When uncertain, ask a clinician or pharmacist rather than changing the dose yourself.

Frequently Asked Questions

When is a weight-loss drug side effect an emergency?

Call local emergency services for trouble breathing, swelling of the face, lips, tongue or throat, fainting, seizure, severe confusion, or any rapidly worsening or potentially life-threatening symptom. Do not delay care to search for information.

When should I call my prescriber about vomiting or diarrhea?

Call promptly if vomiting or diarrhea is repeated, you cannot keep fluids down, urination is markedly reduced, you feel faint, or symptoms are worsening. The WEGOVY label warns that fluid loss can contribute to acute kidney injury.

Should I change my next dose after vomiting?

Do not independently skip, double, restart or change a prescription dose. Ask the prescriber or pharmacist for instructions specific to the medicine, dose and timing of the vomiting.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What symptoms require stopping alli (orlistat)?

FDA advises stopping alli and consulting a health care provider for back or groin pain, painful urination, blood in the urine, swelling of the feet or legs, or less frequent urination. This warning is specific to alli.

What should I do after a compounded semaglutide dosing mistake?

Contact the prescriber, pharmacist, poison control service or compounder promptly. Provide the concentration, intended and injected amounts, syringe or device, timing and product source. Seek emergency care for severe or rapidly worsening symptoms.

Can common nausea or abdominal discomfort still need medical advice?

Yes. A symptom listed as common still warrants a call if it is severe, persistent, worsening, prevents hydration or normal activities, or feels unusually different from prior doses.

The Bottom Line

Emergency symptoms need emergency services. Severe, persistent or worsening effects, dehydration signs, severe abdominal pain and dosing errors need prompt professional advice. Monitor only mild effects that are improving, and always follow the exact medicine’s Medication Guide rather than changing doses on your own.
