Before entering a diagnosis, prescription, test result, image, or any other identifiable health detail, audit the exact chatbot, account, plan, and privacy controls you are about to use. Turn off model-training or activity use where available, check chat history separately, start a temporary or incognito conversation, disable memory and personalization, review connected apps and uploads, and read the provider’s retention and human-review terms. These controls reduce some uses of your data; they do not make disclosure risk-free or prove that a consumer chatbot is suitable for clinical or legally protected information.
The labels and protections differ by product, region, account type, app version, and administrator policy. Treat the following as a checklist to verify in the current app and its current terms, not as a permanent promise from any provider.
Why the exact account matters before you type anything
Privacy settings belong to a particular service and account context. The same chatbot can expose different controls to a personal user, a school account, a company-managed workspace, or a healthcare-oriented product. Your country, age eligibility, subscription, and administrator policy can also change what you see.
Start by recording four details: the product name, the signed-in account, the plan or workspace, and the device or app version. Confirm whether the account is personal, employer-managed, school-managed, or supplied by a healthcare organization. In managed workspaces, an administrator may set retention, memory, compliance, or access rules that you cannot override. OpenAI says ChatGPT control availability depends on sign-in state, plan, and workspace settings; managed workspaces can impose organization-level retention and access policies.

If you cannot identify who controls the account or where its data is governed, do not enter identifiable health information. Use a de-identified description instead—for example, remove names, exact dates, medical-record numbers, addresses, photographs, and rare combinations of facts that could identify a person.
The pre-sharing audit: seven checks in the order you need them
- Confirm the service and account. Verify the product name, signed-in identity, plan, region, and whether an administrator controls the workspace.
- Find the model-training or activity control. This determines whether new conversations may be used to improve models, but it is separate from chat history and deletion.
- Use temporary or incognito mode. Check what the mode excludes and how long the provider may retain content for safety or service operations.
- Inspect memory and personalization. Turn off saved memories or personalization if you do not want earlier health details influencing later answers.
- Read retention, review, and deletion terms. Look for safety windows, human review, legal exceptions, and delays between deleting activity and deleting backend copies.
- Audit feedback, integrations, files, and sharing. A thumbs-up, uploaded document, connected app, browser context, plugin, or public link can create a separate data path.
- Decide whether the use is professional or regulated. Consumer toggles do not establish a healthcare contract, regulatory compliance, or permission to upload protected records.
Training controls are not chat-history controls
First locate the setting that governs model improvement, training, or activity. Then separately inspect the history or activity page. Turning off training generally affects future use of conversations for model improvement; it does not automatically erase conversations already visible in your account.

These are provider-stated policies, not independent audits of every implementation or every exceptional legal and safety circumstance. Read the wording shown in your account before relying on a switch.

Open Settings, choose Data controls, and turn off Improve the model for everyone. OpenAI states: “When Improve the model for everyone is off, your new conversations won’t be used to train OpenAI models.” The setting applies across devices when you are signed in. It does not remove existing conversations from the history list; delete those conversations using the separate history controls.
Do not submit a thumbs-up or thumbs-down on a sensitive conversation after opting out without reading the current warning. OpenAI says the entire conversation associated with submitted feedback may be used to train models, even when the general training setting is off.

Gemini: turn off Keep Activity and check the feedback exception
Open Gemini Apps Activity and inspect Keep Activity. Google says that, when it is off, future chats will not appear in Activity and will not be used to train AI models unless you send feedback. The same policy says Gemini may retain chats for up to 72 hours to provide responses, process feedback, and protect the service. Therefore, “off” means a narrower activity record and training use, not instant zero retention.
Claude: verify the current consumer Privacy Settings
In Claude’s consumer account, open Privacy Settings and review the model-improvement choice for that account. Anthropic says Incognito chats are not used to improve Claude, even when Model Improvement is enabled. Anthropic’s consumer retention notice, dated July 1, 2026, says conversations used for model improvement may be kept in de-identified form for up to five years.

Anthropic says chats and coding sessions from commercial offerings such as API, Console, Team, and Enterprise are not used for training unless the customer joins the Development Partner Program, reports material, or opts in. That policy is distinct from consumer-plan settings.
Temporary and incognito chats still have retention windows
A temporary mode usually changes history, memory, or model-improvement behavior. It does not necessarily prevent short-term storage for safety, abuse detection, response delivery, troubleshooting, or legal obligations. Before using one, open its explanation in the app and note the stated retention period.

On ChatGPT, check personalization when you start a Temporary Chat: OpenAI says an otherwise temporary conversation may use existing memories, custom instructions, and plugins if personalization remains enabled. A temporary label is therefore not a substitute for checking the settings that feed context into the conversation.

Memory and personalization are separate from training
Model training asks whether your conversation may help improve a model. Memory asks whether information from your conversations can be saved and used to personalize later responses. You need to inspect both.
ChatGPT memory
OpenAI describes Memory and model-training controls as separate. Turn memory off in the personalization controls, or start a Temporary Chat and disable personalization at the start if you do not want existing memories, custom instructions, or plugins to contribute context. Also inspect and delete individual saved memories if the interface offers that list; deleting a chat does not necessarily mean every separately saved memory has been removed.
Gemini memory and saved information
Google says Gemini may use sensitive information from prior chats to personalize responses when Memory is on. In the Gemini Privacy Hub, review the separate controls for activity, saved information, and Connected Apps. Turning off Gemini settings does not change other Google settings, including Web & App Activity, so audit those controls independently if they are relevant to your account.
Claude personalization
Use the current Claude account controls to determine whether conversation context or saved preferences can carry into later chats. If you cannot find a clear memory or personalization switch, assume that the temporary-mode and retention statements do not by themselves answer how future responses are personalized; avoid identifiable health details.
Retention, human review, and deletion: what “off” does not promise
Read the provider’s retention language before disclosure. Look for separate periods for ordinary service operation, safety investigations, human review, feedback, policy violations, backups, and model-improvement pipelines.

These periods come from provider-published pages and can change. They are not statistical findings or universal guarantees. A legal hold, abuse investigation, safety review, submitted feedback, or other stated exception can produce a different outcome. If the app’s current notice conflicts with an older help page, follow the current account notice and terms.
Audit feedback, uploads, connected apps, and public sharing
Privacy controls for ordinary text chat do not automatically govern every feature attached to a conversation.
- Feedback: Treat thumbs-up, thumbs-down, ratings, and written reports as a new disclosure decision. OpenAI warns that submitting feedback can make the entire associated conversation available for model training. Google also identifies feedback as an exception to the no-training effect of Keep Activity off.
- Files and images: Remove names, faces, barcodes, metadata, and document identifiers before uploading. Check how long attachments remain and whether deleting the message also deletes the uploaded file.
- Connected health or productivity apps: Review each connection’s permissions and privacy policy. Google says third-party connected apps have their own policies; disconnecting an app or deleting Gemini activity may not delete information the app already received.
- Browser and page context: A chatbot may receive the page, selected text, or other context you intentionally attach. Do not assume the ordinary chat setting covers that external source.
- Plugins, tools, and agents: A tool can send prompt content to another company. Remove integrations that are not needed and inspect the destination’s retention and deletion terms.
- Shared links: Check whether a conversation can be opened by anyone with a link, indexed, copied, or forwarded. Revoke public links before discussing health information.
Google’s Privacy Hub lists files, images, shared browser content, and connected-app information among data Gemini Apps may process. That is why an audit must include the whole prompt path, not just the text box.

A health-branded mode, encryption statement, or training opt-out is not the same as a contractual or regulatory assurance. For work involving identifiable patient records, confirm the exact product, data-processing terms, administrator controls, and agreements required by your organization and jurisdiction.

OpenAI states that connected medical records and Apple Health information in ChatGPT Health, and conversations using that connected data, are not used to train its foundation models or target ads. The same OpenAI statement says: “Health in ChatGPT is not intended for clinical or covered-entity use and does not offer a Business Associate Agreement.” Therefore, that feature should not be treated as a substitute for a healthcare system with the agreements and controls your organization requires.

If you are a clinician, covered entity, school nurse, insurer, researcher, or contractor handling regulated data, ask your privacy or security officer before using any consumer chatbot. Verify whether a Business Associate Agreement or equivalent contract exists for the exact plan, region, and data flow. If the provider cannot give a clear answer, do not upload protected records.
A practical decision rule before sending a health prompt
Use a consumer chatbot only for de-identified, low-consequence questions when
- You have confirmed the exact account and workspace owner.
- Training or activity use is disabled where possible.
- Temporary or incognito mode is enabled and its retention window is acceptable.
- Memory, personalization, feedback, integrations, uploads, and sharing are either disabled or understood.
- The question does not require a person’s identity, exact dates, images, record numbers, or a regulated workflow.
Stop and obtain organizational approval when
- The prompt contains identifiable patient, employee, student, or client information.
- The chatbot is connected to a medical-record system, health app, cloud drive, browser, or third-party tool.
- You need a contractual assurance, audit trail, access-control regime, or deletion guarantee.
- The account is managed by an employer or school and you do not know the administrator’s retention and review policy.
- The answer could affect diagnosis, treatment, eligibility, employment, insurance, or another high-consequence decision.
What to record for your own audit
For a one-off personal question, a quick screenshot or note can prevent confusion later. Record the date, product and app version, account type, plan, region, training/activity setting, temporary-mode setting, memory status, connected apps, upload or sharing status, and the retention language shown by the provider. Keep this record separate from the health information itself.
Recheck these items whenever the provider changes its interface, you switch accounts, an administrator changes workspace policy, or you enable a new integration. A setting that was available in one account may be absent or governed differently in another.
The bottom line for safer health questions
There is no single “private mode” that guarantees an AI conversation will never be stored, reviewed, remembered, or shared with another service. The safest workflow is to minimize the information first, then verify account context, training controls, history, temporary mode, memory, retention, review, integrations, and contractual suitability. If the information is identifiable or regulated, use a system approved for that purpose rather than relying on consumer privacy toggles.
