There is no universally correct order for calibration and ensembling medical-imaging classifiers. Define what each score means, choose and document the aggregation rule, fit calibration on data kept separate from model training, and evaluate the exact combined pipeline on untouched—and preferably external—data. Compare “aggregate then calibrate” with “calibrate then aggregate” under the same split design rather than assuming that calibrating every model first will produce a better ensemble.
What calibration means—and what it does not
A classifier is calibrated when its predicted probabilities agree with observed outcomes. If a group of examinations receives a predicted risk of 0.8, roughly 80% of those cases should have the outcome being predicted, within the population and setting in which the model is used. Calibration is therefore a property of the probabilities, not merely of the class labels.
Accuracy answers a different question: how often the selected labels are correct at a chosen threshold. Discrimination asks whether higher-risk cases tend to rank above lower-risk cases, commonly summarized with measures such as the area under a receiver-operating-characteristic curve. A model can rank patients well yet be systematically overconfident, underconfident, or shifted away from the observed event rate. Conversely, a model can have reasonable average calibration while offering weak ranking ability. A medical-imaging ensemble needs both properties assessed separately.

Calibration also depends on the target population, prevalence, acquisition protocol, reference standard, and clinical workflow. A probability that is reliable in one hospital or research cohort may not retain that meaning after a change in scanners, patient mix, disease prevalence, or labeling practice.

Why the order of calibration and aggregation is unsettled
Combining classifier outputs changes the score distribution that a calibrator sees. Calibrating each component changes the inputs to the aggregation rule. Those operations generally do not commute: applying them in the opposite order can produce different probabilities, slopes, and subgroup behavior.
A 2022 comparative study evaluated four calibration models in natural-image and medical-imaging settings, including lung-cancer risk estimation and balanced versus imbalanced training conditions. Its reported behavior varied by domain. The authors cautioned that combining methods intended to reduce overconfidence can create underconfidence, and that techniques that appear useful in general computer vision can damage calibration in medical imaging. That study is an arXiv preprint; its publication status was not verified here, so its findings should be treated as comparative evidence rather than a universal rule.
The evidence does not identify one correct aggregation rule for an unspecified medical-imaging task. Probability averaging, logit averaging, weighted combinations, stacking, and other schemes make different assumptions about score semantics, labels, dependence between models, and the intended decision. Treat the chosen rule and the calibration procedure as one pipeline, and validate that pipeline as a unit.

Before fitting any calibrator, write down exactly what a score represents. “Confidence” can mean the probability of a disease, the probability that an image is correctly classified, the probability of a finding being present, or a ranking score that was never intended to be probabilistic. A calibration method cannot repair an ambiguous target.
Binary classification
Specify the event coded as one. For example, decide whether the output is the probability of malignancy, the probability of a positive reference-standard diagnosis, or the probability that a particular finding is absent. Record the reference standard and the time horizon if the outcome is time-dependent.
Multiclass classification
List every mutually exclusive class and state whether the classifier outputs a normalized distribution whose components sum to one. Decide whether calibration will be assessed per class, across all predictions, or both. A model can be well calibrated for common classes while failing for rare classes.
Multilabel classification
Clarify whether each label is an independent event and whether labels can co-occur. A single global calibration statement can hide serious errors for a particular finding. Define the unit of evaluation—image, study, series, or patient—and keep that unit consistent across all models.
Clinical meaning and operating point
State who will act on the output and what action thresholds are contemplated. A score used to trigger urgent review has different consequences from one used to prioritize a research queue. This decision context determines which calibration and utility analyses are necessary later.
Step 2: Specify the ensemble before fitting calibration
Document the aggregation rule in a versioned analysis plan. Include the inputs, transformations, weights, missing-output behavior, tie handling, and any thresholding. Do not average hard labels when the intended goal is a probability unless that choice is explicitly justified; converting probabilities to labels discards information.

| Design choice | What to record | Why it matters |
|---|---|---|
| Score scale | Probability, logit, margin, or another score; range and direction | Different scales are not interchangeable. An average has a different meaning on each scale. |
| Aggregation | Unweighted or weighted combination, stacking, voting, or another rule | The rule defines the ensemble output that must ultimately be calibrated. |
| Weights | Fixed values or values learned from data; training source for learned weights | Weights learned on evaluation data can make calibration and discrimination look too optimistic. |
| Label structure | Binary, multiclass, or multilabel; class order and coding | A calibration result is meaningful only for the stated outcome definition. |
| Missing or failed model output | Fallback, exclusion, or explicit missingness policy | Deployment-time failures can change the score distribution from that used for validation. |
The absence of a specified task, modality, prevalence, and score semantics means no particular aggregation scheme can responsibly be declared best in advance.
Step 3: Separate training, calibration, and evaluation data
Do not use the data that trained the classifiers as though it were an independent calibration or performance assessment. A practical design has at least three roles:
- Model-training data: used to fit the imaging classifiers and any feature-learning components.
- Calibration data: held apart from classifier fitting and used to estimate the parameters of the chosen calibration method or to learn ensemble weights.
- Final test data: untouched until the complete aggregation-and-calibration pipeline is frozen.
When possible, add an external dataset from a different institution, time period, scanner configuration, or patient population to assess transportability. The external set should remain untouched while choices are made. If data are clustered by patient, site, or examination series, split at the relevant cluster level to prevent near-duplicates from crossing partitions.
Repeated resampling or cross-validation can help when data are limited, but it does not turn training performance into independent validation. Clinical-prediction guidance summarized by Binuya and colleagues from 70 guidance papers emphasizes evaluating models in appropriate validation data and using external validation when generalizability is important. It also cautions against treating a simple data split as a substitute for stronger validation designs when those are feasible.
Step 4: Compare the two plausible operation orders
If the practical question is “Should I calibrate each model before averaging its predictions?”, the defensible answer is: not automatically. Compare candidate orders using the same data partitions, outcome definitions, and reporting plan.
Workflow A: aggregate first, calibrate the ensemble
- Run each frozen classifier on the calibration set.
- Apply the pre-specified aggregation rule to obtain one ensemble score per evaluation unit.
- Fit a calibration model to those ensemble scores and observed outcomes on the calibration set, using a design that avoids fitting and judging on the same observations when the method requires additional internal validation.
- Apply the frozen aggregation and calibration steps to the untouched test set.
This workflow directly calibrates the quantity that will be shown or consumed by the clinical system.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesWorkflow B: calibrate components first, then aggregate
- Generate each classifier’s raw output on the calibration set.
- Fit a separate calibrator for each classifier using only its own output and the defined target.
- Freeze those component calibrators and transform the component outputs.
- Apply the pre-specified aggregation rule to the transformed outputs.
- Evaluate the resulting ensemble on untouched data. If ensemble weights or a second calibration layer are learned, that fitting requires its own protected data or a carefully designed nested procedure.
This workflow may be attractive when each component score is independently consumed or when the aggregation rule has a clear probabilistic interpretation. It can also alter dependence and variance between models, so component-level calibration does not establish ensemble-level calibration.
Use a common comparison design
For both orders, keep the patient-level partitions, outcome definitions, preprocessing, aggregation rule, and test set identical. Report the complete chain, not only the best-looking metric. If a third workflow—such as calibrating raw ensemble logits or adding a second-stage calibrator—is scientifically plausible, include it only if its fitting data are protected in the same way.
Step 5: Evaluate calibration with complementary measures
No single number describes calibration completely. Present a reliability diagram that compares predicted probability bins with observed outcome frequencies, including the number of cases in each bin. Show uncertainty intervals or sample counts where feasible so that an apparently extreme bin with few examinations is not overinterpreted.

Calibration-in-the-large
Calibration-in-the-large assesses whether predictions are systematically too high or too low overall. A positive or negative shift can reflect a prevalence difference between development and evaluation populations even when ranking remains strong.
The Tool Desk
Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →Outbyte PC Repair FREEClear out junk files and repair common Windows errorsFree Scan →Calibration slope
The calibration slope assesses whether predictions are too extreme or too compressed. A slope below one is consistent with over-dispersed, overconfident predictions; a slope above one suggests insufficient spread. Interpret the estimate with its uncertainty and in the context of the calibration plot.
Expected calibration error
Expected calibration error (ECE) is used in medical-image classification literature, including the systematic review by Kurz and colleagues, but its value depends on binning choices and sample size. Report the binning scheme and do not use ECE as the sole basis for selecting a pipeline.
Discrimination and classification summaries
Report discrimination separately from calibration, using measures appropriate to the task. Include threshold-based metrics only with their operating threshold, prevalence, and confidence intervals. A change in accuracy can result from a threshold shift without any improvement in probability quality.
Subgroups and classes
Where the intended use requires it, inspect calibration by clinically relevant subgroup, site, scanner, age range, sex, disease prevalence, and class. For multiclass or multilabel tasks, show class-specific results rather than allowing aggregate averages to conceal a failing label.

Step 6: Test clinical usefulness, not only statistical calibration
A statistically calibrated score is not automatically useful for care. If the output informs a decision, evaluate the consequences of acting at plausible thresholds. Decision-analytic measures such as net benefit can show whether using the model provides more benefit than treating everyone, treating no one, or using an existing strategy across a range of threshold probabilities.
Describe the action linked to each threshold, the harms of false positives and false negatives, and whether a clinician can override the recommendation. Net-benefit analysis should use the same population and outcome definition as the proposed use; it should not be presented as a universal property of the model.
What the medical-imaging evidence says about methods
Kurz and colleagues’ 2022 systematic review screened medical-image classification studies published from 2016 through 2021 and selected 22 papers for detailed analysis. It found a wide range of uncertainty-estimation approaches. Monte Carlo dropout and deep ensembles appeared most frequently among the reviewed studies, but frequency is not a head-to-head finding that either method is superior or best calibrated.
The review describes reliability diagrams and ECE as common assessment tools. Its coverage is limited to the studies and period examined, so it does not establish a minimum calibration-set size, a universal threshold for acceptable error, or a preferred ensemble recipe for the scenario in this article.

External validation, robustness, and regulatory context
Calibration should be checked in settings that resemble intended deployment and in plausible shifted settings. Examine changes in prevalence, image quality, acquisition protocol, referral pattern, and reference-standard practice. If performance degrades, determine whether the issue is a prevalence shift, covariate shift, label shift, missing model output, or a change in the clinical process before choosing a recalibration strategy.
AAPM Task Group 273’s report on artificial intelligence and machine learning in computer-aided diagnosis in medical imaging stresses proper training and rigorous validation of generalizability and robustness before clinical adoption. That principle applies to an ensemble even when every component has already been evaluated separately.
The U.S. Food and Drug Administration’s September 2022 final guidance, Clinical Performance Assessment: Considerations for Computer-Assisted Detection Devices Applied to Radiology Images and Radiology Device Data in Premarket Notification (510(k)) Submissions, addresses clinical-performance assessment for radiology computer-assisted detection devices, including computer-assisted detection within combined systems. It is not a universal calibration standard. Confirm its current status and applicability to the specific device, jurisdiction, and submission before relying on it for regulatory decisions.
Deployment monitoring and recalibration
Calibration is a lifecycle property. After deployment, monitor the relationship between predicted probabilities and observed outcomes when outcome ascertainment is timely and reliable. Track changes in case mix, prevalence, scanner or protocol, referral pathways, and component-model availability. A recalibration decision should follow a predefined governance process that preserves an independent evaluation set for confirming the change.
The sources do not establish a mandatory monitoring interval or a single regulatory rule for recalibration. Set the interval and alert criteria according to clinical risk, data volume, outcome delay, and the applicable quality system, and document who can approve a change.

Calibrating on training predictions
Symptom: calibration appears excellent in development but deteriorates on new cases. Fix: generate calibration predictions from data not used to fit the classifiers, and reserve untouched data for the final assessment.
Assuming component calibration guarantees ensemble calibration
Symptom: every classifier looks reliable alone, but the combined score is overconfident or underconfident. Fix: evaluate the complete aggregation-and-calibration pipeline and plot its probabilities directly.
Combining overconfidence-reducing methods without checking spread
Symptom: probabilities cluster near the middle and the calibration slope indicates underconfidence. Fix: compare operation orders and inspect calibration-in-the-large, slope, and the reliability diagram rather than stacking methods because each helped separately.

FAQ
Should I calibrate each model before averaging its predictions?
Only if that ordering is justified by the score semantics and performs well in a controlled comparison. Component calibration alone does not establish calibration of the final ensemble; compare it with calibrating the aggregated output on the same protected data design.
Is a perfectly calibrated model necessarily accurate?
No. Calibration concerns probability–outcome agreement. A model can be calibrated while offering weak discrimination, and a highly discriminative model can be badly calibrated.
Is ECE enough to choose between two workflows?
No. ECE depends on binning and can conceal local failures. Use the reliability diagram, calibration-in-the-large, slope, discrimination, subgroup results, and decision-analytic usefulness relevant to the intended action.
Can I validate calibration with a random holdout from the same dataset?
A holdout is better than evaluating on training predictions, but it may not demonstrate transportability. Patient-level separation and, when possible, external validation provide stronger evidence for use in a new setting.
What calibration-set size should I use?
The cited evidence does not establish a universal minimum for this ensemble scenario. Plan around the number of events, classes, subgroups, model complexity, and uncertainty you can tolerate, then report the resulting uncertainty rather than claiming a generic threshold.

Should I calibrate each model before averaging its predictions?
Not by default. Compare component-first and ensemble-first workflows under identical protected data splits, then select based on the complete pipeline’s calibration, discrimination, subgroup behavior, and clinical usefulness.
Is ECE enough to choose a workflow?
No. Pair ECE with reliability plots, calibration-in-the-large, calibration slope, discrimination, and decision-analytic measures.
Can a same-dataset holdout prove transportability?
No. It can provide an internal assessment, but external or otherwise independent validation is needed to evaluate performance in a new setting.
