An effective insider-threat response plan tells your business exactly who assesses a concern, who can act, how evidence and people are protected, and when an incident is escalated. It covers intentional and unintentional behavior, from suspected data theft to an accidental disclosure, without treating an alert as proof of guilt.

Build the plan as a repeatable lifecycle—detection, reporting, assessment, containment, management, recovery and learning—with incident-specific branches. Give a multidisciplinary team clear authority, use confidential reporting, and have counsel adapt the procedures to your jurisdictions, workforce, contracts and regulatory duties.

What an insider-threat response plan does

Insider risk is the possibility that someone with authorized access—an employee, contractor, temporary worker, partner or other trusted person—could intentionally or unintentionally harm people, information, systems, facilities or business operations. The response plan is the operating manual for deciding what a report means and what to do next. It is not a list of suspicious employees and it is not a license for indiscriminate monitoring.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

CISA’s guidance treats insider risk as both intentional and unintentional and recommends a context-sensitive assessment by a multidisciplinary threat-management team. That means an unusual download, a policy violation or a coworker’s concern should trigger fact-finding and proportionate safeguards, not an automatic conclusion about motive.

Your plan should answer these questions before an incident occurs:

  • What behavior, event or condition is reportable?
  • Who receives the report and who can make decisions?
  • How are immediate safety, access, evidence and business continuity assessed?
  • Who may restrict access, preserve records, interview people or contact outside authorities?
  • How are confidentiality, privacy, employment rights and protection from retaliation maintained?
  • How is the case documented, closed and used to improve controls?

Use the plan alongside your broader incident-response, emergency, workplace-violence, business-continuity, privacy and disciplinary procedures. It should make handoffs between those processes explicit rather than creating a parallel system that conflicts with them.

Set scope and policy boundaries first

Write a short scope statement that covers the people, assets and events the plan governs. Include information-security events, physical-security concerns, personnel issues and threats to operations. State that both intentional and accidental conduct can enter the process. A negligent misdirected email, for example, may need the same initial containment as a deliberate disclosure, even though the eventual personnel response differs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Define a reportable concern

Describe observable triggers in plain language. Examples include a suspected unauthorized transfer of sensitive data, unexplained attempts to bypass controls, misuse of approval authority, credible threats to people or facilities, destructive changes to systems, or an accidental event that may expose information. Avoid labels such as “disloyal” or “unstable”; they encourage assumptions instead of evidence.

Set information-handling rules

Specify what information may be collected, who may access it, how it is protected, how long it is retained and when it may be shared. Limit case files to a need-to-know group, use approved storage, and record disclosures. Counsel should review these boundaries for applicable privacy, employment, civil-liberties, labor, records-retention and sector requirements in every relevant jurisdiction.

Connect the plan to other policies

Cross-reference the cyber incident-response plan, access-control standard, physical-security procedure, emergency contacts, code of conduct, whistleblower and anti-retaliation policy, investigation protocol, and business-continuity plan. Identify which document controls when procedures overlap. Include an owner and review date for every cross-reference so a changed policy does not silently invalidate the response plan.

Give a multidisciplinary team real authority

Contacts alone are not enough. Assign roles, alternates, decision rights and escalation thresholds. CISA recommends a threat-management team with varied disciplines. NIST identifies business and system owners, human resources, physical and personnel security, legal, operations and procurement as coordination partners. A smaller company can combine roles, but it should still cover these decisions.

Role Primary responsibility Authority or handoff to define
Executive sponsor Owns the program and resolves business-risk conflicts. Approves major risk acceptance, resources and external escalation.
Incident or case lead Coordinates the case, timeline, decisions and status updates. Opens and closes cases; convenes the right specialists.
Security or insider-risk lead Performs contextual assessment and coordinates protective measures. Recommends urgency, safeguards and investigative scope.
IT and system owners Preserve logs, assess access and protect system integrity. Define who can suspend, narrow or restore access and under what approval.
Human resources Advises on personnel processes, welfare, accommodations and retaliation concerns. Coordinates employment actions with legal and management.
Legal and privacy Reviews lawful collection, privilege, notifications, contracts and jurisdiction. Sets legal holds, disclosure rules and outside-counsel or authority thresholds.
Physical and personnel security Handles facility access, safety and protective measures. Coordinates escorts, credential changes and emergency response.
Business or process owner Explains operational impact and acceptable continuity trade-offs. Approves safe workarounds and restoration priorities.
Recorder or evidence custodian Maintains the case log, decision record and evidence custody. Controls access to the authoritative record.

List alternates for every role, including after-hours coverage. State when outside investigators, specialist forensics, insurers, regulators, customers, service providers or law enforcement may be contacted and who makes that decision. Membership should change with the incident: a suspected financial-fraud case may need finance and procurement, while a facility threat may require physical security and emergency responders.

Make reporting safe, visible and usable

Employees cannot use a channel they do not know about or trust. Publish one obvious route—such as a security or ethics intake address, portal or phone line—and a confidential alternative controlled by an appropriately independent function. Explain what to report, what information is useful, what happens after submission, how confidentiality is protected and how retaliation concerns are handled.

Tell reporters what useful facts look like

Ask for dates and times, locations, systems or records involved, direct observations, witnesses, and any immediate safety or continuity issue. Make clear that a reporter does not need to diagnose intent or prove misconduct. Anonymous reports can be accepted where lawful, but explain that anonymity may limit follow-up.

Protect both sides of a report

CISA recommends a positive reporting culture and confidentiality for reporters and people reported. Restrict case access, avoid gossip, and prohibit retaliation. Do not promise absolute secrecy if disclosure is legally required or necessary to protect someone. Avoid inappropriate monitoring; collection must have a defined purpose, lawful basis and proportional scope.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use a standard intake record

Record the intake identifier, date and time received, channel, source or anonymous status, observed facts, affected people, assets or processes, immediate danger, possible ongoing access, and the staff member who accepted it. Preserve the original submission and note any translation or transcription. Do not put conclusions in the initial facts field.

Triage context, urgency and confidence

Use the same triage sequence for every report, then branch by incident type. Separate observations from interpretations in the case record. A useful first assessment asks:

  1. Is anyone in immediate danger? If yes, follow emergency and workplace-violence procedures first and contact qualified responders as appropriate.
  2. Is harm or access ongoing? Determine whether a person can still reach systems, facilities, data, funds or equipment and whether a destructive action is continuing.
  3. What is at risk? Identify people, safety, operations, information, intellectual property, regulated data, finances and contractual obligations.
  4. Could evidence disappear? Identify volatile logs, messages, video, devices, transaction records or physical evidence that require prompt preservation.
  5. What is known and how reliable is it? Record sources, corroboration, gaps and confidence without assigning a psychological diagnosis.
  6. What is the likely incident class and scope? Consider data disclosure, fraud, sabotage, violence, policy misuse or accidental exposure, and whether other locations or systems are involved.

Adopt a locally tailored risk rubric rather than a universal score or behavioral profile. Define, for example, what your organization means by critical, high, moderate and low urgency, which facts move a case between levels, and who may override a rating. The rubric supports consistent decisions; it does not replace judgment or legal review.

Use a repeatable response lifecycle

The plan’s backbone should remain stable even when the incident branch changes. NIST SP 800-171 Revision 3 states: “Implement an incident-handling capability that is consistent with the incident response plan and includes preparation, detection and analysis, containment, eradication, and recovery.” Apply that capability to insider-related events with the additional personnel, physical-security and privacy coordination they require.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

1. Detect and identify

Accept a report, alert or referral; open a case; capture initial facts; and check for immediate safety or continuity needs. Confirm that the concern falls within scope and assign a case lead. Do not delay urgent protective action while waiting for a complete narrative.

2. Assess and classify

Apply the triage questions, identify affected assets and people, estimate scope and confidence, and select the relevant incident branch. Document why the case was rated at a particular urgency and what information would change that rating.

3. Contain proportionately

Choose the least disruptive effective measure that protects people, evidence and operations. Options can include narrowing permissions, suspending a token, separating duties, moving a process to a controlled queue, securing a facility, preserving a device or assigning temporary work. State who can approve each action and who must be consulted.

Coordinate technical action with HR, legal, business owners and physical security. NIST emphasizes close coordination when disabling accounts for individuals assessed as high risk. Avoid an unreviewed account shutdown that destroys evidence, interrupts critical services or creates an unsafe confrontation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

4. Investigate and manage the case

Set the investigative question, sources, authorized collection methods and review team. Maintain a timeline of actions, timestamps, approvals, findings and unresolved uncertainty. Preserve originals, calculate or otherwise document integrity where your procedures require it, and record every transfer or access to evidence.

5. Communicate on a need-to-know basis

Use approved channels and a named communications owner. Prepare separate messages for affected staff, leaders, customers, suppliers and authorities if notification is appropriate. Do not disclose identities or allegations more broadly than necessary. External reporting depends on applicable law, contracts, insurance terms and the facts; define thresholds and decision authority rather than promising one universal deadline.

6. Recover and close

Restore access, systems and normal duties only after the case lead, system owner, HR, legal and other required functions agree that the risk is controlled. Confirm that backups and system integrity are sound, resolve personnel or process issues through the proper channels, document the outcome and close the case with an approval. Record lessons, control changes, training needs and any follow-up review.

Prepare branches for common incident classes

The following branches are planning examples, not a legal classification system. CISA notes that response processes vary by incident or threat type and by whether conduct was intentional or unintentional.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Incident class Planning emphasis
Data theft or unauthorized disclosure Preserve access and audit records; limit further disclosure proportionately; coordinate legal, HR, IT and business owners; evaluate contractual and legal notifications.
Fraud or misuse of authority Preserve transaction, approval and vendor records; separate suspected access from investigation authority; coordinate finance, legal, HR and security.
Sabotage or destructive activity Prioritize continuity and safe containment; protect backups and system integrity; retain evidence of changes, commands and access.
Threats or potential workplace violence Prioritize immediate safety; use emergency and workplace-violence procedures; involve qualified responders as appropriate.
Accidental or negligent event Contain harm, determine scope and cause, restore services, and distinguish coaching or process fixes from intentional misconduct.

For each branch, list the first safe action, the decision-maker, evidence to preserve, functions to notify, communication limits and recovery criteria. Keep the branch short enough to use under pressure; put detailed technical runbooks in linked or referenced procedures.

Protect privacy, fairness and evidence at the same time

Insider-risk work can involve sensitive employment, health, identity, communications and security information. Build safeguards into collection and access rather than adding them after an investigation starts. Use the minimum information needed for the defined purpose, restrict the case file, log access, and set retention and deletion rules approved by counsel and records management.

Do not infer intent from a single anomaly, protected activity, disability, medical condition, national origin or other sensitive characteristic. Require corroboration and context. Give the person responsible for the relevant process an opportunity to explain operational facts where doing so does not create a safety or evidence risk. HR and legal should determine how interviews, notice, representation, accommodations and disciplinary processes work in each jurisdiction.

Evidence handling should be reproducible. Identify the custodian, source, collection time, storage location, access history and any transfer. Preserve volatile data before making changes when feasible, but do not let evidence preservation delay emergency action needed to protect people.

Define communications and external notification

Write a communications tree with primary and alternate contacts, approved channels, message approval and update frequency. A status update should state what is known, what is being done, current business impact, decisions needed and the time of the next update—without speculation or unnecessary personal details.

Set criteria for notifying law enforcement, regulators, customers, insurers, suppliers or service providers. The trigger may depend on the type of data, physical danger, contractual language, jurisdiction, insurance conditions or a request from qualified responders. Legal counsel should determine which obligations apply; the plan should not state a universal notification period.

Preserve a record of what was communicated, by whom, when, to whom and under which authority. Coordinate public statements through the designated communications function and counsel.

Train people and exercise the plan

Every worker should know how to recognize and report a concern, what confidentiality means and how retaliation is handled. Responders need role-specific training. NIST distinguishes basic user awareness from deeper training in collection, forensics, reporting and restoration. Train managers not to investigate independently, confront a subject unsafely or circulate allegations.

Exercise progressively

  1. Checklist review: Verify contacts, alternates, access to forms and emergency numbers.
  2. Walkthrough: Have each function explain its actions, approvals, records and handoffs for a realistic scenario.
  3. Tabletop: Introduce timed developments such as ongoing access, a safety concern, a media inquiry or conflicting business priorities.
  4. Simulation: Where appropriate and safe, test technical containment, evidence preservation and restoration with production safeguards.

Record each gap, its risk, an owner and a due date. Test both an intentional scenario and an accidental one, and include after-hours coverage. CISA recommends recurring exercises, audits and independent assessment. Revisit the plan after incidents, exercises, audits, organizational changes and relevant legal or policy changes.

Measure readiness without turning people into scores

Track program performance, not employee “risk” ratings. Useful measures include time to acknowledge a report, time to establish a case lead, percentage of staff trained, exercise findings closed by their due dates, percentage of cases with complete decision logs, time to preserve critical evidence, and restoration time for affected services. Review trends by process and control, not by protected characteristics or informal labels.

Use CISA’s Insider Risk Mitigation Program Evaluation (IRMPE) as one structured self-assessment. CISA lists that fillable assessment, developed with Carnegie Mellon University’s Software Engineering Institute, as revised July 29, 2024. It can help identify missing governance, reporting, assessment, response and improvement practices; it does not replace legal advice or an investigation.

Build a usable plan document

Keep the plan in a controlled location with a version number, owner, approval date and next review date. Include these sections:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Purpose, scope, definitions and policy boundaries.
  • Roles, alternates, authority and escalation thresholds.
  • Reporting channels, confidentiality and anti-retaliation rules.
  • Intake form and triage rubric.
  • Response lifecycle and incident-specific branches.
  • Access-restriction, safety, evidence and legal-hold procedures.
  • Communications tree and external-notification decision points.
  • Case-record, retention and evidence-custody requirements.
  • Recovery, closure, lessons-learned and corrective-action process.
  • Training, exercise, audit and independent-assessment schedule.
  • Contact directory with verification date and after-hours coverage.

Run a final review with leadership, security, IT, HR, legal, operations and relevant system owners. Ask each person to identify one decision they are authorized to make, one decision requiring consultation and one handoff that could fail. Fix those ambiguities before an incident exposes them.

Official guidance to keep current

CISA’s Insider Threat Mitigation Guide provides the core program and response guidance; its PDF path is dated November 2022. CISA’s IRMPE and HR’s Role in Preventing Insider Threats Fact Sheet list July 29, 2024, revision dates. NIST SP 800-171 Revision 3 is particularly useful for organizations handling controlled unclassified information, while NIST SP 800-61 Revision 3, published April 3, 2025, integrates incident-response recommendations with the Cybersecurity Framework 2.0 and supersedes Revision 2. Verify the current editions and your legal obligations during each plan review.
