A workable ransomware or data-breach plan is a written, approved procedure that still functions when systems, email and normal phone directories may be unavailable. It assigns decision authority, isolates affected technology, preserves evidence, assesses exposed data, coordinates legal and external notifications, restores clean services and captures improvements. Build it before an incident, keep an offline copy, and rehearse it with realistic outages and uncertain facts.

What an incident response plan must accomplish

Your plan should turn a chaotic event into a sequence of controlled decisions. It is not only a technical runbook: ransomware can interrupt operations while a separate investigation determines whether credentials or personal information were stolen. A data breach can require legal, contractual and regulatory action even when systems remain available.

Use the plan to answer five questions immediately:

  • Who can declare an incident, make containment decisions and authorize outside help?
  • Which services, systems, accounts and data are affected, and what remains uncertain?
  • How will the organization communicate if identity, email or collaboration systems cannot be trusted?
  • What evidence and records must be preserved while containment proceeds?
  • In what order can the business return to a clean, validated operating state?

Keep a separate communications plan, coordinated with the technical plan. CISA advises organizations to “Create, maintain, and regularly exercise a basic cyber incident response plan (IRP) and associated communications plan that includes response and notification procedures for ransomware and data extortion/breach incidents.” See the CISA #StopRansomware Guide.

Use current guidance as the framework

NIST SP 800-61 Rev. 3

NIST SP 800-61 Rev. 3 was published on April 3, 2025, and supersedes Rev. 2. It places incident-response recommendations throughout the Cybersecurity Framework (CSF) 2.0 risk-management cycle, covering preparation, detection, response, recovery and improvement. Use it as the general structure for governance and repeatable processes.

CISA ransomware guidance

The CISA guide is specific to ransomware, data extortion and breach response. Its checklist emphasizes isolating impacted systems, examining security and identity logs, preserving system and memory evidence when appropriate, containing compromised accounts, restoring from offline encrypted backups and documenting lessons learned. CISA’s guide page shows a September 2023 revision; check the current agency page and its linked materials when maintaining your plan. CISA also provides a resource page that may contain updated documents.

FTC breach guidance

The Federal Trade Commission’s August 2023 Data Breach Response: A Guide for Business covers investigation, legal review, notification and communications. It stresses that obligations depend on the applicable federal and state laws, the type of information, the sector and the facts. It also gives a simple evidence rule: “Do not destroy evidence.”

1. Define purpose, scope and activation authority

Write the scope in operational terms

List the entities, subsidiaries, locations, cloud services, networks, endpoints, applications, operational technology and data stores covered by the plan. State whether the same procedure applies to a suspected compromise, confirmed malware, a denial-of-service event, a lost device and a third-party incident. Identify systems that support health and safety, revenue, customer access and legal or regulatory records.

What’s actually slowing this PC down?

Pick the symptom - the matching free tool is one click away.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Set activation triggers

Specify observable triggers rather than relying on a general instruction to “use judgment.” Examples include widespread file encryption, a ransom or extortion demand, confirmed unauthorized access, malware on a domain controller, theft of credentials, suspected exfiltration of personal information, or an outage that threatens an essential service. Allow any employee or service provider to report a suspected event without waiting for technical proof; the incident lead can downgrade or close it after triage.

Name the authority chain

Document the incident commander, deputies and the person who can activate the plan outside business hours. Record who may approve network isolation, disabling accounts, taking a service offline, engaging forensic specialists, invoking continuity procedures, contacting law enforcement, and issuing public statements. Obtain written executive approval and publish the chain of command so responders do not negotiate authority during the crisis.

Maintain an offline quick-reference pack

Keep a current, access-controlled copy outside the production identity system. Include emergency contacts, alternate numbers, escalation rules, facility information, critical-service owners, insurer and service-provider contacts, regulator and law-enforcement contacts, network diagrams, backup locations and the first-hour checklist. Store at least one copy in a form that can be read when internet access and corporate credentials are unavailable.

2. Assign roles and resilient communication paths

Assign responsibilities, not just job titles

Function Primary responsibilities
Incident commander Declares the incident, sets priorities, resolves conflicts, approves major decisions and maintains the decision log.
Security and IT response Detects, scopes, contains and eradicates technical compromise; protects logs and coordinates restoration.
Business-service owners Explain operational dependencies, acceptable downtime, safety implications and restoration order.
Legal and privacy counsel Directs privilege, evidence preservation, contractual analysis and jurisdiction-specific notification decisions.
Communications Maintains approved facts, audience messages, spokesperson arrangements and update cadence.
Executive leadership Sets risk tolerance, allocates resources and approves decisions reserved for senior management.
Forensic and recovery specialists Collect evidence, analyze intrusion, remove persistence and support clean rebuilding when internal capability is insufficient.
HR, facilities and vendor managers Address workforce, site access, insider-risk, supplier and contractual issues as needed.

Design fallback communications

Identify at least two independent channels for each critical role. If corporate email or single sign-on may be compromised, use pre-arranged out-of-band numbers, a separately administered emergency platform or in-person coordination. Define how responders authenticate one another before sharing sensitive information. Record a fallback for executives, service providers, counsel, insurers, regulators and law enforcement.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Set meeting and reporting rules

Define who convenes the response bridge, how often status is reviewed, what the written situation report contains and who receives it. Every update should separate confirmed facts, working assumptions, unknowns, decisions, owners and the next decision time. This prevents an early guess about the attacker or scope from becoming an unsafe “fact.”

3. Prepare the information and controls responders will need

Build an accurate asset and dependency view

Maintain an inventory linking critical services to applications, hosts, identities, network segments, cloud tenants, backup sets and suppliers. Mark systems whose loss affects safety, regulated data, payroll, revenue or customer access. Include asset owners and recovery dependencies; a restoration list without dependencies can bring systems back in an order that recreates the outage.

Pre-authorize evidence and access procedures

Document who can collect disk images, memory captures, endpoint telemetry, authentication records, firewall and cloud logs, malware samples and relevant application data. Specify the evidence repository, access permissions, retention period and chain-of-custody fields. Identify short-retention and volatile sources so they can be preserved before normal log rotation removes them.

Protect recovery material

Record where offline or otherwise isolated backups are kept, how they are encrypted, who can authorize access and how restoration integrity is tested. Separate backup administration from ordinary domain credentials, and document clean recovery environments. A backup that is reachable with the same compromised identity as production is not an independent recovery point.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Prepare contact and decision records

Keep current contracts, insurance instructions, breach counsel contacts, forensic retainers, service-level commitments, data-processing agreements and supplier escalation paths. Add a decision log with fields for time, decision, evidence considered, approver, owner and follow-up. These records support continuity, later legal review and lessons learned.

4. What should we do first after a ransomware alert?

Use the following sequence as a practical outline, while following the approved plan and safety requirements for your environment.

  1. Activate and authenticate. Contact the incident lead through an independent channel, open the incident record and note the first observed time, reporter and symptoms.
  2. Protect people and essential operations. Address health, safety and physical-security consequences before technical convenience. Identify services that cannot be stopped without an executive or operational decision.
  3. Isolate deliberately. Disconnect or quarantine affected devices and network segments as appropriate. Avoid broad destructive actions that erase evidence or interrupt essential services without an authorized decision.
  4. Establish knowns and unknowns. List affected assets, accounts, timestamps, ransom or extortion messages, suspected entry points and gaps in visibility. Assign an owner and next review time for each uncertainty.
  5. Preserve volatile evidence. Capture relevant memory, active connections, logs and system state when qualified responders determine it is safe and useful. Record who collected each item, when and how it was stored.
  6. Secure identities and access paths. Contain compromised accounts, remote-access tools, privileged sessions and exposed credentials in coordination with service owners. Rotate secrets using a sequence that does not lock out essential operations or destroy forensic visibility.
  7. Expand the search. Review endpoint, antivirus, intrusion-detection, identity, firewall, cloud and application telemetry for earlier intrusion, lateral movement and data access. Treat encryption as possible evidence of a prior compromise, not necessarily the beginning of the event.
  8. Coordinate outside parties. Engage leadership, counsel, communications, the insurer, relevant providers, qualified forensic support, law enforcement and government contacts according to the plan.
  9. Run the breach workstream. Determine whether information was accessed or exfiltrated, what categories and people may be involved, and which legal, contractual or regulatory analyses are required.
  10. Recover in priority order. Rebuild or reimage from a clean environment, restore protected backups, validate systems and credentials, and reconnect only after security and business owners approve the result.
  11. Document and improve. Keep the timeline and decisions current, then assign owners and due dates for corrective actions after containment and recovery.

5. Triage, containment and investigation

Prioritize by impact, not by convenience

Rank systems using safety, essential services, legal obligations, revenue, customer dependence and restoration complexity. A less visible identity or backup system may deserve priority because it controls recovery. State explicitly which systems may remain online temporarily and who approved that risk.

Preserve before you clean

Do not wipe, reboot, delete malware or restore over a system merely because it is inconvenient. Coordinate collection with qualified responders and counsel where appropriate. The FTC’s guidance is direct: “Do not destroy evidence.” Preserve original files and metadata, make working copies for analysis, restrict access and record transfers.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Investigate the path to impact

Review authentication anomalies, privileged-account use, remote access, endpoint detections, firewall connections, cloud audit trails, email activity and application access. Look for persistence, newly created accounts, disabled security tools, unusual data transfers and signs that attackers were present before encryption or extortion. Avoid declaring the scope complete until the relevant visibility gaps are documented.

Contain without causing a second outage

Containment can include network segmentation, host isolation, disabling a compromised account, blocking malicious infrastructure and removing exposed remote access. Sequence changes with business owners when a system supports safety or essential operations. Record each action, its expected effect and the rollback or recovery method.

6. Run data-breach assessment and notification in parallel

Identify whose information may be involved

Create a fact record for data categories, systems, records, individuals, business customers, employees and service providers potentially affected. Distinguish confirmed access, suspected access, encryption-only impact and information that has not been assessed. Preserve the evidence supporting each conclusion and update it as forensic findings change.

Route legal analysis to counsel

Build a jurisdiction and sector matrix covering regulators, law-enforcement reporting, contractual partners, affected individuals and other required recipients. The applicable rules vary with geography, data type, industry, contractual terms and the facts of the incident. Do not hard-code one universal deadline into the plan or publish a notification date before counsel confirms the requirement and available facts.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Coordinate suppliers and customers

Identify whether a service provider stores or processes the affected information, who owns the notification obligation, what cooperation the contract requires and how facts will be exchanged securely. Keep one approved chronology so different parties do not send contradictory descriptions.

Give affected people practical help

Notices should state what is known, what remains under investigation, what the organization is doing, what recipients can do now and where they can obtain help. Use plain language, a monitored contact method and an update process. Do not include speculative attacker claims or technical details that would create additional risk.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Support on Ko-Fi

7. Build the communications plan

Prepare audience-specific holding statements

Draft adaptable messages for employees, customers, business partners, regulators, affected individuals and the public. Each template should have an approval owner, a designated spokesperson, a secure publication method and a process for replacing preliminary facts with verified information. Keep the templates factual rather than promising a specific cause, scope or completion time.

Use one source of approved facts

Communications should draw from the incident commander’s situation report and the legal review, not from separate technical conversations. Mark statements as confirmed, unconfirmed or under investigation. Set an update rhythm even when there is no material change, and explain when the next update is expected.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Protect responders and affected people

Do not publish exploitable technical details, credentials, internal diagrams or unverified accusations. Tell employees where to report suspicious messages and how to avoid spreading the incident. Provide affected people with concrete protective steps that are supported by the facts and applicable guidance.

8. Recover to a clean, prioritized state

Define the restoration order

Restoration decision Questions the plan must answer
Safety and essential operations Which services must return first, what manual workaround exists, and who accepts temporary risk?
Identity and administration How will privileged identities, authentication services and remote access be rebuilt or validated before dependent systems reconnect?
Core infrastructure and data Which clean network, servers, applications and datasets are required for the next business service?
Customer and workforce services What dependencies, access controls and monitoring must be verified before users return?
Noncritical systems Can the service wait until investigation, hardening and testing are complete?

Validate backups and the recovery environment

Use protected backups and a clean recovery environment. Confirm backup integrity, restoration permissions, dates, dependencies and malware-scanning procedures before production use. Restore into an isolated segment, rebuild or reimage systems suspected of compromise, rotate credentials that may have been exposed and enable heightened monitoring.

Control reconnection

Require technical and business owners to approve each system before it rejoins a trusted network. Check security tooling, patches, accounts, network paths, logging and data integrity. Keep suspected-compromised systems and accounts separated until validation is complete. Maintain alternate operating procedures while restoration continues.

9. Exercise, review and improve the plan

Run realistic exercises

Use ransomware and data-theft scenarios that include unavailable email, uncertain scope, an essential-service outage, a supplier compromise and possible notification obligations. CISA points organizations toward cyber-exercise resources that can help evaluate or develop a plan. A paid facilitator can be useful for a tailored rehearsal, but it is optional rather than a prerequisite.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Measure decisions, not attendance

Record how long it took to identify the incident lead, reach critical contacts, authorize isolation, preserve logs, locate clean backups, make a notification decision and restore a priority service. Note unclear authority, failed contact numbers, missing dependencies, inaccessible evidence and unsafe assumptions. Assign an owner and due date to every corrective action.

Set review triggers

Review the plan after every exercise and incident, and after major changes to systems, suppliers, organizational structure or applicable law. Revalidate offline copies, contact details, backup access, recovery procedures and communication templates. Obtain renewed executive approval when authority or risk tolerance changes.

10. Decide when to use external responders

Organizations without sufficient in-house capacity should arrange incident-response and managed-security support before an emergency. Evaluate providers against the work your plan requires, not a marketing label:

  • Technical, forensic, identity and recovery capabilities for your environment.
  • Experience with your sector, data types and operating constraints.
  • 24-hour availability, escalation thresholds and named contacts.
  • Evidence handling, secure data transfer and chain-of-custody practices.
  • Ability to coordinate with counsel, insurers, leadership and law enforcement.
  • Clear scope, response times, rates, travel terms, confidentiality and ownership of work product.

Specialized privacy or data-breach counsel can help interpret jurisdictional and contractual duties; technical responders should not make legal notification decisions without the designated counsel. Backup or recovery services may support resilience, but no single storage product replaces isolated copies, restoration testing, identity protection and a practiced procedure.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

11. Common plan failures and their fixes

Failure Why it causes harm Fix
The plan exists only online Attackers or an outage can remove access to contacts and procedures. Keep controlled offline copies and test retrieval without corporate credentials.
Everyone assumes IT is in charge Operational, legal, communications and executive decisions stall. Name an incident commander, deputies and explicit approval rights.
Responders wipe systems immediately Evidence, volatile state and the intrusion path may be lost. Preserve relevant evidence first, using qualified responders and documented custody.
Ransomware is treated as only an outage Earlier credential theft or data exfiltration can be missed. Run a parallel breach assessment and examine logs for prior access.
One communication channel is trusted Email, identity or collaboration services may be compromised. Pre-arrange independent channels and authentication procedures.
Backups are restored without validation Malware or compromised credentials can be reintroduced. Use a clean recovery environment, validate backups and approve reconnection.
Notifications use a fixed universal deadline Legal duties differ by jurisdiction, data, sector and facts. Maintain a legal matrix and have counsel confirm each obligation.
Exercises are table-reading sessions Contact failures and authority gaps remain hidden. Simulate unavailable systems, make participants decide, and track corrective actions.

12. A maintenance checklist for plan owners

  • Scope, activation triggers and authority are approved in writing.
  • Incident commander, deputies, service owners, counsel, communications and external contacts are current.
  • Offline copies open successfully and contain alternate communication methods.
  • Asset, dependency, identity, supplier and data inventories reflect current systems.
  • Evidence collection, storage, access and chain-of-custody procedures are documented.
  • Protected backups, clean recovery environments and restoration priorities have been tested.
  • The breach matrix identifies relevant jurisdictions, sectors, contracts and approval owners.
  • Audience-specific messages, spokesperson authority and update cadence are ready.
  • Exercises include ransomware, data theft, unavailable communications and essential-service disruption.
  • Every exercise or incident produces assigned corrective actions with due dates.

Bottom line

Build the plan around authority, isolation, evidence, parallel breach analysis, resilient communication and clean recovery. Keep it offline, obtain executive approval and rehearse it under the conditions that make real incidents difficult. When ransomware or a suspected breach occurs, follow the sequence, state what is known and unknown, involve counsel early, preserve evidence and restore only after systems and identities have been validated.
