A vulnerability remediation SLA works when it turns risk into an owned work item with a clearly defined clock, a realistic target, documented exceptions, and verified closure. The deadline is only one part of the agreement: teams also need an unambiguous start event, priority rules that account for exploitation and business impact, escalation before a breach, and evidence that the exposure is actually reduced.
This guide shows how to design that system without pretending that one universal timetable fits every organization. Use the framework to set targets from your assets, obligations, change windows, staffing, and measured delivery capacity.
What a meetable vulnerability remediation SLA must accomplish
A remediation SLA is an operational agreement, not merely a table of severity labels and day counts. It should make five decisions explicit for every finding:
Recommended Free Tools
- What is covered: the asset classes, environments, discovery sources, and providers included in the process.
- How priority is determined: technical severity combined with exploitation evidence, exposure, business criticality, potential impact, and local context.
- When the clock starts and stops: the event that creates the obligation, the calendar used, and the evidence required to close it.
- Who is accountable: the operational owner, security contact, risk owner, escalation path, and closure validator.
- What happens when the target cannot be met: temporary controls, risk acceptance or another treatment decision, an expiry or review date, and a path to permanent remediation.
The result should be a repeatable workflow in which a scanner alert or advisory becomes a validated record, a responsible team can act on it, leaders can see overdue exposure, and a closure claim can be checked independently.

Set the scope before setting deadlines
Write down what the SLA covers before choosing targets. Scope decisions affect both workload and risk, so they cannot be left implicit.
List asset and service classes
Decide whether the process includes endpoints, servers, cloud resources, applications, network equipment, containers, open-source and third-party dependencies, and externally managed services. Include production, development, test, and disaster-recovery environments where they create meaningful exposure. If a class is excluded, record the separate process or control that applies to it.
Define accepted finding sources
State whether the system of record receives scanner findings, vendor advisories, penetration-test results, bug-bounty reports, code-analysis results, incident discoveries, and vulnerabilities disclosed in dependencies. A source may require a different validation step, but it should still enter the same ownership and exception workflow.

Establish authoritative asset data
Every finding needs an asset owner and a business-criticality value. Use an authoritative inventory or configuration-management source and define how ownership is resolved when records conflict. Criticality should reflect the service’s role, data sensitivity, safety implications, customer impact, and recovery requirements rather than the technology label alone.
Map obligations and contracts
Identify regulatory requirements, customer commitments, insurance conditions, and supplier contracts that impose security or notification duties. A private organization may use the risk-based ideas in the federal Binding Operational Directive (BOD) 26-04 as a reference, but the directive applies to covered U.S. federal agencies and is not a universal private-sector schedule. CISA’s BOD 26-04 implementation guidance recommends putting suitable remediation timelines into an SLA or contract with a service provider or cloud provider; that guidance does not turn one contract form into a general legal requirement.
Normalize and validate each finding
Do not start an SLA clock on an unverified, duplicated observation. First create a usable record that a remediation team can understand and prioritize.
Deduplicate and identify the issue
Merge repeated scanner observations for the same vulnerability and asset while retaining the affected evidence. Map the issue to a vulnerability identifier when one exists, such as a vendor advisory or common identifier. Record all affected versions and instances so that closing one host cannot be mistaken for closing the entire exposure.

At minimum, capture:
- Validation status and the person or process that confirmed it.
- The severity methodology and version, such as a stated CVSS version or an internal rating method.
- Known exploitation status, including whether exploitation is confirmed, publicly demonstrated, suspected, or unknown.
- Network and identity exposure, including internet reachability, lateral-movement paths, and required privileges.
- Business, data, safety, and availability impact if the vulnerability is exploited.
- Available patch, upgrade, configuration change, workaround, compensating control, or decommissioning option.
- Estimated remediation effort, change dependencies, vendor constraints, and material uncertainty.
NIST’s Secure Software Development Framework (SSDF) recommends gathering enough information to plan remediation, estimating exploitation impact and remediation effort, and considering other relevant planning factors. Its guidance is process-oriented; it does not prescribe a single deadline table.

Keep uncertainty visible
Unknown exploitation or ownership should not silently lower priority. Mark the uncertainty, assign a person to resolve it, and set a short investigation target separate from the final remediation target. When new evidence changes the risk, allow the finding to be reclassified rather than leaving it in its original tier.
Use risk signals instead of severity alone
Technical severity is an input, not the final priority decision. A moderate-severity flaw on an exposed, business-critical service may deserve faster action than a nominally critical flaw on an isolated, disposable system. Conversely, a high technical score with no reachable attack path may permit a planned change window if other safeguards are strong.
| Risk signal | Questions to ask | Possible effect on priority |
|---|---|---|
| Exploitation evidence | Is exploitation confirmed, actively reported, publicly demonstrated, or merely theoretical? | Known or credible exploitation can move a finding into the urgent tier regardless of its nominal severity. |
| Exposure | Is the asset internet reachable, reachable by untrusted users, or exposed through a partner or supply chain? | Greater reachability generally shortens the acceptable response window. |
| Service criticality | Would compromise interrupt a critical service, safety function, revenue process, or recovery capability? | Critical services warrant tighter targets and earlier escalation. |
| Data and safety impact | Could exploitation expose regulated, personal, financial, or operationally sensitive data, or create physical safety consequences? | High consequence can override a lower technical score. |
| Exploit practicality | Does exploitation require unusual access or specialized conditions, or is a reliable technique widely available? | Practical attacks justify faster treatment than difficult, constrained attacks. |
| Available treatment | Is a tested patch available, is a workaround reliable, or is only a temporary control possible? | A ready fix supports a firm remediation target; a missing fix requires a mitigation and review plan. |
Document which signals can override a nominal severity band and who has authority to make that decision. CISA’s BOD 26-04, issued June 10, 2026, structures prioritization around risk for its federal scope. NIST SSDF likewise calls for risk-based remediation decisions. Neither source establishes a universal private-sector deadline.
Design a small set of tiers teams can staff
Four practical categories are usually easier to operate than a dozen arbitrary severity bands. The labels below are a starting framework; your risk owners must set thresholds and targets using actual capacity and obligations.
| Tier | Typical trigger | Required treatment | How to set the target |
|---|---|---|---|
| Urgent active exploitation | Confirmed or strongly credible exploitation, or a rapidly changing threat against an exposed asset. | Immediate triage, an explicit mitigation decision, executive visibility, and the fastest available permanent fix or isolation. | Set a target that matches emergency-change capability, on-call coverage, and the harm of leaving the exposure open. Validate it through exercises and recent incident data. |
| High-risk exposed or critical-system | High-impact vulnerability on an internet-facing asset, critical service, sensitive-data system, or safety-relevant environment. | Named owner, near-term change plan, dependency escalation, and a verified result rather than a ticket update. | Use patch-window lead time, testing requirements, vendor availability, and the criticality of the service to choose a target that teams can repeatedly achieve. |
| Routine material finding | Meaningful weakness without current exploitation or exceptional exposure, with a feasible fix. | Scheduled remediation, normal change control, and verification within the agreed target. | Base the target on observed queue volume, team throughput, and standard maintenance cycles. |
| Lower-risk backlog | Limited-impact issue with constrained exposure and no current exploitation signal. | Risk-based backlog management, periodic review, and escalation if exposure or threat intelligence changes. | Set a review and treatment target that prevents indefinite aging while preserving capacity for higher-risk work. |
For every tier, specify the response or triage target, a mitigation target when one is needed, the final remediation target, the accountable team, the escalation point, and the evidence required for closure. Test the proposed targets against recent ticket counts, patch windows, change-control lead time, vendor response, staffing, and operational blackout periods. A target that is missed routinely is a capacity or design problem, not a reason to hide breaches in an average.
Define the SLA clock and state transitions
A deadline has no meaning until the policy defines its clock. Choose one start event and apply it consistently, with documented exceptions.
Choose a start event
Common choices include scanner detection, vendor disclosure, security validation, or entry of a validated finding into the system of record. The most defensible operational choice is often the validated-record event because it avoids charging a team for duplicate or false observations, but an organization may need an earlier trigger for known exploited vulnerabilities or contractual notifications. State the choice in plain language and identify who records the timestamp.
Define calendar, timezone, and pause rules
- Specify calendar days or working days and name the timezone used for due dates.
- Define treatment of weekends, public holidays, maintenance freezes, and emergency changes.
- Do not pause the clock merely because a ticket is waiting for a meeting, approval, or vendor response unless the policy explicitly allows it and another owner carries the risk.
- State whether a finding is re-timed after reclassification or whether the original clock remains visible alongside the new target.
Define entry and exit evidence
| State | Entry condition | Required record | Exit condition |
|---|---|---|---|
| Detected | Observation received from an approved source. | Source, timestamp, raw evidence, and affected asset reference. | Duplicate or false-positive determination, or validated finding creation. |
| Validated and prioritized | Issue confirmed and risk signals recorded. | Severity method, exploitation status, exposure, impact, owner, tier, and due dates. | Approved remediation or mitigation plan. |
| In remediation | Work has an accountable team and change path. | Planned change, dependencies, test evidence, and escalation status. | Fix deployed, asset removed, or approved temporary control active. |
| Pending verification | Treatment completed according to the plan. | Deployment record, configuration evidence, or other proof of the change. | Verification confirms the vulnerability is no longer present or the approved mitigation is effective. |
| Closed or accepted | Verification succeeded, or an authorized risk decision was recorded. | Validation result, residual risk, approver, and review or expiry date where applicable. | Reopen if evidence fails, the asset changes, or new threat information invalidates the decision. |
Allow dynamic reprioritization. CISA’s federal directive recognizes that timelines can change as facts change; use that as design context, while keeping the directive’s legal scope limited to covered agencies.

Assign ownership and escalation before work begins
“The infrastructure team” is not an owner. Each record should name one accountable remediation owner who can arrange the change, a security triage contact who can interpret risk, a risk owner who can accept residual exposure, and a person or function that validates closure.
Make accountability unambiguous
- Remediation owner: controls the technical work and updates the plan.
- Security triage contact: validates the finding, assigns or reviews the tier, and advises on mitigation.
- Risk owner: owns the business consequence and can approve an exception or alternate treatment.
- Closure validator: checks scan, configuration, deployment, or test evidence independently of the ticket’s status.
- Escalation authority: can approve emergency change, service isolation, additional staffing, or executive risk review.
Escalate before a breach
Trigger escalation when a dependency, failed test, unavailable vendor fix, change freeze, or staffing gap makes the target unlikely. Escalation should occur before the due date, with the remaining risk, decision needed, and proposed next action visible. After a missed target, notify the risk owner immediately, apply or reassess mitigation, and keep the item visible in overdue exposure reporting.
Handle vulnerabilities that cannot be patched on time
When a permanent fix is unavailable or unsafe to deploy within the target, do not silently extend the date. Create an exception or alternate-treatment record before expiry.
Required exception fields
- Vulnerability identifier, affected assets, and current risk tier.
- Reason remediation is blocked, such as no vendor fix, incompatible software, testing failure, or an operational freeze.
- Residual risk statement that explains the likely consequence and exposure.
- Compensating controls, their owner, and evidence that they are active.
- Next action toward permanent remediation, including dependencies and an expected decision point.
- Named risk owner who approved the treatment and the approval date.
- Expiry or review date, with a rule that renewal requires a fresh risk decision.
Distinguish mitigation from remediation
A firewall rule, access restriction, feature disablement, enhanced monitoring, virtual patch, segmentation change, or service isolation may reduce risk without removing the vulnerable code. Label it as temporary mitigation unless policy explicitly recognizes it as the final treatment. Give the mitigation its own verification test and owner, and link it to the permanent fix so that the issue cannot disappear into an indefinite exception queue.
Use authorized risk decisions
Risk acceptance, risk transference, remediation, and another documented treatment can all be valid decisions when made by the appropriate authority. NIST SSDF calls for deciding whether to remediate or address risk through another means and for planning temporary mitigation when a permanent solution is unavailable. Acceptance is not a status that a technical team can grant to itself; it is an accountable business decision with a review point.
Verify closure instead of trusting ticket status
Close the SLA only when a defined result is demonstrated. Depending on the treatment, acceptable evidence may include a verification scan, a configuration check, package or version inventory, a successful test of a disabled feature, a decommissioning record, or evidence that an approved mitigating control is functioning.
Define how to handle partial remediation. If ten instances are affected and only eight are fixed, keep the finding open or split it into accurately scoped records. Record the evidence location, verification timestamp, method, and validator. Reopen the item when a subsequent scan, asset change, or threat update shows that the exposure returned.

| Measure | What it reveals | Useful breakdown |
|---|---|---|
| Time from identification to verified remediation | End-to-end reduction of exposure, not merely ticket movement. | Median and a tail measure such as the 90th percentile, by tier and asset class. |
| Target attainment | Whether teams meet the agreed target reliably. | Percentage within target by tier, team, source, and provider. |
| Overdue high-risk exposure | Current risk that requires leadership attention. | Count, age, affected service criticality, and exploitation status. |
| Exception count and age | Whether temporary treatment is becoming permanent. | Open exceptions by risk owner, expiry date, and compensating control. |
| Reopen rate | Whether verification is sound and fixes persist. | Reopens by root cause, asset type, and remediation method. |
| Dependency delay | Which vendors, change queues, or shared platforms repeatedly block treatment. | Elapsed time attributable to each dependency, with unresolved escalations. |
Show counts and aging alongside percentages. Use medians and tail measures only when timestamp quality is sufficient, and label them as internal measurements rather than industry benchmarks. NIST vulnerability-management guidance identifies measuring the time between identification and remediation; the additional measures above are implementation choices that help expose bottlenecks.
Review the policy on a defined cadence
Review tier targets, exception patterns, and missed-deadline causes on a schedule set by your governance process, and after a material incident, major exposure change, or repeated target failure. Recalibrate targets when asset inventory, staffing, patch windows, supplier terms, or emergency-change capability changes. BOD 26-04 requires formal annual reassessment for agencies within its scope; that annual requirement should not be presented as a universal private-sector rule.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Make provider and cloud responsibilities explicit
If a supplier, managed service, or cloud provider owns part of the remediation path, the contract should identify the same control points as the internal SLA:
- Covered assets and vulnerability sources.
- Notification and clock-start events.
- Risk information the provider must supply.
- Response, mitigation, and final-remediation targets by risk tier.
- Evidence and verification rights.
- Escalation contacts and emergency-change procedures.
- Exception authority, expiry, and reporting obligations.
Align the provider’s commitments with your own clock and closure evidence. A supplier’s “patch available” notice is not proof that your exposed instance is fixed. CISA’s implementation guidance specifically discusses suitable remediation timelines in service-provider or cloud-provider SLA or contract arrangements; treat that as guidance in the BOD 26-04 context and reconcile it with your legal and contractual requirements.
Use a practical implementation sequence
Roll out the SLA in the order teams will use it, testing each decision against real records.
- Inventory the scope. List asset classes, environments, finding sources, owners, criticality data, providers, and applicable obligations.
- Choose the system of record. Ensure it can retain identifiers, timestamps, ownership, risk inputs, plans, exceptions, evidence, and audit history. NIST notes that existing issue-tracking software can support this process; a named security product is not required.
- Run a normalization exercise. Take a representative set of scanner, advisory, test, and dependency findings through deduplication and validation.
- Agree on risk overrides. Document how exploitation, exposure, criticality, data, safety, exploit practicality, and fix availability affect priority.
- Draft the tier matrix. Define triage, mitigation, and remediation targets, owners, escalation, and closure evidence without copying an unrelated generic deadline table.
- Test capacity. Compare proposed targets with historical ticket volume, patch windows, vendor lead times, change approvals, staffing, and emergency capability. Adjust targets or fund capacity gaps before publishing.
- Pilot the clock and state model. Timestamp start events, apply pause rules, test reclassification, and verify that a closed record contains objective evidence.
- Publish exception and reporting rules. Require pre-expiry approval, compensating-control evidence, review dates, overdue visibility, and root-cause analysis for recurring misses.
- Train owners and rehearse escalation. Make sure every participant knows who can change priority, approve risk, authorize emergency work, and validate closure.
- Review and improve. Examine target attainment, overdue exposure, exceptions, reopens, and dependency delays; then update the SLA when the operating environment changes.
Common SLA designs that fail, and how to correct them
Copying a universal “critical in X days” table
Why it fails: It ignores exploitation, exposure, business impact, patch windows, and legal or contractual requirements, and it may be mistaken for a mandatory standard.
Correction: Use a small risk-based tier set, document the inputs that can override severity, and set targets from capacity and consequence data. CISA’s federal timelines, where applicable, must not be presented as a universal private-sector schedule.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsStarting the clock at every raw scanner alert
Why it fails: Duplicates and false positives consume the same deadline as validated risk, while ownership may be unknown.

Correction: Define validation and normalization states, preserve the original detection timestamp for analysis, and state which event creates the contractual or operational obligation.
Using ticket assignment as proof of remediation
Why it fails: A ticket can be assigned, deferred, or marked complete while the vulnerable component remains exposed.
Correction: Require deployment, removal, configuration, or mitigation evidence and an independent verification result.

A concise policy test before approval
Ask the following questions against a real vulnerability record, not just the policy document:
- Can a responder identify the exact asset, owner, vulnerability, exposure, impact, and exploitation status?
- Is the clock-start event, timezone, day convention, pause rule, and stop evidence unambiguous?
- Can priority change when threat intelligence or asset context changes?
- Does one person own remediation, and is a different authorized role able to accept residual risk?
- Will a missed target trigger escalation before or immediately at breach?
- Can a team use an interim mitigation without disguising it as a permanent fix?
- Does closure require evidence that another person or process can verify?
- Will management see overdue high-risk exposure and aging exceptions rather than only an average?
- Does the design fit change-control lead time, staffing, supplier terms, and applicable obligations?
NIST SSDF task RV.1.3 states: “Have a policy that addresses vulnerability disclosure and remediation, and implement the processes needed to support that policy.” A meetable SLA is the operational expression of that policy: risk is assessed in context, work is owned and timed, exceptions are controlled, and the result is verified.
