A secure multi-institutional disease-research workflow is a governed process, not a file-transfer tool. Before any data move, inventory the datasets and intended uses; map each dataset to consent, law, funding and repository rules; choose an access tier; assign decision-makers; sign agreements; de-identify and transmit data in an approved environment; provision least-necessary access; and continuously monitor, respond and reassess. The result should make permitted research practical while keeping participant protections, restrictions and accountability attached to the data.

Scope: what this workflow does—and where it does not decide for you

This implementation guide is primarily aligned with U.S. National Institutes of Health (NIH) policy and guidance. A consortium that includes EU, UK, Tribal, state or other-country participants needs jurisdiction-specific review. Requirements also vary with the dataset, funding award, consent language, institutional policy, repository, and the purpose of a proposed use. Treat this as an operational framework, not universal legal advice.

The workflow applies to clinical records, genomic data, imaging, biospecimen-linked data, phenotypes, exposure data and combinations of those sources. It is especially important for rare-disease cohorts and linked datasets, where a small geography, unusual trait or external dataset can make an individual more recognizable than any single field suggests.

The workflow at a glance

Stage Required outcome Primary owner
1. Inventory A current map of institutions, datasets, purposes, recipients, locations and transfer routes Consortium coordinator and data managers
2. Permission mapping Consent, law, funding, institutional and repository constraints recorded per dataset Data steward, privacy officer and institutional review authority
3. Access-tier decision Open, broadly shared de-identified, controlled-access or enclave-only disposition with a written rationale Authorized governance body
4. Governance Named people and escalation paths for approvals, transfers, changes and incidents Each participating institution
5. Agreement Signed terms that define purpose, security, downstream limits, incidents, retention and destruction Institutional legal and contracting offices
6. Preparation and transfer Documented identifier reduction, risk assessment and approved storage/transfer path Data manager and information-security team
7. Access operations Named users, least privilege, authentication, logging and periodic recertification System owner and data steward
8. Release and continuity Repository, publication timing, curation, retention and end-of-award stewardship plan Principal investigators and repository liaison
9. Monitoring and improvement Training, incident response and a trigger-based review when circumstances change Governance body and security/privacy teams

1. Inventory the collaboration before collecting or moving data

Start with a living inventory that can be reviewed by investigators, data managers, privacy staff and security staff. Do not begin by choosing a platform. First record what exists and why it needs to move.

Record the participants and infrastructure

  • Every participating institution, subcontractor, repository and cloud or other third-party IT environment.
  • Countries and jurisdictions where participants, staff, systems and backups are located.
  • The data steward and technical owner for every source and destination.
  • All transfer routes, including APIs, managed file exchange, repository ingestion and approved compute-to-data connections.

Describe the data at a useful level of detail

  • Clinical, genomic, imaging, biospecimen-linked, phenotype, exposure and administrative fields.
  • Direct identifiers, quasi-identifiers, linkage keys, free text, dates, geography, rare traits and small-cell categories.
  • Whether a dataset can be joined to another consortium dataset, a public register or an external commercial or public source.
  • Whether the data include children, deceased participants, Tribal data or other groups with additional governance expectations.

Write the intended-use statement

For each dataset, state the scientific questions, analyses, users, recipient institutions, expected outputs and whether future secondary uses are anticipated. A vague statement such as “disease research” is difficult to evaluate against consent and access rules. Separate the primary project from possible later uses so that a later change can trigger a new review.

Flag rare-disease cohorts and linked datasets for heightened indirect-identification review. NIH’s Data Sharing Approaches guidance explains why combinations of indirect identifiers and context matter even when direct identifiers have been removed.

2. Map consent and every applicable rule to each dataset

Build a permission record per dataset rather than assuming that one consortium-wide consent statement covers everything. The record should be understandable to an auditor and to a new data steward who joins two years later.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Extract what participants were told

  • Which future uses were described or excluded.
  • Whether sharing with outside researchers, industry, government or international recipients was mentioned.
  • Where data may be stored, how long they may be retained and whether recontact is allowed.
  • Whether identifiable, coded or de-identified data may be shared, and what withdrawal means operationally.

Check external and institutional requirements

Review applicable federal, Tribal, state, local and institutional requirements, along with award terms and the destination repository’s rules. For NIH-funded work, document the approved Data Management and Sharing (DMS) Plan. Determine whether the NIH Genomic Data Sharing (GDS) Policy applies; if it does, controlled-access terms and institutional responsibilities continue after data are received. NIH describes those obligations in Using Genomic Data Responsibly Under the NIH Genomic Data Sharing Policy.

Where GDPR or another non-U.S. privacy regime may apply, obtain advice from the relevant institutional legal counsel. NCATS’ rare-disease consortium guidance specifically advises this step in its RDCRN Suggestions for Establishing Data Sharing and Data Management Guidance for Individual Consortia.

Record a decision, not just a citation

For every proposed use, record the rule or consent clause, the approving body, the date, the permitted recipients, the access tier and any conditions. If the answer is uncertain, stop the transfer and escalate it; do not treat technical de-identification as permission.

3. Choose an access tier using risk, consent and utility

De-identification reduces risk but does not automatically determine whether access should be broad. NIH’s privacy best-practices page says:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

“Researchers and institutions should proactively assess the protections needed for sharing scientific data from participants, including determining whether sharing should be restricted through controlled access, regardless of whether the data meet technical and/or legal definitions of ‘de-identified’ and can legally be shared without additional protections.”

Use a documented assessment of the combined dataset, its context and the intended users. The three common patterns below are choices to justify, not labels to apply automatically.

Access pattern When it may fit Benefits Costs and controls
Open or broadly shared de-identified data Consent, law, risk review and repository rules permit broad reuse, and scientific utility survives identifier reduction Maximum discoverability and reuse; fewer individual access decisions Indirect-identification and linkage risk still require assessment; documentation must state limitations and prohibited uses
Controlled-access repository Secondary use is valuable but users, purposes or security commitments require review Eligibility and purpose can be checked; terms and audit records follow approved users Application review, institutional assurances, user training and continuing oversight are required
Restricted enclave Privacy risk, rare traits, linkage potential or consent limits make wider release inappropriate Researchers can analyze data in a constrained environment without receiving a general-purpose copy Higher compute, transfer and support complexity; output review and environment administration are needed

Compare the options on participant risk and consent fit, scientific utility, access-review workload, auditability, transfer and compute needs, repository requirements and long-term stewardship. NIH’s Principles and Best Practices for Protecting Participant Privacy and Data Sharing Approaches provide the policy context for this assessment.

4. Establish cross-site governance and decision rights

A consortium needs named authorities, not a generic promise that “the team” will review requests. Each institution should appoint a data steward and identify the body or officials who can approve access, transfers, changes and incident actions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Define the minimum decision set

  • Who verifies a requester’s identity, institutional affiliation, training and scientific purpose.
  • Who decides whether a proposed use fits consent, law, the DMS Plan and repository terms.
  • Who authorizes a transfer, a new user, an onward disclosure or a change in storage location.
  • Who receives an incident report, starts containment and coordinates institutional notifications.
  • Who approves a change to variables, linkage, retention, publication or re-identification restrictions.

Give the reviewing body appropriate authority

The institutional body reviewing sharing must have the expertise and authority to evaluate privacy, scientific purpose, security and consent. Write quorum, conflict-of-interest and appeal rules into the governance charter. For rare-disease consortia, include the consent, governance, repository and legal-review considerations described by NCATS in its consortium guidance.

Maintain a change register

Log membership changes, new data types, new countries, new recipients, infrastructure changes, consent updates and repository-policy changes. A change register makes it possible to re-open a decision instead of allowing an old approval to silently cover a new risk.

5. Put the rules in a data-sharing or data-use agreement

Execute the agreement before access, not after the first transfer. The agreement should connect the dataset’s permission record to operational duties at every institution and should make restrictions travel with copies, derived files and approved users.

Essential clauses

  • Purpose and scope: the specific project, variables, analyses, recipients and approved outputs.
  • Oversight assurances: the responsible institutional authorities and how continuing review will occur.
  • Security, privacy and confidentiality: each party’s required safeguards, personnel duties and approved environments.
  • Access and onward transfer: named or eligible users, sub-recipient approval and a prohibition on unapproved redistribution.
  • Re-identification and recontact: explicit prohibitions or tightly defined exceptions where applicable.
  • Consent and use limitations: how restrictions, withdrawal information and permitted purposes are communicated to downstream users.
  • Incident notification: reporting channels, response times as set by the parties’ policies, cooperation and evidence preservation.
  • Retention and destruction: the end date, approved extensions, return or destruction method and treatment of derived data and backups.
  • Publication and acknowledgment: repository-required acknowledgments, attribution and disclosure of approved data sources.

NIH’s NOT-OD-22-213, Protecting Privacy When Sharing Human Research Participant Data, sets out agreement elements, downstream restrictions, de-identification methods and risk-assessment considerations. For NIH controlled-access genomic datasets, treat the NIH Data Use Certification (DUC) as binding terms; the investigator and institution remain responsible for confidentiality, integrity, security and oversight, including when cloud or other third-party IT systems are used.

6. Prepare the dataset and transmit it safely

Reduce identifiers deliberately

  1. Remove direct identifiers that the approved analysis does not require.
  2. Review indirect identifiers such as detailed geography, dates, rare diagnoses, unusual procedures, free text and small groups.
  3. Separate any re-linkage key from the research dataset and restrict its custodians and access path.
  4. Assess the combined dataset and plausible external linkages, not each column in isolation.
  5. Document the method, reviewer, date, residual risks, permitted users and downstream instructions.

The appropriate de-identification method depends on the data and purpose. A coded dataset may still require controlled access when linkage or context creates meaningful risk. NIH’s Data Sharing Approaches discusses indirect identifiers, restricted environments and data-use terms.

Match the environment to the access tier

Use an institutionally approved transfer and storage environment with access control, authentication, logging, backup and recovery arrangements appropriate to the dataset. The exact controls depend on the current NIH requirements, agreement and institutional infrastructure; do not treat this guide as a substitute for the applicable security-baseline document. NIH’s Requirements for NIH Controlled-Access Data Repositories and Users includes security expectations and identifies an effective date of February 25, 2026.

Do not make removable drives the default transfer mechanism. If an exceptional offline transfer is approved, document the authorization, chain of custody, encryption, receipt verification, storage and destruction steps separately.

Verify receipt before deleting a source copy

Confirm the recipient, dataset version, file integrity, access permissions and audit record. Preserve the agreed source and version metadata so analysts can reproduce which data were used. Only then apply the retention or destruction terms in the agreement.

7. Provision least-necessary access and keep it current

  1. Verify eligibility: confirm the user’s identity, institution, role, required training, approved purpose and any repository certification.
  2. Approve a named account: do not use shared credentials; record the approving authority, scope and expiry or review date.
  3. Apply role-based permissions: grant only the variables, operations, projects and environments necessary for the approved purpose.
  4. Authenticate users: use the institutionally required strong authentication and account-lifecycle controls.
  5. Log activity: retain access, administrative, transfer and relevant output-review records for the period required by policy and agreement.
  6. Review regularly: recertify users and permissions, and remove access promptly when a person leaves, changes role, finishes training or no longer needs the data.
  7. Enforce purpose restrictions: prohibit attempts to identify participants or recontact them when the consent or agreement forbids it.

For NIH controlled-access data, responsibility does not shift to a cloud provider or other IT contractor. NIH’s controlled-access guidance places continuing confidentiality, integrity, security and oversight duties on the investigator and institution.

8. Plan repository release, publication timing and continuity

Select a repository by policy fit

Evaluate an established repository’s access model, documentation, curation, retention, user verification, auditability and ability to carry consent and use limitations. A repository is suitable only if its operating rules match the dataset’s risk and permission record. The reviewed NIH guidance does not identify a universally best commercial platform.

Set the release deadline

NIH’s Final NIH Policy for Data Management and Sharing says shared scientific data should be accessible no later than the associated publication or the end of the performance period, whichever comes first, subject to applicable restrictions. Record the expected date, the person responsible and the reason for any restriction in the DMS policy implementation record.

Keep stewardship alive after the project

  • Name the institution that maintains access decisions, documentation and user support if a site closes or an award ends.
  • Define migration, repository transfer and preservation procedures before infrastructure is decommissioned.
  • Specify how active users, derived datasets, backups, keys and audit records are handled at project end.
  • Keep a contact route for incident reports and permission questions after publication.

9. Monitor, respond and improve

Train every approved user

Training should cover the approved purpose, prohibited re-identification or recontact, confidentiality, secure handling, output rules, incident reporting and repository acknowledgments. Keep completion records tied to the user’s access approval.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use a defined incident path

  1. Receive and record the report, including the dataset, account, time, systems and suspected action.
  2. Contain the event under the institution’s incident procedures without destroying evidence.
  3. Notify the designated data steward, security lead, privacy officer and other authorities named in the agreement.
  4. Assess whether data were accessed, copied, disclosed, altered or used outside the approved purpose.
  5. Coordinate required institutional, funder, repository, participant or regulator notifications through authorized officials.
  6. Document corrective actions, access changes, retraining and the decision about whether the workflow must be re-approved.

Use the incident and near-miss record to improve controls; a workflow that never changes despite new users, data or infrastructure is not being actively governed.

Define review triggers

  • New variables, linkage, rare traits or a material change in data volume.
  • New recipients, institutions, countries, cloud environments or repositories.
  • Changes to consent, law, funding terms, DMS plans or repository policy.
  • Evidence of misuse, an access anomaly, a security incident or a failed control.
  • A change in scientific purpose, publication plan, retention period or recontact need.

Reassess when any of these triggers occurs, rather than waiting for an annual calendar review.

Implementation checklist for consortium launch

  • Inventory: every dataset, field class, linkage key, institution, jurisdiction, recipient, location and transfer route is recorded.
  • Permission: consent, law, funding, institutional review and repository terms are mapped to each dataset and intended use.
  • Tier: the selected access pattern and residual-risk rationale are written and approved.
  • Governance: each institution has a named steward, approver, security contact and incident escalation route.
  • Agreement: purpose, security, onward transfer, re-identification, retention, destruction and incident terms are signed before access.
  • Preparation: direct and indirect identifiers, linkage and residual risk have been reviewed and documented.
  • Transfer: the environment is approved, receipt is verified and version metadata are preserved.
  • Access: users are named, authenticated, least-privileged, logged, trained and periodically recertified.
  • Release: repository, publication timing, curation, retention and post-award stewardship are assigned.
  • Operations: incident handling, change triggers and reassessment responsibilities are tested and understood.

Common failure modes and the corrective decision

Failure mode Why it fails Corrective action
“The fields are de-identified, so they can be openly posted.” Indirect identifiers, rare traits and linkage can leave meaningful residual risk; consent or policy may still limit sharing. Perform a contextual risk assessment and select controlled access or an enclave when warranted.
One blanket consent interpretation for every dataset Participants may have received different future-use, recipient, storage or recontact information. Map consent and restrictions per dataset and escalate uncertain uses.
Security delegated entirely to a cloud or IT provider Institutional and investigator duties continue even when infrastructure is outsourced. Verify the environment against applicable requirements and retain institutional oversight and audit evidence.
Agreement signed after the first transfer Early copies may lack enforceable purpose, retention, incident and downstream restrictions. Execute and approve terms before provisioning access or transmitting data.
Access granted indefinitely People change roles, projects end and permissions accumulate. Use named accounts, expiry or recertification dates, logs and prompt revocation.
No plan for an ending institution Repositories, keys, documentation and incident contacts can disappear with a site or award. Assign continuity ownership, migration steps and post-award support before launch.

What “secure” means in practice

Security is the combination of a defensible permission decision, explicit obligations and functioning controls. A technically strong environment cannot cure incompatible consent, an undefined purpose or missing authority. Conversely, a carefully written agreement is ineffective if accounts are shared, access is unlogged or data are copied into unapproved locations.

Use the workflow as a repeatable gate: no inventory, no transfer; no permission decision, no access tier; no signed terms, no provisioning; no verified receipt, no source deletion; and no reassessment after a material change. This sequencing turns participant protection from a one-time compliance document into an operating system for the consortium.
