A repeatable mortgage quality control (QC) checklist is an operating instrument inside a lender’s written QC plan—not a generic list of documents. Build the plan first, then translate each risk, timing rule, evidence requirement, exception path, and reporting duty into a version-controlled checklist that another qualified reviewer can execute and an auditor can reconstruct.

The framework below follows Fannie Mae Selling Guide guidance current April 1, 2026. Fannie Mae requirements are examples for that investor; confirm the applicable rules for your investors, products, jurisdictions, underwriting methods, and contractual obligations before putting the checklist into production.

Start with the written QC plan

Your checklist should be the working layer of a documented QC plan. The plan defines what “quality” means, which risks are material, who owns the program, how independence is protected, how files are selected, what reviewers must verify, how findings are corrected, and how management receives evidence that controls operate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Fannie Mae describes an effective QC program as part of the lender’s overall control environment because quality risk can create financial impacts for the lender and may affect a borrower’s sustainability. Its Selling Guide requires documented standards, systems, and procedures addressing risks such as fraud, negligence, errors, and omissions.

  • Read Before You Buy — No Video Output: These adapters support charging and USB 2.0 data transfer, but cannot transmit video signals. Except for standard USB webcams (which use USB data only), they are not compatible with HDMI/DisplayPort cables, video-capable USB-C hubs, or docking stations with video output.
  • Convert USB-A Ports to USB-C: Designed to connect USB-C earphones, cables, flash drives, card readers, and other USB-C accessories to standard USB-A ports. Plug-and-play with no drivers or software required.
  • Aluminum Alloy Housing: Built with a sturdy aluminum alloy shell that aids in heat dissipation and protects against daily wear and scratches. Designed to maintain a stable and secure connection.
  • Compact & Travel-Friendly: The ultra-compact design allows the adapter to stay plugged into your device without blocking adjacent ports or adding bulk, reducing wear and tear on your original USB ports.
  • 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.

Define measurable objectives

Write objectives that a reviewer can test rather than broad statements such as “ensure loan quality.” Examples include:

  • Confirm that the underwriting decision is supported by the file and applicable investor requirements.
  • Verify that approval conditions were satisfied and documented before closing or acquisition.
  • Detect material discrepancies in borrower, property, transaction, and closing data.
  • Identify recurring process failures and assign corrective action with a due date.
  • Demonstrate that reviews, rebuttals, findings, and remediation were completed within the required cycle.

Map each objective to a measurable result, such as pass, finding, unable to verify, or not applicable. Define defect-severity levels in the plan before reviewers begin scoring files; otherwise similar issues will be rated inconsistently.

Assign ownership and independence

Name the accountable QC owner, reviewers, business-area contacts, escalation authority, and report recipients. Document conflicts and the controls used to prevent production staff from judging their own work. Fannie Mae says prefunding QC should operate independently of production where practical. Even when staffing makes complete separation difficult, record the rationale, compensating review, and approval.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Set scope and version control

The plan and checklist should identify every channel, mortgage product, underwriting method, closing method, and relevant third party in scope. A checklist for one product cannot silently become the checklist for another. Give each release a version number, effective date, approving authority, change summary, and retirement date for the prior version. Preserve the version used for each completed review.

Separate prefunding and post-closing review

These stages answer different questions and require different evidence. Put the timing, sample source, reviewer actions, escalation route, and completion deadline in separate sections of the checklist.

Dimension Prefunding QC Post-closing QC
When Before closing or acquisition, as applicable After closing or acquisition, within the documented cycle
Primary purpose Prevent an avoidable defect from reaching funding or acquisition Measure delivered-loan quality, identify trends, and drive remediation
Selection Document timing, population, selection method, and verification procedures Use representative random review plus discretionary targeted review, with methodology and size informed by risk and production
Evidence available Current underwriting file, conditions, verifications, and pre-closing data Final loan file, closing package, post-closing reverifications, and delivery data
Escalation Hold, condition, correct, or elevate before funding or acquisition Record finding, seek rebuttal, assess severity, correct, and escalate trends
Fannie Mae timing reference Review must occur before the applicable closing or acquisition event Complete the post-closing cycle within 90 days from the month of disbursement for an originated loan or acquisition for an acquired loan

Do not copy a sample percentage from another lender. Your documented process should explain the population, risk profile, production characteristics, selection method, and sample size. Selection should be scheduled monthly for post-closing work so the 90-day clock can be tracked from selection through review, rebuttal, and reporting.

Combine random and targeted selection

Selection type What it is for What to record
Representative random Provides coverage of the production population and supports meaningful measurement of delivered-loan quality Population definition, randomization method, selection date, period, channel, product, and sample size
Discretionary targeted Tests loans exposed to a known or emerging risk, unusual transaction feature, process change, complaint, or prior finding Specific risk signal, selection rationale, affected population, reviewer, and whether results are excluded from representative-rate calculations

Targeted results can guide training and controls, but they should not be presented as a population-wide defect rate. Keep random and discretionary samples identifiable in the workpaper and management report.

Design the checklist at the file-review level

Each row should tell a reviewer what requirement or risk is being tested, where the evidence is found, what action to perform, what result to record, and what happens next. A useful row contains at least these fields:

  • Control or risk: the requirement, decision, or failure mode being tested.
  • Source: document, data field, system record, reverification, or third-party report.
  • Reviewer action: compare, recalculate, authenticate, trace, reconcile, or confirm.
  • Result: pass, finding, unable to verify, not applicable, or pending.
  • Evidence reference: page, document name, system identifier, date, or secure location.
  • Follow-up: owner, due date, rebuttal status, correction, and escalation.

Eligibility and underwriting decision

  • Does the file support the underwriting decision under the applicable investor and product requirements?
  • Are the selected underwriting method, program, occupancy, loan purpose, and transaction terms eligible?
  • Were all approval conditions satisfied, dated, and documented before the relevant closing or acquisition event?
  • Do automated findings, manual underwriting analysis, compensating factors, and final approval agree?

Record the exact evidence supporting the conclusion. “Underwriter approved” is not evidence that conditions were met.

Credit, income, employment, assets, liabilities, and occupancy

Test the information used to qualify the borrower, not merely whether a document exists. Compare application data, underwriting data, source documents, and closing records. Recalculate material figures where the control requires it.

  • Confirm that credit liabilities and payment amounts used in qualification match the available credit evidence.
  • Check income calculations, employment history, stability, and documentation against the underwriting decision.
  • Trace assets and required funds to acceptable sources and verify that deposits, transfers, and reserves are supported.
  • Review occupancy representations and the evidence supporting the selected occupancy type.
  • For every required reverification, confirm that it was obtained, dated, and reconciled with the original documentation in the origination file.

A reverification that conflicts with the original file is a reconciliation issue requiring a documented resolution, not a box to mark complete.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Collateral and property data

  • Confirm that the property supports the loan amount, eligibility, occupancy, and transaction structure.
  • Review appraisal or property-data concerns under the applicable rules, including the resolution of material discrepancies.
  • Check comparable-property adjustments and other collateral inputs that materially affect value or eligibility.
  • Document who performed the collateral work, what source was used, and how exceptions were resolved.

Data integrity and closing package

Compare the application, underwriting system, disclosures, closing documents, delivery data, and final conditions. Look for mismatches in borrower identity, property address, loan amount, interest rate, term, cash to close, seller or interested-party contributions, liabilities, and payment calculations. Confirm that final documents reflect the approved transaction and that any approved change was re-underwritten or otherwise handled under the applicable process.

Legal, program, and third-party requirements

List the laws, regulations, investor rules, and program overlays that apply to your footprint; do not assume a Fannie Mae checklist covers every jurisdiction or investor. Include controls for broker, processor, appraiser, settlement agent, verification provider, and other third-party contributions. Record what the vendor supplied, how the lender validated it, and how vendor quality is monitored.

Run every review through the same workflow

  1. Freeze the population. Record the review period, population definition, channel, product, underwriting method, selection date, and whether each loan was randomly or discretionarily selected.
  2. Assign the reviewer. Confirm competence, independence, conflict clearance, and access to the current checklist version.
  3. Gather the file set. Identify missing documents, system extracts, reverifications, closing records, and vendor work before scoring controls.
  4. Review the underwriting decision. Start with eligibility, conditions, and the rationale for approval; then test the borrower, transaction, and collateral evidence that supports it.
  5. Trace and reconcile. Compare source documents with application, underwriting, closing, and delivery data. Recalculate material figures and document the method.
  6. Record evidence as you go. Enter a result and evidence reference for every applicable row. Do not rely on memory or a later narrative summary.
  7. Open findings immediately. Describe the condition, requirement, evidence, affected loan, provisional severity, and containment action.
  8. Obtain a rebuttal. Give the responsible business area a controlled response path and preserve the response, supporting evidence, and decision-maker.
  9. Finalize severity and disposition. Distinguish a corrected documentation issue from a decision defect, eligibility breach, or systemic control failure. Record whether the loan was corrected, repurchased, escalated, or otherwise resolved under the applicable process.
  10. Close the cycle. Approve the workpaper, update trend data, include the result in the management report, and confirm retention and any corrective-action monitoring.

Make exceptions actionable

A finding is not closed because it was logged. Use one controlled record for each issue with these fields:

  • Loan identifier and review stage
  • Requirement or control tested
  • Objective evidence and document location
  • Defect description written in factual terms
  • Category and severity
  • Immediate containment or loan-level correction
  • Root-cause hypothesis and affected process
  • Responsible owner and due date
  • Rebuttal, response, adjudicator, and decision date
  • Final resolution and verification of completion
  • Escalation status and management visibility

Separate loan defects from trends

A single inaccurate payment calculation is a loan-level finding. Repeated inaccurate calculations across a processor, product, system release, or time period indicate a trend. Track both levels: the loan record preserves the correction, while the trend record identifies the broader control change, owner, expected resolution, and implementation or completion timing. Fannie Mae calls for written corrective action when trends are identified.

Use consistent severity rules

Define severity using impact and decision relevance, not the reviewer’s personal judgment. Your plan should explain how to treat eligibility failures, material misrepresentation, unsupported underwriting, missing evidence, data discrepancies, and harmless clerical issues. Require escalation when a threshold is met, when a defect may affect other loans, or when a response is late.

Report, retain, and independently audit the program

Management reporting

Under Fannie Mae guidance, prefunding and post-closing results should be reported to management at least monthly. A useful report has both loan-level and trend-level views:

  • Population, sample composition, review stage, and completion status
  • Findings by category and severity
  • Random versus targeted results, clearly labeled
  • Recurring procedural or operational problems
  • Responsible business area and aging of corrective actions
  • Rebuttal outcomes, unresolved items, and escalations
  • Remediation status and evidence that completed actions worked

Do not combine a targeted sample with a representative sample and call the result a defect rate. Explain the denominator, selection method, review period, and any exclusions beside every metric.

Retention

Retain QC reports, completed workpapers, findings, reviewed loan files, reverification documents, rebuttals, and corrective-action records for at least three years under Fannie Mae requirements. Apply the longer period when another investor, law, contract, or litigation hold requires it. Make retained records searchable, access-controlled, and tied to the checklist version used.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Independent audit

Audit whether staff followed the documented procedures and applied assessments consistently. Test reviewer assignments, evidence references, severity decisions, timing, rebuttal handling, trend escalation, and report accuracy. Distribute audit results to management and track remediation to verified completion; an audit that only identifies problems without confirming correction is incomplete.

Oversight of QC vendors

Outsourcing review work does not outsource the lender’s responsibility. Define vendor scope, selection method, reviewer qualifications, security, turnaround time, defect taxonomy, rebuttal handling, and reporting format. Validate the vendor’s work and assess whether its methods produce reliable results. Fannie Mae guidance calls for the lender to review at least 10% of the post-closing QC sample reviewed by the vendor each month. Record which loans were independently checked, what discrepancies were found, and what action followed.

Use current defect observations without treating them as universal rankings

Fannie Mae’s June 29, 2026 Quality Insider article describes initial significant defects from a discretionary sample of reviews completed October 2025 through March 2026. The listed areas include undisclosed liabilities, borrower employment, interested-party contributions, undisclosed mortgages, occupancy, monthly-payment calculations, debt documentation, and comparable-property adjustments.

Use those areas to challenge your risk assessment, training, and targeted-selection rules. Do not present them as random-sample prevalence, a ranking for every lender, or proof that one process is defective in your organization. For each risk signal, define the affected population, evidence to inspect, escalation threshold, and whether the resulting findings remain separate from representative performance metrics.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Practical checklist record

A row can be implemented in a spreadsheet, workflow system, or controlled form. Keep the structure stable even when requirements change:

Field Example entry
Control ID and version INC-04, checklist v2026.04, effective April 1, 2026
Risk Qualifying liabilities may be incomplete
Source evidence Credit report, application, underwriting findings, payoffs, and reverification
Reviewer action Compare liabilities and payment amounts; investigate discrepancies; document reconciliation
Result Pass, finding, unable to verify, or not applicable
Evidence reference Document name, page or system record, and review date
Follow-up Owner, due date, rebuttal, final severity, resolution, and escalation

Fannie Mae’s Loan Quality resources include an Action Plan Template, Reverification Tracking Process and Templates, Loan Defect Taxonomies, and Form 1032, Post-Closing Loan File Document Checklist. Treat these as investor-specific aids and confirm their current availability and scope before adopting them.

Common implementation failures and fixes

  • One checklist for every loan. Fix it by branching controls by product, channel, underwriting method, investor, and jurisdiction.
  • Checklist rows that say only “review file.” Fix it by naming the source, comparison, expected result, and evidence reference.
  • Random and targeted samples blended together. Fix it by labeling selection type and reporting denominators separately.
  • Conditions marked complete without proof. Fix it by requiring dated evidence and a reviewer reconciliation.
  • Findings closed after a correction with no trend analysis. Fix it by checking for recurrence across the relevant population and opening corrective action when warranted.
  • Vendor output accepted without validation. Fix it by documenting vendor oversight and the required monthly independent review of at least 10% of the vendor-reviewed post-closing sample.
  • Reports issued after the review window. Fix it with a monthly calendar that tracks selection, review, rebuttal, reporting, and the 90-day post-closing completion deadline.
  • Old checklist versions left in circulation. Fix it with controlled distribution, effective dates, approval records, and retention of the version used for each loan.

Implementation sequence

Build the plan, define risks and severity, assign independent ownership, map prefunding and post-closing timing, design random and targeted selection, convert every control into an evidence-based row, establish rebuttal and corrective-action records, and set monthly reporting, retention, vendor validation, and audit checks. The result is repeatable when two qualified reviewers can follow the same instructions, reach supportable conclusions, and leave a record that management and an auditor can verify.
