To determine whether your project contains a potentially compromised npm release, preserve the exact lockfile first, inventory every resolved package and integrity value, then run npm audit signatures, inspect provenance against the expected source and CI workflow, and run npm audit as a separate known-vulnerability check. None of those checks alone proves that a release is safe: signatures address artifact authenticity, provenance describes how and where a package was built, and audit reports match known advisories. A package can pass one check and still require incident-specific code and release analysis.

What this audit can—and cannot—establish

A trusted-publishing incident is a supply-chain investigation, not an ordinary dependency update. Your first objective is to establish exactly what your project consumed. Your second is to compare each artifact with the publishing path that maintainers expected. Your third is to separate evidence of a known vulnerability from evidence that a particular release was altered.

No incident, package, or affected version is identified here. Therefore, you must obtain the incident owner’s primary advisory, including its release window and indicators, before declaring any version affected or safe. Generic npm documentation cannot supply that list.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Check What it answers What it does not answer
Lockfile and integrity review Which exact package versions and registry artifacts your build resolved Whether the publisher’s source or workflow was malicious
npm audit signatures Whether npm can verify registry signatures and available provenance attestations for installed packages Whether the package code is benign or free of an undisclosed compromise
Provenance inspection Which source repository, commit, workflow, build run, and publishing identity are linked to an artifact Whether the linked source itself contains malicious code
npm audit Whether dependency paths match vulnerabilities known to npm’s advisory data Whether a release was deliberately poisoned or contains a new malicious payload
Source and artifact comparison Whether the release contents differ unexpectedly from a known-good version or expected build A universal guarantee when reproducible inputs or a trustworthy baseline are unavailable

npm describes audit as submitting a description of project dependencies to the default registry for a report of known vulnerabilities; it is not a malicious-release detector. Read the npm audit documentation alongside the incident advisory.

1. Freeze the evidence before changing dependencies

Do not start by upgrading, reinstalling, or regenerating the lockfile. Those actions can erase the exact state you need to investigate.

Identify the incident scope

Obtain the incident report from the package maintainer, npm, or the organization that disclosed the event. Record the named package(s), affected release range, publication timestamps, indicators of compromise, and any explicitly excluded versions. If the report does not identify versions, treat that as an unresolved question rather than inferring a range from the date alone.

Preserve project artifacts

  • Copy package-lock.json, npm-shrinkwrap.json, or the lockfile used by your package manager into write-protected incident storage.
  • Preserve the repository commit, CI logs, build logs, deployment records, and the original installation command.
  • Keep the existing node_modules tree when policy permits, or record its location and hash before removal. If you must rebuild, document the time, CLI version, Node.js version, registry URL, and command used.
  • Save the output of every npm verification command as an evidence file, including errors and warnings.

Inventory resolved packages

For every direct and transitive occurrence of a package named in the incident report, record the package name, exact version, registry, tarball URL if present, integrity value, and dependency path. The same version can appear through multiple parents, while different versions of one package can coexist. The lockfile is the authoritative starting point for this inventory because npm audit uses it by default.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not replace a vulnerable-looking entry with a range such as ^1.2.3 in your notes. Record the concrete version that was actually resolved. Preserve the original file before making a remediation branch.

2. Establish a reproducible installation context

Signature and provenance checks are meaningful only when they examine the dependency tree that your project actually installed. npm’s provenance-verification guidance requires dependencies installed with npm install or npm ci, and recommends npm CLI 9.5.0 or later. Registry-signature documentation specifies npm CLI 8.15.0 or later. Use a current npm CLI that meets the newer 9.5.0 threshold for the combined verification, and record the versions used.

  1. In a clean, isolated checkout, verify the tool versions with node --version and npm --version. Record both outputs. Do not silently switch Node or npm between evidence runs.
  2. Copy the preserved lockfile into the checkout. If the project’s normal process uses a frozen lockfile, retain that setting so npm does not rewrite dependency selections.
  3. Install from the preserved state with the project’s documented command. For npm-managed projects, npm ci is appropriate when a compatible lockfile is present; npm’s provenance guidance specifically recognizes installation through npm install or npm ci.
  4. Record the registry configuration and scope mappings used for the run. A private mirror may have different signature support from the public npm registry.

If installation changes the lockfile, resolves a different tarball, or uses a different registry, stop and preserve both states. Do not mix results from the original and reconstructed trees.

3. Run registry-signature and provenance verification

Run the combined npm check

With the exact dependency tree installed, run:

npm audit signatures

npm documents this command as verifying registry signatures and provenance attestations for downloaded packages. Save the complete terminal output, exit status, npm version, registry configuration, and the lockfile hash alongside the evidence.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A successful result means npm verified the evidence available for the packages it examined. It does not certify the package’s behavior. A failure, missing signature, or missing attestation is a lead for investigation, not a diagnosis of the incident.

Understand missing or invalid signatures

npm states that when a registry supports signatures but a package has missing or invalid signatures or attestations, the CLI returns an error. That condition can indicate an attempt to circumvent signature verification, but it can also reflect registry configuration, an unsupported mirror, or a release that predates the relevant publishing mechanism. Preserve the exact error and establish which registry served the artifact before drawing conclusions.

  • Confirm that the command contacted the registry you intended, not an internal proxy with different metadata.
  • Check the exact package version and tarball integrity value against the lockfile and registry record.
  • Repeat the check in an isolated environment using the same resolved version, not a newly selected version.
  • Ask the incident owner or package maintainer whether signatures and attestations were expected for that release and provider.

Do not “fix” an error by deleting the lockfile or installing the latest version; that destroys the comparison you are trying to make.

4. Inspect provenance as a build-chain statement

npm says provenance provides a verifiable link to a package’s source code and build instructions so developers can audit and decide whether to trust it. Treat that link as evidence about origin and process, not as a code-safety verdict. npm explicitly warns that provenance does not guarantee the absence of malicious code.

  • Repository: Is the organization and repository the project normally uses?
  • Immutable source commit: Does the commit correspond to the intended release tag or reviewed merge?
  • Workflow file and identity: Is the workflow the approved build definition, and is the actor or identity authorized to publish?
  • Build run: Does the run exist in the expected CI system, at the expected time, with the expected inputs?
  • Publishing path: Was the package published by the configured trusted publisher rather than an unexpected account or token?
  • Public-ledger entry: Is the attestation present and internally consistent with the package metadata?

npm’s provenance viewer checks linked source commits and repositories and displays a warning when those links can no longer be found. A missing repository can mean deletion or a change to private visibility; it is unavailable evidence, not proof that the package was compromised. Record the warning and seek the maintainer’s archived source or build logs.

Compare source and package contents

After validating the identity fields, compare the release with a known-good artifact or a independently retained build. Review source diffs, generated files, lifecycle scripts, bundled dependencies, and unexpected network or credential access. The comparison should be tied to the incident’s indicators and the project’s normal build process. Provenance can show that a workflow built a commit; it cannot tell you that the commit was free of malicious changes or that generated output was harmless.

Use npm’s official explanations of viewing package provenance and generating provenance statements when interpreting the attestation fields.

5. Run npm audit separately

Run the ordinary advisory check against the preserved dependency set:

npm audit

The command analyzes dependencies configured in the project and, by default, uses the lockfile. Review each affected path, advisory severity, vulnerable range, and remediation recommendation. Save the report without allowing an automatic update to overwrite your evidence checkout.

A clean report means npm found no matching known advisories in the submitted dependency description. It does not mean that a release was not compromised, that its publisher account was secure, or that its code is safe. Conversely, a reported vulnerability does not prove that the package was part of the publishing incident; it may be an unrelated, older defect. Keep advisory findings and compromise indicators in separate incident fields.

6. Compare candidate releases systematically

If the incident report names several possible releases, use the same evidence fields for every candidate. Do not compare one version using a public registry and another using a private mirror without recording that difference.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Comparison axis Evidence to record Interpretation
Exact release and artifact Package version, registry, tarball URL, and lockfile integrity value Confirms that the artifact under review is the one the project consumed
Registry signature Valid, invalid, missing, or unsupported result from npm audit signatures Shows whether registry-level signing evidence verifies for that artifact
Provenance source Repository and immutable commit Tests whether the artifact points to the expected source
Build and publisher identity Workflow file, CI run, provider, and publishing identity Tests whether the release followed the authorized build path
Content difference Source diff, generated output, lifecycle scripts, and bundled files versus a known-good release Identifies unexpected changes that require code-level investigation
Incident indicators Matches to hashes, timestamps, accounts, files, or behaviors in the primary incident report Connects technical evidence to the independently reported event
Known advisories npm audit paths and advisory identifiers Provides vulnerability context, not compromise attribution

Mark each field as verified, contradicted, unavailable, or not applicable. “Unavailable” is more accurate than “safe” when an attestation or repository cannot be retrieved.

7. Review trusted-publisher controls

Verification of an installed package should be followed by a review of the account and CI controls that could have produced it. Inspect the npm package’s trusted-publisher configuration and the corresponding repository and workflow identity. Confirm that the configured provider, repository, workflow, and environment still match the project’s intended release process.

Check token-based access

Inventory traditional publish tokens, their owners, scopes, creation dates, last use, and expiration or revocation state. Remove unused publish tokens and restrict token-based publishing where the project supports a safer alternative. npm recommends preferring trusted publishing over long-lived tokens when available, restricting traditional token publishing access, auditing trusted-publisher configurations regularly, and removing unused tokens.

Account for configuration prerequisites

Current npm documentation lists npm CLI 11.5.1 or later and Node.js 22.14.0 or later for configuring trusted publishing. These are configuration prerequisites, not requirements for every consumer running npm audit signatures. Keep the two version questions separate:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • For consumer-side provenance verification, use npm CLI 9.5.0 or later and install dependencies with npm install or npm ci.
  • For configuring trusted publishing, use the current npm and Node.js versions documented by npm, including npm CLI 11.5.1+ and Node.js 22.14.0+ where those docs apply.

Automatic provenance depends on documented conditions such as the supported provider, a public repository, and a public package. npm’s documentation states that CircleCI trusted publishing does not generate provenance. Therefore, an absent attestation is not automatically suspicious: first confirm whether the provider and package configuration were eligible to create one at publication time.

Read the current trusted publishing documentation for provider-specific support and configuration details before changing release credentials.

8. Decide whether to contain, remediate, or continue investigating

Contain when identity or content evidence is adverse

If the incident report implicates the exact version, the artifact integrity value differs from the retained lockfile, an unauthorized publisher or workflow appears in provenance, or code review finds the reported malicious indicators, treat the release as suspect. Prevent new builds and deployments from selecting it, quarantine affected artifacts, and notify the incident owner and downstream teams. Preserve the suspect package and logs for forensic analysis rather than deleting all traces.

Remediate only after evidence capture

Once the original state is preserved, move to a remediation branch. Pin a version that the incident owner identifies as unaffected, regenerate the lockfile deliberately, reinstall in a clean environment, and rerun signature, provenance, and vulnerability checks. Record why the replacement was selected and retain the old-to-new dependency graph. Do not describe a replacement as safe solely because npm audit is clean; its provenance and incident indicators still need review.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Escalate ambiguous results

Escalate when signatures are missing on a registry that should support them, provenance points to an unexpected repository or workflow, a source repository is unavailable, or package contents cannot be reconciled with the attested commit. Ask the package maintainer or incident owner for archived workflow runs, release artifacts, and a definitive affected-version statement. Until that evidence arrives, classify the version as unresolved rather than safe.

Practical evidence checklist

  • Primary incident advisory and its affected-release indicators
  • Original lockfile and a cryptographic hash of the preserved file
  • Package name, exact version, registry, tarball, integrity value, and dependency paths
  • Node.js and npm CLI versions plus registry and scope configuration
  • Installation command and date, with relevant CI and deployment logs
  • Complete npm audit signatures output and exit status
  • Provenance repository, commit, workflow, build run, provider, and publishing identity
  • Complete npm audit output and advisory paths
  • Source, generated-file, lifecycle-script, and package-content comparisons
  • Trusted-publisher settings, token inventory, and any revocations or configuration changes
  • Decision for each version: verified, contradicted, unavailable, or unresolved

Frequently misunderstood results

“The signatures passed, so the package is safe.”

Signatures establish that registry metadata verifies for the artifact. They do not judge the code, the source commit, or the publisher’s intent. Continue with provenance and content review.

“There is provenance, so the build was trustworthy.”

Provenance links an artifact to source and build instructions. A compromised source commit or workflow can still produce a validly attested package. Review the linked inputs and the incident indicators.

“npm audit found no vulnerabilities.”

That result only says no matching known advisory was returned for the submitted dependency description. It cannot exclude a new malicious release.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

“The repository link is gone, so the release was malicious.”

npm warns when linked commits or repositories cannot be found, but deletion or private visibility can cause the same warning. Treat the provenance as unavailable and seek archived evidence.
