A Node.js login endpoint can request an SMS one-time passcode, but a provider status cannot prove that the intended user saw the message. To evaluate an OTP flow honestly, log each stage separately: challenge creation, provider attempt, delivery status, and the user’s verification result. A managed verification API handles more of that lifecycle; a custom endpoint makes your team responsible for defining and operating it.
What an OTP endpoint needs to prove
An SMS login challenge has several distinct events. Treating them as a single “sent” or “delivered” result makes both troubleshooting and audit claims unreliable.
- Challenge initiated: Your application asks a service to start a verification and records its own request timestamp or correlation ID.
- Provider action: The API accepts or rejects the request, then may create a channel-specific attempt. An accepted API request establishes that the provider processed an API operation, not that a text reached a phone.
- Delivery status: A status may describe the provider’s progress in handing the message to a carrier. Twilio says SMS is a “best effort protocol” and that it can only confirm successful delivery to the sending carrier; this is not proof of handset receipt or that the intended person saw the SMS. See Twilio’s troubleshooting guidance.
- Code checked: The user submits a code and the verification service or application records whether it matched, expired, or failed. A successful check establishes a match in that verification flow; it does not establish who physically read the text.
Keep those outcomes distinct in dashboards and incident reports. A single successful test message is not a delivery-rate or reliability measurement.

How to send an OTP SMS from Node.js
With a managed verification API, the documented flow is to create a Verify Service, request a verification for the destination using the SMS channel, and then submit the code entered by the user to the Verification Check API. Twilio’s Verify API documentation includes Node.js client examples and states that its REST API is served over HTTPS; unencrypted HTTP is not supported. Store credentials in environment-based configuration or a secrets manager, not in source control.

- BUILT TO LAST: Made from tough, waterproof, and crush-resistant materials. Manufactured in Sweden and programmed in the USA with the highest security standards.
A general SMS sending API can transmit a message, but sending a text is not the same as managing a verification lifecycle. If you build the OTP lifecycle yourself, your application must define code generation, expiration, one-time use, retry and resend behavior, verification checks, rate limits, and abuse handling. Do not imply that an SMS send API supplies those controls unless its documentation says it does.
For Twilio Verify specifically, product documentation describes a maximum of five verification attempts to the same entity within ten minutes, with configurable service limits. Confirm the current service settings before an experiment; this limit is not a rule for other providers or for custom endpoints.

What to log for an auditable experiment
Use an application-generated run or request ID to connect events without relying on the phone number as a public identifier. The following is a proposed audit schema, not a vendor-mandated format. Provider fields are available only to the extent exposed by the chosen API; application fields must be recorded by your own code.
| Field | Record | Where it comes from |
|---|---|---|
| Run/request ID and request timestamp | Unique correlation value and when the application initiated the challenge | Application |
| Provider verification and attempt IDs | Identifiers returned for the verification and each channel attempt, when available | Provider response or attempt records |
| Channel and destination region | SMS and a region or country-level destination; avoid exposing the full number in public logs | Application and provider metadata, depending on API |
| Provider response/status | Request acceptance or error, attempt status, and subsequent status updates when available | Provider |
| Retry count and timestamps | Resend or verification attempts, with event times | Application; attempt detail may also be provider-supplied |
| Verification-check outcome | Completed, failed, expired, or other documented result | Provider response, with application event logging |
| Elapsed time | Time between selected events, such as request and check | Calculated by application from timestamps |
| Message segments and recorded price | Segment count and charge when exposed or available in billing data | Provider, billing records, or application reconciliation |
Twilio’s Verification Attempts documentation describes channel-specific data, conversion status, and price. The exact fields and their availability depend on the API and implementation. Record failed, expired, and completed flows separately. Do not log raw OTP values, and limit access to phone numbers and other personal data.

What SMS verification costs
Twilio’s pricing page, marked current as of August 2026, lists $0.05 per successful verification plus standard channel fees. Its US SMS row lists an additional $0.0083 per SMS. These are vendor-published figures for the stated pricing page, not a market average or a guarantee of total cost. Check the current Verify pricing for the destination and channel you plan to use.
For the listed US SMS figures, a simple charge estimate is: successful verifications × $0.05, plus SMS messages sent × $0.0083, before any other applicable charges. For example, one successful verification that uses one SMS corresponds to $0.0583 using those listed rates. Resends, failed flows that still send messages, and message segments can change the total. Twilio’s developer best practices explains that SMS is priced by segment and character set affects segment length.

There is no evidence here to conclude that a custom implementation is cheaper overall. A fair comparison needs the same geography, message volume, retry pattern, segment counts, and engineering and operations costs. A managed service charges for its verification workflow as well as the channel; a DIY design still incurs SMS charges and requires someone to build and maintain the security and abuse controls.

Custom endpoint or managed verification API?
| Decision area | Custom Node.js endpoint | Managed verification API |
|---|---|---|
| Code and lifecycle | Your application owns code generation, expiry, one-time use, retries, and checking. | The service provides a verification lifecycle; your application initiates the challenge and checks the submitted code through its API. |
| Abuse controls | You must implement and operate rate limits and fraud controls. | Provider capabilities and service limits may help, but must be checked and configured for the account and use case. |
| Audit visibility | You choose the event schema; provider delivery information may come from a separate messaging API. | Attempt and conversion data may be available, but exact fields depend on the API. |
| Cost basis | Message and segment charges plus engineering and operating effort. | Verification charge plus channel fees and any segment or retry-related charges. |
| Coverage and operations | You select and integrate delivery services and maintain the workflow. | The provider supplies its documented channels and geographic coverage; verify availability and requirements for the destination. |
Choose a managed flow when reducing lifecycle implementation work and using provider attempt data are priorities. Consider a custom flow only when there is a concrete requirement for control and your team can securely implement, test, monitor, and maintain every lifecycle and abuse-handling step. Neither option guarantees handset receipt, and price alone does not establish which is cheaper for a particular login volume.

Is SMS 2FA secure enough?
SMS codes add a possession check, but they are not phishing-resistant and should not be treated as suitable for every account or threat model. OWASP identifies risks including SS7 interception, SIM swapping, and number porting, and notes that SMS/PSTN codes are a restricted authenticator under NIST SP 800-63B-4. See the OWASP Multifactor Authentication Cheat Sheet.
Assess the value and sensitivity of the account, the consequences of number takeover, and whether stronger alternatives are practical for your users. Where SMS is retained, limit attempts, monitor unusual resend and failure patterns, and avoid presenting a carrier delivery status as proof of identity.
Consent and sending requirements
Twilio’s Verify SMS overview says to obtain and document recipient consent under its Messaging Policy before sending OTPs. That is vendor policy guidance, not a universal legal determination. Check the rules that apply to your geography, recipients, and use case before sending.
