Use CVSS to understand a vulnerability’s standardized technical severity, then assess your own risk separately. A sound decision combines the CVSS version and vector, evidence of exploitation, whether the affected software is actually deployed and reachable, the importance of the assets, likely business or safety consequences, and the mitigations available. A high CVSS score can describe a dangerous flaw that is irrelevant to your environment; a modest score can demand urgent action when the vulnerable system is exposed, critical, or already being attacked.
Severity and risk answer different questions
Severity describes the characteristics and potential impact of one vulnerability under standardized assumptions. Risk describes the likelihood and consequences for a particular organization. CVSS is designed primarily for the first question; it does not know whether your company runs the affected product, whether an attacker can reach it, how valuable its data is, or what controls already limit exploitation.
NIST Interagency Report 7946 states: “Vulnerability assessment via the CVSS can assist in conducting risk assessments, but the CVSS scores should not be the sole factor when determining risk.” The report was published on April 28, 2014 for CVSS 2.0, so use current CVSS documentation for scoring mechanics while retaining its warning about local context.

| Question | Evidence to use | What it tells you |
|---|---|---|
| How severe is the flaw technically? | CVSS version, numerical score and vector | Standardized attack conditions and potential impacts |
| Is it being exploited? | Known Exploited Vulnerabilities (KEV), credible incident reports and EPSS | Observed exploitation or an estimate of future exploitation likelihood |
| Are we exposed? | Asset inventory, installed version, configuration, reachability and controls | Whether an attacker can use the flaw in your environment |
| What could we lose? | Asset criticality, data classification, business continuity and safety analysis | Organization-specific consequences |
| What should we do first? | Combined evidence and available fixes or workarounds | Documented remediation priority |
Read the CVE record, not just the number
Confirm the CVSS version
Record whether the score is CVSS 3.1, CVSS 4.0 or another version. Scores from different versions are not directly interchangeable: each version models metrics differently and can produce different numerical results for similar descriptions. Put the version beside the score in tickets, dashboards and reports.

Capture the complete vector and publisher
Save the vector string, score provider (such as the responsible CNA or vendor), record date and modification date. The vector exposes assumptions hidden by a single number: the attack path, required privileges, user interaction, scope or changed authority, and the confidentiality, integrity and availability impacts. Explain those assumptions in plain language rather than copying the score into a priority field.
Use CVSS 4.0’s metric groups correctly
CVSS 4.0 separates metrics into four groups:
- Base: intrinsic technical characteristics of the vulnerability.
- Threat: factors that can change over time, including threat intelligence such as exploitation maturity.
- Environmental: values that reflect your deployment, asset importance and modified impact requirements.
- Supplemental: additional characteristics that provide context but do not change the base score in the same way.
If an authority publishes only a base score, do not present it as an organization-specific risk rating. Where your process supports it, calculate or document threat and environmental considerations separately, preserving the original vector and provider score.
How do I know if a CVE is actually dangerous?
Use a sequence that moves from applicability to consequence. A CVE becomes urgent when several independent signals align: the vulnerable component is present, the attack path is reachable, exploitation is observed or likely, the asset is important, and mitigation is available or time-critical.

1. Confirm that the product and version are affected
- Read the CVE record’s description and affected-product data.
- Open the current vendor advisory and compare its affected and fixed-version statements with your inventory.
- Verify the exact installed version, build, edition, optional module and configuration. A package name alone may not prove that the vulnerable code is enabled.
- Check whether the component is bundled inside another product, container image, appliance or managed service.
Vendor statements can be newer or more precise than an initial vulnerability record. Treat an unverified product match as an investigation lead, not proof of exposure.
2. Determine whether an attacker can reach the vulnerable function
- Is the service directly reachable from the internet, from a partner network, from an employee network or only locally?
- Does exploitation require a particular protocol, port, endpoint, feature, file format or authentication path?
- Are network segmentation, access-control lists, reverse proxies, endpoint controls or application allow-lists blocking that path?
- Does exploitation require a logged-in user, administrator privileges or a victim to open content?
Do not infer reachability from a product’s presence. A vulnerable library on an isolated build server has a different attack path from the same library in an internet-facing gateway.

3. Identify what the affected asset can expose or change
Map the vulnerable process to the data and systems it can access. Consider credentials, secrets, personal information, source code, payment data, operational technology, safety functions and downstream management interfaces. Include effects that require chaining with another weakness or stolen account; a single CVSS record may not represent that chain.
4. Check exploitation evidence
Look for inclusion in the Cybersecurity and Infrastructure Security Agency’s Known Exploited Vulnerabilities catalog, credible incident reports, vendor warnings and telemetry from your own detection systems. KEV inclusion is meaningful evidence of known exploitation, but a CVE’s absence from KEV is not proof that it is safe.

What should I look at besides the CVSS score?
| Dimension | Questions | High-risk indications |
|---|---|---|
| Exposure | Where is the asset and who can reach it? | Internet-facing, broadly reachable or weakly segmented |
| Exploit conditions | What privileges, user action and prerequisites are needed? | Unauthenticated, low-complexity or reliable remote attack path |
| Threat activity | Is exploitation observed or predicted? | KEV listing, confirmed incidents or rising credible reports |
| Asset importance | What business or safety function depends on it? | Identity, payment, production, healthcare or safety-critical systems |
| Impact | What can an attacker read, alter, destroy or disrupt? | Privilege escalation, sensitive data loss, remote code execution or safety consequences |
| Controls | What reduces likelihood or blast radius? | No compensating control, weak monitoring or untested isolation |
| Mitigation | How quickly can exposure be removed? | Patch available but deployment blocked, or workaround unavailable |
Does a critical CVSS score mean I need to patch immediately?
It means the flaw has severe technical characteristics under the published assumptions; it does not, by itself, set your deadline. Patch immediately when the affected, reachable asset is important and exploitation evidence or plausible impact makes delay unacceptable. A critical score on software that is not installed, is unreachable and is not part of a recovery path may be lower priority than a medium-scored flaw in an exposed identity system.
Use a documented decision rather than an arbitrary score threshold. State the evidence, the chosen deadline, the temporary controls and what would cause the decision to change.

| Situation | Typical decision logic |
|---|---|
| KEV-listed, internet-facing, vulnerable version confirmed | Emergency remediation or isolation; monitor continuously until fixed |
| High or critical score, exposed asset, no exploitation observed | Accelerated patching based on asset criticality and exploit prerequisites |
| High score, vulnerable package present but unreachable and strongly isolated | Verify isolation, apply a scheduled fix and watch for topology changes |
| Score unavailable because the record is not enriched | Assess from the vendor advisory, vector if supplied, exposure and impact; do not treat missing data as low severity |
How to check whether a CVE is being exploited
Use KEV as an observed-exploitation signal
Check whether the CVE appears in the current KEV catalog and note the catalog’s date and any remediation deadline or required action. Inclusion means exploitation is known to have occurred; it does not tell you whether your specific instance has been attacked.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsUse EPSS as a forecast, not a fact
Capture the EPSS value and timestamp. Compare it with exposure, exploit prerequisites and your own telemetry. A high estimate without a reachable vulnerable asset does not create exposure; a low estimate does not eliminate it.
Search for organization-specific indicators
- Web, authentication, endpoint, DNS, firewall and cloud audit logs for the relevant attack path.
- Unexpected process launches, child processes, file changes, outbound connections or account activity.
- Vendor detection guidance, signatures and incident-response advisories.
- Threat-intelligence reports that identify exploit code, campaigns or affected versions.
Document “no evidence found” with the time range and data sources examined. That statement is narrower and more defensible than “not exploited.”
A practical CVE risk-assessment workflow
- Confirm applicability. Match the product, version, configuration and vulnerable component against the vendor advisory and your asset inventory. Verify reachability.
- Interpret the CVSS evidence. Record version, score, complete vector, provider and record date. Translate attack conditions and impacts into plain language. Add threat and environmental context where supported.
- Check active-threat evidence. Review KEV, credible exploitation reporting, EPSS and internal telemetry. Mark evidence as observed, predicted, absent or uncertain.
- Assess exposure and impact. Identify privileges, user interaction, reachable paths, affected data, dependent systems, business downtime and safety consequences. Include likely vulnerability chains.
- Set and document priority. Combine severity, exploitation, exposure, asset criticality, consequences and mitigation availability. Write the reason for the deadline and the evidence that would change it.
- Apply and verify mitigation. Patch, upgrade, disable the feature, restrict access or isolate the asset according to the vendor’s guidance. Confirm the fixed version and test that the attack path is closed.
- Recheck changing evidence. Refresh the vendor advisory, CVE record, NVD status, KEV membership and threat estimate before relying on an old assessment.
Why CVE sources and scores can disagree
A vendor or CNA may publish a score while the National Vulnerability Database has a different score, no score or a later enrichment. Compare the underlying assumptions instead of choosing whichever number looks more alarming.
| Compare | What to verify |
|---|---|
| CVSS version | Whether both scores use the same version and metric definitions |
| Metric assumptions | Attack path, privileges, user interaction, scope and impact values |
| Publisher and date | Who calculated the score and when the record or advisory was updated |
| Observed versus predicted threat | KEV or incident evidence versus EPSS-style likelihood estimates |
| Affected versions | Whether each source describes the same editions, builds and configurations |
| Local consequences | Asset criticality, downstream effects and safety implications |
| Mitigation | Patch availability, workaround quality and deployment constraints |
If a CNA has already supplied a score, NIST says it will no longer routinely provide a separate score. A difference therefore may reflect source responsibility rather than an error. Preserve both values and vectors when they materially differ, then explain which assumptions fit your deployment.

Current NVD limitations and data checks
Enrichment is now prioritized
In an April 15, 2026 announcement, NIST said NVD enrichment would prioritize KEV-listed CVEs, software used within the federal government and critical software defined by Executive Order 14028. Other CVEs remain listed but may not receive immediate enrichment. NIST also acknowledged that those criteria can miss other potentially high-impact cases. An unenriched record or missing NVD score therefore says little about safety.
Refresh stored CVSS 4.0 scores
NIST reported an error in calculated CVSS 4.0 numerical scores: approximately 4,500 records, or 19% of records with a v4.0 score, were higher than the correctly calculated value; fewer than 30 were lower. NIST corrected the underlying calculation error and planned automated verification. If you rely on synchronized or cached NVD data, inspect modification history and refresh the record before making a high-impact decision.
Use the newer feed fields
On June 17, 2026, NIST reported that NVD feeds and APIs added SSVC v2.0.3 and CVE Affected v1.0 information. The supplied SSVC data includes a computed score showing how a vulnerability priority was decided. Treat that score as an additional decision aid, not a replacement for validating your own exposure and consequences.
Interpret the volume of CVEs carefully
NIST reported a 263% increase in CVE submissions between 2020 and 2025 and nearly 42,000 CVEs enriched in 2025, 45% more than any prior year. More records and changing enrichment coverage make automation useful, but they also increase the need to verify product matching, dates, status and source assumptions.
How to document a defensible priority
For every material CVE, keep a short assessment that another analyst can reproduce:

Never add CVSS scores together to create a system score. Aggregate organizational risk from assets, attack paths, dependencies and consequences, while retaining each vulnerability’s technical assessment.
Common assessment mistakes
Treating the score as a patch queue
A numerical sort ignores deployment, reachability and asset value. Use the score to start triage, then apply local evidence.
Assuming absence from KEV means safety
KEV is valuable evidence of known exploitation, not a complete inventory of attacks. Absence can mean that exploitation is unknown, unreported or outside the catalog’s coverage.
Calling an EPSS estimate proof of exploitation
EPSS predicts likelihood and can be inaccurate. Pair it with observed telemetry and exploit prerequisites.
Ignoring record dates and status
Scores, affected-version statements and enrichment can change. A stale synchronization can preserve a corrected CVSS 4.0 error or miss a newly published fix.
Overlooking chains and safety effects
A CVSS assessment may not model a multi-step attack, downstream identity compromise or physical safety consequence. Add those effects in the organizational assessment.
Bottom line: make CVSS the starting point, not the verdict
Read the CVSS version and vector, verify the vendor’s affected-version statement, check exploitation evidence, and map the attack path to real assets and consequences. Prioritize the CVE when the vulnerable software is deployed and reachable, the asset matters, exploitation is observed or plausible, and mitigation is available or urgent. Recheck the record as NVD enrichment, vendor guidance, threat intelligence and your own environment change.
