Assess off-target toxicity by measuring what the complete degrader changes in relevant cells and tissues, not merely whether its ligand binds selectively. A defensible workflow combines target and whole-proteome measurements, concentration- and time-dependent exposure studies, functional toxicity assays, mechanism-appropriate animal models, and iterative chemistry or dosing changes. A candidate is safer only when an unwanted signal improves while intended degradation and efficacy-relevant activity remain.
This guide applies to heterobifunctional PROTACs and molecular glues during drug development. It is a practical decision framework, not a universal regulatory checklist or a clinical prescribing guide.
Why binding selectivity does not predict a degrader’s safety profile
A PROTAC links a target-binding ligand to an E3-ligase recruiter through a linker. A molecular glue changes an E3 ligase’s substrate recognition without the same bivalent architecture. Both can induce ubiquitination and proteasomal removal of proteins. Because the degrader acts catalytically, one molecule can promote turnover of multiple target molecules; continuous occupancy is therefore not the only determinant of biological response. The European Medicines Agency describes this as event-driven pharmacology and highlights non-linear exposure-response, tissue-specific E3 biology and species differences as development challenges (EMA horizon-scanning report).
Testing the target ligand, E3 recruiter and linker separately can miss effects created by the linked molecule. The ternary complex may engage an unintended protein, recruit an unexpected substrate or perturb a pathway at concentrations where the individual components appear inactive. The safety question is consequently: which proteins and pathways change in the actual treated cell or tissue, at what exposure and for how long, and do those changes produce a functional injury?

Build a candidate-specific risk hypothesis first
Before ordering broad safety studies, record the biological assumptions that the assays must test. This prevents a proteomics screen from becoming an unprioritized list of altered proteins.
Define the molecule and intended pharmacology
- Target protein, target abundance and the intended depth of depletion.
- E3 ligase, recruiter chemistry, warhead and linker series.
- Expected active concentration range and anticipated exposure in each intended tissue.
- Whether the therapeutic effect requires brief depletion, sustained depletion or repeated pulses.
- Normal tissues in which the target is essential or highly expressed.
Describe the expected pharmacodynamic window as a range, not a single potency value. Include the exposure at which target degradation plateaus, the time needed to reach maximal depletion and the time required for recovery after compound removal.
List plausible liabilities before testing
Include risks from the complete bivalent chemistry, unintended protein engagement, excessive intended degradation in normal tissue, recruited-ligase biology and pathway perturbation. For cereblon (CRBN)-recruiting agents, explicitly list plausible neosubstrates and developmental or reproductive concerns. A 2026 review links these concerns to the developmental-toxicity history of thalidomide-related immunomodulatory drugs (IMiDs), while emphasizing that individual CRBN degraders do not necessarily share the same neosubstrate profile (Nature Reviews Drug Discovery review).

Quantitative proteomics is the broadest practical way to discover protein changes that were not predicted from ligand selectivity. Use a relevant cellular context and measure the intended target and the wider proteome in the same experiment. The 2026 Nature Chemical Biology study used high-throughput data-independent acquisition (DIA) mass spectrometry and proteome-trained machine learning to connect protein changes with toxicity mechanisms; in its HepG2 workflow it consistently quantified 3,331 proteins under the described acquisition conditions. That count is a study-specific technical result, not a guaranteed performance level for every laboratory or tissue (Nature Chemical Biology study).
The study’s authors describe their approach this way: “Here, we developed a drug discovery framework that exploits high-throughput proteomics to map and mitigate toxicity mechanisms of emerging drug modalities.” Use that framework as a model for integrating molecular and functional data, not as evidence that one platform replaces all other safety assays.

Design the concentration and time matrix
Run multiple concentrations spanning inactive exposure, intended pharmacology and potential supratherapeutic exposure. Collect early and late time points and, where feasible, samples after compound removal. This separates direct degradation from delayed pathway effects, identifies exposure thresholds and shows whether a change recovers.
| Dimension | What to measure | Why it matters |
|---|---|---|
| Concentration | Below-active, pharmacologically active and high-exposure conditions | Distinguishes on-target effects from liabilities that appear only above the intended window |
| Time | Early, maximal-effect, late and recovery samples | Captures event-driven, delayed or persistent toxicity |
| Context | Relevant disease cells plus normal-cell or organ-relevant models | Tests whether a signal is selective for diseased tissue |
| Controls | Vehicle, target ligand, recruiter, linker or inactive analogues when informative | Helps attribute an effect to the complete degrader rather than a component |
Interpret proteomic hits as hypotheses
A changed protein is not automatically a toxic event. Prioritize hits by magnitude, reproducibility, concentration dependence, temporal ordering and biological plausibility. Confirm important changes with an orthogonal method such as immunoblotting, targeted mass spectrometry or another validated protein assay. Then test whether the change depends on the recruited E3 ligase or the proteasome. A change that disappears when the relevant degradation machinery is blocked is mechanistically different from a nonspecific stress response.

Connect molecular changes to functional toxicity
Proteomics tells you what changed; functional assays establish whether the change harms a cell or organ-relevant process. Select endpoints from the risk hypothesis rather than using viability as the only readout.
Use a tiered functional panel
- Cell health: viability, apoptosis or necrosis, proliferation and recovery after washout.
- Mechanism-specific function: mitochondrial respiration, electron-transport activity, lysosomal function, DNA-damage response or other pathway endpoints indicated by the degradome.
- Organ-relevant function: hepatic injury markers and transporter function for a liver hypothesis; contractility or electrophysiology for a cardiac hypothesis; barrier or neuronal function where appropriate.
- Exposure linkage: intracellular or free-drug concentration, target depletion and functional response measured on matched samples.
A hepatic cell model can reveal a mechanism and rank analogues, but it is not equivalent to whole-organism or clinical safety evidence. Interpret every result in the context of model maturity, exposure, protein expression and the ability of the system to recover.
Account for non-linear and delayed behavior
Degrader concentration-response curves may not be monotonic. At high concentrations, the hook effect can reduce productive ternary-complex formation, making apparent potency fall even as total exposure rises. Event-driven pharmacology can also produce delayed onset or effects that persist after plasma or media concentrations decline. Include sufficient observation time, washout and recovery measurements before concluding that a high dose is inactive or that a short study is negative.

Choose nonclinical models that represent the liability
Species selection must begin with biology, not convention. Determine whether each candidate species expresses the recruited E3 ligase, intended target and suspected unintended substrates at comparable levels and in comparable tissues. A negative animal result is weak evidence if the species lacks the human-relevant substrate or uses a different E3-ligase distribution.
Evaluate tissue and species comparability
- Compare E3-ligase and substrate expression across human and animal tissues.
- Check whether the recruiter forms the same productive complexes in the test species.
- Assess whether metabolism changes the active degrader or creates a species-specific metabolite.
- Use human primary cells, organoids or other translational models when an animal cannot reproduce the relevant biology.
For CRBN-recruiting compounds, developmental and reproductive assessment should reflect the mechanism, exposure and program context. The CRBN review and EMA report identify species expression and potential developmental effects as issues requiring explicit consideration, but neither establishes one universal species or test battery.
CRBN-specific questions that belong in the safety plan
CRBN recruiters can alter substrate recognition and cause unintended neosubstrate degradation. Do not infer a common neosubstrate panel or a guaranteed adverse effect across all CRBN degraders. Instead:
- Profile the complete candidate, not only the recruiter, in CRBN-expressing relevant cells.
- Measure known or plausible neosubstrates across concentrations and time, including recovery.
- Use CRBN perturbation, proteasome inhibition or recruiter-displacement controls to test causality.
- Compare neosubstrate changes with functional developmental, reproductive or tissue-specific endpoints appropriate to the program.
- Check human-versus-animal expression and complex-formation differences before relying on a negative species result.
The rationale for this emphasis is historical and mechanistic: thalidomide-related IMiDs established that CRBN-mediated substrate changes can have developmental consequences. That history supports rigorous assessment; it does not prove that every modern CRBN degrader produces the same profile.
Use an evidence matrix to decide whether a signal is actionable

| Observation | Interpretation | Next decision |
|---|---|---|
| Protein change only at concentrations far above intended exposure, with no functional effect | Possible pharmacology outside the projected window or assay artifact | Confirm free-drug exposure, repeat orthogonally and define a safety margin |
| Reproducible protein loss that depends on the recruited E3 ligase | Likely degrader-mediated off-target or neosubstrate event | Confirm substrate identity and test chemistry or recruiter changes |
| Protein change precedes a concentration-matched functional injury | Mechanistically credible toxicity signal | Investigate causality and set a development decision threshold |
| Functional injury without a corresponding protein-loss signature | Possible pathway perturbation, metabolite effect or non-degradative engagement | Expand target-engagement, transcriptomic, metabolite and cellular-function studies |
| Signal appears after prolonged dosing or fails to recover | Delayed or persistent event-driven liability | Extend study duration and reconsider accumulation, schedule and tissue exposure |
| Animal study is negative but the species lacks the human substrate | Non-informative negative result | Use a biologically relevant species or human-derived model |
Reduce toxicity without losing intended degradation
Mitigation is empirical and candidate-specific. Every change must be tested against the same target-degradation, proteome and functional endpoints used to identify the liability.
Redesign the chemistry
Modify the linker, warhead, recruiter or ternary-complex geometry to reduce unwanted engagement while preserving productive target degradation. Linker changes can alter permeability, cooperativity, residence time and the set of proteins recruited. A 2026 study of experimental androgen-receptor (AR) degraders found that modest linker changes reduced electron-transport-chain complex I engagement and hepatotoxicity while preserving AR degradation and improving antitumor activity in a castration-resistant prostate-cancer xenograft model (Nature Chemical Biology study). This is a useful case study in iterative optimization, not proof that a particular linker strategy generalizes to every target or scaffold.
Change the recruited ligase when biology drives the liability
If the unwanted effect tracks with the E3 ligase or its tissue distribution, evaluate a different recruiter or ligase. This can remove a neosubstrate liability, but it may also change tissue exposure, potency, degradation kinetics and species translation. Re-profile the complete molecule rather than assuming that a new recruiter is intrinsically safer.
Optimize exposure and schedule
Use exposure-degradation-response data to determine whether the desired target effect is maintained with a lower peak, shorter pulse or longer interval. Schedule changes can reduce normal-tissue exposure while retaining tumor or disease-tissue degradation. They can also fail if the target remains depleted after the dose is gone or if repeated dosing causes accumulation. Measure target recovery and functional recovery before selecting an intermittent regimen.
Demonstrate retained benefit
- Intended target degradation remains within the efficacy-relevant range.
- Unintended protein or pathway changes decrease at matched active exposure.
- Functional toxicity improves in the same model and exposure window.
- Efficacy-relevant cellular and in-vivo activity is preserved or improved.
- The revised candidate does not simply appear safer because it is less exposed or less active overall.
Compare degrader candidates consistently
When several molecules are available, compare them on the same axes and at matched free-drug exposures. A single potency number cannot capture event-driven or tissue-specific risk.

Common failure modes and corrective actions
Relying on component testing alone
Failure: the ligand and recruiter look clean individually, so the linked molecule is advanced without broad profiling. Correction: test the complete degrader and use component controls only to answer attribution questions.
Calling every proteomic change toxic
Failure: a long list of altered proteins is treated as a safety conclusion. Correction: confirm priority hits, establish E3 or proteasome dependence and connect them to function and exposure.
Using one concentration or one time point
Failure: delayed, persistent or high-exposure effects are missed. Correction: sample a concentration-time-recovery matrix that includes the projected clinical or translational window.
Recommended Free Tools
Ignoring the hook effect
Failure: reduced degradation at high concentration is interpreted as lack of target engagement. Correction: measure ternary-complex or target-degradation behavior across the full range and interpret non-monotonic curves mechanistically.
Overgeneralizing a CRBN or linker result
Failure: one neosubstrate profile or one successful linker change is assumed to apply to every candidate. Correction: reproduce the finding in the new molecule, context and exposure range.
What the current development landscape implies
The EMA’s 2026 horizon-scanning report estimated more than 40 TPD candidates in clinical development at the time of publication. That is a dated landscape estimate, not a live pipeline count (EMA report). As the modality expands, standardized thinking about degradome-wide effects, E3-ligase biology, timing and species relevance becomes more valuable, but the reviewed industry perspective does not establish one universal regulatory protocol (PubMed: Hemkens et al., 2023; Drug Discovery Today highlights).
Quick Recap
A practical go/no-go checklist
- The candidate’s target, recruiter, linker, intended tissues and exposure range are documented.
- CRBN or other E3-specific neosubstrate hypotheses are explicit where relevant.
- Proteomics covers multiple concentrations, time points and recovery, with controls that address the actual attribution question.
- Priority protein changes are confirmed orthogonally and tested for degradation-machinery dependence.
- Functional toxicity endpoints are matched to the molecular risk and measured at matched exposure.
- Species and tissue models express the relevant E3 ligase and substrates sufficiently for translation.
- Any chemistry or schedule mitigation is re-tested for target degradation, unintended effects, functional safety and efficacy.
- Conclusions state the exposure, duration, model and uncertainty rather than implying a class-wide safety claim.
