An AI behavior is a security vulnerability when an attacker can use it to cross a security boundary or cause a concrete loss of confidentiality, integrity, or availability. The practical way to assess one is to examine the deployed system—not just model weights—then preserve a safe, reproducible record, report privately through the right channel, coordinate remediation, and publish only after the exposure is controlled.
What counts as an AI security vulnerability?
Start with the security consequence, not with how strange the model’s answer looks. A model that is inaccurate, offensive, biased, or contrary to a provider’s usage policy may be a quality, safety, or governance problem without being a security vulnerability. It becomes a security issue when an attacker can exploit the behavior to obtain unauthorized data, alter protected information or actions, bypass an access control, execute an unintended tool operation, consume service resources, or otherwise create a material loss.
The relevant asset is often the complete deployed system: model, API or user interface, prompts and system instructions, retrieval data, tools, plug-ins, identity and permission layers, logging, and downstream integrations. A model may be harmless in a local experiment yet vulnerable when an application gives it access to customer records or an administrative tool. Conversely, a troubling output with no path to unauthorized action may belong in a quality or safety report rather than a vulnerability queue.
Security, quality, safety, and governance are different queues
| Observation | Likely classification | What would make it security-relevant? |
|---|---|---|
| A factual error or fabricated citation | Quality or reliability failure | The error enables an attacker to bypass a control, corrupt a protected workflow, or cause unauthorized action. |
| Offensive, biased, or policy-prohibited text | Safety, policy, or trust issue | The behavior exposes protected data, defeats an access boundary, or can be reliably weaponized against a system or person. |
| Prompt injection changes an agent’s behavior | Potential security vulnerability | The injected content causes the agent to reveal secrets, change records, send messages, or invoke tools outside the user’s authority. |
| Model memorizes or reveals a private record | Potential confidentiality vulnerability | An unauthorized party can reproduce or extract the record under realistic conditions. |
| Very expensive or resource-heavy requests | Potential availability or cost-abuse issue | An attacker can trigger material resource exhaustion or financial loss without equivalent authorization. |
State the boundary in one sentence: “A user with these permissions can cause that protected asset or action to be reached.” If you cannot identify the boundary, asset, attacker capability, and loss, the report may still be useful feedback, but it is not yet a substantiated vulnerability claim.

Scope the affected asset before testing
Record exactly what you tested and under what conditions. NIST’s initial preliminary AI Cybersecurity Framework Profile, published in December 2025, recommends maintaining inventories of models, APIs, agents, keys, data, integrations, and permissions, and tracking changes that could alter risk. Apply that discipline to your own evidence.
Minimum asset record
- Provider and product: name the organization, application, API, model family, and interface.
- Version or release: capture a model identifier, application build, endpoint version, and region when displayed. If no fixed version is exposed, record the date and time and say that the provider can change the service.
- Deployment: distinguish a local model, hosted API, chat application, retrieval-augmented system, autonomous agent, or an integration inside another product.
- Connected assets: list the data stores, tools, plug-ins, browser or code execution capabilities, webhooks, queues, and external systems the model can reach.
- Identity and permissions: document the test account’s role, tokens or keys used, tenant boundaries, and whether the action required a second approval.
- Configuration: record system instructions, safety settings, retrieval filters, tool schemas, temperature or sampling controls, and relevant feature flags.
Do not include live secrets in a report. Replace tokens with a clear marker in your private notes, revoke any credential that was exposed, and tell the provider that a redacted secret was present.
Define the security property and boundary
Classify the primary property at risk as confidentiality, integrity, availability, or a combination.

Examples include disclosure of another user’s conversation, retrieval of restricted documents through an agent, leakage of system prompts containing credentials, or output of memorized personal data to an unauthorized requester. Specify whose data was exposed, how authorization was bypassed, and whether the output contained a complete record or only a fragment.
Integrity
Integrity is affected when a model-mediated action changes data or decisions without the required authority. Examples include an agent approving a transaction, editing a ticket, changing a deployment setting, or writing a database record after an untrusted prompt. Demonstrate the resulting state change with a reversible test record whenever possible.

Availability and resource use
Availability issues include a repeatable way to exhaust model context, queue capacity, tool quotas, or downstream services. A single slow response is not enough. Describe the request rate, account limits, resources consumed, and whether ordinary users were affected. Stop before causing a real outage.
Security boundary
Name the boundary crossed: tenant isolation, role authorization, data-classification rule, tool permission, network boundary, or administrative approval. “The model ignored its instructions” is an observation; “an unauthenticated user obtained a restricted customer record through the retrieval tool” identifies a security claim that can be tested.

Reproduce the behavior safely
Your goal is independent confirmation without creating a second incident. Test only systems and accounts for which you have authorization, and use synthetic data unless the provider explicitly gives you a safe test environment.
- Prepare a minimal case. Reduce the input to the smallest prompt, document, tool call, or sequence that still produces the behavior. Remove unrelated customer data and avoid probing other tenants.
- Capture the environment. Save the model and application identifiers, endpoint, account role, configuration, date and time with time zone, and any relevant feature or policy setting.
- Record inputs and outputs. Preserve the exact prompts, retrieved snippets, tool arguments, model response, HTTP status, and resulting state. Redact personal data and credentials while keeping enough structure for the provider to reproduce the issue.
- Repeat conservatively. Run only the number of trials needed to establish whether the result is deterministic, probabilistic, or dependent on a particular sequence. Note temperature, seed, sampling, context length, and tool availability where those controls exist.
- Use reversible actions. If the test writes data or sends a message, use a designated test record, a non-production recipient, or a dry-run mode. Do not delete, encrypt, publish, or alter real customer information.
- Stop at the boundary. Once you have demonstrated unauthorized access or action, do not continue exploring the full dataset or chaining additional exploits. Notify the provider promptly if further confirmation would require risky activity.
CISA’s VINCE-NT reporting form asks for enough information to independently confirm a finding. Screenshots or video can support a report, but they may not be sufficient by themselves: include the exact steps, inputs, account conditions, and expected versus actual result.
Separate observation from inference
AI services can be nondeterministic, and providers can change a model, prompt, filter, or tool policy without notice. Write what you observed separately from what you think caused it.

| Evidence level | Example wording |
|---|---|
| Direct observation | “On 16 October 2026 at 14:10 UTC, a standard user account submitted the following prompt and the agent returned a document belonging to another tenant.” |
| Reproduction result | “The result occurred in 3 of 5 trials with temperature 0; it did not occur when retrieval was disabled.” |
| Hypothesis | “The behavior may indicate a tenant-filter failure in the retrieval layer; the model’s internal cause has not been established.” |
Do not assert that the model weights are defective when the evidence points to an application prompt, retrieval filter, identity check, tool wrapper, or integration. NIST’s December 2025 preliminary profile notes that the effects of AI-system changes can be difficult to predict without testing the changed system. Record the version and rerun a minimal case after a provider reports a change.
Assess impact, exploitability, and scope
A useful impact statement answers five questions:
- Who is the attacker? State whether exploitation requires an anonymous visitor, an authenticated user, a low-privilege employee, a tenant administrator, or access to a connected system.
- What must the attacker do? Describe the prompt, file, message, API call, or sequence and any prerequisites such as a victim opening content or an administrator enabling a tool.
- What can be gained or changed? Identify the data, account, workflow, model resource, or external system affected.
- How broad is the effect? Distinguish one test record from an entire tenant, one endpoint from all deployments, and a theoretical chain from a demonstrated one.
- How reliable is it? Report trial counts and conditions, failure cases, rate limits, and whether another authorized tester independently reproduced it.
Frame impact as attacker gain or victim loss, as requested by CISA’s VINCE-NT form. Avoid labels such as “critical” unless the provider’s policy defines them and your evidence meets that definition. A single anecdotal response does not establish a systemic data breach.
Build an evidence package
Organize the report so a triage analyst can confirm the issue without guessing.
Recommended report contents
- Title: use a precise statement such as “Cross-tenant retrieval returns restricted document to standard user.”
- Affected system: provider, product, model or endpoint, version or date observed, region, and deployment architecture.
- Summary: two or three sentences describing the security property, boundary, and consequence.
- Prerequisites: account role, permissions, feature flags, required document or message, and any victim interaction.
- Reproduction steps: numbered, minimal steps with sanitized inputs and expected and actual results.
- Evidence: redacted logs, request and response identifiers, timestamps, screenshots or video, and the resulting state. Explain what each artifact proves.
- Impact and scope: affected users or systems, attacker gain, victim loss, reliability, and limits of your test.
- Mitigation ideas: suggest containment—such as disabling a tool or tightening authorization—without prescribing an unverified root-cause fix.
- Disclosure contact: your name or handle, secure reply address, preferred encryption method if offered, and a safe way to reach you.
Keep a private chronology of discovery, tests, submissions, replies, and changes in behavior. Hashing or access-controlled storage can help demonstrate that evidence was not altered, but never retain sensitive data merely to make a report look complete.
Choose the right disclosure route
Look for the provider’s published security or vulnerability disclosure policy before sending details. Check whether model behavior, prompt injection, data leakage, abuse of tools, and denial-of-service testing are in scope; which tests are permitted; which channels accept sensitive material; and whether the policy offers safe-harbor language. A bug-bounty program may exclude model behavior, and a general support address may not protect confidential evidence.

| Route | Best fit | Strengths | Limits to verify |
|---|---|---|---|
| Provider security or vulnerability channel | Issue confined to one provider or product | Direct access to engineers, logs, mitigations, and affected release controls | Policy scope, testing permission, response expectations, and safe-harbor terms vary. |
| Competent third-party coordinator, such as CISA’s VINCE-NT for eligible new-technology cases | No suitable provider channel, multiple organizations, or broader infrastructure risk | Can help coordinate identification, remediation, and disclosure across parties | Eligibility, jurisdiction, case handling, and authority must be confirmed; it is not an automatic route for every report. |
| Coordinated disclosure with affected parties | Several vendors, integrators, or downstream users share the exposure | Aligns mitigations and communication so one party does not publish while others remain exposed | Requires agreement on contacts, evidence sharing, timelines, and who owns each fix. |
| Academic proposal or community practice | Designing policy for AI and machine-learning flaws | Can provide concepts such as “Coordinated Flaw Disclosure” | The 2024 proposal is scholarly guidance, not a regulator requirement or universal standard. |
CISA’s January 14, 2025 JCDC AI Cybersecurity Collaboration Playbook describes voluntary information-sharing processes for AI-related vulnerabilities and incidents. “Voluntary” matters: the playbook is a collaboration model, not a mandated disclosure deadline or a substitute for the provider’s policy.
Submit a private report
- Use the designated channel. Follow the provider’s current policy and encryption instructions. If the form limits detail, submit a concise summary and ask for a secure method for logs or samples.
- Identify one vulnerability at a time. CISA’s VINCE-NT form asks reporters to distinguish multiple vulnerabilities. Separate unrelated findings so each can be validated and tracked.
- State the requested handling. Ask for acknowledgement, a technical contact, validation of the affected versions, and confirmation of any temporary mitigation. Do not demand a public deadline that the evidence cannot justify.
- Limit access to evidence. Share only the minimum data needed for confirmation. Keep a redacted copy and a dated record of what was sent.
- Respond to validation questions. Reproduce in an approved test environment, clarify assumptions, and promptly report if a model or configuration change stops the behavior.
If the provider has no usable channel and the issue affects multiple organizations or critical infrastructure, contact a competent coordinator and explain why direct reporting is unavailable or insufficient. Confirm the coordinator’s current eligibility and handling rules before sending sensitive material.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Coordinate remediation and communication
Use a managed lifecycle: receive, assess, coordinate, remediate, and communicate. NIST SP 800-216, Recommendations for Federal Vulnerability Disclosure Guidelines (May 2023), recommends formal processes for accepting, assessing, and managing reports and for communicating mitigation or remediation. Its federal framework is a useful process model, not a universal rule for every private AI provider.
During assessment
- Agree on a technical contact and a secure channel.
- Confirm the affected model, application versions, and deployment conditions.
- Decide whether a temporary control—such as disabling a tool, retrieval source, endpoint, or account role—is needed.
- Define what evidence can be shared with other affected organizations.
- Track provider-side changes and retest the minimal reproduction after each relevant change.
When planning publication
There is no single deadline for all AI vulnerabilities. The appropriate date depends on the provider’s policy, jurisdiction, severity, affected parties, mitigation readiness, and whether public details would enable exploitation. Agree on a case-specific plan rather than applying a universal countdown.
Publish only after a risk review with affected parties. Describe the affected versions and conditions, concrete impact, available mitigations, validation status, and important limitations. Avoid releasing prompts, data, tool calls, or chain details that would expose real secrets or provide a ready exploit before protections are available. If coordination fails, document the attempts and explain the remaining risk without overstating certainty.

Common failure modes and how to correct them
Calling every jailbreak a vulnerability
A jailbreak demonstrates policy circumvention, not automatically a security flaw. Connect it to unauthorized data access, tool execution, account impact, or another protected asset. Otherwise route it as safety or policy feedback.
Testing production too aggressively
Large-scale extraction, high-rate requests, destructive tool calls, or probing other tenants can turn research into an incident. Use synthetic data, low request rates, reversible actions, and explicit authorization; stop once the boundary is proven.
Reporting only a screenshot
A screenshot can omit the model version, account role, hidden context, and exact sequence. Add sanitized inputs, outputs, configuration, timestamps, expected and actual results, and a minimal reproduction.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Assuming a fixed model is permanent
Hosted systems can change prompts, filters, retrieval indexes, routing, or weights. Record dates and identifiers, report nondeterminism, and avoid claiming a root cause that has not been demonstrated.
Overstating scope from one interaction
One surprising answer does not prove every user or tenant is affected. State the number of trials, affected roles, boundaries tested, and what remains unknown.
Publishing before mitigation
Public exploit material can increase harm. Coordinate privately, request containment, and publish a bounded description only when the residual risk is acceptable and the affected parties have had a fair opportunity to respond.
A practical decision checklist
- Have you named the exact model, application, endpoint, version, and deployment conditions?
- Can you identify the confidentiality, integrity, or availability property and the boundary crossed?
- Is there a concrete attacker gain or victim loss rather than only an objectionable output?
- Are the reproduction steps minimal, authorized, safe, and independently confirmable?
- Did you record prompts, outputs, configuration, timestamps, and nondeterministic conditions while redacting secrets?
- Did you distinguish observed facts from hypotheses about the model or application’s cause?
- Did you check the provider’s policy, permitted testing, reporting channel, and safe-harbor terms?
- Have you separated multiple vulnerabilities and preserved a dated submission record?
- Have you agreed on validation, mitigation, affected-party coordination, and a case-specific communication plan?
- Would publication details create avoidable exposure before protections are available?
What a strong report sounds like
“On 16 October 2026, a standard tenant user on the hosted agent’s retrieval-enabled endpoint caused the agent to return a document from a second tenant. The account had no permission to view that tenant, and the result occurred in three of five trials using the same sanitized query. Retrieval disabled the behavior. No production data was modified; testing used a designated synthetic document. The evidence includes redacted request identifiers, timestamps, model and endpoint identifiers, and the exact steps. We have not established whether the defect is in retrieval filtering, authorization, or model instruction handling. Please confirm the affected versions, advise on a temporary containment measure, and provide a secure channel for additional logs.”
