Export the access-review decision report as a complete, multi-page PDF, then preserve its filters, dates, decisions, decision makers, metadata, review history, and integrity controls in a managed records repository. The summary page alone is not sufficient evidence: the detailed pages for every review, certification entity, item, decision, and decision maker must remain available and legible.
What a complete access-review evidence package contains
An access review is usually demonstrated by more than a screenshot or a single approval message. A defensible package lets an independent reviewer answer five questions: what was reviewed, which population and dates were used, what decision was made for each item, who made and reviewed the decision, and whether the record has remained complete and unaltered.
- The full report: the summary page plus every detailed page for reviews, certification entities, items, decisions, and decision makers.
- Run context: report name, system or application, review period, filter values, run time, operator, and a unique record identifier.
- Approval context: the approver or reviewer, the sign-off date, and the relationship to the review.
- Related evidence: links or identifiers for the underlying review, audit logs, emails, confirmations, tickets, and digital signatures.
- Control evidence: repository access history, retention or legal-hold status, backup information, and any hash or other tamper-evidence record.
The U.S. National Archives and Records Administration describes trustworthy web records in terms of content, context, and structure, with integrity meaning that the content is complete and unaltered. Apply that test to the PDF and to the metadata stored with it.

Before exporting, record the exact criteria that define the report population. A later reviewer should be able to distinguish a deliberate scope from an accidental default.

Record the scope-defining fields
- Creation-date range.
- Signed-off-date range, if the system separates completion from creation.
- Due-date range.
- Application or system.
- Manager, certification owner, or other responsible person.
- Tags or labels used to include or exclude reviews.
- Any status, department, identity, entitlement, or risk filters shown by the report interface.
Save the filter values in a run log before selecting Export. If the application provides a report ID, query ID, or run timestamp, copy it exactly. If it does not, create your own unique identifier and associate it with the report name, system, and run time.
Capture the report at the correct lifecycle point
Run the report after the required decisions are complete and signed off, but before the underlying data is likely to change. If the system records a separate completion or sign-off event, note that event time rather than relying only on the PDF creation time. A PDF generated later can still be valid, but the evidence package must distinguish the review period from the export event.
Keep the original system reference
Record the URL or internal navigation path to the report, the application name and version if displayed, and the identifier of the access-review campaign or certification. Do not rely on a URL alone: links can expire, permissions can change, and a future administrator may need the campaign ID to locate the record.

Export every page to PDF
The access-review decision report documentation identifies PDF export as the best option for large report results. Use the product’s own export control where available because it preserves the report’s pagination and fields more reliably than copying visible text.
- Open the access-review decision report.
- Enter and review the creation, signed-off, due-date, application, manager, and tag filters.
- Run the report and wait until the result count and all detail sections have loaded.
- Choose the report’s Export or Export to PDF action. Do not export only the summary or the currently visible page.
- Choose a full-report or all-pages option if the interface offers page ranges.
- Wait for the export to finish, then download the resulting PDF without editing it.
- Rename the file using a stable convention such as
access-review_{system}_{period}_{run-id}.pdf. Keep the run ID in a separate metadata record as well.
If the export dialog offers paper size, orientation, or scaling, choose settings that keep tables readable. Landscape orientation is often appropriate for decision and entitlement columns. Avoid a scale setting that shrinks text below a practical reading size.
Check the PDF before treating it as evidence
Export completion is not the same as a complete record. Perform a documented quality check on the downloaded file.
Page and content checks
- Compare the PDF page count with the report’s indicated page count or expected sections.
- Confirm that the summary is followed by detailed pages for each review, certification entity, item, decision, and decision maker.
- Check the first, middle, and final pages for missing content, blank pages, clipped columns, overlapping text, and unreadable fonts.
- Verify that report title, filters, date ranges, application, and manager fields are visible.
- Confirm that every decision is paired with the correct item and decision maker.
- Check that headers, footers, page numbers, and continuation markers make page order clear.
- Search the PDF for a known review, item, and decision maker to confirm that text is selectable and searchable where expected.
Integrity checks
- Open the file from the location where it will be archived, not only from a browser download preview.
- Record the file size, creation time, and a cryptographic hash if your records process supports hashes.
- Do not overwrite the exported file after hashing. Any corrected export should receive a new record identifier and a new hash.
- Keep a note of who performed the check, what was checked, and when.
A hash demonstrates that the stored bytes have not changed after capture; it does not prove that the source report was correct. Scope, reviewer attribution, and links to the originating system provide that additional context.

Create a metadata record beside the PDF
Store the contextual information in a structured record, not only in a filename. The following fields cover the evidence questions most reviewers ask.

Store the PDF in a controlled repository
A normal shared folder may hold a file, but an audit repository must also preserve its context and control later access. Choose a records-management archive or document repository that can demonstrate the following capabilities.
- Restricted access: grant access only to people with an explicit business need.
- Access logging: record who viewed, downloaded, changed metadata, placed a hold, or attempted deletion; review those logs.
- Retention and legal hold: apply a policy-driven retention period and suspend disposal when a hold exists.
- Backup and migration: maintain recoverable copies and document migrations or format conversions.
- Version control: preserve the original export and identify every later version rather than silently replacing it.
- Date and time accuracy: use a consistent time zone and synchronized clocks for run and review events.
- Quality control: retain the check performed after export, including exceptions and corrective actions.
- Integrity protection: use hashes, write-once controls, digital signatures, or other cryptographic controls where the risk warrants them.
- Search and retrieval: index record ID, system, review period, manager, application, and tags so an independent reviewer can locate the evidence.
- Independent export: allow the complete record and its metadata to be provided to an auditor without changing the original.
Access to audit trails should itself be limited and audited. A person who can alter or erase the trail can undermine the evidence even when the PDF is unchanged.
Connect the PDF to the audit trail
The Canada Revenue Agency defines an audit trail as information needed to recreate a sequence of events related to a business transaction. For an access review, preserve references to the events around the report rather than presenting the PDF as an isolated artifact.
- Link to the access-review campaign or review record.
- Identify the decision maker for each item, as shown in the detailed report.
- Attach or reference approval emails, workflow confirmations, tickets, and digital signatures where policy permits.
- Record changes made after the original decision, including who changed them and why.
- Keep the report-run log and repository access log associated with the same record ID.
These references let a reviewer move from the final decision to the event that created it, the person who approved it, and any subsequent change.
Document performance and review
PCAOB AS 1215 states that audit documentation should show who performed the work, who reviewed it, and the date of that review. Apply that principle to the export and quality-control steps.
- The operator records the filters, runs the report, exports the PDF, and logs the run time.
- A second person, where segregation of duties is required, checks page completeness, legibility, filters, dates, and decision-maker fields.
- The reviewer records their name or unique identity, review date and time, result, and any exceptions.
- Resolve exceptions with a new export or an explanatory record; never overwrite the original without preserving its history.
Keep the review worksheet or checklist with the PDF’s metadata so the evidence package demonstrates both the result and the control performed on it.

Apply the correct retention schedule
Retention is determined by the law, contract, industry rule, litigation hold, and internal schedule governing your organization. Do not treat one published period as a universal default.

For your record, define when retention starts: report run, sign-off, closure of the review, or another policy event. Record the disposal-review date, check for legal holds before destruction, and document whether the record was destroyed or permanently preserved.
Choose an archive using evidence-focused criteria
Compare repositories against the controls your reviewers will actually test, not only storage capacity.

When the application has no PDF export: print the full page
Use the browser only as a fallback when the report system cannot produce its own complete PDF.
- Open the report in a controlled browser session and apply the documented filters.
- Wait for all rows and lazy-loaded sections to appear. Scroll through the report if the interface loads content on demand.
- Use the browser’s print command and choose Save as PDF.
- Select all pages, enable background graphics only when they carry meaning, and choose a readable scale and orientation.
- Disable browser headers and footers only if doing so does not remove report identity or page numbers; otherwise retain them.
- Open the saved PDF and perform the same page, field, search, and integrity checks described above.
Browser printing can omit content hidden behind tabs, virtualized tables, collapsed sections, authentication prompts, or scripts that have not finished running. If any of those conditions apply, use the application’s export or capture a documented exception rather than presenting an incomplete printout as the full report.

Troubleshoot common capture and evidence failures
The PDF contains only the summary
Cause: the export used a summary action or a limited page range. Fix: rerun the report and select the full report or all-pages export. Compare the resulting page count and section sequence with the report’s detail view.
Tables are clipped or unreadable
Cause: portrait orientation, excessive scaling, or a narrow print area. Fix: choose landscape, a suitable paper size, and a readable scale; verify several pages after export. If the product’s own PDF export still clips columns, preserve the original export and document the limitation rather than silently editing the file.
Rows or images are missing
Cause: lazy loading, virtualized tables, a timeout, or content that appears only after interaction. Fix: wait for the report to finish loading, use the application’s export, or use a capture workflow that waits for a selector, delay, or network idle condition. Check the first and last detail pages before archiving.
The report requires authentication
Cause: the capture session has no valid session cookie, header, user agent, or authorization token. Fix: use the application’s authenticated export, or configure an approved capture service with the required headers or cookies. Never place credentials in a public URL or an unprotected filename.

A bot check or blank page appears
Cause: the target system challenged the capture session or failed to render. Fix: capture from an authorized interactive session, investigate the target application’s allow-list requirements, and retain the failure result in the run log. Do not archive a blank page as evidence of a completed review.
The stored file no longer matches the export
Cause: replacement, conversion, or post-export editing. Fix: preserve the original immutable file, calculate a new hash for every derivative, and record the transformation and person responsible.

A reviewer cannot find the evidence later
Cause: missing metadata or inconsistent naming. Fix: index the record ID, system, review period, manager, application, tags, and campaign ID, then test retrieval using only those fields.
Operational performance and cost considerations
- Large reports: allow time for every detail page to render and verify that the export job has completed before closing the session.
- Repeat captures: use a stable naming and record-ID scheme so a rerun is a new version, not an overwrite.
- Network reliability: keep the original system export where possible and log failures, retries, and completion times.
- Storage cost: retain the PDF, metadata, review checklist, and required attachments together; deleting context to save space weakens evidentiary value.
