You can add review structured data to a healthcare website only when it accurately describes reviews visitors can see and the reviewed entity is eligible under Google’s rules. A medical practice generally cannot use its own reviews to qualify for Google review stars: Google excludes pages using LocalBusiness or another Organization type when the reviewed entity controls the reviews. An embedded review widget does not get around that restriction.

First decide whether the page is eligible

Structured data is machine-readable information about the content on a page. For reviews, it can describe an individual review with Review, or a collection of ratings with AggregateRating. Google may use eligible, valid review markup to show a search result with stars and other review information. “May” is important: accurate markup does not guarantee a rich result.

For healthcare sites, the first question is not which plugin to install or which code to paste. It is who is being reviewed, and who controls the reviews. Google’s Review Snippet documentation lists the types it supports for review snippets. A type existing in Schema.org does not by itself make it eligible for stars in Google Search.

Google’s documented examples of supported review-snippet types include Product, Recipe, Event, and SoftwareApplication, among others. This list is useful because it separates two questions that are easy to conflate: whether a vocabulary defines a type, and whether Google supports that type for a particular search feature. Check the current supported-type list for the exact entity you intend to describe rather than assuming that a medically relevant type qualifies.

If the page is about your own practice or organization

If the practice, clinic, or organization controls reviews about itself, its own page is ineligible for Google’s review star feature when marked up as LocalBusiness or another Organization type. Google explicitly includes reviews marked up directly on the site and reviews displayed through an embedded third-party widget. In other words, moving the review display into a widget does not change who controls the reviews.

This is a feature-eligibility rule, not a syntax error. Your page can contain valid structured data and still be ineligible for review stars. Do not change the entity type merely to try to qualify: a healthcare-related type such as Physician or MedicalClinic is not independently eligible for Google review stars unless Google’s current documentation says so.

Google’s wording focuses on whether the entity being reviewed controls reviews about itself. That makes the review source and the relationship between publisher and reviewed entity essential parts of the decision. A practice cannot turn its testimonials into independent reviews simply by adding markup, embedding a feed, or changing how the reviews are formatted. Google’s documentation specifically names embedded Google Business and Facebook review widgets as examples of embeds that do not bypass the policy.

This restriction is about qualifying for the review star feature; it does not mean a practice must remove patient feedback from its website. A practice can present genuine feedback for visitors, subject to its other obligations, but should not expect review structured data on its own controlled LocalBusiness or Organization page to produce Google stars.

If your site reviews other healthcare organizations

A site that captures reviews about other local businesses is a different case from a practice publishing reviews about itself. Google supports LocalBusiness for review snippets only for sites that capture reviews about other local businesses. Its Local Business structured data guidance likewise says review and aggregateRating are recommended only for sites capturing reviews about other local businesses.

That distinction may matter to a healthcare directory or another independent review publisher. It does not mean every directory qualifies automatically: the content still needs to meet Google’s eligibility and review-content rules, and the markup must describe what visitors can actually see.

For a directory, check each page rather than treating the entire site as eligible by default. Identify the business the page reviews, establish that it is not controlling the reviews about itself, and confirm that the publisher actually captures the user ratings being represented. A directory page with a profile description, an editor’s assessment, or a display of scores copied from elsewhere is not the same thing as a page capturing authentic user reviews.

Choose the markup that matches the visible content

What the page displays Structured data concept Healthcare-site consideration
One individual review Review Use it only for a genuine review visible on the page, and only where the reviewed entity and site meet Google’s eligibility rules.
A summary of multiple user ratings AggregateRating Represent the actual aggregate and its applicable count. A practice’s self-controlled reviews do not become eligible for stars by being averaged.

For aggregate ratings, Google’s guidance calls for the rating and the applicable count, expressed with ratingCount or reviewCount. Use the count that accurately describes the displayed rating data; do not substitute a count of unrelated reviews or use a number that visitors cannot verify from the page.

Use Review when the page presents a particular review and the markup describes that review. Use AggregateRating when the page presents a combined rating drawn from multiple user ratings. An aggregate is not a shortcut for adding stars when the page has no visible reviews, and it should not be used to disguise an editorial score as a user rating.

The name of the reviewed item also matters when review information is nested: the page and markup should make clear which practice or organization is being reviewed. Keep the rating value and count consistent with the visible summary. If the page shows a rating and a number of reviews, but the markup describes a different number or a different entity, the structured data is not a faithful description of that page.

Prepare the page and its review data

Before writing markup, document what the page says and where each value comes from. This is especially useful for a healthcare directory, where an organization’s reviews may be spread across multiple pages or systems.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Identify the reviewed entity. Name the specific practice, clinic, or organization the review concerns. Do not confuse the reviewed entity with the website publisher or the author of the review.
  2. Establish control of the reviews. Determine whether the entity being reviewed controls, selects, or supplies the reviews. If it does, the self-serving review restriction applies to its own LocalBusiness or Organization page, including a page showing reviews in an embedded widget.
  3. Check Google’s supported types. Confirm the exact entity type against the current Review Snippet documentation. Schema vocabulary support alone is not evidence that Google supports a type for review stars.
  4. Choose an individual review or an aggregate. Mark up a single visible review with Review, or a real summary with AggregateRating. Include the reviewed item’s name when the review information is nested.
  5. Verify the data against the page. Confirm the review text, rating, aggregate value, and applicable count match the user-visible content and the underlying user submissions.

Keep a simple source-of-truth record for each value used in the markup: which page displays it, which user submissions support it, and how the aggregate and count are maintained. This is a practical way to catch discrepancies when a rating changes or a page is updated. If the content changes, update the structured data at the same time or remove the stale markup.

Represent authentic reviews, not just available ratings

Google’s review guidance requires ratings to come directly from users for the relevant review features. Do not create a rating from an editor’s judgment, combine scores from other websites into an aggregate, or mark up reviews that are not actually shown on the page. A page should not claim a review count or average that it cannot substantiate from its own displayed review content.

  • Keep review text and scores visible. A rating present only in markup, but hidden from visitors, does not meet Google’s content guidance.
  • Do not fabricate or silently alter reviews. The structured data should faithfully represent actual user-submitted ratings and the content published to visitors.
  • Do not mark up undisclosed incentivized reviews. The content must not disguise an incentive in a way that makes the review misleading.
  • Do not import ratings from elsewhere into your own aggregate. External reviews and a widget displaying them do not establish that your page qualifies for a review snippet.
  • Do not use human-edited compilations as user ratings. For the relevant LocalBusiness/Organization review features, Google says ratings must be direct from users and publishers must not rely on human editors to create, curate, or compile local-business ratings.

Visibility means visitors can find the review text and rating or aggregate rating on the same page that the markup describes. A value tucked into JSON-LD but not presented to users is not a substitute for visible content. Likewise, a page should not use markup to imply that a third-party score is a rating collected by the publisher, or that an editor’s summary was submitted by a patient.

For local-business and organization review features, the direct-user requirement also limits how a publisher may produce a rating. Editors can present and organize review content, but they should not create, curate, or compile a rating on behalf of users. Keep the distinction between a user’s submitted rating and an editorial opinion clear both in the page content and in the structured data.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Add markup only after the eligibility check

For an eligible publisher that reviews other local businesses, JSON-LD is one way to express review information on a page. The following is a shape example, not publish-ready data. Replace each code placeholder with the exact value supported by the visible page and actual user submissions. Do not place the example literally on a live page.

{
  "@context": "https://schema.org",
  "@type": "LocalBusiness",
  "name": "{{reviewed_business_name}}",
  "aggregateRating": {
    "@type": "AggregateRating",
    "ratingValue": "{{visible_aggregate_rating}}",
    "reviewCount": "{{visible_review_count}}"
  }
}

This example uses LocalBusiness only to illustrate the documented case of a site capturing reviews about other local businesses. It is not a recommendation that a healthcare provider mark up its own testimonials this way. For a page containing an individual review, use the Review concept and ensure the review, rating, and reviewed item represented in the markup are all present and accurate on that page.

Do not add properties simply because a generator offers them. Each value needs to describe the page truthfully, and the chosen type must have a documented basis for eligibility. If your website is a practice’s own site and its reviews are self-controlled, stop here for the purpose of Google review stars: changing the markup format or adding more properties will not make the page eligible.

The code example is intentionally narrow: it shows the aggregate rating and count for a reviewed local business. It does not supply review values, prove eligibility for a particular page, or cover every property that might be used in a complete implementation. Do not copy the placeholder strings or invent a rating to make the example validate. Replace them only with real, visible data, then check the current Google documentation for the required and recommended properties for the type you are using.

Compare implementation options by eligibility, not star promises

Schema plugins, markup generators, manual JSON-LD, and implementation services can differ in convenience, but none can override Google’s eligibility rules. Compare an implementation on five practical questions: Does the documented Google type fit the reviewed entity? Who controls the reviews? Are ratings direct and authentic user submissions? Is the exact marked-up content visible? Can Google crawl and process the deployed page?

A plugin or generator may help format data, but it cannot decide that a self-serving review is eligible or make an invisible rating acceptable. Manual code gives a publisher control over the output, but also requires care to keep values accurate as page content changes. Whichever approach you use, verify the result with Google’s tools and inspect the live URL rather than relying on a product claim that it can “get stars.” Google identifies the Rich Results Test and URL Inspection as validation steps; it does not endorse a paid third-party tool or promise a particular display from using one.

Validate the markup and inspect the deployed page

  1. Run Google’s Rich Results Test. Test the page or markup to check whether Google detects supported review structured data and to identify errors. Fix critical issues before deploying. Passing this test checks markup eligibility and syntax; it does not override the self-serving review policy or promise that stars will appear.
  2. Deploy to a small set of eligible pages. Start with pages whose visible reviews, reviewed entity, and aggregate values have been verified. Make sure the published page renders the same information the structured data describes.
  3. Use URL Inspection in Search Console. Inspect the deployed URL to see how Google can access and process it. Confirm the page is not blocked by robots.txt, marked noindex, or behind a login requirement.
  4. Allow for recrawling. Google says recrawling and reindexing can take several days. A sitemap can help Google learn about future changes, but it does not force a recrawl or guarantee a rich result.
  5. Review the actual search appearance. Google may display a result differently from a test preview, or may not show stars at all. Treat the live search appearance as an outcome Google controls, not as a promised result of implementation.

Use the test as a technical check, not as a policy verdict. A page may pass validation because its structured data is syntactically valid while still failing the eligibility test for self-controlled reviews. Conversely, an eligible page still needs accurate, visible content and access for Google’s crawler. Keep those questions separate when diagnosing a problem: first confirm eligibility and content, then troubleshoot markup and crawling.

Troubleshoot the common reasons stars do not appear

What you see What to check What to do
The test reports an error Look at the specific field or structured-data issue reported for the page. Correct the markup so it accurately represents the visible review content, then test again.
The test passes but the page is a practice’s own site Check whether the practice controls the reviews and whether the page uses LocalBusiness or another Organization type. Do not expect Google review stars for self-serving reviews. A passing test does not remove the policy restriction.
The test passes but stars are absent Confirm the live page is crawlable and indexed, and that the markup still matches content visitors can see. Use URL Inspection, address access or indexing problems, and allow time for Google to recrawl. Valid markup still does not guarantee stars.
The rating or count looks wrong Compare the marked-up aggregate with the rating and count shown on the page and the underlying submissions. Correct or remove unsupported values. Do not combine third-party ratings or editor-curated scores into the aggregate.
The result differs from the test preview Remember that the preview is not a promise of the final appearance. Confirm the page remains eligible and valid; Google controls whether and how the enhancement appears.

When stars are missing, avoid responding by changing to a different schema type simply because it appears in the vocabulary, adding more ratings, or pulling in outside reviews. Those changes do not fix a policy restriction and may make the structured data less accurate. Revisit the reviewed entity, review source, visibility, count, and crawl access first.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use the right success criterion

For a healthcare practice’s own site, success is accurate, transparent presentation of patient feedback—not obtaining Google stars from reviews the practice controls. For an independent site that captures reviews of other local businesses, success begins with documented eligibility, continues with authentic and visible data, and includes crawlable implementation and validation. Google states that when it finds valid review or rating markup, it may show a rich snippet; the displayed search result can differ from a preview.

Frequently Asked Questions

Can I show patient review stars in Google search for my own medical practice?

Generally, not from reviews the practice controls on its own LocalBusiness or other Organization page. Google’s self-serving review policy makes those pages ineligible for its review star feature, including when reviews appear in an embedded third-party widget. Accurate markup does not change that eligibility rule.

Is physician or MedicalClinic review schema allowed?

A Schema.org type being available does not establish that Google supports it for review stars. Check Google’s current Review Snippet documentation for the exact type. Do not use Physician, MedicalClinic, or another type simply to try to obtain stars unless Google documents it as eligible.

How do I add AggregateRating to a healthcare website?

First confirm that the reviewed entity and page are eligible. Then use AggregateRating only for a genuine aggregate of user ratings that is visible on the page, with the accurate rating value and applicable ratingCount or reviewCount. A practice’s own controlled reviews do not become eligible by being averaged.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Why do my review stars not show even though the Rich Results Test passes?

Passing the test does not guarantee a rich result. The page may be ineligible under the self-serving review policy, may not be crawlable or indexed, or Google may simply choose not to display the enhancement or may display it differently from the preview. Check eligibility and the live page with URL Inspection, then allow time for recrawling.

Can I use reviews from Google, Facebook, or another review website in my markup?

Do not combine ratings from other websites into your own aggregate. An embedded third-party review widget does not bypass the self-serving review restriction for a business’s own page. Mark up only accurate, user-visible content that meets Google’s rules for the reviewed entity and review source.

How long does it take for Google to recognize review markup?

Google says recrawling and reindexing may take several days. A sitemap can help Google learn about changes, but it does not force a recrawl or guarantee that stars will appear.

The Bottom Line

Start with eligibility, not code: identify the reviewed entity, determine who controls the reviews, and verify that Google supports the entity type for review snippets. Mark up only authentic user ratings that are visible on the page, validate with the Rich Results Test, and inspect the deployed URL in Search Console. Even valid, crawlable markup may not produce stars, and a practice’s own controlled reviews cannot qualify its page through a widget or a different schema type.
