Use three control layers around every model call: moderate the request before generation, moderate the response before showing it or allowing an action, and enforce independent request and token budgets at your API boundary. Send ambiguous, high-severity, appealed, or high-impact cases to a reviewer with the complete context needed to decide. Keep policy versions, scores, actions, retries, and reviewer outcomes in an audit log so thresholds can be changed without losing accountability.

Start with a control flow, not a single filter

An AI endpoint is safest when safety decisions happen at several points in the request lifecycle. A practical flow is:

  1. Authenticate the caller and identify the user, API key, project, organization, IP address, or tenant that owns the request.
  2. Apply request-size limits and rate-limit checks before spending model capacity.
  3. Run input moderation on text and supported images.
  4. Classify the request under your policy and choose allow, block, redact, rephrase, authentication, or review.
  5. Generate only when the request passes those checks.
  6. Moderate generated text, images, tool arguments, and tool results before release or execution.
  7. Apply deterministic authorization and tool allowlists before any external side effect.
  8. Deliver the answer, hold it in a review queue, or return a safe refusal.
  9. Record the policy version, model version, scores, action, retry history, and any reviewer decision.

This separation matters because a moderation score is a signal for your application policy, not a universal blocking rule. OpenAI’s moderation guidance uses the same distinction: “Treat moderation scores as signals for your application’s policy, not as an automatic blocking decision.”

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Define policy and risk tiers before choosing thresholds

Write down what your service will allow, restrict, block, and escalate. Include both content categories and the consequences of an incorrect answer. A harmless conversational response and an instruction that changes a customer’s account should not share the same release path.

Classify content and actions

  • Disallowed: content your service will never generate or process. A deterministic block or refusal is appropriate.
  • Restricted: content that may be permitted only with additional safeguards, such as authentication, age or tenant checks, a narrower feature, or a reviewer.
  • High-impact: outputs that can affect a person’s finances, access, safety, employment, health, legal position, or account state. Treat these as an action risk even when the words themselves are not classified as harmful.
  • Uncertain: borderline scores, missing context, policy conflicts, user appeals, and cases in which the model or classifier is not confident enough for an automatic decision.

Map each tier to one deterministic action

Policy result Automatic action What to retain
Allowed Continue to generation or delivery Scores, policy version, and model version
Blocked Refuse, stop tool execution, and return a concise reason Category, score, rule, and response shown to the user
Restricted Redact, request rephrasing, require authentication, or route to a narrower flow Condition that triggered the restriction and the resulting path
Borderline or high impact Hold for trained human review Escalation reason, deadline, and reviewer outcome

Keep the policy separate from code where practical. A policy version such as policy-2026-01 lets you explain why an earlier decision was made after thresholds or category definitions change. Do not silently replace a rule while an old review queue is still open; reviewers need to know which version produced the escalation.

Filter inputs before generation

Use a moderation pass for text and supported images

OpenAI documents the omni-moderation-latest model for text and image inputs. Image files can be up to 20 MB. The model does not classify audio, so an audio upload needs a separate speech or safety pipeline before you treat its contents as trusted. The alias has an omni-moderation-2024-09-26 snapshot documented in 2026; record the resolved model or snapshot in your logs so a later alias change is visible.

The moderation endpoint is free for OpenAI API users and is excluded from monthly usage limits, according to OpenAI’s 2026 Help Center information. Free does not remove operational costs around image transfer, storage, retries, review labor, or the model call that follows a successful check.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Validate the request before calling moderation

  1. Authenticate the caller and attach a stable subject identifier to the request.
  2. Reject payloads over your own byte, character, image-count, and nesting limits. A provider’s 20 MB image maximum is an upper bound, not a reason to accept arbitrarily large uploads.
  3. Normalize encodings and extract only the fields your policy permits. Preserve the original bytes or text under your retention rules for an audit or appeal.
  4. Send text and supported images to the moderation operation. Treat an unsupported modality, such as audio, as a separate safety path rather than as an automatically clean result.
  5. Store the returned flagged value, category fields, scores, model identifier, and policy version.
  6. Evaluate those values against your tier rules. Do not let a caller override the decision by passing a different threshold in the request.

A language-neutral implementation can look like this:

request = authenticate_and_size_check(incoming_request)
moderation = moderate_input(request.text, request.images)
log_input_signal(request.id, moderation, policy_version, moderation_model)

decision = policy.evaluate_input(moderation, request.actor, request.operation)
if decision.action == 'block':
    return safe_refusal(decision.reason)
if decision.action == 'review':
    enqueue_review(request, moderation, decision)
    return review_pending_response()
if decision.action == 'restrict':
    request = apply_restriction(request, decision)

response = generate(request)

The function names above are deliberately conceptual: use the SDK or HTTP client for your provider, but preserve the ordering and the data recorded at each boundary.

Handle borderline scores deliberately

Scores are useful for routing and analysis, not as a claim that a particular numeric value is universally safe. Start with the provider’s flagged result, inspect category and score fields, and tune thresholds against your own traffic, policy, false-positive reports, and reviewer outcomes. Keep separate rules for categories with different harm levels instead of applying one global cutoff.

When an input is blocked, tell the user what they can do next: remove the disallowed material, rephrase the request, authenticate, or use a supported feature. Avoid echoing harmful text in the refusal. For restricted content, preserve the permitted portion only when your policy explicitly defines how redaction works.

Moderate generated output before release or execution

Input screening does not guarantee a safe response. The model can transform benign-looking context into a risky answer, follow malicious instructions embedded in retrieved material, or produce a tool argument with an unintended side effect. Place an output gate after generation and before the answer is shown, stored as a trusted record, or passed to a tool.

Inspect both content and action data

Inline moderation can return input and output results. For each generated response, inspect the output flagged value, categories, and scores; then apply the same policy actions used for incoming content. If the model returns structured data, moderate the human-readable fields and the values that could influence an operation. If it proposes a tool call, do not execute it merely because the conversational text passed moderation.

output = generate(request)
output_signal = moderate_output(output.text, output.images, output.tool_arguments)
log_output_signal(request.id, output_signal, policy_version, generation_model)

release = policy.evaluate_output(output_signal, request.operation)
if release.action == 'block':
    return safe_refusal(release.reason)
if release.action == 'review':
    enqueue_review(request, output, output_signal, release)
    return review_pending_response()
if release.action == 'restrict':
    output = redact_or_narrow(output, release)

return release_to_user(output)

Use different actions for display and execution

A response that is acceptable to display may still be unsafe to execute. Require an explicit authorization check for operations such as sending a message, changing an account, issuing a refund, deleting data, or publishing content. Keep execution behind a second gate that verifies the authenticated actor, resource ownership, allowed operation, and parameter bounds.

Moderation covers tool-call arguments and tool outputs when they appear in conversation content. It does not cover tool names, descriptions, schemas, or response-format schemas. Maintain a separate allowlist of callable tools, validate arguments against a strict schema, and deny by default when a tool or field is not permitted.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Defend against prompt injection and untrusted context

Pattern-based filters alone do not reliably catch indirect prompt injection. An attacker can place instructions in a document, web page, ticket, or image that your retrieval layer later supplies to the model. Treat retrieved material as data, not as an authority that can change your policy or grant tool access.

Reduce the attack surface

  • Limit user input, retrieved text, and model output sizes before they reach the model or a parser.
  • Narrow open-ended fields to a purpose, schema, or set of permitted operations wherever possible.
  • Prefer responses grounded in validated backend material instead of unrestricted browsing or arbitrary execution.
  • Parse untrusted content in a quarantine path with zero tool access, then pass only the extracted, validated fields to the decision flow.
  • Keep secrets, system instructions, authorization tokens, and tool credentials outside model-visible content unless they are explicitly required.
  • Re-check authorization after generation; a model’s statement that a user is entitled to an action is not proof of entitlement.

OWASP recommends rate limiting per user or IP and highlights quarantine-style handling as a defense pattern. Combine those controls with output moderation because injection can produce a plausible, non-obvious response rather than a clearly flagged phrase.

Design rate limits around abuse and fairness

Separate request and token budgets

Count requests and tokens independently. A caller can stay below a request-per-minute limit while sending very large prompts, or send many short requests that consume little token capacity but overwhelm queues. Depending on your workload, add daily request and token budgets and modality-specific budgets such as image minutes or audio minutes.

Enforce each limit at the boundary that represents the risk: user, API key, project, organization, IP address, tenant, or a combination. A public endpoint may need an IP limit before authentication, while an authenticated enterprise endpoint may need a tenant and user limit to prevent one customer from consuming shared capacity.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use a token bucket or leaky bucket

A token bucket allows controlled bursts while enforcing an average rate. Store a token count and last-refill time for each limit key. Refill at the configured rate up to the bucket’s maximum, charge one unit for a request and an estimated or actual token cost for model usage, and reject when the balance is insufficient. A leaky bucket is an alternative when you want a steadier queue rather than bursts.

now = clock()
bucket = load_bucket(limit_key)
bucket.tokens = min(bucket.capacity,
                    bucket.tokens + (now - bucket.updated_at) * bucket.refill_rate)
bucket.updated_at = now
cost = request_unit + estimated_input_tokens + reserved_output_tokens

if bucket.tokens < cost:
    retry_after = time_until_available(bucket, cost)
    return http_429(retry_after)

bucket.tokens -= cost
save_bucket(limit_key, bucket)

Reserve capacity for safety and review paths. If every token is consumed by ordinary traffic, users cannot submit an appeal, reviewers cannot fetch context, and blocked requests may compete with generation for the same queue. Keep limits and reservations observable so operators can distinguish abuse from a legitimate traffic increase.

Retry 429 responses safely

  1. Honor a valid Retry-After value from the server.
  2. If it is absent or invalid, use bounded exponential backoff with jitter.
  3. Cap both the number of retries and total retry time.
  4. Retry only operations that are safe to repeat. Use idempotency controls for requests that could create an external side effect.
  5. Check whether the official SDK already retries. Do not add a second retry loop around it.
  6. Record each retry and the final outcome. Repeated unsuccessful requests still consume rate-limit capacity.

Return a useful 429 response to clients: identify that capacity is temporarily unavailable, include the retry timing when known, and avoid exposing internal bucket keys or other tenants’ limits.

Build a human-review operation, not just a queue

Choose escalation triggers

Route borderline scores, high-severity categories, high-impact outputs, user appeals, policy conflicts, and suspected injection to trained reviewers. OpenAI’s safety guidance states: “Wherever possible, we recommend having a human review outputs before they are used in practice.” For a high-stakes workflow, make review a release prerequisite rather than an after-the-fact audit.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Include enough context to make a decision

A reviewer should not have to reconstruct the event from scattered logs. Present:

  • Original user input and any uploaded or retrieved material that influenced the response.
  • The model output, including proposed tool arguments and tool results.
  • The operation the system was about to perform and the authenticated actor or tenant.
  • Relevant moderation categories and scores, policy version, model version, timestamps, and prior decisions.
  • The precise reason for escalation and the deadline or service-level target.

Restrict reviewer access to the minimum data needed, protect sensitive content, and record who viewed or changed a case. Redact secrets before displaying context, while preserving enough information to verify the decision.

Prioritize and resolve cases

Give each queue item a severity, confidence, policy category, customer or tenant, creation time, and SLA. Sort urgent safety and high-impact cases ahead of low-risk ambiguity. Support four outcomes: approve, reject, edit, and escalate. An edit should preserve both the original model output and the reviewer’s final text; never overwrite evidence.

Send a clear user notification when a result is held or denied. Explain the enforcement reason at an appropriate level, avoid revealing detection thresholds, and provide an appeals path. Appeals should create a new case linked to the original decision so a second reviewer can see what changed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Turn decisions into policy improvements

Aggregate reviewer outcomes by category, score range, model version, policy version, customer segment, and modality. Use the data to tune routing thresholds and identify categories that need a narrower feature or a stronger deterministic rule. Do not use reviewer agreement as an accuracy benchmark unless you have defined a reliable labeling protocol; no independent performance statistic is established by the authoritative material used for this guidance.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Support on Ko-Fi

Log an auditable decision record

At minimum, create one correlated record for each request and append events as it moves through the pipeline. Include:

  • Request identifier, parent request identifier for retries, tenant or project, authenticated actor, and network key used for rate limiting.
  • Input and output modality, byte and token counts, and size-limit results.
  • Moderation model or snapshot, category fields, scores, and flagged status.
  • Generation model, tool name and arguments, tool result, and authorization decision.
  • Policy version, action taken, refusal or redaction reason, and release status.
  • Rate-limit bucket, retry count, Retry-After value if present, and final HTTP outcome.
  • Review case identifier, reviewer, approve/reject/edit/escalate result, timestamp, and appeal linkage.

Separate operational telemetry from content retention. Encrypt sensitive content, define deletion periods, and ensure that logs cannot become an unreviewed second copy of private user data. Use immutable or append-only storage for the decision fields that support an audit.

Compare implementation approaches using the same questions

Whether you build the controls in application code, an API gateway, or a managed safety service, compare the design on the following axes:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Axis Questions to answer
Category and modality coverage Does it handle the text, image, audio, structured data, and tool content your product accepts? What requires a separate pipeline?
Latency, throughput, and cost What work occurs before generation, what can run in parallel, and how are bursts, image processing, review holds, and retries budgeted?
False-positive and false-negative handling Can you route borderline scores, redact, rephrase, appeal, and tune by category without changing historical decisions?
Human escalation Can reviewers see complete context, meet severity-based SLAs, edit safely, and send an appeal through a tracked workflow?
Rate-limit dimensions and retries Can limits be applied per user, key, project, organization, IP, and tenant, with burst controls and correct Retry-After behavior?
Logging and auditability Are model, policy, score, action, tool authorization, retry, and reviewer events correlated and retained under your privacy rules?

OWASP lists AWS Comprehend, Azure Content Safety, and the OpenAI Moderation API as examples of services that can participate in a safety architecture. The important choice is not the product name; it is whether the resulting system covers your modalities, preserves policy-version control, and gives people a workable review and appeal path.

Test the failure paths before launch

Blocked input still reaches the model

Trace every request identifier from the edge to the generation worker. A blocked moderation result must terminate the generation path, not merely add a warning to a log.

Safe text hides a dangerous tool call

Capture structured tool arguments and apply schema validation, allowlists, authorization, and output moderation before execution. Review the operation, not only the prose surrounding it.

Retries amplify an outage

Simulate 429 responses with and without Retry-After. Verify bounded backoff, jitter, idempotency, SDK interaction, and that retries are charged consistently.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Reviewers lack the evidence

Open a held case in the review interface and confirm that the original input, retrieved context, output, tool data, scores, policy version, and reason are present without exposing unnecessary secrets.

A policy change makes old cases unintelligible

Replay a historical decision and verify that it still displays the model and policy versions active at the time. Never rely on a mutable “current policy” label alone.

An unsupported modality slips through

Submit audio or another unsupported format and confirm that it is routed to its dedicated safety path or rejected explicitly. An absent moderation result must not be interpreted as approval.

Roll out in controlled stages

  1. Shadow: run input and output moderation without changing user-visible behavior; measure category distribution, latency, queue volume, and false-positive reports.
  2. Guard: enforce hard size limits, authentication, rate limits, tool authorization, and deterministic blocks.
  3. Review: hold borderline and high-impact cases, train reviewers on the policy, and set severity-based SLAs.
  4. Tune: use approve, reject, edit, escalate, and appeal outcomes to adjust category-specific routing while retaining policy versions.
  5. Audit: periodically sample allowed, blocked, and reviewed events; verify that logs, retention, access controls, and recovery procedures still work.

This staged approach lets you find latency and workflow problems without treating an untested classifier threshold as a final safety decision.
