The Pal Surgery Table is Palworld’s deterministic facility for changing an existing Pal’s gender or modifying its passive skills. You unlock it at Technology level 38 for 3 Technology Points, then build it with Refined Ingots, High Quality Cloth, and Corrosive Solvent. Gender changes consume a Pal Reverser; passive-skill operations use surgery entries, matching implants when required, and a displayed gold fee.
What the Pal Surgery Table does
The Surgery Table is a base facility that applies specific changes to a Pal you already own. Its two established functions are gender conversion and passive-skill surgery. It is not a general stat editor: the available operation is determined by the table’s current menu, the items in your inventory, and the gold fee shown for that operation.

Unlike systems that rely on a random result, surgery presents a selectable operation for the listed entries. That makes it useful when you want to improve a particular Pal instead of repeatedly catching or breeding replacements. The table does not eliminate the need for other systems, however. Breeding remains a separate way to obtain passive skills, and a passive you cannot select in the current surgery menu may still require breeding or another acquisition route.
Unlock requirements and construction recipe
The current Palworld records list the Surgery Table at Technology tier 38. Unlocking the technology costs 3 Technology Points. After unlocking it, gather the following construction materials:

A current version 1.0 database entry also lists 1,000 build work for the structure. That value describes the construction workload; it is not an additional item cost.
Build the table inside a base, then interact with it while the Pal you intend to modify and the required items are available. The available records establish the recipe and facility behavior, but they do not define a universal worker assignment or placement rule. Do not assume a particular Pal, work suitability, or pathing setup is required unless your game version shows that requirement.
Changing a Pal’s gender
Gender conversion is the simpler of the table’s two uses. Choose the gender-change function, select the target Pal, and provide a Pal Reverser. The Reverser is consumed when the operation succeeds, so verify the target and the selected result before confirming.
Where Pal Reversers can come from
The Surgery Table record identifies salvage, treasure chests, and bounty or arena traders as sources for Pal Reversers. Availability can depend on your world progress and current game version, so use the source that is accessible in your save rather than assuming one fixed vendor or drop location.
What to check before confirming
- Make sure the selected Pal is the one you intend to change.
- Check that the displayed result is the desired gender.
- Confirm that the Pal Reverser is present and that you accept its one-use cost.
- Do not treat the operation as reversible for free; another change would require another Reverser.
The documented gender function changes gender only. It should not be used as evidence that the table changes a Pal’s passive slots, individual values, or condensation level.

Changing passive skills with surgery
Passive surgery lets you work on an existing Pal’s passive configuration through the entries exposed by the table. The documented operations include adding, replacing, or fine-tuning passive skills. The exact choices depend on the current menu and whether the relevant passive is available by default or requires an implant.

Some surgery entries are available without an implant. Other passives require a matching implant before the operation can be selected. An implant represents the module for a particular passive skill. Once you have obtained an implant, the implant record states that it can be reused for future surgeries; obtaining it is not the same as consuming it on every subsequent operation.
Gold is charged in addition to any required implant. The fee varies with the passive’s rarity or strength. Because the available material does not provide a complete authoritative price table, use the amount displayed in your current Surgery Table menu instead of applying an older list or assuming every passive costs the same.
What a passive operation changes
When an entry is available, the table lets you select the relevant passive operation and the passive slot to replace or modify, then confirm the change after supplying the listed requirements. The system is deterministic for the operation shown: it is not a roll that can produce a different passive from the one selected.
“Add” in the menu should not be interpreted as proof that surgery increases the Pal’s total number of passive slots. The available records establish passive modification, but they do not establish an increase to slot count. Treat slot expansion as unverified unless your current game build explicitly documents it.

How to use the table: complete workflow
- Reach Technology level 38. Open the Technology screen and spend 3 Technology Points on the Pal Surgery Table.
- Collect the construction materials. You need 30 Refined Ingot, 20 High Quality Cloth, and 10 Corrosive Solvent. The current 1.0 record lists 1,000 build work for construction.
- Build the facility in your base. Place the table where you can interact with it. The available records do not prescribe a specific worker or pathing arrangement.
- Choose your objective. Decide whether you are changing gender or performing passive surgery before selecting an entry.
- For gender conversion, obtain a Pal Reverser. Select the target Pal, review the resulting gender, and confirm only when you accept that the Reverser will be consumed.
- For passive surgery, inspect the menu. Select the operation and passive slot you want to modify. The menu will identify whether the entry is available directly or requires a matching implant.
- Supply the implant when requested. Use the implant associated with the selected passive. An obtained implant can be reused for later surgeries according to the implant record.
- Pay the displayed gold fee. Confirm that you have enough gold and read the fee shown for that specific passive; prices vary by rarity or strength.
- Confirm the operation. Check the Pal and the selected passive one last time, then accept the surgery.
- If the desired passive is absent, stop before spending resources. Use breeding or another acquisition method for a passive that is not listed in the current table menu.
What the current menu contains
A current version 1.0 overview reports 54 surgery entries. Of those, 19 are described as gold-only options and 35 require a matching implant. This count is version-sensitive: updates can add, remove, rename, or reclassify entries.

The overview does not reproduce a complete authoritative list of all 54 entries or a verified fee for every passive. For a purchase decision, the in-game menu is the controlling reference: it shows whether your chosen operation is unlocked, which implant it needs, and how much gold it costs in your build.
Implants, reuse, and resource planning
Think of an implant as an access module for one passive rather than as a consumable payment on every surgery. The implant record says that, after an implant is obtained, it can be reused for future surgeries. The operation can still require gold each time, and the amount depends on the passive’s rarity or strength.
This distinction matters when planning several Pals. One implant may support repeated surgeries for the same passive, while your gold reserve becomes the recurring cost. A Pal Reverser follows a different rule: it is consumed by a gender change. Keep those two item behaviors separate when deciding whether you can afford an operation.

The table is most valuable when you already have a Pal worth keeping and want a controlled change. Breeding and capture remain alternatives, but they solve different problems.

Surgery therefore trades an upfront facility and item investment for control. Breeding and capture can be appropriate when the table does not list the passive you want, when you do not yet have a suitable Pal, or when you prefer to build a new individual rather than alter an existing one.

Do not assume the Surgery Table performs functions that are not documented for the current menu. The available records do not establish that surgery:

- guarantees access to every passive in the game; or
- uses one universal gold price for every passive.
Those points may change with a future update, but they should be treated as unverified until the game’s current records or menu explicitly show them. The same caution applies to the reported total of 54 entries and its 19-versus-35 split: it describes a version 1.0 overview, not a permanent promise for every later build.
Common mistakes and how to avoid them
Spending Technology Points too early
The table is a tier-38 unlock. If you are below that tier, gathering the recipe materials will not let you build it. Save the 3 Technology Points and unlock the technology first.
Confusing an implant with a Reverser
Implants and Pal Reversers have different roles. An implant supports a passive entry and can be reused after acquisition; a Pal Reverser is consumed for gender conversion. Check the operation label before confirming.

Using an old price list
Passive fees vary by rarity or strength, and a complete authoritative list is not established here. Read the gold amount in your current menu rather than copying a pre-1.0 figure.

The table mixes default entries with implant-gated entries. If the passive is missing or locked, do not spend gold or a Reverser trying to force it. Check whether the menu calls for a matching implant; if the entry is not present at all, consider breeding or another acquisition route.
Confirming the wrong Pal
Both gender and passive operations act on the selected Pal. Pause at the confirmation screen, verify the name and intended change, and remember that a successful gender operation consumes its Reverser.
Practical decision checklist
- Is your character at Technology level 38, with 3 Technology Points available for the unlock?
- Do you have 30 Refined Ingot, 20 High Quality Cloth, and 10 Corrosive Solvent for construction?
- Are you changing gender, or are you modifying a passive?
- For gender, do you have a Pal Reverser and accept that it will be consumed?
- For a passive, is the desired entry visible in the current menu?
- If it is implant-gated, do you have the matching reusable implant?
- Have you checked the exact gold fee displayed for that passive?
- If the entry is unavailable, would breeding or obtaining another Pal be more appropriate?
Once these checks pass, the Surgery Table gives you a controlled way to modify the Pal you already own instead of replacing it through a random acquisition process. Its value is precision: choose a listed operation, provide the stated requirement, and confirm only after the game shows the exact change and cost.
