Most medicines in the bloodstream do not reach the brain at a useful concentration because the blood–brain barrier (BBB) is a selective, living interface rather than an open filter. Specialized endothelial cells seal the spaces between them, transport systems admit only certain molecules, and efflux pumps can return drugs to the blood after they enter.

That protection preserves a tightly controlled neural environment, but it turns brain treatment into a delivery problem. A successful therapy must find a viable route across the barrier, avoid being expelled, spread through the relevant tissue and reach its molecular target without unacceptable injury.

What the blood–brain barrier is

The BBB is formed mainly by the endothelial cells lining brain capillaries, together with pericytes, astrocyte end-feet, basement-membrane structures and signaling systems collectively called the neurovascular unit. This arrangement regulates exchange between circulating blood and the brain’s extracellular fluid. It controls nutrients, ions, water, signaling molecules and waste while limiting sudden changes in the neural environment.

Rucha Pandit, Liyu Chen and Jürgen Götz describe it in their 2020 review as “a dynamic structure that functions as a gatekeeper, reflecting the unique requirements of the brain.” “Dynamic” matters: barrier properties can change with development, inflammation, injury, tumors, infection and other disease states. The BBB is restrictive, but it is not an impermeable sheet that behaves identically in every person or brain region.

Why most drugs cannot simply diffuse into brain tissue

Tight junctions close the route between cells

In many tissues, a substance can move through gaps between adjacent endothelial cells. At the BBB, specialized tight-junction proteins link neighboring cells and greatly reduce this paracellular pathway. A circulating drug therefore cannot rely on slipping through intercellular spaces; it generally has to cross an endothelial cell or use a specific transport mechanism.

This cellular seal is one major reason that compounds present in blood may show little exposure in brain tissue. It also explains why increasing the dose is not a straightforward fix: a higher blood concentration can increase toxicity elsewhere without producing a proportional rise in the concentration at the brain target.

The barrier is selective, not absolutely sealed

Brain endothelial cells contain controlled routes for essential nutrients and other physiological cargo. Carrier-mediated transport uses membrane proteins that recognize particular substrates. Receptor-mediated transport can move a larger cargo after it binds to a receptor and is taken across the cell in a vesicle. These routes allow the brain to obtain what it needs while restricting unrelated molecules.

A drug can sometimes be designed, chemically linked or formulated to take advantage of one of these routes. That does not make transport automatic. Capacity may be limited, different molecules may compete for the same transporter, and changing a transporter’s normal activity can have biological consequences.

Passive diffusion depends on more than size or fat solubility

Some molecules cross the endothelial cell membrane by passive transcellular diffusion. Molecular size, charge, polarity, lipophilicity, hydrogen bonding and stability all influence this process. However, no single property predicts clinical brain exposure by itself. A molecule that appears well suited to membrane diffusion may be rapidly pumped out, bind to proteins in blood, dissolve poorly, or fail to reach a high enough concentration at the intended target.

Conversely, a larger or more polar therapeutic may still reach the brain if it uses a carrier, a receptor-mediated route, a formulation or a delivery procedure. The relevant question is not simply whether a molecule can cross a model membrane, but whether it can reach and maintain a therapeutically useful concentration in the correct brain compartment.

Efflux pumps can remove a drug after it gets in

Entry is only one part of the problem. BBB endothelial cells express ATP-binding cassette (ABC) efflux transporters. These proteins use cellular energy to move many drugs and other compounds from the endothelial cell back toward the blood. A compound may therefore cross in small amounts yet still show low net brain exposure because export outpaces entry.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Efflux also complicates drug combinations. One medicine can inhibit or compete with a transporter and alter the brain exposure of another medicine, while disease-related changes in transporter expression can change exposure over time. Drug developers must measure both influx and efflux rather than treating the BBB as a one-way wall.

What determines whether a brain drug reaches its target

A useful concentration depends on several linked steps:

  • Blood exposure: the drug must circulate at a concentration and for a duration that provide an opportunity for delivery.
  • Barrier crossing: it needs a viable passive, carrier-mediated, receptor-mediated or procedural route.
  • Efflux avoidance: transporters must not remove most of the drug before it accumulates.
  • Distribution: the drug must move beyond the vessel into the relevant extracellular space, cells or organelles.
  • Target engagement: enough active drug must remain at the disease target for long enough to produce benefit.
  • Safety: exposure in healthy brain tissue and other organs must stay within an acceptable range.

These steps explain why a promising laboratory molecule can fail in clinical development. A blood test showing adequate systemic exposure does not prove that the molecule reaches the human brain, and a measured brain concentration does not prove that it reaches the right cells or is pharmacologically active there.

How researchers try to improve brain delivery

Broad approaches fall into four classes. They solve different parts of the problem and cannot be ranked as universally best.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Approach Primary aim Important advantages Key trade-offs
Molecular or formulation design Change the drug or its formulation so it can cross more effectively or use an endogenous route Can remain systemic; may be incorporated early in development; avoids an invasive procedure Changes can reduce potency, stability or selectivity; distribution and transporter dependence still require testing
Carrier- or receptor-mediated transport Attach or formulate cargo to use a native endothelial transport pathway Uses a biological route that the brain already recognizes; can support cargo that would otherwise cross poorly Transport capacity can saturate; competing substrates and receptor biology may alter exposure; cargo compatibility is limited
Focused ultrasound with circulating microbubbles Temporarily increase permeability in a selected region Spatial targeting and a potentially repeatable, non-surgical procedure under imaging guidance Requires specialized imaging and operators; tissue effects, repeatability and off-target exposure must be controlled
Direct CNS or alternative-route delivery Bypass some systemic BBB constraints by placing drug in or near the central nervous system Can produce high local exposure when the target is close to the administration site More invasive or procedure-dependent; coverage may be limited and distribution can be uneven; local toxicity is a concern

Designing the molecule or formulation

Medicinal chemists can adjust properties that affect passive diffusion, stability, solubility and transporter recognition. Formulations may protect a payload, alter its release or present it to a receptor or carrier. The design goal is not maximum lipophilicity or minimum size in isolation; it is a balance that produces adequate brain exposure while preserving potency, selectivity, pharmacokinetics and tolerability.

Using endogenous carriers and receptors

Carrier-mediated transport is suited to molecules that resemble a physiological substrate. Receptor-mediated transport can move a cargo attached to a ligand or antibody-like component that binds an endothelial receptor. Both approaches require careful control of binding affinity and cargo ratio. Very tight binding can trap a construct at the vessel wall, while excessive receptor engagement can disturb the receptor’s normal function. Saturation means that increasing the dose may eventually produce little additional transport.

Bypassing the barrier

Direct CNS administration, such as delivery into cerebrospinal-fluid spaces or infusion into a selected brain region, avoids some endothelial restrictions. It can be valuable when a target is local and a procedure is justified. The same localization is a limitation: drug movement through brain tissue is not uniform, and a dose placed in one compartment may not reach distant or infiltrative disease. Procedure-related infection, bleeding, inflammation and device complications must be weighed against the expected benefit.

Can focused ultrasound help drugs cross the BBB?

Focused ultrasound paired with circulating microbubbles is being studied as a localized, temporary permeability-enhancement method. The ultrasound focus is directed at a selected brain region while microbubbles in the bloodstream respond to the acoustic field. Their mechanical interaction with nearby vessel walls can produce a controlled, short-lived increase in cerebrovascular permeability, allowing a co-administered drug or biological cargo greater access than it would have under baseline conditions.

The attraction is spatial control: treatment can be aimed at a defined region rather than opening the barrier throughout the brain. Reviews describe the method as entering human testing primarily to assess safety. That evidence supports calling it an investigational clinical approach, not a routine universal solution or a generally available treatment for every disease and drug.

What must be controlled

  • Acoustic exposure and microbubble dose: excessive mechanical effects can damage vessels or tissue, while insufficient exposure may not produce useful permeability.
  • Targeting: the focus must cover the intended anatomy without exposing critical off-target structures.
  • Timing: the permeability increase is temporary, so drug concentration in blood and the timing of administration matter.
  • Distribution: opening a vascular interface in one region does not guarantee uniform delivery throughout a tumor, network or degenerating circuit.
  • Monitoring: imaging and clinical observation are needed to detect hemorrhage, edema or other tissue effects.

A 2016 review highlighted translational questions about repeatability, effects on target and off-target tissue, dependence on MRI systems and the need for specially trained operators. These are practical development considerations; the review does not establish a current approval, availability or efficacy claim for a particular disease or drug.

What the often-quoted “98%” figure really means

A 2013 review, Noninvasive and Targeted Drug Delivery to the Brain Using Focused Ultrasound, repeated an estimate that 98% of current pharmaceutical agents cannot cross the BBB. That figure is a broad, historical review-level estimate attributed to earlier literature, not a newly measured universal percentage. Other reviews use related estimates for small molecules and discuss large-molecule therapeutics separately, but definitions and denominators differ.

There is no single current percentage that applies to every drug, disease, dose or meaning of “crosses.” A molecule might enter in trace amounts yet fail to reach a therapeutic concentration, or cross only in a disease-altered region. The useful conclusion is qualitative: the BBB excludes or limits a large fraction of drug candidates, and successful delivery must be demonstrated for the specific molecule and clinical context.

How a development team evaluates a brain-delivery strategy

  1. Define the target and required exposure. Identify the relevant cell type, brain region, molecular target, therapeutic concentration and duration.
  2. Characterize the molecule. Measure properties affecting passive diffusion, solubility, stability, protein binding, metabolism and transporter interactions.
  3. Test transport and efflux. Use complementary barrier models and transporter studies to distinguish entry from net brain exposure.
  4. Measure distribution in relevant disease models. Determine whether drug reaches the intended compartment and whether disease changes barrier function.
  5. Compare delivery options. Weigh systemic design, endogenous transport, focused ultrasound and direct administration against invasiveness, coverage, repeatability and safety.
  6. Confirm pharmacology, not just presence. Link brain concentration to target engagement, biological response and toxicity.
  7. Translate cautiously. Human barrier properties, anatomy, imaging requirements and clinical risks may differ from animal or laboratory models.

Common misconceptions

“If it is in the blood, it is available to the brain.”

Blood concentration and brain concentration are different measurements. Tight junctions, selective transport and efflux can keep brain exposure low even when systemic exposure is high.

“Making a drug more fat-soluble solves the problem.”

Greater lipophilicity may aid membrane diffusion but can also increase nonspecific binding, metabolism, toxicity or efflux. It is one design variable, not a universal solution.

“Opening the BBB everywhere would be better.”

Broad disruption could expose neural tissue to unwanted compounds, alter homeostasis and increase injury risk. Localized or route-specific approaches aim to limit that trade-off, though they introduce their own technical constraints.

“Focused ultrasound is already a standard delivery method.”

Focused ultrasound with microbubbles remains a research and clinical-testing approach in the cited review literature. Its suitability depends on the disease, target location, drug, imaging resources and safety data; it is not a blanket recommendation.

Bottom line for patients and drug developers

The BBB limits brain drug delivery through a combination of sealed intercellular junctions, selective influx pathways and active efflux. Crossing is therefore a systems problem: the drug must enter, avoid removal, distribute to the right cells and remain active at a safe concentration. Current strategies either redesign the cargo, recruit transport biology, temporarily alter permeability or bypass the barrier. Each trades simplicity against selectivity, coverage, invasiveness and risk, so the appropriate method must be established for the specific therapy rather than inferred from a single molecule property or a headline statistic.
