Targeted therapy focuses on particular cancer-related targets or processes; chemotherapy acts on fast-growing cells, including some healthy cells; and immunotherapy helps the immune system fight cancer. They are different approaches, but not mutually exclusive ones: some treatments fit more than one category, and doctors may combine treatment types. There is no universal winner. The right option depends on the cancer, its stage, relevant test results, expected benefits, side effects, and the patient’s circumstances.

How the three treatments work

The main difference is what each approach acts on. Chemotherapy acts on cell growth and division, targeted therapy acts on particular cancer-related targets or processes, and immunotherapy works through the immune system. These are broad descriptions, not a guarantee of how a specific drug will behave or which treatment will be suitable.

Approach What it acts on How selection may work Important qualification
Chemotherapy Drugs stop or slow the growth of cancer cells, which often grow and divide quickly. Some healthy cells that also grow quickly can be affected. Use depends on the cancer type and how extensive it is; chemotherapy may be used alone or alongside other treatments. It may be used to cure or control cancer, or to ease symptoms. Its role varies by situation.
Targeted therapy Proteins or other processes that help cancer cells grow, divide, or spread. Some treatments block growth signals or blood-vessel formation; others deliver a cell-killing substance to cells with a particular target. A tumor may need testing to determine whether it has a target for which a treatment is available. A possible target does not guarantee response, and cancer cells can become resistant.
Immunotherapy The immune system: some treatments release immune “brakes” or increase immune activity; some antibodies mark cancer cells or bring immune cells close to them. Biomarker testing can identify features that may support use of certain immunotherapies, but a match is not a promise of benefit. Immune activity can affect healthy tissues as well as cancer.

The National Cancer Institute describes targeted therapy as treatment that targets proteins controlling how cancer cells grow, divide, and spread. In practice, targeted therapy is not one single mechanism: treatments may interfere with different proteins or processes, and an individual drug’s effects and risks depend on that drug. Chemotherapy: broad action on rapidly growing cells

Chemotherapy is often described as a broad-acting treatment because it can affect rapidly growing cells without being limited to one cancer-specific target. That helps explain both its usefulness and a familiar source of side effects: healthy fast-growing cells in the mouth and intestines, as well as cells involved in hair growth, can also be harmed. The particular effects depend on the treatment; the broad category alone cannot predict exactly what a patient will experience. NCI: Chemotherapy and You

“Broad-acting” does not mean chemotherapy is used in the same way for every cancer or that it is only a last resort. Depending on the cancer type and extent, it can be used with the aim of curing or controlling cancer, or easing symptoms. It may be given on its own or as part of a plan that includes surgery, radiation, targeted therapy, or immunotherapy. The purpose of chemotherapy in a particular plan is therefore a useful question to ask, rather than something to infer from the treatment name.

Targeted therapy: action tied to a cancer-related target

Targeted treatments are designed to interfere with features that help cancer cells grow, divide, or spread. Some block signals that tell cells to grow; some interfere with tumor blood-vessel formation; and some carry a cell-killing substance to cells bearing a target. Because the intended target matters, testing the tumor is often part of deciding whether a targeted treatment could be considered. A target’s presence is not the same as proof that treatment will work.

The word “targeted” describes the treatment’s intended focus, not a promise that it affects only cancer cells or has few side effects. NCI lists diarrhea and liver problems among common effects of targeted therapies; fatigue, high blood pressure, skin changes, mouth sores, and other effects may also occur depending on the drug. Ask about the specific medicine under consideration, since a general description of the class cannot substitute for its individual risks.

Immunotherapy: engaging the immune system

Immunotherapy helps the body’s immune system fight cancer. Different immunotherapies do this in different ways: some release restraints on immune activity, while some antibodies mark cancer cells or bring immune cells closer to them. Since the immune response can reach healthy tissue too, side effects can involve inflammation in organs or tissues. NCI: Immunotherapy Side Effects

Immunotherapy is not a single treatment mechanism, and its side effects are not limited to one predictable pattern. The type and dose of treatment, as well as a person’s health and cancer, affect what may happen. Effects can arise during treatment or after it, so ask the oncology team which symptoms matter for the proposed therapy and what to do if they occur.

How targeted therapy differs from immunotherapy

The terms describe different dimensions of treatment, so the distinction is useful but not absolute. Targeted therapy refers to acting on a particular cancer-related target or process. Immunotherapy refers to helping the immune system fight cancer. Some monoclonal antibodies are targeted therapies, and some are also immunotherapy because they help turn the immune system against cancer. Thus, a treatment may fit both descriptions rather than belonging to only one box. NCI: Monoclonal Antibodies

That overlap also means a label alone is not enough to predict side effects or decide how a treatment works in a particular case. The specific medicine, its intended action, and the person receiving it matter. Ask the oncology team to explain what a proposed drug is intended to do and what risks or symptoms are relevant for that treatment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The three labels also should not be treated as a ranking from “least specific” to “most advanced.” They describe different ways of intervening: chemotherapy acts on fast-growing cells; targeted therapy focuses on cancer-related targets or processes; immunotherapy engages immune defenses. A particular treatment plan may include one approach or more than one, and the existence of a narrower-sounding mechanism does not establish that it is the better choice for an individual cancer.

What biomarker testing can—and cannot—tell you

Biomarker testing looks for features of a cancer that may help guide treatment. In this comparison, it matters particularly because a tumor may need to be tested to see whether a target for a targeted therapy is present; biomarkers can also identify features that may support certain immunotherapies. Whether testing is appropriate, and which test is relevant, depends on the cancer and the treatment being considered. NCI: Biomarker Testing for Cancer Treatment

A biomarker result is most useful when considered alongside the diagnosis and treatment question it is meant to inform. Ask what feature the test looks for, whether a result could make a particular treatment an option, and whether there is an available treatment for that finding. A test is not a general forecast of how every treatment will work, and a result does not select a therapy independently of the rest of the clinical picture.

A possible match is a starting point, not a promise

A result that identifies a possible treatment match does not establish that the treatment will work. Not every cancer cell necessarily has the same biomarkers; other features can affect response; and there may be no available treatment that matches the result. A test therefore contributes information to a treatment decision rather than making the decision by itself.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

This qualification applies even when a test identifies a feature associated with a treatment option. The result may support a discussion, but it cannot by itself account for all the features of the cancer, the patient’s circumstances, or the expected benefits and risks in that situation. If the result does not lead to a treatment option, ask what other approaches are available to consider.

Questions to ask about a test result

  • What feature did the test find, and what treatment decision could it inform?
  • Does the finding make a treatment an option, or does it establish that the treatment is likely to benefit me?
  • Are there other cancer or patient factors that could change how the result is interpreted?
  • If there is no match—or the matched treatment is not appropriate—what alternatives should we discuss?

Side effects: compare the causes, not a safety ranking

There is no sound blanket rule that targeted therapy is safer than chemotherapy, or that immunotherapy has fewer side effects. Each category includes different treatments, and the relevant risks vary with the drug, the cancer, and the patient. Mechanism can help explain the kinds of effects clinicians watch for, but it cannot provide an individual prediction.

Approach Why side effects can occur Examples or considerations established by NCI
Chemotherapy It can harm healthy fast-growing cells as well as cancer cells. Healthy cells in the mouth and intestines, and cells involved in hair growth, are examples of affected fast-growing cells. Effects depend on the chemotherapy used.
Targeted therapy Effects depend on the drug and the target or process it affects; targeted does not mean harmless to the patient. Common effects listed by NCI include diarrhea and liver problems. Fatigue, high blood pressure, skin changes, mouth sores, and other effects may occur depending on the drug.
Immunotherapy Immune activity can affect healthy organs and tissues, causing inflammation-related effects. Effects vary with the type and dose of immunotherapy and the person’s health and cancer. They may occur during treatment or after it.

A practical comparison is therefore not “Which class has the fewest side effects?” but “Which effects are relevant to this specific treatment, and how will we respond if they occur?” Ask the care team which effects are expected, which symptoms need prompt attention, and whom to contact. For immunotherapy in particular, the team should explain that relevant effects can appear during treatment or later. Do not assume that a symptom is unimportant simply because treatment has ended; use the instructions your clinicians give you. NCI: Immunotherapy Side Effects

Monitoring and communication are part of the treatment discussion, not an afterthought. Before starting a proposed treatment, ask how the care team wants you to report symptoms and whether there are effects that need a prompt call. The answer should be specific to the medicine and your situation; a general comparison cannot provide a personalized symptom checklist or tell you to change or stop a treatment.

How doctors decide which approach to use

Treatment choice starts with the actual cancer and the person being treated, not a general ranking of drug categories. Cancer type and how advanced it is matter; test results may inform whether a targeted treatment or certain immunotherapies are options; and expected benefit, side effects, and the patient’s circumstances also belong in the discussion. Some approaches are combined with one another or with surgery or radiation. NCI: Cancer Treatment

  1. Establish the treatment context. Ask how the cancer type and stage shape the goals and available treatment options.
  2. Review relevant testing. Find out whether biomarker testing could inform the decision, what a result means, and whether a possible match has an available treatment.
  3. Discuss likely benefits and uncertainties. Ask what benefit is expected for this situation and what factors could affect the chance of response. A biomarker match alone does not answer that question.
  4. Compare risks and monitoring. Ask about likely side effects for the specific proposed treatment, symptoms that need prompt attention, and how the care team will guide or monitor treatment.
  5. Consider the full set of options. Ask whether treatments may be combined, what alternatives exist, and whether a clinical trial is relevant to discuss.

These questions help distinguish eligibility from suitability. A biomarker may indicate that a treatment is worth considering, but the decision still involves the cancer’s type and extent, the treatment’s expected benefit in that circumstance, possible side effects, and the patient’s context. Similarly, a treatment being available does not mean it is automatically the preferred option.

This is a clinician-led decision. Patients can ask their oncology team to explain why a proposed treatment fits their particular cancer, what the test results do and do not establish, and how the team will handle side effects. These questions help make the reasoning clear without assuming that any one approach is best for everyone.

Resistance, combinations, and why categories are not interchangeable

Cancer cells can become resistant to targeted therapy, so the presence of a target does not ensure that a treatment will remain effective. Resistance is one reason a biomarker result cannot be treated as a complete forecast of response. The evidence summarized here does not establish a universal order in which these therapies should be tried or a general success rate for one category versus another.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Doctors may use chemotherapy alone or combine it with surgery, radiation, targeted therapy, or immunotherapy, depending on cancer type and extent. Combination treatment is not automatically preferable: whether it makes sense depends on the individual treatment plan. Similarly, targeted therapy and immunotherapy are not interchangeable simply because they may both be considered after testing; they work through different mechanisms, with some overlap in particular treatments.

Combination use does not erase the differences between the treatments. A plan that includes two approaches still has a specific rationale based on the cancer and its extent, and each medicine has its own intended action and risks. If a combination is proposed, ask what each part is meant to contribute, what the expected benefits are, and how side effects and monitoring will be handled across the plan.

Common comparison mistakes to avoid

  • Calling targeted therapy “mild” by definition. It can cause side effects, including diarrhea or liver problems, with other effects depending on the drug.
  • Assuming immunotherapy affects only cancer. Immune activity can affect healthy tissues and cause inflammation-related side effects.
  • Treating a biomarker match as a guarantee. Tumor cells may not all share the same biomarkers, other features may affect response, and a matching treatment may not be available.
  • Choosing by category name alone. The specific cancer, stage, medicine, expected benefit, and patient circumstances shape the decision.
  • Assuming the categories are mutually exclusive. Some monoclonal antibodies are both targeted therapies and immunotherapies, and treatment types can be combined.
  • Assuming one approach always comes first. These broad treatment categories do not establish a universal sequence; the order and combination depend on the individual plan.

What to take to an oncology appointment

A concise set of questions can help focus a treatment discussion:

  • What is the goal of the proposed treatment in my case?
  • Is a biomarker test relevant, and what would a positive, negative, or inconclusive result mean for my options?
  • What benefits are expected for my cancer type and stage, and what uncertainty remains?
  • Which side effects are most relevant to this specific treatment, and what symptoms should prompt me to call?
  • Could another treatment be combined with it, or are alternatives or clinical trials appropriate to discuss?
  • If a test identifies a possible target, is there an available treatment that matches it, and what might make that treatment unsuitable?
  • How should I contact the team about symptoms during treatment or after it ends?

The National Cancer Institute advises patients to discuss treatment choices and side effects with their clinicians. These questions are a way to make that conversation specific to the proposed treatment rather than trying to select a therapy from its category name alone.

Does targeted therapy require biomarker testing?

A tumor may need testing to determine whether it has a target for which a targeted treatment is available. Whether testing is appropriate and which test matters depend on the cancer and treatment being considered. A possible match does not guarantee that treatment will work.

Can immunotherapy and targeted therapy be the same treatment?

Some monoclonal antibodies are targeted therapies, and some are also immunotherapy because they help turn the immune system against cancer. The categories overlap in some cases, but they describe different aspects of a treatment.

Is immunotherapy safer than chemotherapy?

Neither category is universally safer. Chemotherapy can harm healthy fast-growing cells; immunotherapy can cause inflammation when immune activity affects healthy organs or tissues. Risks vary with the specific treatment, cancer, and patient, so ask the oncology team about the proposed medicine.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can chemotherapy, targeted therapy, and immunotherapy be combined?

Treatment approaches may be combined, and chemotherapy may also be used with surgery or radiation. Whether a combination makes sense depends on cancer type and extent and the individual treatment plan; it is not automatically preferable.

The Bottom Line

Chemotherapy, targeted therapy, and immunotherapy work in different ways, but their labels do not determine which is best or safest. Cancer type and stage, relevant biomarkers, expected benefit, side effects, and the patient’s circumstances guide the choice. Discuss the specific treatment, its risks, and alternatives with the oncology team.
