Scientists investigate bacteriophages, or phages, as precision tools against antibiotic-resistant bacteria. Therapeutic phages are lytic viruses: they recognize susceptible bacterial cells, infect them and destroy them without infecting human cells. Because each phage usually has a narrow host range, researchers must match it to the strain causing an infection, sometimes combine several phages in a cocktail and sometimes use the result with antibiotics. This is a promising but still limited medical approach, not a generally available replacement for antibiotics.

Why bacteriophages matter in antibiotic resistance

Antibiotic resistance (AMR) makes bacterial infections harder to treat when the drugs that normally kill or suppress them no longer work. The scale of the problem explains the interest in alternatives. A 2025 fact sheet from the World Health Organization Regional Office for Europe reports 133,000 deaths directly attributable to AMR each year in the WHO European Region and 541,000 deaths indirectly linked to it. It estimates annual costs of about €11.7 billion to the European Union and European Economic Area through health spending and lost workforce productivity. The estimation year is not specified in that fact sheet, so these figures should not be read as measurements made in 2025.

In the United States, estimates cited by the U.S. Food and Drug Administration from the Centers for Disease Control and Prevention describe at least 2.8 million antibiotic-resistant infections and more than 35,000 related deaths annually. The FDA page does not present those figures as newly measured in 2026.

Phages offer a different biological strategy from a conventional antibiotic. A lytic phage attaches to a compatible bacterial host, infects that cell and causes its destruction. Human cells are not the target. The same specificity that makes phages attractive also creates the central practical problem: a clinician needs a phage that recognizes the particular bacterial strain in the infection.

How scientists match a phage to an infection

Phage work is usually a matching and monitoring process rather than a standard prescription selected from a universal list. The broad workflow is:

  1. Identify the bacterium. A clinical laboratory determines which bacterial species, and where possible which strain, is associated with the infection. This matters because phages often target a single species or strain.
  2. Test available phages. Researchers expose the patient’s isolate to candidate phages to determine whether the bacteria are susceptible. A suitable candidate may not be found.
  3. Choose one phage or a combination. A single matched phage may be considered when the isolate is susceptible. A cocktail can be considered when more than one phage is needed, including situations involving strain variation or a complex infection.
  4. Consider combination treatment. Investigators may pair phages with an antibiotic. The aim is not to assume that either component always works, but to test whether the combination improves activity against the particular infection.
  5. Monitor the response and resistance. Bacterial susceptibility can change during treatment. Researchers therefore track the infection, the patient’s response and any evidence that the bacteria have become phage-resistant.

This workflow explains why phage therapy is described as personalized or precision treatment in some case reports. It also explains why it cannot be treated as an ordinary consumer medicine that works for every resistant infection.

Phage therapy compared with antibiotics

The differences are practical as well as biological. The comparison below uses the distinctions described by WHO/Europe, the FDA and a peer-reviewed review in the Annual Review of Medicine.

Question Phage therapy Antibiotics
Target breadth Highly specific to susceptible bacterial hosts; a phage may target one species or strain. Can act more broadly, depending on the drug.
Selection and matching Requires a susceptible bacterial isolate and a suitable phage; a match may not be available. Selected according to the organism, drug susceptibility and clinical context, without requiring a virus matched to the exact strain.
How combinations are used May be delivered as a multi-phage cocktail or investigated alongside an antibiotic. May be used alone or in combinations selected by clinicians.
Resistance Bacteria can evolve phage resistance, so susceptibility may need reassessment. Bacteria can evolve antibiotic resistance, a major driver of AMR.
Evidence and access Broad routine human use is limited by clinical evidence, monitoring and regulatory questions; WHO/Europe says phages are not approved medicines in most countries. Many antibiotics are established medicines with country-specific approvals and prescribing systems.

Specificity can be an advantage when a resistant pathogen must be targeted while minimizing broad antibacterial activity. It is not proof that phages have no effects beyond the intended pathogen. An FDA Science Forum abstract in 2023 noted that potential effects on the host microbial community warrant assessment.

Can phages be used against all bacterial infections?

No. WHO/Europe’s FAQ asks this question directly because a phage must have the biological properties needed for the infection at hand.

When no suitable match is found

A laboratory may not identify a phage that infects the patient’s isolate, or the available phage may not have properties suitable for therapeutic use. Phage collections are therefore not interchangeable with a universal antibiotic formulary.

When more than one bacterium is involved

Mixed or complex infections can contain several bacterial targets. One phage may not cover them all, so researchers may investigate combinations. The need for a cocktail is a response to the organisms present, not a guarantee that a cocktail will succeed.

When the bacteria change

A strain that is initially susceptible can become phage-resistant. The 2022 review by Graham F. Hatfull, Rebekah M. Dedrick and Robert T. Schooley identifies phage resistance as a major challenge, along with variation among clinical isolates and possible limits imposed by host immune responses.

How scientists manage resistance and safety concerns

Phage resistance is an active research problem

Using a phage creates evolutionary pressure on the bacterial population. If resistant bacteria emerge, a previously effective phage may lose activity. Researchers study resistance mechanisms and investigate whether changing the phage, using a cocktail or combining phages with antibiotics can preserve useful activity. These are evidence-dependent strategies, not automatic solutions.

Endotoxin and inflammatory responses

WHO/Europe cautions that phages may release endotoxins from bacterial cell walls. Inflammatory responses can be especially concerning when large numbers of bacteria are involved. The available source does not establish reliable incidence rates for this risk, so a precise percentage would be misleading.

Host immune responses

The patient’s immune system can affect how a phage behaves in the body. The 2022 medical review lists host immune responses among the challenges that may limit phage therapeutics. This is one reason clinical monitoring and further study remain necessary even though phages do not infect human cells.

Are phages substitutes for antibiotics?

Not at present. WHO/Europe’s FAQ uses this question to emphasize that phages should not be framed as automatic replacements. Researchers investigate phages alongside antibiotics, and the peer-reviewed literature describes the potential for a combination to enhance antibiotic efficacy in some circumstances. Whether that happens depends on the bacteria, the phage, the antibiotic and the clinical setting.

For a patient, the practical implication is that an antibiotic should not be stopped or replaced with a phage without a qualified clinical decision. Phage use is considered in selected situations, often when standard options are inadequate, and requires evidence that the proposed phage is relevant to the infection.

What the current human evidence and access look like

WHO/Europe’s fact sheet published on 17 February 2025 describes phage therapy as promising but not ready for routine broad human availability. In most countries, phages are not approved medicines. The fact sheet says they are primarily used on compassionate grounds in life-threatening situations after other treatments have been exhausted. Approval and access rules differ by country; the available evidence does not establish a single worldwide pathway or imply that a reader can obtain therapeutic phages as an ordinary medicine.

WHO/Europe states: “Further evidence from clinical studies is needed before phages can become available more widely for therapeutic use in humans.” No individual speaker is identified; the statement is issued by the organization.

The FDA lists bacteriophage therapy among nontraditional antimicrobial products on its antimicrobial-resistance regulatory-science page. Its work includes clinical-trial design, combination antimicrobials and resistance mechanisms. That listing demonstrates regulatory-science interest, not approval of a particular phage treatment.

How the research field is building evidence

The 2022 Annual Review of Medicine article by Hatfull, Dedrick and Schooley describes renewed interest after clinical successes in personalized-phage case studies and discusses clinical trials in progress in that publication context. Because the review is from 2022, it should not be treated as a current inventory of trials, phases or results.

Current development questions include:

  • How to design trials that account for strain-specific matching rather than assuming one product fits every infection.
  • How to evaluate phage–antibiotic combinations and decide which pairings are biologically and clinically useful.
  • How to measure and respond to phage resistance during treatment.
  • How to monitor inflammatory reactions, immune responses and possible effects on the wider microbial community.
  • How regulatory systems can evaluate treatments whose composition may need to reflect an individual bacterial isolate.

These questions explain why a promising case report is not the same as proof of broad effectiveness. The available sources do not establish universal success rates, a current trial count, manufacturing standards or country-by-country approval status.

Those uses should not be merged with claims about human treatment. The organisms, delivery methods, regulatory requirements, exposure patterns and evidence standards can differ by sector. A result in an agricultural or aquaculture setting does not establish that a phage is safe or effective for a human infection.

What a realistic clinical pathway means for patients

A person with an antibiotic-resistant infection cannot safely self-select a phage from an online listing. A realistic pathway, where local rules permit consideration, involves clinical confirmation of the bacterial cause, laboratory testing for phage susceptibility, review of available treatment options and oversight under the applicable medical and regulatory framework.

Patients should ask their treating team whether the organism has been identified, whether a suitable phage has actually shown activity against that isolate, whether the proposed use is authorized or compassionate in that jurisdiction, how antibiotics fit into the plan and how resistance or inflammatory reactions will be monitored. The answers will vary by country and by infection.

What scientists can and cannot claim today

Scientists can say that lytic phages provide a targeted way to attack susceptible bacteria, including some strains resistant to antibiotics. They can report individual case successes, study phage cocktails and combinations, and test how resistance and immune responses affect outcomes.

They cannot yet claim that phages work against every resistant infection, that they eliminate the need for antibiotics, or that a promising case study establishes routine effectiveness. The current evidence supports continued clinical research and carefully monitored use in selected settings, while broader access remains dependent on stronger evidence, safety monitoring and workable regulation.
