Residual bone height (RBH) is a key measurement when planning implants in the posterior maxilla, but it is not a stand-alone rule for choosing a sinus-augmentation technique. More remaining bone often makes a transcrestal (crestal) approach feasible, while very limited bone may favor lateral access. Sinus health, Schneiderian-membrane condition, sinus shape, septa, the amount of elevation required, and the surgeon’s ability to control the graft and membrane can change that choice.
What residual bone height means
Residual bone height is the vertical thickness of native bone between the edentulous ridge and the floor of the maxillary sinus, measured on three-dimensional imaging. In the posterior maxilla, tooth loss and long-term sinus pneumatization can leave too little vertical bone for predictable implant anchorage. Sinus augmentation creates additional vertical space by elevating the Schneiderian membrane and, when indicated, placing graft material beneath it.
RBH matters because it affects both the amount of augmentation needed and how much initial implant support may remain. A site with substantial native bone may need only a modest membrane elevation. A severely resorbed site may require a larger augmentation and, in some cases, delayed implant placement. The measurement is therefore a planning input, not a diagnosis that automatically selects one operation.
Historical RBH thresholds—and why they are not a universal rule
A 2023 anatomical review reproduces categories attributed to the Academy of Osseointegration’s 1996 sinus-graft consensus. They are useful historical context, but the review does not establish them as a currently binding guideline, and modern imaging and techniques do not make every case fit a fixed interval.

| Residual bone height | Historical approach described in the 2023 review | How to interpret it today |
|---|---|---|
| 10 mm or more | Conventional implant placement without sinus grafting | May be suitable when implant length, position, bone quality and sinus anatomy are otherwise favorable. |
| 7–9 mm | Osteotome (transcrestal) technique with simultaneous implant placement | Represents a historical category, not a guarantee that a crestal lift will provide the required elevation. |
| 4–6 mm | Lateral access and grafting, with immediate or deferred implant placement | Current reviews also consider a transcrestal option in selected healthy sinuses; anatomy and treatment goals decide. |
| 1–3 mm | Lateral access and grafting with deferred implant placement | Very limited support may make staged treatment more practical, but the decision remains patient- and anatomy-specific. |
The table should not be read as “one millimeter equals one operation.” It describes how older consensus categories linked bone height to technique. A contemporary plan must verify whether the sinus can be safely accessed, whether the membrane is healthy enough to elevate, whether septa or an unfavorable contour increase difficulty, and whether the planned implant can be stabilized.
How anatomy changes the decision
Sinus and membrane health
Inflammation, fluid levels, mucosal disease or other sinus pathology can make membrane elevation less predictable. The 2023 anatomical review’s proposed decision tree favors transcrestal access when RBH is above 6 mm. At 4–6 mm, it favors transcrestal access when the sinus and membrane appear healthy, but lateral access when they are unhealthy. This is the review authors’ framework, not a universal recommendation or a substitute for medical assessment.
Sinus width, contour and floor shape
A narrow or favorable sinus contour may allow the membrane to lift with less lateral spread. A broad sinus can require a larger area of elevation and graft distribution. The shape of the floor and the angle between the walls influence how instruments reach the site and how evenly a graft can be placed. RBH alone cannot describe these three-dimensional differences.

Antral septa
Bony septa divide portions of the sinus and can obstruct instrument paths or create areas where the membrane is stretched unevenly. Their location should be identified before surgery. A septum may lead a clinician to modify the window position, choose a different access route or stage treatment, even when the measured RBH would otherwise suggest a crestal approach.
Membrane stretching and perforation risk
Perforation risk is related to how much the Schneiderian membrane must stretch and how the sinus geometry distributes that tension. Greater planned elevation, sharp contours, adhesions or septa can increase technical difficulty. A lateral window can provide direct visual access for membrane management and graft placement, while a transcrestal route limits the working field. Neither route eliminates perforation risk.
Transcrestal and lateral approaches compared
| Planning question | Transcrestal (crestal) access | Lateral-window access |
|---|---|---|
| Typical reason to consider it | Modest-to-moderate elevation through the implant osteotomy when remaining support and anatomy are favorable. | Need for broader access or a larger augmentation, especially when very little bone remains or anatomy makes a crestal route difficult. |
| Amount of native support | Usually more attractive when enough bone remains to stabilize the implant or when the planned elevation is limited. | Can be used in severely resorbed sites and when implant placement is deferred, subject to the individual plan. |
| Sinus contour and septa | More dependent on a direct, unobstructed instrument path and a contour that permits controlled elevation. | Window position can be adapted to sinus shape and septa, although septa can still complicate membrane elevation. |
| Membrane and graft control | Access is through a narrow osteotomy, so direct inspection and management are limited. | Provides a larger opening for visual assessment, membrane handling and graft distribution. |
| Invasiveness and operative burden | Generally considered less invasive in suitable cases, but suitability depends on anatomy and operator technique. | Creates a lateral bony window and may involve greater surgical exposure and recovery demands. |
| Evidence certainty | Randomized comparisons do not establish superiority for sites with RBH ≤6 mm; individual studies have important bias. | Likewise, no clear superiority over transcrestal treatment has been demonstrated for several measured outcomes in those comparisons. |
This comparison describes decision factors rather than a ranking. A less invasive route is not automatically safer if it cannot deliver the required elevation or if the membrane cannot be controlled.

What comparative evidence shows for RBH of 6 mm or less
Shi and colleagues’ systematic review in Clinical Oral Implants Research (2023) compared transcrestal sinus-floor augmentation with lateral sinus-floor augmentation in randomized and controlled studies. In seven randomized comparisons involving sites with RBH of 6 mm or less, the meta-analysis found no statistically significant difference between approaches in implant survival, Schneiderian-membrane perforation or marginal bone loss.
“No significant difference” does not prove that the procedures are equivalent. The review identified a high risk of bias in individual studies, and the number and design of trials limit confidence in applying the pooled result to every patient, sinus shape or surgical method.
Across the broader set of studies included in that review, pooled implant survival was 96.5% (95% confidence interval, 93.2%–98.9%) at least one year after transcrestal sinus-floor augmentation. The pooled sinus-membrane perforation estimate was 5.4% (95% confidence interval, 2.7%–8.8%). These are study-level estimates, not a personal prediction. They combine different patients, operators, follow-up periods and protocols; an individual risk can be higher or lower.

A 2025 review comparing osseodensification with lateral-window or osteotome approaches found short-term randomized evidence with high risk of bias and low GRADE certainty. Its conclusion was that the evidence was inadequate for evidence-based clinical-guideline implications. Therefore, a newer instrument or preparation method should not be treated as a proven replacement for anatomy-based planning.
A practical planning workflow
- Measure RBH on three-dimensional imaging. Record the shortest relevant vertical dimension at the intended implant site, rather than relying on a panoramic image alone. The measurement must be associated with the exact implant position and planned implant dimensions.
- Define the restorative objective. The needed augmentation depends on the planned implant position, length, diameter, prosthetic emergence and available native support. A site needing only limited elevation is a different problem from one requiring a large vertical gain.
- Examine the sinus and membrane. Assess signs of sinus disease, membrane thickening or fluid, and any condition that could make elevation unsafe or unpredictable. Medical or otolaryngology evaluation may be appropriate when pathology is suspected.
- Map the three-dimensional anatomy. Review sinus width and contour, the shape of the floor, wall thickness, and the number and position of antral septa. These findings can outweigh a borderline RBH value.
- Match access to the required control. Consider whether the surgeon can elevate the membrane and distribute graft material adequately through a crestal osteotomy. If direct visualization or broad graft placement is needed, lateral access may offer better control.
- Decide on simultaneous or staged implantation. Immediate placement requires sufficient primary stability under the selected plan. When native support is inadequate or the augmentation is extensive, grafting first and placing the implant later may reduce dependence on initial fixation.
- Discuss alternatives and contingencies. The consent discussion should include possible membrane perforation, changes in the planned access, delayed implant placement, treatment of sinus disease, and the possibility that the final technique will change after the anatomy is inspected.
How the main RBH ranges are commonly interpreted
RBH of 10 mm or more
Historical guidance placed these sites in the conventional-implant category. If the planned implant can be positioned with adequate bone support and the sinus is healthy, augmentation may not be necessary. The clinician still evaluates bone width, quality, implant position and prosthetic requirements; the 10-mm value is not a promise that grafting is never needed.

RBH of 7–9 mm
These sites historically corresponded to an osteotome or other transcrestal technique with simultaneous placement. The likely elevation requirement, membrane condition and sinus contour determine whether that approach is sensible. A site near the lower end of this range with an unfavorable contour may be more challenging than a site with slightly less bone but a healthier, more favorable sinus.

RBH of 4–6 mm
This is the range in which the choice is most clearly anatomy-dependent. The 2023 review’s proposed tree favors transcrestal access when the sinus and membrane are healthy and lateral access when they are unhealthy. The randomized evidence summarized above does not show a significant difference in several outcomes at RBH ≤6 mm, but its high risk of bias prevents a blanket conclusion that both approaches perform identically.
RBH of 1–3 mm
Very little native bone can make simultaneous implant stability difficult. Historical categories favored lateral grafting with delayed placement. A clinician may still consider other individualized strategies, but the smaller starting platform increases the importance of surgical access, graft containment, membrane management and a staged plan when primary stability cannot be assured.
Common pitfalls in using RBH
- Treating a cutoff as an automatic prescription. Thresholds describe populations and historical practice patterns; they do not account for every sinus contour, septum or membrane condition.
- Ignoring sinus health. A healthy membrane and an inflamed or pathologic sinus are not equivalent surgical environments.
- Assuming a less invasive route is always preferable. If the required elevation or graft control cannot be achieved through a crestal access, choosing it solely to reduce exposure may increase technical difficulty.
- Reading pooled survival as an individual guarantee. The 96.5% estimate is a pooled, at-least-one-year study result with a confidence interval, not a personalized forecast.
- Interpreting “no significant difference” as proof of equality. The randomized comparisons had substantial risk-of-bias concerns and may be underpowered to detect clinically important differences.
- Overpromising newer techniques. The 2025 osseodensification review reported low-certainty, short-term evidence that was insufficient for guideline-level conclusions.
Questions to ask at a consultation
- What is the measured RBH at the exact proposed implant position, and how was it measured?
- How much membrane elevation is required for the restorative plan?
- Are there septa, an unfavorable sinus contour or signs of membrane or sinus disease?
- Why is a transcrestal or lateral approach better suited to this anatomy?
- Can the implant be placed at the same appointment, and what would make staged placement safer?
- How would a membrane perforation or unexpected sinus finding change the plan?
- Which parts of the recommendation are based on the individual scan, and which are general historical thresholds?
Bottom line for patients and clinicians
RBH is a starting measurement for deciding how much augmentation may be needed and whether a crestal route is technically reasonable. It cannot, by itself, select the operation. Historical categories associate greater bone height with transcrestal treatment and very limited height with lateral grafting, but modern planning adds sinus health, membrane condition, contour, septa, required elevation, graft control and the possibility of staged implantation. For RBH of 6 mm or less, current comparative evidence has not shown a significant difference in several major outcomes, yet the certainty is limited by biased studies. The safest choice is therefore the approach that fits the complete three-dimensional anatomy and the restorative goal, selected by a qualified implant clinician after appropriate imaging and medical evaluation.
