Researchers identify neuroinflammation drug targets by combining evidence from human disease, genetics, cell biology and experimental perturbations. They validate a candidate by testing whether changing it in a defined direction alters disease-relevant biology—and whether that effect can plausibly be achieved safely in the right tissue, cell state and disease stage. A signal found in diseased tissue is a reason to investigate a target, not proof that treating it will help patients.
Identification and validation answer different questions
Target identification nominates a biological component that might influence disease. It can begin with a genetic association, a change in gene or protein activity in diseased tissue, a pathway implicated by prior work, or a perturbation signature. These findings help researchers choose what to test, but association and differential expression alone do not establish that changing the candidate will change disease.
Target validation tests a specific intervention hypothesis: if researchers increase, decrease or otherwise alter the target in a defined context, does relevant biology change in a reproducible way? Strong validation connects the intervention to target engagement, a downstream mechanism and a meaningful functional outcome. It also asks whether the direction and degree of modulation are appropriate, and whether the proposed intervention could reach the relevant part of the nervous system.

Start by defining the disease and the proposed intervention
“Neuroinflammation” describes biology involved in multiple diseases and contexts, not one disease or one uniform cell state. Before interpreting a candidate signal, researchers need to specify the condition and the question they want to answer. A useful target hypothesis states:
- Disease and stage: Which disease, and which phase or form of it, is the target meant to address?
- Location and cell type: Is the relevant biology in the brain, spinal cord, another compartment, or a particular cell population?
- Direction of intervention: Should activity be increased, reduced or changed in another defined way?
- Timing and outcome: When would the intervention act, and what biological or clinical outcome should change?
This framing matters in multiple sclerosis (MS), where peripheral inflammatory mechanisms associated with relapses are not interchangeable with CNS-compartmentalized inflammation associated with progression. Progression may involve several concurrent mechanisms. In their 2026 review, Alica Blenkle, Anastasia Geladaris and Martin S. Weber write that “Disease progression is probably driven by a combination of several mechanisms rather than a single factor.” A target proposed for one disease process, lesion context or stage should not automatically be treated as a universal MS target.
Build a candidate from evidence that converges
Researchers can draw on several evidence streams. Each answers a different question, so agreement among them is more informative than repeating the same type of observation in several datasets.

Genetic evidence can strengthen a target hypothesis, but it is not itself a test of a drug intervention.
- Human disease tissue: Expression, protein and other molecular measurements can show which pathways differ in affected tissue. Cell-resolved or spatial measurements can help locate the signal. A difference can still be a consequence of disease, a response to injury or a feature of a changing cell mixture rather than a cause.
- Pathway knowledge: A candidate can become more plausible when it fits a coherent mechanism connecting the observed signal to disease biology. A pathway label alone does not demonstrate that the candidate is the controlling or therapeutically useful node.
- Perturbation signatures: Results from experimentally changing a candidate can show whether downstream effects align with the proposed mechanism. These results are strongest when the perturbation is specific, the controls are appropriate and the outcome is relevant to the intended disease context.
The key question is not simply whether a molecule appears in diseased tissue. It is whether the evidence supports a causal role, in which cells and circumstances that role matters, and whether a particular intervention can reproduce the desired effect.

Resolve which cells and states carry the signal
Bulk tissue measurements can blur differences between cell types and between states within a cell type. Single-cell and single-nucleus transcriptomics, chromatin-accessibility methods and spatial multi-omics help researchers investigate which cells carry a signal, where those cells are located and what molecular programs are present.
These methods have made clear that microglia are not adequately described by a simple binary of “resting” and “activated,” or by treating M1/M2 as a complete account of their biology. Microglial states are multidimensional and can vary with age, brain region, sex, genotype, environment and prior exposures. A disease-associated marker therefore does not, by itself, show whether the marked cells are causing damage, responding to it, supporting repair or performing more than one role.
Researchers need to establish that the relevant state and location are present in the disease context their intervention is meant to address. They also need to consider function: microglia contribute to homeostasis, immune defense and repair as well as disease-associated processes. Broadly suppressing a pathway that appears elevated could impair beneficial functions if the target acts in several states or contexts.
Test causality with deliberate perturbations
After nomination, researchers test whether manipulating the candidate changes the proposed mechanism and outcome. A useful design makes the intervention, controls and readouts fit the hypothesis rather than relying on detection of the target alone.

- Choose the intended direction and context. Define whether the experiment will increase or decrease target activity, in which cell or model, and at what stage. The intended drug effect should be clear enough to compare with the experimental perturbation.
- Use orthogonal approaches where feasible. Genetic perturbation and pharmacological modulation provide different ways to test the hypothesis. Agreement across methods can reduce the chance that an effect is peculiar to one reagent or experimental approach.
- Include suitable controls and test reproducibility. Controls help separate target-specific effects from handling, delivery or off-target effects. Replication, and rescue experiments where appropriate, can test whether the result depends on the proposed target.
- Measure target engagement. Establish that the intervention changed the target as intended. Without that evidence, a negative outcome may reflect failure to perturb the target, while a positive outcome may be difficult to attribute to it.
- Connect engagement to mechanism and function. Measure downstream biology and a disease-relevant functional outcome, not just a molecular marker. The result should support the chain from intervention to target effect to a meaningful change.
Robert M. Plenge, Edward M. Scolnick and David Altshuler, in their 2013 review “Validating therapeutic targets through human genetics,” caution that “Existing preclinical models have a limited ability to test ‘therapeutic hypotheses’”. That limitation is one reason to distinguish a well-controlled mechanistic result from evidence that a treatment will benefit people.
Use human genetics as one part of causal triangulation
Human genetic evidence can help researchers assess whether naturally occurring differences in a target plausibly influence disease. When a causal gene and the direction of effect are clear, genetics can strengthen prioritization and inform hypotheses about dose-response or safety. Mendelian randomization is one approach used to study the effects of perturbing drug targets; it adds evidence rather than replacing laboratory experiments or clinical trials.
Interpreting a genetic result requires scrutiny of the link between the genetic instrument and the proposed target. Researchers should examine whether the variant acts through the target or through other pathways (pleiotropy), whether the instrument is strong enough, which tissue and cell type the evidence represents, and whether the population context is appropriate. Genetic proxies may differ by tissue and population, so an association does not necessarily describe the target’s effect in the relevant CNS cells.
There is also a timing difference. Inherited perturbations can act throughout life, whereas a drug is administered at a particular dose, for a limited period, and at a selected disease stage. A genetic result may therefore support the direction of a hypothesis without establishing the effect or safety of a timed pharmacological intervention.

Choose models for the question they can answer
Experimental systems are complementary, not interchangeable. Each offers a different view of mechanism and has limits that matter when judging how far a result can be translated.
| Model or evidence source | What it can contribute | What it cannot establish by itself |
|---|---|---|
| Human genetics | Evidence from naturally occurring perturbations; potential support for causal prioritization and hypotheses about direction or dose-response. | That a drug given at a particular stage, dose or duration will reproduce the genetic effect or be safe. |
| Animal models | Tests of mechanisms and interventions in an organism-level setting. | That the model captures human disease biology or predicts clinical benefit; predictive value can be limited by species differences, disease-model fidelity, design and endpoints. |
| Human iPSC-derived microglia and multicellular models | Access to human cell biology and opportunities for high-throughput perturbation, including investigation of microglia-related mechanisms in progressive MS. | The full CNS environment or the full complexity of MS progression. |
| Human disease tissue and cell-resolved data | Evidence about where signals occur and which cell populations or states are associated with disease. | Causality: a signal can reflect a response to disease rather than a cause that can be changed therapeutically. |
Researchers should match the model to the question. A cell model may help test whether a candidate changes microglial biology, while an organism-level model can test consequences in a broader system; neither alone answers whether the intervention works in people. Replication across complementary systems and eventual human evidence make the overall case more informative. Human iPSC-derived models can help study progressive-MS mechanisms, but they do not reproduce the complete disease.

| Question | Why it matters |
|---|---|
| How strong is the causal evidence? | Separates a candidate linked to disease from one for which intervention evidence supports a causal role. |
| Which cells and tissue are implicated? | Tests whether the signal is specific to the intended biological context or may reflect broader effects. |
| At what stage and time should it be modulated? | Accounts for disease mechanisms that differ between stages, lesions or clinical processes. |
| What direction and degree of modulation are supported? | Clarifies whether the proposed treatment should increase or decrease activity, and what evidence supports that choice. |
| Does the result reproduce across methods and models? | Shows whether the case depends on one assay, perturbation or model system. |
| Can target engagement be measured? | Provides a way to determine whether an intervention reaches and affects its intended target. |
| Are the endpoints functionally relevant? | Distinguishes a change in a convenient biomarker from an effect connected to disease biology or patient outcomes. |
| What are the likely on-target safety effects? | Highlights the possibility that a target also supports homeostasis, immune defense or repair. |
| Can the intervention reach the relevant CNS compartment? | Addresses whether the intended biological location is accessible to the proposed treatment. |
For candidates supported by genetics, add questions about instrument validity, pleiotropy, ancestry, tissue context and whether lifelong perturbation meaningfully informs the proposed treatment. A strong candidate on one dimension may remain weak on another; for example, a causal signal is not enough if its direction is unclear or target engagement cannot be assessed.

Plan translation before a clinical test
Before clinical testing, the target hypothesis needs a practical measurement and treatment plan. Researchers must define how they will measure target engagement and downstream pharmacodynamics, determine whether the intervention reaches the relevant CNS compartment, and select the patient subgroup, disease stage and clinical outcome suited to the proposed mechanism.
There is no universal target-engagement threshold established across neuroinflammatory diseases and target classes. A measurable change must be interpreted in relation to the specific target, intervention, compartment and intended biological effect. Likewise, a pharmacodynamic marker can show that biology changed without proving that patients experienced a meaningful clinical benefit.
These decisions are particularly important when a target could have both harmful and beneficial roles. A treatment intended to reduce a disease-associated function should be evaluated for effects on other functions of the same cell or pathway, and for whether the timing and patient selection fit the hypothesized mechanism.
Why early target confidence does not guarantee an effective drug
Drug-development figures illustrate the attrition problem, but their denominators and scope must not be conflated. Plenge, Scolnick and Altshuler’s 2013 review reported that more than 90% of compounds entering clinical trials fail to demonstrate sufficient safety and efficacy to gain regulatory approval. The review attributed most failures to limited predictive value of preclinical models and uncertainty about the long-term effects of target perturbation in humans. This is a broad historical estimate, not a neuroinflammation-specific trial failure rate.
A 2018 Genome Medicine article gave a separate estimate that 4% of drug-development programs yield licensed drugs. That figure refers to programs, not the same denominator as compounds entering trials in the 2013 review, so the percentages are not directly comparable. Neither number estimates the success rate for neuroinflammation target validation specifically.
The practical lesson is not that target validation is futile, but that evidence at one stage does not settle the next question. A biological association, a causal perturbation result, target engagement, a safe intervention and clinical benefit are distinct claims that require distinct evidence.
A concise way to judge a target hypothesis
Before treating a neuroinflammation candidate as therapeutically credible, ask whether the case specifies the disease and stage, identifies the relevant cell state and location, supports a causal role rather than an association, tests the intervention in the intended direction, and connects target engagement to functional biology. Then assess whether findings converge across human evidence and complementary models, whether safety and CNS access are plausible, and whether a clinical test can measure engagement and evaluate an appropriate outcome. If several of these links are missing, the candidate may still be worth studying—but it has not yet been validated as a treatment target.
