Ransomware is malicious software that blocks access to files, systems or networks until attackers demand payment. Some operations also steal data and threaten to publish it, so restoring encrypted files may not resolve the full incident. There is no single way attackers get in: official advisories describe exploited internet-facing systems, stolen or brokered credentials, phishing, drive-by downloads and social engineering.

What is ransomware?

The FBI defines ransomware as malicious software that prevents access to files, systems or networks and demands a ransom. An attack can disrupt access to business-critical systems as well as individual files.

Encryption and data theft are different risks

In a double-extortion operation, attackers steal data and threaten to publish it in addition to encrypting files. The July 2025 Interlock advisory describes actors encrypting after exfiltrating victim data. An August 2026 Medusa update likewise describes encryption paired with threats to publish stolen material. These are documented tactics of those operations, not a rule that every ransomware incident includes data theft.

How do ransomware attacks get in?

Ransomware is a changing set of financially motivated operations, not one malware strain with one fixed entry point. The CISA #StopRansomware Guide groups prevention around common initial-access vectors and recommends vulnerability scanning, particularly for internet-facing devices. Recent joint advisories illustrate how different those paths can be.

Exposed or vulnerable internet-facing services

An attacker may exploit a weakness in a service reachable from the internet. The CISA guide warns against exposing remote services such as Remote Desktop Protocol (RDP) to the web without appropriate compensating controls. The practical issue is not only whether a vulnerability has a high severity rating: whether a vulnerable system is exposed and whether exploitation is known also affect remediation priority.

Compromised credentials and access brokers

The August 2026 Medusa update reports access through brokers as well as phishing and exploitation of newly disclosed, unpatched internet-facing vulnerabilities. Brokered access means attackers may acquire an entry point from another party rather than exploit the victim’s system themselves. The specific routes in that advisory are observations about Medusa, not a ranking of how ransomware groups generally enter.

Phishing, drive-by downloads and social engineering

Phishing can be used to obtain credentials or induce someone to open or run malicious content. The July 2025 Interlock advisory documents drive-by downloads from compromised legitimate websites, as well as a technique called ClickFix: a social-engineering approach that tricks a person into executing a malicious payload. These examples show why defenses need to address both technical weaknesses and user-directed deception.

Known exploitation after a vulnerability is disclosed

A June 2025 joint advisory on Play reported that multiple ransomware groups, including initial-access brokers with ties to Play operators, exploited CVE-2024-57727 in the SimpleHelp remote monitoring and management tool after its disclosure. This is a specific, dated example—not evidence that the vulnerability remains actively exploited today or that SimpleHelp is the only affected technology. Check current official advisories before making decisions about live threats.

Which defenses address the main weaknesses?

No single control prevents every route into an organization or replaces recovery planning. The controls below address different parts of the problem; the CISA guide and joint advisories recommend layered prevention and preparation.

Control What it addresses Practical action
Exposure reduction Internet-facing systems and unnecessary remote services that attackers may exploit. Inventory internet-facing assets, scan for vulnerabilities, remove services that are not needed and prioritize known exploited vulnerabilities. Avoid exposing RDP without appropriate compensating controls. (CISA #StopRansomware Guide; June 2025 Play advisory)
Identity protection Compromised or brokered credentials and unauthorized account access. Apply identity and access controls and require multifactor authentication (MFA) where possible, especially for remote access and accounts that can reach critical systems. (July 2025 Interlock advisory; June 2025 Play advisory)
Containment Movement from an initially compromised device to other systems. Segment networks and constrain remote-service traffic so a compromised device cannot freely reach the rest of the environment. (July 2025 Interlock advisory; August 2026 Medusa update)
Recovery Loss of access to data or systems, including failure of a backup to restore what is needed. Back up data regularly, confirm jobs completed, protect copies from the systems they back up, and test restoration as part of a continuity and recovery plan. (FBI ransomware guidance; June 2025 Play advisory)
Detection and response Recognizing and handling an incident while preserving useful information. Maintain monitoring appropriate to the environment, preserve useful incident information and report the incident to the FBI or IC3; relevant joint advisories also list CISA and sector-specific reporting channels. (FBI ransomware guidance; July 2025 Interlock advisory)

How can you protect backups from ransomware?

A backup is useful only if it is intact and can be restored. The FBI recommends keeping backup copies disconnected from the systems they protect when they are not in use. A permanently connected copy may remain reachable from a compromised environment, so isolation is an important part of the plan.

  • Schedule regular backups and verify that each job completes.
  • Keep backup copies protected from the systems they back up; disconnect copies when they are not in use.
  • Test restoring data rather than assuming that having a backup proves it is complete or recoverable.
  • Include restoration and continuity steps in a recovery plan, so the organization knows how to resume needed operations.

An offline drive can serve as one backup destination, but it is not a complete recovery strategy by itself. The FBI supports the practice of disconnected backups, not a particular brand or model.

What should you know about ransom payment and reporting?

The FBI states: “The FBI does not support paying a ransom in response to a ransomware attack.” The agency says payment does not guarantee data recovery and may encourage perpetrators to target additional victims. This is FBI guidance, not a claim that every incident has identical circumstances or legal implications.

Preserve useful incident information and report the incident to the FBI or IC3. Depending on the incident and sector, the relevant joint advisory may also identify CISA or a sector-specific reporting channel.

What do the current actor examples show?

Official advisories provide concrete examples, but they describe named actors and observed periods—not a complete census of ransomware activity or a global prevalence estimate. One scoped figure in the August 2026 CISA, FBI and HHS Medusa update is more than 500 victims as of April 2026, across multiple named critical-infrastructure sectors. That number refers to Medusa actors and those sectors; it is not a count of all ransomware victims.

Treat actor tactics and exploited-vulnerability details as time-sensitive. The Medusa, Interlock and Play examples above are tied to their respective advisory dates; consult current official advisories when evaluating an active exposure or incident.
