Radiologists do not accept an artificial-intelligence flag as a diagnosis. They verify it by reviewing the original images, the examination’s clinical purpose, and the patient’s relevant information, then judging whether the software’s output is plausible, useful, and applicable to that case. The exact sequence varies by modality, indication, software, and local protocol. Verification is a human-led interpretation supported—not replaced—by AI.
What an AI finding actually represents
Medical-imaging AI is built for a defined task. A tool may mark a suspected pulmonary nodule on a CT scan, estimate a measurement, prioritize a worklist, or flag a possible intracranial hemorrhage. Its output can be a label, probability, measurement, bounding box, segmentation, heat map, or other annotation. That output is evidence generated for the task the developer evaluated; it is not an independent diagnosis and it does not establish what is happening in a particular patient.
Before relying on a result, a radiologist needs to know the tool’s intended use: which finding it detects or measures, on which modality and acquisition protocol, and in what clinical setting. A model evaluated on noncontrast head CT for emergency triage should not automatically be treated as validated for contrast-enhanced studies, postoperative scans, children, or a different scanner protocol. The RSNA’s 2023 guide to evaluating clinical AI publications recommends examining the study question, clinical context, training and test data, measured performance, usefulness, interpretability, and reproducibility.
Regulatory clearance indicates that a product met the requirements of a particular review pathway. It does not mean the output is correct for every patient, scanner, population, or workflow. The radiologist remains responsible for interpreting the images in context.

How verification works at the workstation
There is no single universal checklist that applies to every product. The following sequence describes the common reasoning tasks rather than a mandatory order for every examination.
1. Confirm the examination and the tool’s scope
The radiologist first establishes what study is being viewed and why it was performed. The body region, modality, contrast status, acquisition quality, and clinical question determine whether the AI result is relevant. The tool’s output is considered only within its labeled or locally approved intended use. If the scan is outside that scope, the radiologist may still inspect it, but the output carries less evidentiary weight and should not be presented as validated performance.
2. Review the source images, not only the annotation
The original image data remain the primary evidence. The reader examines the finding in the appropriate planes, window settings, series, and neighboring anatomy used for that examination. An annotation can focus attention, but it can also be misplaced, incomplete, or caused by an artifact. The radiologist checks whether a visible structure, motion artifact, beam-hardening effect, foreign object, or normal variant explains the alert.

Software interfaces differ. Some show an overlay, a list of candidate findings, a measurement, or a priority status; others return a result in a separate viewer. A radiologist should not assume that a missing overlay means a normal study or that a prominent overlay is necessarily important. The product’s instructions and local protocol define what its symbols and confidence information mean.
3. Compare the output with clinical information
Symptoms, timing, prior examinations, laboratory data, and the reason for imaging can change the interpretation of an apparently plausible mark. Clinical context can support or weaken a hypothesis, but it must not be used to force the image to match a preconception. The radiologist integrates the patient information with the complete study and documents the clinically relevant interpretation.
4. Test plausibility and completeness
The reader asks whether the flagged abnormality is genuinely present, whether its location and extent are correct, and whether other important findings are absent from the AI output. A tool designed to detect one abnormality is not a general-purpose second reader. A negative result therefore does not rule out every disease, and a positive result does not establish severity, cause, or urgency without image interpretation.

5. Decide how the result changes the report
If the finding is confirmed, the radiologist incorporates the appropriate observation into the report and reaches a conclusion using standard clinical judgment. If the finding is an artifact or normal anatomy, it is disregarded or, where local policy requires, recorded as a discordant case. If the AI suggests something uncertain, the radiologist resolves the uncertainty using the images, available prior studies, clinical information, and any approved follow-up pathway.

Why a plausible flag can still be wrong
AI systems can fail because of image artifacts, unusual anatomy, incomplete clinical context, or data that differ from the population and protocols used for development. Limited transparency can also make it difficult to understand why a model produced a particular output. Bias is possible when training data do not represent the people and equipment in the facility where the tool is used.
A case summarized by the RSNA from a 2024 review of FDA-cleared radiologic algorithms illustrates the risk: an algorithm labeled a finding as intracranial hemorrhage, while the patient was ultimately diagnosed with ischemic stroke. This is evidence that the failure mode has occurred; it is not an error rate for all radiology AI systems and does not predict what will happen in an individual examination.
The practical consequence is that agreement between a flag and a radiologist’s impression is reassuring but not self-validating. Disagreement is not automatically proof that the software failed, either. The radiologist may have identified a subtle abnormality the tool was not designed to detect, or the model may have highlighted a real but clinically insignificant feature. Each situation must be judged against the intended task and the complete image.
What happens when the radiologist disagrees
The image appears normal
The radiologist reviews the relevant source images and confirms that the marked area is artifact, normal anatomy, or outside the tool’s task. The report follows the image interpretation, not the alert. If the local quality program collects disagreements, the case can be submitted for monitoring without changing the clinical conclusion.
The image confirms the finding but the AI missed it
A missed finding demonstrates that the tool is not comprehensive. The radiologist reports the abnormality using the usual clinical standards. Facilities may retain the case for performance review, especially when the miss could affect safety or when it reveals a protocol, population, or workflow gap.

The output is delayed or absent
Result generation and presentation are operational issues as well as technical ones. A delay may make an alert unavailable when a patient is being read, while a transmission or interface failure can make a completed analysis appear missing. The radiologist should follow the local procedure for unavailable results and must not delay urgent interpretation while waiting for an optional AI output. A missing result is not equivalent to a negative result.
The AI and the images are both plausible but conflict
The radiologist resolves the conflict by considering the full study, prior imaging, clinical question, and the limits of the tool. When uncertainty remains, local escalation or a second human opinion may be appropriate. The correct response is not to average two opinions mechanically; the software and the radiologist do not have equal roles or equal access to context.
How a facility evaluates an AI tool before use
Verification of an individual case sits inside a larger governance process. The 2024 multi-society statement from the American College of Radiology, Canadian Association of Radiologists, European Society of Radiology, Royal Australian and New Zealand College of Radiologists, and RSNA emphasizes evaluation by purchasers and users rather than relying only on vendor or publication claims.
Define the clinical job
Leaders specify the intended task, patient population, modalities, protocols, urgency, and users. A tool for worklist prioritization has different safety and workflow requirements from one that produces a quantitative measurement or supports a treatment decision. The facility should also define what the tool is not intended to do.
Check data relevance and representativeness
Evaluation includes the sources of the training and test data, inclusion and exclusion criteria, disease prevalence, scanner and protocol mix, and whether the tested population resembles the facility’s patients. A high performance result from a different setting may not transfer unchanged. Data drift can occur when equipment, protocols, referral patterns, or patient demographics change.
Examine performance and clinical usefulness
Headline accuracy is insufficient. Decision-makers should review the performance measures appropriate to the task, the test-set design, false-positive and false-negative consequences, and whether the output improves a real clinical or operational outcome. A model can perform well on a curated dataset yet add little value if its alerts arrive too late, duplicate existing work, or are difficult to interpret.

Assess interpretability and failure handling
Users need to understand what an output means, how confidence or measurements are defined, and what happens when the system cannot analyze a study. Documentation should explain supported protocols, known limitations, and expected behavior for incomplete or poor-quality data. Facilities should have a way to record and investigate clinically meaningful disagreements.

Images and associated metadata may leave the local environment for processing, depending on the architecture. Governance therefore covers data handling, access control, retention, contracts, and compliance obligations. It also identifies who is responsible for reviewing alerts, maintaining the integration, responding to outages, and approving changes to the tool or its use.
Monitoring after deployment
Performance is not a one-time acceptance test. The ACR’s May 2026 announcement about its ACR-SIIM imaging-AI practice parameter describes selection, evaluation, monitoring, privacy, and continuous quality improvement as parts of an ongoing program. The announcement also describes ACR Forensics for local investigation of discordant cases and Assess-AI for monitoring.
A practical monitoring plan can track whether results are generated, delivered, and viewed as intended; whether processing delays interfere with care; and whether disagreement patterns change over time. Facilities can sample confirmed findings and misses, examine cases from different scanners and protocols, and investigate unexpected behavior. Monitoring should distinguish technical availability from clinical usefulness: a system may be online while its output is poorly aligned with the users’ workflow.
When a tool, scanner, protocol, or patient population changes, the facility should reassess whether previous evidence still applies. Updates require change control, communication, and, when appropriate, renewed local validation. Continuous quality improvement is intended to identify and correct problems rather than to treat the original evaluation as permanent proof of suitability.

Workflow details that affect verification
The RSNA’s 2026 primer on interacting with AI results highlights practical factors that can change how safely a result is used:

- 【Space Saving & Keep Organized】Desk cable management tray holds all your power cords, outlet strips, USB hubs and computer transformers hidden from your feet, and the data cables will not enter the vacuum cleaner to keep your desk clean and tidy. Desk wire organizer is also suitable for use in the kitchen and outdoors.
- 【Reduce Safety Risks】Cable tray under desk keeps all plugs away from your kids, no more worry about tripping. Using 2 holes wire tray saves extra space, helps you easily cabling and wire protection. The mesh design is not easy to accumulate dust, but avoids safety accidents caused by messy wires.
- Generation: The time required to process a study and the conditions that cause an analysis to fail or be skipped.
- Presentation: Where the result appears, how it is labeled, and whether it can be distinguished from a radiologist’s annotation or a prior report.
- Alignment: Whether the alert arrives at the point in the reading process where it can help rather than interrupt or duplicate work.
- Storage: How annotations, measurements, and audit information are retained and linked to the study.
- Medicolegal use: What the organization’s policy says about documenting, retaining, and acting on algorithmic outputs.
These details are not cosmetic. A technically strong model can be unsafe if results are routed to the wrong worklist, appear after a decision has been made, or are mistaken for a definitive diagnosis.
How to compare two radiology AI tools
When a practice evaluates alternatives, it should compare the whole use case rather than a single accuracy number.
| Comparison area | Questions to answer |
|---|---|
| Intended task and setting | What finding or measurement does the tool address, on which modality, protocol, urgency, and patient group? |
| Training and test data | Are the data representative of the facility’s scanners, protocols, disease prevalence, and population? |
| Measured performance | Were appropriate test data used, and are false positives and false negatives relevant to the intended decision? |
| Clinical usefulness | Does the output improve interpretation, triage, measurement, or another defined outcome? |
| Interpretability | Can users understand the annotation, measurement, confidence information, limitations, and failure states? |
| Workflow fit | Are generation time, presentation, routing, storage, and downtime behavior compatible with local reading practice? |
| Privacy and governance | How are images and metadata handled, who has access, and who owns operational accountability? |
| Monitoring plan | How will stability, safety, disagreement, delays, and population or protocol changes be detected after launch? |
This framework reflects the evaluation areas identified in RSNA guidance, the 2024 multi-society statement, and the ACR-SIIM practice-parameter announcement. It also prevents a common mistake: selecting a tool because it reports a favorable result in one study without checking whether that result applies locally.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsQuestions radiologists and patients commonly ask
Can a radiologist disagree with an AI result?
Yes. The radiologist’s interpretation of the complete study and clinical context governs the report. A disagreement can be clinically appropriate and may be useful for quality monitoring.
Does a cleared AI tool verify a diagnosis?
No. Clearance does not make an output correct for every patient or setting. It indicates that the product passed a particular regulatory review; local suitability and human interpretation remain necessary.
Does a negative AI result rule out disease?
No. Most tools address a defined task and can miss findings or encounter unsupported images. A negative output cannot replace a complete radiologist interpretation.
Is there one required verification sequence for every image?
No universal sequence was established. The operational details depend on the modality, indication, software instructions, and local protocol. The consistent principle is to treat AI as an adjunct and assess its output against the source images and clinical context.
