Public-health teams track Ebola outbreaks by linking five jobs: surveillance that detects unusual illness, rapid investigation and laboratory diagnosis, contact tracing, epidemiological analysis, and continuous work with affected communities. A contact is monitored actively for 21 days after the last known exposure, with daily follow-up and immediate assessment if symptoms appear.

The World Health Organization’s Surveillance, case investigation and contact tracing in filovirus disease outbreaks, published 9 October 2026, is the current operational framework. It covers Ebola disease caused by Ebola, Sudan, Bundibugyo and Taï Forest viruses, as well as Marburg virus disease, and updates WHO’s 2015 Ebola contact-tracing guidance. It supports national response plans; it does not replace the case definitions, reporting rules or treatment protocols set by a country’s health authorities.

What the outbreak-tracking workflow is designed to do

Filoviruses can cause severe disease and spread when infectious people or contaminated materials come into contact with others. The public-health objective is therefore operational: detect a signal early, determine whether it represents a filovirus case, find people who may have been exposed, monitor them through the risk window, and use the resulting data to stop further transmission.

WHO summarizes the purpose of its 2026 guidance as follows: “This operational guidance helps countries detect filovirus disease outbreaks early, find all cases, interrupt transmission and use data to guide the response.” The workflow is not a single database or an app. It is a coordinated system involving surveillance officers, clinicians, laboratories, epidemiologists, trained community health workers, local leaders and, when needed, teams in neighboring countries.

Connected function What teams do Operational output
Routine surveillance and early warning Watch human and wildlife health information and collect alerts about unusual illness or deaths. An alert register with each signal’s investigation status.
Investigation and contact tracing Assess suspected cases, arrange testing, reconstruct activities and identify exposed people. Case line lists, contact lists and transmission links.
Epidemiological analytics Combine case, laboratory and follow-up records to locate transmission and gaps in response. Indicators, maps or other analyses that direct resources.
Community engagement Work with trusted local actors throughout detection, referral, tracing and protective measures. Earlier reporting, safer referrals and greater acceptance of response actions.
Cross-boundary coordination Share and reconcile information when people move between districts, countries or mass gatherings. Continuity of follow-up despite mobility.

1. Detect signals before transmission becomes widespread

Surveillance starts with routine information, not only with a confirmed Ebola diagnosis. Teams monitor reports from health facilities, laboratories, community networks and, where relevant, wildlife surveillance. A cluster of unexplained illness, several deaths with a similar pattern, or a report from a community worker can all be an alert requiring investigation.

An alert is not automatically a case. It is a prompt to verify what happened, where it happened and whether the event meets the current national or WHO alert criteria. Maintaining a live register matters because it shows which signals are new, which are being investigated, and which have been resolved. In its 24 August 2026 temporary recommendations for Bundibugyo virus disease in the Democratic Republic of the Congo, WHO specifically called for a current register of signals and the status of each investigation.

Community reporting can shorten the time between the first symptoms and a professional assessment. Local networks may notice an unusual death or a group of people becoming ill before a formal facility report is generated. Their role is to alert trained responders and support safe referral, not to make a laboratory diagnosis.

2. Investigate and classify suspected cases

Once an alert is received, an investigation team gathers a clinical history, exposure history and location information, arranges an appropriate assessment and collects specimens under the applicable infection-prevention procedures. Laboratory testing is essential because symptoms can overlap with other diseases. The exact case definition and testing algorithm depend on the virus, the phase of the outbreak and current national technical guidance.

Case categories and line lists

Investigators record people in a case line list using the categories required by the response: suspected, probable and confirmed. A line list is a structured record, not a public ranking. It commonly connects an identifier with dates, locations, clinical status, laboratory results, exposure information and outcome so that the response can be updated as evidence changes.

WHO’s August 2026 DRC recommendations called for updated lists of suspected, probable and confirmed cases and for building chains of transmission. A chain of transmission links cases or exposures in time and place. It can reveal an overlooked household exposure, a health-care-associated event or a gap between two geographic areas.

Why classification can change

A person can move from suspected to probable or confirmed as epidemiological and laboratory information becomes available. Conversely, an investigation may determine that an alert was caused by another illness. Teams should therefore treat line lists as living operational records and record the reason and date for important status changes.

3. How does contact tracing work?

Contact tracing identifies people who may have been exposed to a case, informs them of what to do, and follows their health during the relevant monitoring period. WHO’s public explainer describes the basic sequence as identifying contacts from a person’s activity history, listing and informing them, and following up.

  1. Reconstruct activities. A trained interviewer asks the case, relatives or caregivers where the person went, whom they spent time with, who provided care, and which funerals, health facilities, workplaces, schools, transport journeys or gatherings they attended during the period relevant to the investigation.
  2. Assess possible exposure. Investigators determine which people had an exposure that meets the response definition. The assessment is based on the circumstances and the virus-specific protocol, not simply on whether someone lives nearby.
  3. Create and verify the contact record. The team records each person’s identity or assigned identifier, location, exposure details, preferred means of communication and the responsible follow-up worker. Duplicate records are reconciled so one person is not counted twice.
  4. Inform the contact. A contact is told why the team is calling, the dates that matter, how follow-up will occur, which symptoms require immediate reporting and where assessment will take place. Communication should use a language and channel the person can understand and access.
  5. Link the record to response services. If a contact becomes ill, the follow-up team must be able to alert the clinical and infection-prevention teams quickly, arrange safe transport or referral under the local protocol, and update the case and contact records.

Contact tracing is therefore an active response function rather than a one-time notification. A list that is never revisited cannot show whether people remained well, moved away, became unreachable or developed symptoms.

Cross-border travel and mass gatherings

People may be exposed in one district and become symptomatic in another country, or a single gathering may involve participants from many locations. WHO’s 2026 operational guidance includes investigation across borders and at mass gatherings. Response teams need agreed procedures for notifying the receiving area, transferring the minimum information needed for follow-up, and confirming that responsibility has been accepted. Without that handoff, a person can disappear from the original team’s list while remaining at risk elsewhere.

4. How long are Ebola contacts monitored?

WHO says identified Ebola contacts are monitored for 21 days after exposure. The 2026 DRC recommendations specify counting from the person’s last known exposure, because a later exposure resets the relevant end date. Teams record each contact’s status throughout the period and may reprioritize follow-up repeatedly when outbreak conditions or available staff require a risk-based approach.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What daily follow-up includes

Daily follow-up can confirm whether the person is well, ask about symptoms, verify location and contact details, and identify barriers to reaching a clinical service. The exact channel may be an in-person visit, telephone call or another approved method, depending on security, geography and the response plan. Missed follow-ups are recorded as gaps to be resolved, not silently treated as evidence that the person is healthy.

What happens if a contact develops symptoms?

A symptomatic contact should report the symptoms immediately through the channel provided by the response team. The team arranges prompt assessment and referral under the applicable national or outbreak protocol, using trained personnel and the required infection-prevention measures. WHO’s 2015 Ebola contact-tracing guideline explains the urgency: “A person with EVD can spread the disease to others as soon as he or she begins to have symptoms; therefore, it is crucially important to identify and isolate symptomatic persons immediately to stop the disease from spreading.”

People being followed should not self-present to a crowded clinic without first using the instructed contact route when the local protocol provides one. That route allows responders to plan a safer assessment and protect health workers, other patients and transport staff.

5. Turn records into decisions

Case and contact records become useful when teams analyze them together. Laboratory results can confirm or exclude suspected cases; exposure dates establish monitoring windows; and linked records show where transmission is continuing. WHO’s 2026 guidance explicitly includes epidemiological analytics and digital data standards, while the DRC recommendations call for indicators that monitor contact-tracing performance.

Questions the analysis should answer

  • How quickly are alerts investigated and specimens collected?
  • How many suspected cases have a current status and laboratory result?
  • How complete is the contact list for each case, and how many contacts have a documented daily follow-up?
  • Where are contacts becoming unreachable or crossing administrative boundaries?
  • How many symptomatic contacts were assessed promptly, and were they linked to a case or transmission chain?
  • Which locations, events or populations need additional investigation or community support?

These measures help managers assign teams and supplies. For example, a district with many listed contacts but a high proportion of missed follow-ups may need more field workers or a different communication route, while a cluster of alerts with delayed testing may require laboratory or transport support.

Community engagement is part of every stage

Community engagement is not a publicity step added after the technical work. WHO’s current framework places it throughout detection, investigation, referral, contact tracing and protective measures. Trusted local actors can help identify signals, explain why a responder is asking about activities, support a safe referral and make it more likely that contacts continue daily follow-up.

The DRC recommendations emphasize community networks and locally led, community-anchored response. This matters where conflict, displacement, population mobility or constrained access make repeated visits difficult. A technically complete form cannot compensate for a person who does not trust the team, cannot safely reach a phone, or has moved beyond the area of responsibility.

How to compare outbreak-response approaches

WHO’s framework does not endorse a consumer tracking product as a substitute for public-health work. A stronger response is the one that performs well on operational measures:

Measure What good performance looks like Warning sign
Speed Alerts are investigated, diagnosed and acted on quickly. Signals remain unresolved or specimens wait for transport.
Completeness Investigators find contacts across households, care settings, events and travel routes. New cases have no plausible exposure link or contacts are missing from records.
Continuity Every contact has a current status through the 21-day window, including after movement. Follow-up stops when a person changes district or phone number.
Analytical use Line lists, laboratory results and links guide where teams work next. Data are collected but not reviewed or reconciled.
Community access and trust People report illness, accept referral and can communicate with follow-up workers. Rumors, fear, insecurity or practical barriers delay reporting and care.

What the 2026 Democratic Republic of the Congo figures show

A WHO disease-outbreak report recorded 10,821 contacts identified and under follow-up in the Democratic Republic of the Congo as of 1 July 2026; 8,954 had been followed up. Those numbers are a dated situation snapshot, not a current count or a guarantee that every contact completed monitoring.

The same report described surveillance, testing and diagnostic capacity being scaled up while conflict, displacement, population mobility and constrained access complicated operations. It also emphasized a locally led, community-anchored response. The example illustrates why the size of a contact list alone cannot measure success: teams must also know whether follow-up was timely, continuous and connected to assessment when symptoms appeared.

Limits of a single global procedure

The WHO guidance applies across Ebola and Marburg virus disease but does not make every outbreak identical. National authorities determine the legally applicable case definitions, reporting channels, clinical pathways, data-governance rules and movement or gathering measures. Temporary recommendations may also be specific to an outbreak and affected countries, as with the 24 August 2026 Bundibugyo virus disease recommendations.

For readers who are contacted by a response team, the safest practical rule is to cooperate with the assigned follow-up process, provide accurate exposure and location information, monitor as instructed for the full period counted from the last known exposure, and report symptoms immediately through the supplied channel. Public-health teams use that timely information to assess the person, protect others and update the wider transmission picture.
