Pharmaceutical companies can reduce China-linked supply risk without stopping validated production by working in sequence: map dependencies below the finished-drug supplier, rank the nodes that could threaten patients or release schedules, qualify alternate sources before they are needed, and add carefully sized inventory, capacity and response options. The objective is controlled resilience—not an abrupt relocation of every operation.
China exposure often sits upstream in active pharmaceutical ingredients (APIs), key starting materials (KSMs), excipients, packaging inputs or logistics. A manufacturer may have a non-Chinese contract manufacturer yet still depend on a single Chinese KSM supplier. Finding and qualifying those hidden dependencies while routine supply continues is the central risk-reduction task.
What “China supply-chain risk” actually includes
Country of origin is only one variable. A useful assessment asks what each material does, how many qualified sources exist, how quickly a replacement could be approved, and whether losing the input would stop manufacture or delay release. Risks can arise from a single API route, a KSM made at one site, a critical excipient, a shared utility, a port or freight lane, or a supplier whose quality status is uncertain.

They do not establish a current percentage for China’s share of all U.S. medicines or APIs. Any company-wide statement should therefore identify the product, material tier, geography, time period and denominator rather than cite a broad national percentage.

Why the named finished-product supplier is not enough
Supplier questionnaires and audits often stop at the company that makes the finished dose. That can miss an API manufacturer, KSM producer or excipient distributor several tiers upstream. Trace each critical input to the manufacturing site where feasible, and record intermediaries, transport routes and storage locations. The map should show where a disruption would first appear and how long it would take to affect release-ready product.
Separate concentration risk from quality risk
A material can be low-risk from a geographic perspective but high-risk because of poor deviation control or weak testing. Conversely, a China-based source may be technically reliable yet create a severe continuity problem if it is the only qualified source. Score geographic concentration, supplier quality history, substitutability and recovery time separately before combining them into a priority.
Step 1: Build a material-level dependency map
Create a product-by-product bill of critical inputs that extends beyond the finished dosage form. Include the API, KSMs, critical excipients, processing aids, primary packaging where it affects sterility or stability, and manufacturing and logistics sites. Link each item to its approved supplier, actual manufacturing location and any known sub-tier source.

- Material name, specification, grade and the products that use it.
- Supplier legal entity, manufacturing site and distribution path.
- Country and region for each production and transit step.
- Number of approved and technically usable sources.
- Lead time, minimum order, transport mode and storage constraints.
- Available inventory, expiry or retest limits and current demand rate.
- Change-control, comparability, validation and regulatory-file requirements for a switch.
- Inspection history, major deviations, recalls, import alerts or other quality signals.
Ask suppliers to disclose relevant sub-tier dependencies and to notify the company of site, process or ownership changes. Information may remain incomplete; mark confidence levels and create actions to close the most consequential gaps rather than treating an unknown as “low risk.”
Identify chokepoints
Flag sole-source materials, shared sites serving several products, long transfer routes, inputs with no practical substitute, and suppliers whose capacity is already committed. A “second supplier” that buys the same KSM from the same upstream factory does not remove the underlying chokepoint. Trace common sub-suppliers and utilities before counting sources as independent.
Step 2: Rank risks by patient and production impact
Use a documented quality-risk-management process aligned with ICH Q9 principles. FDA’s May 2022 Risk Management Plans to Mitigate the Potential for Drug Shortages guidance describes a proactive framework for identifying, prioritizing and mitigating hazards; it is draft, non-binding and marked “Not for implementation. Contains non-binding recommendations.” It does not prescribe one scoring formula.

A practical scoring model
Rate each dependency against five questions, using definitions approved by quality, manufacturing, regulatory and supply-chain leaders:
The Tool Desk
Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →- Likelihood: How plausible is a supply, quality, geopolitical or logistics failure?
- Impact: Which patients, markets, batches or release commitments would be affected?
- Detectability: How early would warning signals appear, and can the company verify them?
- Time to recovery: How long would it take to obtain conforming material and release it?
- Alternatives: Is a qualified source or approved process already available?
Document the owner, early-warning indicators, escalation trigger, contingency action and review date for every high-priority item. Useful indicators include missed supplier milestones, worsening right-first-time performance, unusual test failures, capacity reductions, freight cancellations, regulatory restrictions and changes in a supplier’s inspection status. The score should drive action, not become a static dashboard.
Step 3: Qualify alternate suppliers before a disruption
Diversification only improves resilience when an alternate source can legally and technically supply conforming material. FDA-hosted remarks on supply-chain vulnerabilities emphasize that component substitutions are complex and cannot easily be made. A source still undergoing testing, validation or filing work is an option under development—not interchangeable production capacity.
Qualification workstream
- Quality assessment: Review the supplier’s pharmaceutical quality system, GMP history, data integrity controls, change notification practice, laboratory capability and capacity evidence.
- Material comparability: Define critical quality attributes, impurity profiles, particle or physical properties and analytical methods. Test representative lots against the approved source.
- Process and facility assessment: Determine whether the material behaves equivalently in the drug-product process. Run engineering, demonstration or validation batches as applicable.
- Regulatory assessment: Identify affected applications, master files, specifications, stability commitments and filing or notification requirements in every market supplied.
- Approval and control: Complete quality-system approvals, update procedures and establish ongoing monitoring before routine use.
Plan qualification while the incumbent source is performing normally. That preserves time for failed comparability studies, capacity corrections or regulatory questions without forcing an emergency switch during a shortage.
Design for genuine independence
Compare the alternate’s upstream materials, utilities, workforce dependencies and transport lanes. Two suppliers in different countries may still share one KSM producer or one specialized manufacturing step. Independence should be assessed at the chokepoint level, not inferred from supplier names or corporate addresses.

Step 4: Balance inventory, capacity and shelf life
Inventory can bridge a temporary interruption, but it is not a substitute for qualification. Set buffers using product criticality, demand variability, replenishment time, shelf life, storage cost, expiry risk and the time required to transfer or approve a source. FDA materials do not establish a universal number of months of inventory; a historical observation from 2020 should not be presented as a current benchmark.

Use layered protection
- Physical stock: Hold additional API, KSM or finished-product inventory where stability and release controls permit.
- Reserved capacity: Contract for capacity at a qualified site or maintain an approved site that can run when needed.
- Multiple qualified sources: Keep more than one source active enough to retain technical knowledge and supply history.
- Flexible allocation: Define how limited material will be allocated among products, markets and patient groups.
Model the time from a supplier stoppage to the first missed batch, then compare that window with available stock and the fastest credible recovery path. Include testing, quarantine, quality release and transportation in the calculation; “days of supply” based only on warehouse quantity can be misleading.
Step 5: Add manufacturing and network flexibility
FDA’s final March 2023 Q13 guidance provides scientific and regulatory considerations for continuous manufacturing of drug substances and drug products, including development, implementation, operation and lifecycle management. Continuous manufacturing can reduce some transfer or scale constraints, but it does not by itself remove upstream API or KSM dependence. Its value depends on process suitability, facility design, control strategy, economics and the company’s ability to qualify the technology.
Evaluate flexibility options against the real bottleneck
| Option | Risk it can reduce | Readiness work and limits |
|---|---|---|
| Qualified second supplier | Single-source or site concentration | GMP assessment, comparability, validation and regulatory work; may share upstream inputs. |
| Additional inventory | Short interruptions and transport delays | Uses cash and storage; constrained by shelf life, stability and expiry. |
| Alternate manufacturing site | Loss of a plant, region or utility | Technology transfer, validation, inspection and filings can take substantial time. |
| Reserved external capacity | Surges or temporary internal outages | Requires contracts, scheduling and quality oversight; capacity may not be immediately available. |
| Continuous manufacturing | Some batch, scale and process constraints | Requires a suitable process and Q13-aligned control and lifecycle strategy; does not eliminate material sourcing risk. |
FDA’s supply-chain review associated with Executive Order 14017 also identifies broad resilience pillars: strengthening quality-management maturity, supporting innovative manufacturing, improving data use, fostering international cooperation and boosting local production. These pillars support a diversified network; they do not imply that moving every step to one replacement country removes risk.

Step 6: Prepare notification and disruption response
Resilience depends on information and decisions as much as on physical supply. FDA’s February 2024 draft guidance on notifying the agency about certain discontinuances or manufacturing interruptions proposes timely, informative notifications for affected finished products, biologics and APIs. It is draft Level 2 guidance and not for implementation, so companies must check the current statutory and regulatory duties that apply to their product and situation.
Build a response playbook
- Name decision owners for supply, quality, regulatory, medical, commercial and communications functions.
- Maintain current contacts for suppliers, contract manufacturers, testing laboratories, logistics providers and regulators.
- Define evidence needed to release material from an alternate source or use a substitute process.
- Set allocation rules that prioritize patient need and prevent inconsistent market decisions.
- Prepare internal and external messages that distinguish confirmed facts, assumptions and expected dates.
- Record decisions, deviations, temporary controls and post-event corrective actions.
The FDA APEC supply-chain toolkit covers the product lifecycle from raw materials to patients and includes quality practices, surveillance, track-and-trace, prevention, detection and incident response. Use those practices to test whether a company can detect a problem early, locate affected lots and coordinate a controlled response.
How to compare resilience investments

- Risk reduction: Does it remove the actual single-source or geographic chokepoint?
- Quality and regulatory readiness: What qualification, validation, comparability and filing work remains?
- Time to usable supply: How quickly can conforming, releasable material arrive after a disruption?
- Continuity and scale: Can the option maintain output during qualification and expand if demand rises?
- Economics: What are carrying costs, reserved-capacity fees, transfer costs, testing costs and expiry losses?
- Residual concentration: Does the project shift dependence to another country, utility, sub-tier supplier or freight lane?
Use a cross-functional review rather than a procurement-only decision. The cheapest alternate may have the longest approval path; the fastest buffer may expire before it is used; and a technically sound source may not be approved for every market.

What the FDA statistics do—and do not—show
FDA’s Report on the State of Pharmaceutical Quality FY2024, published in 2025, reports a narrowly defined supplier subgroup covering FY2020–FY2024. In that analysis, 72% of API manufacturing sites subject to the specified regulatory actions exclusively supplied compounding pharmacies. Those sites represented 18% of API manufacturers in FDA’s Site Catalog. Within the violative sites in that subgroup, 51% were located in China and 30% in India.
These figures are not China’s share of all U.S. API production, all finished medicines or every pharmaceutical supplier. They describe a particular set of API sites and a particular regulatory-action analysis. The same report says 40% of quality-related import alerts were based on section 704(a)(4) record requests to sites that FDA had not inspected; that is a surveillance statistic, not a China-specific sourcing measure.
FDA urges compounders to know their bulk-drug-substance and excipient suppliers and whether those suppliers test components for appropriate quality. The principle applies broadly: visibility and verification are prerequisites for sensible diversification.
Common mistakes that create disruption
Switching after the shortage starts
Emergency sourcing compresses supplier audits, comparability work, regulatory review and logistics into the period when every function is under pressure. Develop and maintain alternatives during normal operations.
Counting an unqualified source as available
A supplier with a quotation or memorandum of understanding cannot replace an approved source until applicable GMP, technical and regulatory requirements are complete.

Using country alone as the risk score
Country flags can hide a shared upstream factory, or unfairly prioritize a source that has strong quality and multiple routes. Assess concentration, quality, substitutability and recovery time together.
Buying inventory without a release plan
Stock that has not been tested, quarantined, stored correctly or assigned a release pathway may not protect patients. Include quality-release timing and expiry in the buffer calculation.
Assuming a new country equals diversification
Moving production can transfer dependence to another single KSM, utility or shipping lane. Re-map the full chain after every network change.
A governance cycle that keeps resilience current
Assign an accountable executive, a quality-system owner and a cross-functional review team. Revisit the dependency map after supplier changes, process changes, inspections, demand shifts and significant geopolitical or logistics events. Track leading indicators, test response plans and document whether each mitigation is qualified, in progress or merely conceptual.
The practical test is simple: if a critical China-linked node failed tomorrow, could the company identify affected products, estimate the time to shortage, activate a qualified option, meet release requirements and communicate accurately? If not, the next investment should close that specific information or qualification gap before adding more general inventory.
