Osteoarthritis (OA) is often diagnosed from your symptoms, medical history and a physical examination. A scan is not routinely needed when an adult has a typical pattern of joint pain. In UK guidance, the National Institute for Health and Care Excellence (NICE) says a clinician can diagnose OA in someone aged 45 or older who has activity-related joint pain and either no morning stiffness or stiffness lasting no more than 30 minutes. Tests may be considered when symptoms are unusual or another condition is possible; there is no single test that confirms OA in every case.
How clinicians diagnose osteoarthritis
OA is a condition affecting joints. Its diagnosis is usually based on how symptoms behave, the history of the problem and what a clinician finds when examining the affected joint. NICE guideline NG226, published on 19 October 2022, recommends that clinicians in the UK do not routinely use imaging to diagnose OA unless there are atypical features or signs that suggest another or an additional diagnosis.
For adults aged 45 or older, NICE describes a typical clinical pattern as joint pain related to activity, with either no morning joint-related stiffness or stiffness lasting no more than 30 minutes. These are diagnostic criteria, not a guarantee that a person has OA: the clinician still considers the whole presentation and whether another explanation fits better. NICE guidance covers people over 16, but the age-45 criterion is specific to this clinical pattern.
There is no universal test sequence. The US National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS) likewise explains that there is no single test for OA. Depending on the symptoms and examination, a clinician may make a diagnosis without imaging or may use imaging, blood tests or a joint-fluid sample to assess the joint or investigate another cause. Those options are not a package that everyone needs.

What to expect at an appointment
Questions about symptoms and daily activities
A clinician may ask where the pain is, when it began, how it has changed and which activities make it better or worse. They may ask how the problem affects function—for example, which everyday tasks have become difficult—and whether there is stiffness in the morning or at other times. The aim is to understand the pattern and impact of the symptoms, not simply to match one symptom to one scan result.
Relevant medical history
Expect questions about other health conditions and medicines that could matter when assessing joint pain. A clinician may also ask whether joint problems run in your family. NIAMS includes medical history as part of OA assessment. There is no single standard questionnaire or fixed appointment script established by the cited guidance, so the questions and their order can vary.
Examination of the joint
The clinician may assess your general health and examine the affected joint. What they check depends on the joint and symptoms. History and examination help determine whether the presentation is typical for OA, whether further investigation is warranted and whether a different or additional diagnosis needs consideration.

It can help to describe what you can and cannot do, when symptoms occur and how long any morning stiffness lasts. This is useful information for the assessment; it does not replace the examination or establish a diagnosis on its own.
Which tests might be used—and what they show
Each test answers a different question. Imaging can show structural features, while blood work or fluid analysis may help investigate another explanation for pain or inflammation. A clinician chooses among them based on whether the result is likely to clarify the diagnosis or affect management; they are not interchangeable options for patients to select themselves.

X-rays: structural changes, not a direct view of cartilage
An X-ray can provide information about bones and the space between them. Narrowing of that space may indicate cartilage loss, but cartilage itself is not directly visible on an X-ray. A normal-looking X-ray also does not rule out early joint damage, because early changes may not appear. These limits are one reason clinicians interpret images alongside symptoms and examination rather than treating a scan as the whole diagnosis.

MRI and ultrasound: more detail is not automatically more useful
MRI can show soft tissues around a joint, which may be useful for a particular question—for example, when a joint locks or gives way. Ultrasound can show some tissue changes, including fluid and synovial changes. Neither fact means every person with joint pain needs one of these scans. Under NICE guidance, imaging is not routine for a typical OA diagnosis; the relevant question is whether an atypical feature or suspected alternative diagnosis makes imaging useful.
Blood work and fluid sampling: investigating other causes
Blood tests do not confirm OA. A clinician may order them when the symptoms or examination raise a question about another cause of pain or inflammation. Joint aspiration involves drawing fluid from the joint so it can be analysed; this can help investigate conditions such as gout or infection. These tests serve a different purpose from imaging and should not be interpreted as proof that OA itself has a specific laboratory marker.
When a clinician may recommend further testing
NICE advises against routine imaging for a typical presentation, but says imaging may be appropriate when features are atypical or suggest another or additional diagnosis. The guideline’s rationale gives inflammatory arthritis, such as rheumatoid arthritis, and malignancy as examples of possible alternatives clinicians may need to consider. The point is not that these conditions are likely whenever someone has joint pain; rather, an unusual presentation can make it important to investigate beyond the usual OA pattern.

Further assessment may also be considered when the clinical picture leaves an unanswered question. The type of test depends on that question: an X-ray can provide structural information, MRI can show soft tissues, and blood or joint-fluid tests can help investigate other causes. The clinician’s judgment about the joint, symptoms and examination guides the choice. The available guidance does not establish a universal rule for which scan or laboratory test a particular person will receive.

Not receiving a scan does not mean your symptoms have been dismissed. For the typical adult presentation described by NICE, a clinical assessment can be sufficient to diagnose OA. NICE’s rationale says its committee found no evidence that imaging benefits routine OA diagnosis and considered history plus examination sufficient for typical cases. Imaging can reveal structural changes, but a result is useful only if it helps answer a clinical question or changes what happens next.

Early changes may not show on X-ray, and the image has to be considered alongside the person’s symptoms and examination. If the presentation suggests another diagnosis, tests may be chosen to explore that possibility rather than to confirm OA.

Imaging during follow-up or when surgery is considered
NICE recommends against routine imaging for follow-up and non-surgical management of OA. Its rationale notes that imaging usually does not guide a person’s response to non-surgical treatment. Symptoms and their effect on function remain important to clinical review; repeating a scan simply as a routine check is not recommended by this guidance.
Imaging may be relevant when surgery is being considered or planned, but that does not mean every surgical referral requires a particular scan. Local pathways and clinicians differ, and the appropriate imaging depends on the case and the decision being made. The cited guidance does not specify one scan that every person must have before surgery.
Questions to ask if tests are discussed
If a clinician suggests a test—or explains why one is not needed—these questions can make the purpose clearer:

- What question is this test intended to answer?
- Is it looking for OA-related structural change, or checking for another cause of my symptoms?
- How could the result change the diagnosis or the next step in my care?
- If the result is normal, what would that mean given that some early changes may not show on an X-ray?
- Is this test being considered because my symptoms are atypical, or because of a follow-up or surgical decision?
These questions do not prescribe a test or predict what a clinician will order. They help distinguish tests that investigate different possibilities and clarify how a result will be used.
How guidance varies by location
The age-and-symptom criteria and recommendation against routine imaging described here come from NICE NG226, UK guidance published on 19 October 2022. They should not be assumed to represent an identical pathway in every health system. NIAMS is a US government source describing the components a clinician may use, while the Arthritis Foundation provides practical descriptions of tests. Across these sources, the central distinction is consistent: symptoms and examination can support diagnosis, and tests may be used selectively to assess the joint or investigate another cause. The exact decision for an individual depends on the clinical assessment and local practice.
