Oncogenic proteins are altered or overactive proteins that keep cancer-driving signals switched on. They can tell a cell to divide, avoid death, migrate, build a blood supply, or resist treatment when those instructions should be off. Targeted drugs work by matching a treatment to a specific protein or molecular alteration: small-molecule inhibitors can enter cells, antibodies can bind targets on the cell surface or nearby tissue, and antibody–drug conjugates can carry a toxic payload to cells bearing a marker. The match is useful only when the tumor depends on that target, the drug can reach it, and the cancer has not developed a way around the blockade.
What is an oncogenic protein?
An oncogenic protein is a protein whose abnormal activity contributes to cancer. A gene may be mutated, copied too many times, or fused to another gene after a chromosomal rearrangement. The resulting protein can be permanently active, produced in excessive amounts, located in the wrong cellular compartment, or newly connected to a signaling partner.
Normal cells use signaling networks to coordinate growth, division, survival, movement, and communication with neighboring tissue. These signals are normally turned on and off in response to conditions such as nutrients, tissue damage, or developmental instructions. An oncogenic change can make the signal autonomous. The cell behaves as though growth factors are always present, death signals do not matter, or a route for invasion is permanently available.
The National Cancer Institute describes targeted therapy as treatment aimed at proteins that control how cancer cells grow, divide, and spread. Blocking a critical abnormal protein can slow uncontrolled growth, but the result depends on the cancer type, the exact alteration, previous treatment, and whether other pathways can substitute for the blocked one.

How an altered protein drives a tumor
- Persistent growth signaling: a receptor or downstream signaling protein remains active without its normal stimulus.
- Excess protein dosage: gene amplification produces too much of a growth-promoting receptor, increasing the cell’s response to ordinary signals.
- New fusion activity: a chromosomal rearrangement joins parts of two genes, creating a hybrid protein with abnormal signaling behavior.
- Failure of braking systems: a mutation can weaken a protein that normally restrains division or promotes controlled cell death.
- Support for spread: altered signaling can help cells migrate, invade surrounding tissue, or encourage blood-vessel formation.
These effects are molecular drivers, not diagnoses by themselves. A mutation found in a tumor does not automatically prove that a particular drug will work, because the alteration may be incidental, the protein may be unreachable, or no approved treatment may exist for that cancer context.
How targeted drugs interrupt cancer signaling
Targeted treatment begins with a dependency: the tumor must rely on a protein or pathway that can be inhibited. The drug then interferes with that dependency in a way suited to the target’s location and structure.
Small-molecule inhibitors
Small molecules can enter cells and reach intracellular enzymes or signaling proteins. Many inhibit kinases, enzymes that add phosphate groups to other proteins and thereby transmit growth signals. By occupying an active site or another critical pocket, a small molecule can reduce signaling through the abnormal protein. Because these medicines circulate through the body and can enter cells, their effects and toxicities are not limited to the tumor.
Monoclonal antibodies
Monoclonal antibodies are engineered proteins that bind a defined target, usually on the outside of a cancer cell or in its surrounding environment. Binding may block a receptor’s interaction with growth factors, mark a cell for immune attack, or interfere with a signal that supports blood-vessel formation. Antibodies generally do not cross cell membranes in the way small molecules do, so the target’s location matters.
Antibody–drug conjugates
An antibody–drug conjugate combines target recognition with a toxic payload. The antibody binds a marker on the cancer cell, and the conjugate is internalized or otherwise releases its payload near the target cell. This approach can concentrate a cytotoxic agent in cells carrying the marker, but it can still damage normal tissues that express the same target or are exposed to the released payload.

Other ways a target can be blocked
Targeted drugs may inhibit an abnormal growth signal, block angiogenesis, help the immune system recognize cancer cells, or interrupt a downstream pathway. The practical effect is not determined by the protein name alone. The same protein can have different clinical meaning in different tumors, and a drug approved for one alteration or cancer type is not automatically interchangeable with another drug that acts on the same pathway.
Mutation, amplification, fusion, and protein target: what is the difference?
These terms describe different levels of the biology.

|---|---|---|---|
| Mutation | A change in the DNA sequence of a gene. | EGFR mutations or the KRAS G12C substitution. | The change may alter the protein’s shape or activity and create a drug-sensitive site. |
| Amplification | Extra copies of a gene, often producing excess protein. | HER2/ERBB2 amplification in a molecular subtype of breast cancer. | High surface-protein abundance may make antibody binding or another targeted approach useful. |
| Fusion | A rearrangement joins parts of two genes. | The BCR-ABL fusion protein or ROS1 rearrangements. | The hybrid protein can acquire continuous kinase activity or a new regulatory context. |
| Protein target | The molecule a drug is designed to inhibit, bind, or use as a delivery address. | BCR-ABL, HER2, EGFR, KRAS G12C, or ROS1. | A target is actionable only if the tumor depends on it, the drug reaches it, and the indication supports its use. |
A DNA test may identify a mutation, while an assay may measure protein abundance or activation. Those results answer related but different questions. A tumor can carry a gene alteration without producing enough abnormal protein to be clinically important, and protein expression alone may not reveal the exact DNA change that determines drug sensitivity.
Examples of oncogenic proteins and matched drugs
The following examples show why treatment decisions require the alteration, tumor type, test, and regulatory indication together.
| Target and alteration | Where the target acts | Illustrative targeted drugs | Clinical context |
|---|---|---|---|
| BCR-ABL fusion protein | Intracellular tyrosine kinase created by a characteristic chromosomal rearrangement. | Imatinib. | Leukemia driven by the BCR-ABL fusion. |
| HER2/ERBB2 | Cell-surface receptor tyrosine-kinase pathway. | Trastuzumab and other HER2-directed treatments. | HER2-defined breast cancer and selected HER2-altered non-small-cell lung cancer (NSCLC) and other cancers. |
| EGFR mutations | Cell-surface receptor whose abnormal activation sends growth signals inward. | Erlotinib and gefitinib. | A subset of lung cancers with activating EGFR mutations. |
| KRAS G12C | Intracellular signaling protein with a specific amino-acid substitution. | Sotorasib and adagrasib. | Biomarker-selected NSCLC; the exact indication and treatment line depend on the current label. |
| ROS1 rearrangements | Fusion-driven receptor tyrosine-kinase signaling. | Crizotinib, entrectinib, and repotrectinib. | ROS1-positive NSCLC; ROS1 rearrangements occur in approximately 1% to 2% of NSCLC patients, according to the National Cancer Institute’s current PDQ page accessed in 2026. |
BCR-ABL: treating a fusion-created kinase
In BCR-ABL-positive leukemia, a chromosomal rearrangement creates a fusion protein with overactive tyrosine-kinase activity. Imatinib inhibits that kinase. This is a clear example of a fusion being both a biological driver and a treatment target: the clinically relevant question is not simply whether either gene is present, but whether the tumor contains the disease-defining fusion.

HER2: receptor abundance and altered HER2 biology
HER2-targeted treatment is used in molecularly defined settings, including a subtype of breast cancer and selected HER2-altered NSCLC. HER2 status can reflect amplification, overexpression, or a sequence variant, and those categories are not interchangeable. In an NCI-cited study of trastuzumab deruxtecan for HER2-variant NSCLC, 91 treated patients had a confirmed objective response rate of 55%; median progression-free survival was 8.2 months and median overall survival was 17.8 months. Those figures describe that study population and regimen, not every person with a HER2 result.

EGFR: a receptor activated by a mutation
Some lung cancers carry EGFR mutations that make the receptor signal abnormally. Erlotinib and gefitinib are examples of drugs designed to restrict that activation. A result showing an EGFR alteration must still be interpreted by mutation subtype, cancer type, prior treatment, and the indication in force when treatment is considered.
KRAS G12C: a mutation-specific inhibitor
KRAS is an intracellular signaling protein. The G12C alteration creates a specific chemical feature that can be targeted by drugs such as sotorasib and adagrasib in selected NSCLC settings. In the NCI’s current NSCLC PDQ page accessed in 2026, a phase II sotorasib study reported a 37.1% overall response rate among 124 evaluable patients, median progression-free survival of 6.8 months, and median overall survival of 12.5 months. These are study-level outcomes; they are not a prediction for an individual patient.
ROS1: a fusion-driven target in a small NSCLC subgroup
ROS1 rearrangements create an abnormal signaling protein in approximately 1% to 2% of NSCLC patients. NCI cites crizotinib, entrectinib, and repotrectinib as FDA-approved options in ROS1-positive disease. Testing must establish the rearrangement with a clinically appropriate assay, and the choice among options depends on the current indication, disease sites, prior therapy, and resistance profile.
How biomarker matching works
Biomarker testing examines genes, proteins, or other tumor features that can guide treatment. The sample may come from a tumor biopsy or, in some circumstances, blood. The purpose is to connect a measurable feature with a therapy that has evidence and a regulatory pathway for the patient’s cancer.

- Define the clinical question. The oncology team identifies the cancer type, stage, prior treatments, and whether tissue is available.
- Select an assay. Testing may look for DNA mutations, gene fusions, copy-number changes, RNA signals, protein expression, or several categories at once. NCI identifies Guardant360 CDx and FoundationOne Liquid CDx as FDA-approved liquid-biopsy tests.
- Interpret the result in context. A report may identify an FDA-approved treatment, an off-label option, a clinical trial, a resistance alteration, or a finding with no currently useful match.
- Verify the indication. The target, alteration, cancer type, treatment line, test requirement, and approval status must align with the current FDA label or trial protocol.
- Reassess when the disease changes. A resistant tumor may acquire a new alteration or stop depending on the original target, so a repeat biopsy or liquid biopsy may be considered by the treating team.
A “positive” biomarker is therefore not the same as a guaranteed response. Testing can miss an alteration if the sample is inadequate, and a detected alteration may not have an available drug. Conversely, a liquid biopsy that is negative may require careful interpretation because a blood sample can contain too little tumor DNA to exclude a tumor alteration.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. Why targeted therapy can stop working
Resistance means that cancer grows despite a drug that previously controlled it. It can be present before treatment in a small subpopulation or emerge during therapy as cells adapt.
Changes in the target
A second mutation can alter the drug-binding site or change the target’s shape. The original inhibitor may no longer bind effectively even though the oncogenic protein is still present.
Bypassing the blocked pathway
Cancer cells can activate a parallel signaling route, amplify another growth driver, or restore downstream signaling. The tumor then receives a similar instruction without relying on the inhibited step.

Drug exposure and tumor location
Insufficient drug concentration, altered drug metabolism, or a sanctuary site such as the central nervous system can allow resistant cells to survive. These issues are distinct from a target mutation and may require a different strategy.
When progression is suspected, clinicians interpret scans, symptoms, treatment exposure, and sometimes a new molecular test together. A single resistance result does not automatically identify the next treatment; the available options depend on the cancer, prior drugs, organ function, safety considerations, and current approvals or trials.
Why targeted therapy is not a universal cure
- Not every tumor has an actionable alteration. Testing may find no matching drug, or only an off-label option or a clinical trial.
- A target may not be the dominant driver. Tumors contain multiple interacting pathways, and blocking one may leave another intact.
- Responses vary by cancer context. The same protein name can represent different biology in different organs or alteration types.
- Resistance is expected in some settings. Initial control can be followed by progression through target changes, bypass signaling, or loss of dependence.
- Toxicity limits treatment. Normal tissues may use the targeted pathway or express the target, and antibody or payload exposure can cause significant adverse effects.
- Approvals change. FDA notices in 2026 include molecularly defined oncology approvals or accelerated approvals involving RAS-family inhibition, HER2 alterations, and ESR1-mutated disease. The current label and date must be checked for any specific claim.
