Environmental noise can disturb sleep, interfere with cognitive performance, and contribute to health problems beyond hearing loss. The effects depend on the source, timing, exposure pattern, and the person affected; a brief sound or a single decibel reading cannot predict an individual health outcome. The World Health Organization’s noise guidance and burden estimates focus chiefly on the WHO European Region, so its figures should not be read as global counts or personal risk forecasts.

Noise can affect more than hearing

Noise is not only a hearing-protection issue. The World Health Organization (WHO) identifies sleep disturbance, annoyance, cognitive impairment, reduced performance, cardiovascular and metabolic effects, and hearing impairment among the health concerns associated with excessive environmental noise. These outcomes do not all arise in the same way, and they do not affect every exposed person equally. WHO describes evidence and population-level patterns, not a formula that turns one sound level into an individual diagnosis.

Environmental noise includes more than traffic. WHO’s European guidance considers road traffic, railways, aircraft, wind turbines, and leisure activities as separate source categories; industry is also among the sources covered by WHO’s broader noise materials. Sources differ in when and how they are heard: a relatively steady sound, repeated events, and a nighttime interruption are not interchangeable exposures. WHO/Europe reports that approximately 100 million people—about one in five EU residents—are exposed to unhealthy road-traffic noise levels. It also estimates that noise from road, rail, and air transport accounts for at least 392,000 healthy years of life lost annually in the EU. These are regional estimates, not worldwide totals or a measure of any one person’s risk. [WHO/Europe overview]

Why nighttime noise matters for sleep

Noise can disrupt sleep, making sleep disturbance a major outcome in WHO’s assessment of environmental noise. Nighttime exposure can matter even when a person is not consciously awake for every sound: the relevant public-health question is not simply whether a sound was noticed, but how exposure relates to sleep and health across a population. The available guidance does not mean every interruption has the same effect, or that a particular sound level guarantees poor sleep for an individual.

WHO’s 2009 Night Noise Guidelines for Europe use Lnight, outside, an outdoor annual-average nighttime exposure indicator. The guideline sets 40 dB as a target to protect the public, including vulnerable groups, and 55 dB as an interim target where the main target cannot be achieved in the short term. These values belong to a specific outdoor, long-term measurement framework. They are not readings for a bedside sound meter, indoor bedroom limits, or personal thresholds that predict whether someone will sleep well on a given night. [WHO Night noise guidelines for Europe]

A separate WHO/Europe fact sheet from 2011 says community-noise guidance recommends less than 30 dB(A) in bedrooms at night for good-quality sleep. This is an indoor bedroom figure and should not be compared as though it were the same metric or setting as outdoor annual-average Lnight. Neither number replaces applicable local rules. Sleep disruption can also be relevant to people whose schedules do not follow a conventional day-night pattern. WHO identifies shift workers as a group who may be more affected by noise. A neighborhood’s annual nighttime average describes the exposure framework used by the guideline; it does not, on its own, describe a particular worker’s sleep period or personal circumstances.

What noise can do to concentration and learning

Noise can interfere with cognitive performance and reduce performance, but the evidence needs to be described with its population and scope intact. WHO’s 2011 fact sheet reports consistent evidence that noise exposure harms children’s cognitive performance. This supports concern about children’s learning and cognitive functioning; it does not establish that every child, every sound, or every short exposure causes lasting harm. [WHO/Europe noise fact sheet]

WHO’s European burden assessment estimated 45,000 disability-adjusted life years (DALYs) from cognitive impairment in children in EU Member States and other western European countries. DALYs combine years of life lost and years lived with disability in a population-level burden measure. This 2011 estimate is not a count of children or a forecast of an individual child’s outcome. It was based on evidence available for that assessment and on the report’s stated geography and assumptions. [WHO/Europe, Burden of disease from environmental noise, 17 October 2011]

For classrooms, the same 2011 WHO/Europe fact sheet gives a separate recommendation: less than 35 dB(A) for teaching and learning. It is a classroom setting value, not the outdoor nighttime metric and not a universal concentration cutoff for every setting. The distinction matters: a classroom recommendation and a neighborhood’s annual average answer different questions.

Other health outcomes and what the burden figures mean

WHO’s European assessment quantified several health outcomes associated with environmental noise. For EU Member States and other western European countries, its 2011 report estimated 903,000 DALYs attributable to sleep disturbance, 61,000 from ischaemic heart disease, 45,000 from cognitive impairment in children, 22,000 from tinnitus, and 587,000 from annoyance. The report described the estimates as using conservative assumptions. They are historical regional burden estimates using evidence available at the time, not current global totals and not individual predictions. [WHO/Europe burden of disease report, 2011]

These categories show why noise is considered a public-health issue even when it does not cause obvious hearing damage. Sleep disturbance, annoyance, cardiovascular disease, tinnitus, and children’s cognitive impairment are distinct outcomes; the numbers should not be added or interpreted as though they counted the same kind of event. A DALY is a health-burden measure, not a tally of exposed people.

WHO’s work on measurement has continued since that report. In 2024, WHO/Europe described 82 new or updated disability weights for noise-related health states, including sleep disturbance and annoyance, derived from public judgments. Disability weights are assessment parameters used to quantify health burden; they are not a count of people affected. The updated methods signal ongoing work on how to assess burden, but they do not make an older estimate current without its original date, geography, and method. [WHO/Europe, 4 August 2024]

How to interpret noise guidance and measurements

Decibels are not a standalone health verdict. The WHO values cited here apply to different places, time periods, and purposes: outdoor annual-average nighttime exposure, bedrooms at night, and classrooms. A momentary reading cannot be substituted for an annual average, and a reading taken indoors cannot automatically be compared with an outdoor indicator.

Guidance value Setting and metric What it means
40 dB Outdoor annual-average nighttime exposure, Lnight, outside; WHO Night Noise Guidelines for Europe, 2009 Target intended to protect the public, including vulnerable groups.
55 dB Outdoor annual-average nighttime exposure, Lnight, outside; WHO Night Noise Guidelines for Europe, 2009 Interim target when the main target cannot be achieved in the short term.
Less than 30 dB(A) Bedroom during the night; stated in WHO/Europe’s 2011 fact sheet as community-noise guidance Recommendation for good-quality sleep; it is not the outdoor annual-average metric.
Less than 35 dB(A) Classroom; stated in WHO/Europe’s 2011 fact sheet Recommendation for teaching and learning.

The figures are dated WHO guidance for the European Region, not one universal personal safe-exposure threshold. Local standards may differ. To make a comparison meaningful, identify the sound source, whether exposure is daytime or nighttime, whether it is continuous or intermittent, where and over what period it was measured, and which outcome is being discussed. Those details determine whether two readings or situations are actually comparable. [WHO Night noise guidelines for Europe; WHO/Europe noise fact sheet]

Who may be more vulnerable to noise

Exposure and vulnerability are not distributed evenly. WHO identifies children, older people, people with chronic illness, and shift workers as groups who may be more affected. Its materials also note socioeconomic differences in harmful exposure. These are reasons to consider who is exposed and under what circumstances, rather than assuming that an average for a neighborhood describes everyone’s experience. [WHO/Europe overview; WHO/Europe noise fact sheet]

WHO’s 2018 European guidelines were underpinned by eight peer-reviewed systematic reviews. The reviews covered cardiovascular and metabolic effects, annoyance, sleep, cognitive impairment, hearing impairment and tinnitus, adverse birth outcomes, quality of life, mental health and well-being, and the effectiveness of interventions. The guideline addresses average exposure from road traffic, railways, aircraft, wind turbines, and leisure activities. This breadth reflects a developing evidence base; it does not mean that every listed outcome is equally established for every source or person. [WHO/Europe release, 10 October 2018; WHO Environmental noise guidelines for the European Region, 2018]

What the evidence can—and cannot—tell you

WHO’s evidence is useful for understanding population health, setting environmental guidance, and distinguishing types of exposure. It is not a personal medical assessment. Population-level associations and burden estimates do not establish that a particular sound caused an individual’s symptom, nor do they predict the result of a single night’s exposure.

When comparing two noise situations, keep five dimensions separate:

  • Source: road, rail, aircraft, wind turbine, leisure, or industry.
  • Time pattern: daytime or nighttime, steady or intermittent.
  • Measurement context: indoor bedroom or classroom versus an outdoor annual-average indicator.
  • Outcome: sleep, cognition, cardiovascular health, annoyance, hearing, or another assessed health state.
  • Population: the people exposed, including children, shift workers, older adults, and people with chronic illness.

Those distinctions prevent the most common misreadings: treating a long-term environmental average as a momentary reading, treating a bedroom figure as an outdoor limit, or treating a regional DALY estimate as an individual forecast. They also clarify why claims that one sound-level number guarantees safety or harm go beyond the guidance described here.
