Nerve-stimulation implants are clinician-managed devices for selected people whose chronic pain remains disabling despite appropriate nonsurgical treatment or whose medicines cause unacceptable effects. The best-known type, spinal cord stimulation (SCS), sends controlled electrical pulses through leads placed near the spinal cord. Peripheral nerve stimulation (PNS) places leads beside a particular nerve outside the brain and spinal cord. Neither is a universal treatment or a guaranteed cure; the appropriate system depends on the pain diagnosis, location, prior treatment, health factors, and the device’s regulatory labeling.
What “nerve-stimulation implant” means
The term covers several systems rather than one standard implant. Most discussions concern SCS, but PNS is an important alternative when a clinician can identify a suitable peripheral-nerve target. In both approaches, a pulse generator delivers programmed electrical stimulation through leads. The exact lead design, programming options, approved uses, MRI conditions, and follow-up requirements vary by system and country.
| Feature | Spinal cord stimulation (SCS) | Peripheral nerve stimulation (PNS) |
|---|---|---|
| Target | Leads are positioned in the epidural space near the spinal cord. | One or more leads are positioned near a selected peripheral nerve outside the brain and spinal cord. |
| Typical reason for considering it | Selected chronic, difficult-to-manage trunk, back, leg, or limb pain patterns, depending on the system’s labeling. | A pain pattern with a sufficiently well-defined nerve target; evidence differs by nerve and diagnosis. |
| Power and control | An implanted pulse generator sends pulses; an external controller may allow the patient and clinician to adjust permitted settings. | System-specific implanted or temporary components and external controls; configuration depends on the device. |
| Evidence | Guidelines and device studies support selected indications, but long-term response varies. | Evidence is condition- and target-specific and should not be generalized from one nerve to every PNS use. |
In the United States, FDA summaries describe systems such as Abbott SCS, Nevro Senza, Saluda Evoke, and Boston Scientific WaveWriter. A 2024 Abbott overview says, “The Abbott SCS System may help to treat chronic, difficult to manage pain of the torso, arms, and legs.” That wording applies to that labeled system, not to every stimulator or every pain condition. US PNS guidance discusses systems including Freedom, StimRouter, SPRINT, Nalu, and ReActiv8; availability and indications can change, so your team should verify current local labeling.
How spinal cord stimulation may reduce pain
During SCS, thin leads deliver programmed electrical pulses near the spinal cord. The implanted pulse generator is commonly placed under the skin near the hip or another location selected for comfort and safe operation. An external programmer lets the clinician configure stimulation, and some systems provide patient controls within limits set by the care team.

Clinicians and manufacturers often describe SCS as modulating pain signaling as it travels through the nervous system. The precise biology is not settled into one mechanism that explains every waveform, diagnosis, or patient response. Newer systems can use different stimulation modes, including settings intended to produce sensations that may or may not be felt. Whether a particular mode helps depends on the system, lead position, programming, and the individual’s condition.
SCS is not the same as a surface TENS unit. A TENS device places electrodes on the skin and does not involve implanted leads or a pulse generator. Do not substitute one for the other when discussing an implant recommendation.
How peripheral nerve stimulation differs
PNS aims at a named peripheral nerve associated with the painful area. The clinician identifies the nerve and places a lead close enough to deliver stimulation without directly damaging the nerve. A well-defined target can make PNS selective in some cases, but not every chronic pain diagnosis has a single nerve that can be safely or meaningfully targeted.

The 2024 American Society of Interventional Pain Physicians (ASIPP) guideline and the American Society of Pain and Neuroscience (ASPN) guideline describe PNS as a group of techniques rather than one uniform therapy. Results depend on the diagnosis, nerve, lead design, stimulation schedule, and study quality. Evidence for one indication is not proof that PNS will work for another.
Who may be evaluated for an implant
Implants are generally considered after a specialist has confirmed the pain diagnosis, addressed treatable causes, and tried reasonable alternatives. They are not automatically appropriate because pain is severe or has lasted a long time. Referral and selection decisions commonly consider:
- Pain type and distribution: neuropathic or neuropathic-like features, exact location, spread, intensity, triggers, and whether the pattern matches an SCS or PNS target.
- Previous treatment: medicines, physical rehabilitation, injections or surgery when appropriate, and the amount of relief or functional improvement each produced.
- Daily impact: sleep, walking, work or caregiving, mood, and activities you want to regain. Improvement in function may be a more useful goal than a zero-pain score.
- Medical and anatomical factors: infection risk, bleeding risk, spine or nerve anatomy, other implanted devices, medication use, and ability to attend programming and follow-up visits.
- Psychosocial factors: expectations, coping, untreated depression or other conditions, substance-use concerns, social support, and practical ability to protect the implant during recovery.
The 2023 American Society of Regional Anesthesia and Pain Medicine (ASRA) consensus guideline states: “All patients must be screened with an objective validated instrument for psychosocial factors, and this must include depression.” Screening is part of risk reduction and planning, not a judgment that pain is psychological. The European expert consensus likewise describes referral decisions as depending on pain characteristics, anatomy, prior treatment response, and psychosocial considerations. These criteria support a clinical evaluation; they are not a self-diagnosis checklist.

What happens during an SCS trial
A trial lets you and the clinical team test stimulation before deciding on a permanent generator and leads. Protocols differ by clinician, device, and country, but a traditional trial usually lasts several days.
- Planning and screening: The team confirms the diagnosis and target, reviews medicines and bleeding or infection risks, records baseline pain and functional goals, and completes psychosocial screening. You should agree in advance on what counts as a useful response, such as walking farther, sleeping better, reducing rescue medicine, or achieving a specified pain reduction.
- Lead placement: Temporary leads are inserted through the skin and positioned near the spinal cord under sterile conditions. Some protocols use surgically placed paddle leads instead; the clinician explains why one approach is suitable.
- External stimulation: The leads connect to an external pulse generator, rather than a fully implanted generator. A patient handbook for the WaveWriter system describes this arrangement with leads positioned along the spinal cord and an external trial stimulator.
- Daily observation: You record pain, activity, sleep, sensations, medication use, and any unwanted stimulation. Follow the team’s restrictions on bathing, bending, lifting, and lead protection; movement can change temporary lead position.
- Review: After the trial period, the team removes temporary equipment and compares your agreed goals with baseline. A strong trial response may support permanent implantation, while an inadequate or unclear response may lead to reprogramming, another evaluation, or stopping the process.
ASRA’s consensus guideline recommends a trial before definitive implantation for the patient groups it addresses, with an exception noted for some chronic anginal-pain cases. A successful trial improves the decision, but it cannot guarantee durable benefit: permanent lead position, healing, disease progression, programming, and everyday activity can all affect later results.
Permanent implantation and ongoing care
If the expected benefits outweigh the risks, the team implants the long-term leads and pulse generator. The generator location is chosen to protect it from pressure and make charging or control practical. You receive device-specific instructions about wound care, activity restrictions, charging, controller use, and when to report symptoms.

Follow-up is an active part of treatment. Programming may be adjusted as pain changes or as you learn which settings support walking, sleep, or other goals. Keep the device identification and emergency information supplied by the manufacturer. Tell every healthcare professional that you have an implant, especially before procedures, diathermy, electrosurgery, radiation treatment, or imaging.
Potential benefits—and what the evidence does not promise
Some patients obtain meaningful pain relief, better function, improved sleep, or lower reliance on rescue medication. The size and durability of improvement vary by diagnosis, target, device, programming, and patient. Pain relief is not assured, and an implant does not repair every underlying cause of pain.
The 2023 ASRA consensus guideline reports that nearly 30%–40% of patients describe inadequate relief beyond 24–36 months. This is a guideline-reported range, not an individual forecast and not a head-to-head comparison of all current devices.

A separate example comes from the FDA’s 2024 WaveWriter supplement summary. In the SOLIS study described there, about 56 patients receiving SCS therapy experienced a 50% or greater reduction in average overall pain intensity at one year. That finding belongs to that device and study context; it should not be treated as the expected result for every system, diagnosis, or patient.

Implantation combines a procedure, implanted hardware, and ongoing electrical stimulation. FDA clinician labeling for SCS systems discusses the following possible problems:
| Risk or limitation | What it can mean | What to clarify with the team |
|---|---|---|
| Procedure-related pain | Temporary pain or tenderness at the incision or implant site. | Expected duration, activity limits, and pain-control plan. |
| Infection | May require antibiotics, removal of hardware, or additional treatment. | Risk factors, wound-care instructions, and symptoms that require urgent contact. |
| Cerebrospinal-fluid leak | Can cause positional headache or other complications after lead placement. | How the team monitors and treats a suspected leak. |
| Bleeding, seroma, hematoma, or rare paralysis | Uncommon but potentially serious complications, including rare epidural hemorrhage. | How blood-thinning medicines and neurological symptoms will be managed. |
| Lead movement or failure | Stimulation may change, stop working, or require revision. | How loss of benefit is evaluated and when revision is considered. |
| Unwanted stimulation | Changes in sensation, uncomfortable jolts, or stimulation in an unintended area. | Which settings can be changed and when to turn the system off. |
| Electromagnetic or MRI hazards | Some equipment or scan conditions can heat, damage, or interfere with the system. | Exact MRI eligibility, body-region limits, scanner settings, and required device mode. |
The probability and seriousness of each event depend on your health, the procedure, and the exact system. Use the current device-specific patient and clinician labeling rather than assuming that every listed event is common. Seek urgent medical advice for fever, spreading redness or drainage, new weakness or numbness, loss of bladder or bowel control, severe headache after a procedure, or rapidly worsening neurological symptoms.

FDA summaries show that SCS systems differ, but the available evidence does not establish one system as best for every patient. Ask the team to compare the systems they actually offer against your diagnosis and goals:
- Indication and evidence: What exact condition and body area is covered by the device’s current labeling? Which studies apply to someone like me?
- Target and leads: Where will the leads sit, and why is that configuration appropriate for my pain map?
- Stimulation modes: Will stimulation be felt? What programming options are available if the first setting is ineffective or uncomfortable?
- Trial design: How long will the trial last, what restrictions apply, and what measurable result would justify permanent implantation?
- Power and control: Does the generator recharge, how often, and what happens if charging or the controller is inconvenient?
- MRI and other procedures: Which scans and medical treatments are permitted, under what conditions, and where is that information documented?
- Follow-up and revision: Who programs the device, how quickly can you be seen if benefit fades, and what is the plan for lead migration, infection, or generator replacement?
- Team experience: How often does the treating team implant and manage this system for the proposed indication?
Questions to take to a pain specialist
- What is the working diagnosis, and why is stimulation being considered now rather than another treatment?
- Is SCS or PNS the more logical target for my pain pattern, and what alternatives remain?
- What improvement should we measure during the trial: pain score, walking, sleep, medication use, or a specific daily task?
- What would make you stop the trial or decide against permanent implantation?
- Which risks are more relevant because of my medicines, medical conditions, anatomy, or previous surgery?
- What are the exact MRI rules for the proposed system?
- How often will follow-up and programming occur, and whom do I contact after hours?
- What is the plan if the device helps at first but benefit declines?
Bottom line
Nerve-stimulation implants can be a reasonable option for carefully selected people with chronic, difficult-to-manage pain, but they are not first-line treatment for every diagnosis and they do not guarantee lasting relief. SCS stimulates the spinal-cord region; PNS targets a named peripheral nerve. A structured evaluation, validated psychosocial screening, a clearly measured trial when indicated, and a discussion of device-specific labeling, MRI rules, risks, and follow-up are the foundation for an informed decision.
