Nanoparticles may help drugs reach the brain by carrying a payload to the blood–brain barrier (BBB) and interacting with transport processes in its cells. Some are designed to bind endothelial receptors and be carried through the cells; others use different surface or carrier designs. These are experimental strategies, not a single mechanism or a guarantee that a drug reaches brain tissue. Crossing the barrier, reaching the intended brain cells, and improving a patient’s health are separate milestones.

Why is the blood–brain barrier hard for drugs to cross?

The BBB is a selective, multicellular interface between blood and the central nervous system. Its brain endothelial cells are joined by tight junctions, limiting passage between cells, while other features of the brain’s vascular environment help regulate what enters. This protection is essential: it helps keep potentially harmful substances out of the brain, but it also restricts many medicines.

Many small molecules do not readily pass through the barrier, and large biological medicines such as proteins face particular difficulty. A drug that works in a dish or in another part of the body may therefore fail to reach enough of its target in the brain. Disease can also affect the barrier and the surrounding tissue, so delivery conditions may differ by illness, brain region, and disease stage.

A nanoparticle is a carrier, not a key that automatically unlocks the BBB. It can package or associate with a drug and be engineered to interact with cells at the barrier. Whether that design delivers an active dose to the intended site depends on the carrier, its payload and release behavior, the transport route, and the biology of the disease being treated.

How can a nanoparticle use receptor-mediated transport?

One prominent strategy is receptor-mediated transcytosis. The carrier’s surface is equipped with a ligand—a molecule selected to bind a receptor on brain endothelial cells. After binding, the cell may internalize the receptor–carrier complex into a vesicle. The intended route is for that vesicle to move through the endothelial cell and release the carrier, or its payload, on the brain-facing side.

  1. Attach a targeting ligand. A candidate ligand is displayed on the nanoparticle surface so it can bind a receptor on brain endothelium. Reviews discuss transferrin receptor, low-density lipoprotein receptor-related protein 1 (LRP-1), and insulin receptor as possible targets.
  2. Bind and enter the cell. Receptor binding can prompt cellular uptake, but uptake alone only shows that material entered an endothelial cell.
  3. Move through the endothelium. The desired outcome is transport across the cell rather than retention or breakdown within it. A useful carrier must emerge on the brain side in a form that can deliver its payload.
  4. Reach the relevant brain compartment. The drug must then distribute to the brain region and target cells that matter for the disease, while retaining enough activity to have an effect.

Each step can limit the next. Strong receptor binding does not by itself prove successful transport across the cell; material measured in a brain-associated sample does not necessarily show that active drug reached brain parenchyma. A design also has to avoid unacceptable peripheral uptake and toxicity. The details of the ligand, particle surface, payload, and release mechanism all matter.

What other nanoparticle and transport strategies are studied?

Receptor-mediated transport is not the only concept. Reviews also discuss adsorptive transport, transporter-related approaches, and surface modification. These labels describe ways a carrier may interact with cells or transport processes; they do not establish that a particular material crosses the BBB in a useful amount.

Candidate carrier families include liposomes, polymeric particles, lipid-based systems, micelles, dendrimers, and inorganic systems. These categories describe materials or structures, not a ranking of brain-delivery performance. A lipid carrier, for example, does not inherently cross the barrier simply because it is lipid-based. Performance depends on formulation and biological context, including particle size, surface charge, ligand density, payload chemistry, and how the drug is released.

Some brain-delivery methods instead change barrier permeability or bypass the barrier by another route. Those approaches should not be described as a nanoparticle independently crossing an intact BBB. When comparing a proposed treatment, ask whether the carrier relies on an endogenous transport route, uses a physical method to open the barrier, or reaches the brain through a different route. The mechanism determines both what “delivery” means and which safety questions need attention.

How do the main approaches differ?

Approach What it is intended to do What must be demonstrated
Receptor-mediated transcytosis Use a surface ligand to bind an endothelial receptor and promote transport through the cell. Transport to the brain-facing side, suitable payload release, access to the intended brain compartment, and acceptable selectivity and safety.
Adsorptive or transporter-related approaches Use surface interactions or transporter-related processes to engage endothelial cells. That the interaction results in useful delivery rather than cell uptake, retention, or unintended distribution.
Physical barrier opening Increase barrier permeability so a therapeutic or carrier can enter. That opening is controlled, reaches the relevant site, and does not cause unacceptable effects.
Bypass routes Deliver material to the central nervous system without relying on a nanoparticle crossing an intact BBB. That the route reaches the right tissue and produces an adequate, safe exposure.

These approaches are not interchangeable, and the table does not imply that each has equal maturity or clinical evidence. The reviews do not establish a universal best carrier. For a specific candidate, the meaningful comparison is its mechanism and formulation, the payload it carries, the disease and model studied, the brain region and cell types reached, the quality of efficacy evidence, and the safety and development stage.

What counts as evidence that a drug reached the brain?

“Brain delivery” can refer to several different observations, and they should not be treated as equivalent:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Endothelial uptake: material has entered cells lining brain blood vessels. This is evidence of cellular interaction, not proof that it passed through the barrier.
  • Transport across an endothelial layer: a laboratory model suggests material can move from one side of a cell layer to the other. Such a result is useful for screening, but does not by itself establish delivery in a living brain.
  • Brain-associated signal or biodistribution: a measurement detects carrier or payload in brain-associated samples. It may not establish whether the signal is in blood vessels, other barrier-associated compartments, or brain parenchyma.
  • Parenchymal and target-cell access: the active payload reaches the relevant brain tissue and cells. This is closer to the therapeutic goal, but still does not prove that the treatment works.
  • Functional benefit: treatment produces a meaningful effect in an appropriate disease model or clinical setting, with safety that supports its use.

Evidence should therefore be read in context: was the work conducted in a cell model, an animal, or people; what disease and barrier state were studied; what material was measured; and was efficacy assessed separately from distribution? A result in one species or model does not automatically predict delivery in people. Reviews describe CNS delivery as complex, with physiological and disease-related factors that complicate translation.

Why is clinical translation difficult?

Getting a particle into or through endothelial cells is only an early part of the problem. A useful therapy must deliver an adequate active dose to the intended site, release it at the right time or retain it appropriately, preserve its activity, and produce a meaningful effect. It must also meet safety and manufacturing requirements. Off-target distribution, effects on endothelial cells, immune responses, and the consequences of repeated dosing can all matter.

The evidence base also varies by carrier and indication. A formulation that appears promising for one payload, disease, or model may not work for another. Reviews emphasize a substantial translation gap between preclinical findings and clinical use. A 2016 review noted that relatively few preclinical in vivo studies had assessed nanocarrier efficacy against brain disease at that time; that is historical context, not a current count.

Scale matters when interpreting the field’s progress. A 2026 review in the Journal of Controlled Release, “Clinical translation of nanomedicines for brain diseases: Current challenges and future directions,” reports that over 50 nanomedicines had received regulatory approval for various diseases and more than 100 were undergoing clinical trials. Those figures cover nanomedicines overall; they are not counts of products shown to cross the BBB or treat neurological disease.

The same review’s graphical abstract says NanoTherm was the only EMA-approved nanomedicine for brain tumors among the nanomedicines discussed in that review. This is a scoped secondary-source statement, not an independently verified, exhaustive inventory of current authorizations. It should not be generalized to every jurisdiction, indication, or nanoparticle-based brain treatment. A complete current list broken down by indication and jurisdiction cannot be established from these review-level figures alone.

How should you evaluate a claimed brain-delivery system?

Use these questions to distinguish a promising design from a demonstrated therapy:

  • What is the route? Is the system intended to use receptor-mediated transport, another interaction with endothelial cells, barrier opening, or a bypass route?
  • What does the carrier contain? Identify the carrier class, surface design, ligand if any, payload type, and release behavior. Carrier name alone does not show performance.
  • Where was delivery measured? Separate cell uptake, transport across a model, brain-associated signal, parenchymal distribution, and access to the intended target cells.
  • What evidence shows benefit? Look for functional efficacy in the relevant disease context, not just a higher delivery signal. Note whether results come from in vitro work, animals, or human testing.
  • What are the safety and selectivity findings? Consider endothelial effects, off-target distribution, immune response, and repeat-dose questions alongside the claimed benefit.
  • What is the development status? Distinguish a research formulation, preclinical candidate, clinical-trial product, and regulator-authorized treatment. A general nanomedicine trial or approval statistic is not evidence that a specific carrier crosses the BBB.

There is no single carrier class or transport mechanism that can be called best on the available review evidence. A meaningful comparison must match the design to its payload, disease, target tissue, evidence quality, and safety profile.

Can nanoparticles get medicine into the brain?

They may help, and researchers are investigating multiple ways to use them, including receptor-mediated transport. But “can” is not the same as “reliably does”: results depend on the specific formulation and biological setting, and evidence of endothelial uptake or a brain-associated signal is not by itself proof of therapeutic delivery. The review literature describes an active field, not a routine nanoparticle treatment for neurological disease.
