Decision fusion combines predictions made independently from pathology and MRI at the output stage. A pathology model analyzes a whole-slide image, an MRI model analyzes one or more sequences, and a fusion rule or learned combiner turns their scores into one patient-level prediction. The approach preserves modality-specific models and can work with different architectures, but it may miss relationships that are visible only when pathology and imaging features are learned together.

Whether fusion improves performance depends on the disease task, cohort, data pairing, model design, validation strategy, and what happens when one modality is unavailable. Results from recent glioma and breast-cancer studies are encouraging, but they do not establish routine clinical benefit or approval for any particular model.

What decision-level fusion means

In a decision-fusion, also called late-fusion, system, each modality is processed separately until it produces a prediction. The pathology branch receives a digitized biopsy or resection slide and may output a class probability, risk score, or vector of scores. The MRI branch receives the selected sequences and produces its own output. A final operation combines those outputs into one joint decision.

The combiner can be a fixed aggregation rule or an additional learned module. Its exact form is implementation-specific: the important distinction is that it operates on modality outputs rather than on raw pixels or early feature maps. The final result is informed by both streams, while the two experts remain independently trainable and replaceable.

This answers the practical question, “How are the separate pathology and MRI predictions combined?” They are not usually merged as images. Each model first produces a decision-level representation; the fusion stage then uses those representations to calculate the multimodal prediction.

How a pathology–MRI fusion pipeline works

  1. Prepare patient-level inputs. Associate a pathology whole-slide image (WSI), the relevant MRI sequences, and the target label for each case. The label might be a molecular subtype, treatment response, or another clinically defined endpoint.
  2. Run the pathology expert. A pathology model converts the WSI, often through tiles or a foundation-model embedding, into a score or probability distribution. Slide quality checks, tissue selection, and stain variation handling occur in this branch.
  3. Run the MRI expert. A radiology model processes the available sequences and outputs a score or probability distribution. Registration, sequence harmonization, and scanner differences are handled here rather than forced onto the pathology branch.
  4. Combine the outputs. A decision-level rule or learned fusion head receives the two outputs. It may learn when one modality should carry more influence, but the article’s cited studies do not justify assuming equal contribution or a particular weighted-average formula.
  5. Produce and assess the joint prediction. The fused output is compared with the ground-truth label, and its calibration, discrimination, error pattern, and effect on a clinical decision are evaluated separately from the two unimodal models.

In deployment, the system must also record which inputs were present, because a prediction made with both modalities is not automatically equivalent to one made with pathology alone or MRI alone.

Early, intermediate and late fusion compared

Fusion level What is combined Strengths Typical limitations
Early (input-level) Raw inputs or very early features from pathology and MRI Can expose cross-modal relationships from the start Requires compatible preprocessing, alignment or harmonization, and a model that can handle very different data structures
Intermediate (representation-level) Learned representations after each modality has been encoded Allows the model to exchange richer features while retaining modality-specific encoders More complex training and greater risk of overfitting or unstable cross-modal representations
Late (decision-level) Predictions, scores, or output representations from separate modality experts Modular, easy to swap modality-specific architectures, and naturally preserves specialized processing May discard low-level or intermediate interactions that are needed for the task; handling a missing input requires explicit design

These labels describe where information is joined, not one fixed architecture. A mixture-of-experts model, for example, can retain separate experts and learn how to combine them without being identical to a simple late-fusion head.

Why use late fusion for pathology and MRI?

It respects the differences between modalities

A WSI contains gigapixel-scale tissue morphology, while MRI consists of three-dimensional, sequence-dependent measurements. Separate experts can use preprocessing, feature extractors, and quality controls appropriate to each source instead of forcing both into a single input format.

It is modular

An institution can update the MRI branch without rebuilding the pathology encoder, or reuse a validated pathology model in a new fusion experiment. This modularity is useful when the modalities arrive on different schedules or are stored in different systems.

It can be practical with limited paired data

Some projects have many labeled pathology cases and a different number of MRI scans. Decision fusion can make use of modality-specific training, but the fusion head still needs a defensible strategy for learning relationships between outputs. Evidence that unpaired training works in one experiment should not be treated as a general guarantee.

The trade-off is lost interaction information

If the clinically useful signal is a relationship between a microscopic pattern and a spatial imaging feature, combining only final scores may not expose that relationship. Intermediate or early fusion can learn such dependencies, although it demands more harmonization and usually a more involved model.

What the 2026 glioma study found

Saueressig and colleagues’ article, “Multimodal fusion of pathology and radiology foundation models for WHO 2021 glioma subtyping,” appeared in npj Precision Oncology in 2026. The authors open by noting that molecular subtyping of gliomas is common clinically but difficult to perform from histology or radiology images alone. Their target labels came from genetic testing of biopsy or resection tissue.

Study setup

The study used pathology and multiparametric MRI foundation models as fixed feature embedders, then compared three multimodal strategies: late fusion (MM-LF), early fusion (MM-EF), and a mixture-of-experts design (MM-MoE) with linear and sequence-based expert variants. Training included 772 pathology cases and 959 MRI scans. The independent test set contained 171 unseen, patient-matched cases.

Reported performance

Result Model and cohort Reported value What the metric means here
Validation discrimination MM-MoE on the study validation set AUC 0.98 Ranking performance in that validation cohort
Independent-test discrimination MM-MoE on the independent test set AUC 0.94 Ranking performance on unseen cases; not a clinical-impact estimate
Overall accuracy Best-performing MoE-MM architecture on held-out TCGA cases 85% Share of correct classifications at the study’s selected decision rule
Matthews correlation coefficient MM-MoE on the held-out TCGA test set 0.73 ± 0.05 Correlation-style score that accounts for all confusion-matrix cells
Matthews correlation coefficient MM-LF on the held-out TCGA test set 0.70 ± 0.07 Not directly interchangeable with AUC or accuracy
Matthews correlation coefficient MM-EF on the held-out TCGA test set 0.71 ± 0.02 Reported for the same held-out comparison

The authors reported that multimodal models consistently outperformed their unimodal counterparts in the study’s evaluation. However, the pathology-only model performed better than the MRI-only model on the TCGA test comparison. That finding shows why the value of adding MRI cannot be assumed to be equal for every task or cohort.

What the result does and does not establish

The AUC values, the 85% accuracy, and the MCC values summarize different properties and must not be presented as interchangeable percentages. They are results from a retrospective model-development study, not estimates of prospective clinical benefit, regulatory status, or safety in routine care. The cohort and split attached to each number matter as much as the number itself.

What pairing and unpaired data mean

Patient-matched data means the pathology and MRI records belong to the same person and can be linked to one ground-truth label. Unpaired data can mean that modalities share labels or a disease population but do not come from the same individuals. The distinction affects what a fusion model can learn and how leakage is controlled.

In the glioma study, randomly pairing labels was nearly as effective as patient-matched training in the reported setup. Fully unpaired training was broadly comparable for the early-fusion and mixture-of-experts approaches. Late fusion was an exception: its joint output head could not be trained in the same unpaired manner. These observations are evidence from one task and architecture, not proof that paired records are unnecessary.

When records are paired, splits must be made at the patient level. A slide from a patient in training and that patient’s MRI in testing can produce deceptively high scores. When data are unpaired, investigators should document how labels are shared, how class balance is controlled, and which parameters can be learned without a true cross-modal correspondence.

A second example: predicting breast-cancer treatment response

A retrospective, cross-institutional study evaluated an attention-based multimodal fusion transformer called MuFi for response to neoadjuvant therapy. It used pathology slides, MRI, and clinical data from 567 biopsy-confirmed patients across two institutions.

The abstract reported AUCs of 81.9% in discovery, 78.5% in validation, and 79.3% in external testing. In that analysis, the multimodal model outperformed clinical, single-modality, and late-fusion comparison models. This is a different endpoint, population, and study design from the glioma work, so its AUCs should not be ranked directly against the glioma AUCs as though they measured the same problem.

How to evaluate a fusion model responsibly

Use an independent or external cohort

Internal validation can show whether a model generalizes within the development distribution. An independent or external test, ideally from different institutions, scanners, laboratories, and time periods, is more informative about transportability. Report the number of patients, modality availability, class distribution, and patient-level split.

Keep metrics tied to the decision

AUC measures ranking across thresholds. Accuracy depends on a selected threshold and class balance. MCC summarizes binary classification quality using all confusion-matrix cells and is useful when classes are imbalanced. None of these alone tells a clinician whether using the model improves care.

Check calibration

A model can rank cases well yet produce probabilities that are too high or too low. Calibration curves, calibration error, and recalibration procedures should be reported when the output is used as a risk estimate or treatment aid.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Measure missing-modality behavior

Test the complete-input pathway and each realistic missing-input pathway separately. A system should specify whether it refuses to predict, falls back to a unimodal expert, uses a modality-dropout-trained combiner, or applies another documented policy. Performance and calibration must be reported for each policy.

Assess clinical decision value

Ask whether the fused prediction changes a biopsy decision, treatment selection, monitoring plan, or other action in a way that is accurate and safe. A benchmark improvement that does not alter decisions, or that increases harmful false positives, is not evidence of clinical utility.

Missing modalities, scanners and sites

Pathology and MRI are often acquired at different times, and one may be absent, delayed, unusable, or outside quality standards. Late fusion can be attractive because each branch can be built around its own availability rules, but the fusion layer still needs explicit training and testing for missing inputs.

  • Define an input contract: list required MRI sequences, acceptable slide quality, and the maximum time between specimens and imaging.
  • Record provenance: retain scanner, sequence, staining, laboratory, and acquisition-site metadata for auditing and shift analysis.
  • Separate fallback outputs: label a pathology-only or MRI-only prediction as such instead of presenting it as equivalent to a complete multimodal result.
  • Monitor drift: compare score distributions and calibration across sites, scanners, staining protocols, and demographic or clinical subgroups.

Interpreting attention maps and SHAP values

The glioma authors analyzed attention and SHAP explanations to inspect which image regions or features influenced predictions. Such tools can reveal patterns for quality review, hypothesis generation, and error analysis. They are not causal explanations, proof of biological validity, or evidence that a highlighted region is the reason a patient has a subtype.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Interpretability checks should therefore be paired with pathology and radiology review, controlled perturbation or ablation tests where appropriate, and external validation. The glioma article also noted that one commonly confused subtype pairing was not represented in its dataset. Coverage of difficult clinical populations is a prerequisite for claiming broad reliability.

A practical blueprint for building and testing decision fusion

  1. Define the clinical question and label source. Specify the endpoint, timing, reference standard, and whether labels come from biopsy, resection, follow-up, or treatment response.
  2. Audit each modality independently. Measure pathology-only and MRI-only performance, calibration, failure modes, and missing-data rates before adding a combiner.
  3. Choose the fusion level deliberately. Use late fusion when modular experts and simpler integration are priorities; consider intermediate or early fusion when cross-modal interactions are central and the data support the added complexity.
  4. Prevent leakage. Split by patient, keep repeated scans and slides together, and freeze preprocessing decisions that were learned from the training set.
  5. Train the combiner with an availability plan. Include realistic missing-modality cases or define a tested fallback rather than assuming both inputs will always arrive.
  6. Compare against meaningful baselines. Report both unimodal experts, the selected fusion model, and a clinically understandable decision rule.
  7. Validate outside the development distribution. Use an independent or external cohort and report confidence intervals or variability where available.
  8. Evaluate utility and governance. Test calibration, subgroup performance, human workflow, error consequences, audit logging, and procedures for declining to issue a prediction.

Common mistakes and how to avoid them

  • Calling any multimodal model “decision fusion.” Verify where the modalities meet. If features are combined before outputs, it is early or intermediate fusion, not late fusion.
  • Combining incomparable metrics. Keep AUC, accuracy, and MCC labeled with their cohort, split, and decision rule.
  • Assuming both modalities contribute equally. One branch may dominate, as the pathology-versus-MRI comparison in the glioma test set illustrates.
  • Generalizing from one dataset. A retrospective result can support further validation but cannot establish prospective benefit or approval.
  • Treating explanations as proof. Attention and SHAP are inspection aids, not causal evidence.
  • Ignoring absent inputs. A model that works only when every sequence and slide is present is not ready for an environment where those inputs are intermittent.

Bottom line

Multimodal decision fusion runs separate pathology and MRI experts, then combines their predictions at the decision stage. It offers modularity and modality-specific processing, while risking the loss of cross-modal interactions. The 2026 glioma study and the cross-institutional breast-cancer study show promising performance in their own cohorts, but reliable adoption requires patient-level validation, external testing, calibrated probabilities, explicit missing-modality behavior, and proof that the prediction improves a real clinical decision.
