Hospitals can integrate a real-time capacity-management platform with an electronic health record (EHR) by connecting the operational system to admission-discharge-transfer (ADT) messages, FHIR APIs, or both. The reliable approach is to define the decisions the capacity team must make, map patients and locations to a single set of identifiers, agree which system owns each status, configure and test the interface with real clinical scenarios, and monitor stale or failed updates after launch.
An integration is not successful merely because a vendor supports “EHR integration.” A useful bed board must show the right patient, encounter, unit, room and bed state at the right time, while preserving privacy, auditability and a clear recovery path when messages are delayed or corrected.
What the integration should accomplish
Capacity software turns clinical and administrative events into an operational view. The EHR remains the source for many patient and encounter facts, while the capacity platform organizes those facts around decisions such as where to place a patient, when a transfer can occur, whether an expected discharge is blocking flow, or when emergency-department boarding requires escalation.
Before selecting an interface, write down the decisions and users involved. A hospital may need one or more of the following views:

- Economical &Reusable: Built durable materials with a long-lasting battery, our CAGIWIRU air duster it's a sustainable alternative to disposable air cans.
- Bed placement and admission matching.
- Unit-to-unit transfers and step-down or escalation capacity.
- Expected discharges and the dependencies delaying them.
- Emergency-department boarding and inpatient demand.
- Environmental-services, transport or isolation constraints.
- Command-center coordination across several campuses.
Each view needs named users, an update expectation and a defined action. For example, a bed manager may need a current assignment and blockage reason, while a nurse manager may need a forecast of discharges and pending transfers. This prevents an implementation from importing large amounts of data that no one uses.
Choose the interface pattern
Most implementations use HL7 version 2 ADT feeds, FHIR APIs, or a combination. The label alone does not prove that two products exchange the same fields or interpret them the same way; confirm the installed EHR version, interface engine, supported message or resource profiles, authentication and write-back behavior.
| Pattern | Typical use | Questions to verify |
|---|---|---|
| HL7 v2 ADT feed | Streaming admission, transfer, discharge and demographic or encounter changes from an EHR or registration system. | Which ADT events are supported? Is delivery through the hospital interface engine or direct MLLP? How are acknowledgements, duplicates, corrections and out-of-order events handled? |
| FHIR API | Querying or subscribing to structured patient, encounter and location data, and sometimes sending updates back. | Which FHIR release and implementation profiles are supported? Which resources and search parameters are available? What authorization scopes, rate limits and subscription behaviors apply? |
| ADT plus FHIR | Using event feeds for rapid notification and APIs for reconciliation, enrichment or selected write-back. | Which path is authoritative when values disagree? How are identifiers correlated and how is an API outage detected while ADT continues? |
One vendor example, Dualis TrackER, states that it uses FHIR R4 for named electronic medical records and HL7 v2 over MLLP for other ADT-capable systems. It also advertises EHR write-back and integration diagnostics. Those are product claims that must be demonstrated in the hospital’s own environment, not assumed from a standards name.

Identity and encounter fields
- Patient identifier and the identifier system that issued it.
- Encounter or visit identifier, including how an observation encounter differs from an inpatient encounter.
- Legal or operational name and demographic fields needed for safe matching.
- Admission, discharge, transfer and cancellation timestamps, with timezone rules.
Location fields
- Campus, facility, unit or ward, room and bed identifiers.
- Local code mappings and the display name used by staff.
- Whether a location is staffed, temporarily closed, isolation-capable or otherwise restricted.
Operational status fields
- Occupied by a patient.
- Assigned to an incoming patient but not yet occupied.
- Unassigned and available for placement.
- Blocked, such as for maintenance, infection-control restrictions or another local reason.
- Dirty or awaiting cleaning, if environmental-services workflow is included.
GE HealthCare describes a capacity view using occupied, assigned, unassigned and blocked beds. Those labels are useful starting points, not a universal standard. A hospital should document whether “assigned” means a placement decision has been made, whether a bed can still be offered to another patient, and who may change each state.
Step-by-step implementation plan
1. Inventory the current environment
List the EHR, registration or ADT source, bed-management application, interface engine and any systems that affect availability, such as staffing, transport, environmental services or a command center. Record the installed versions, hosting model, network zones and existing interfaces. Ask both vendors for a conformance statement specific to those versions rather than accepting a generic integration brochure.
2. Define ownership and write-back boundaries
Assign one authoritative source for patient identity, encounter status, physical location and operational bed status. Decide whether the capacity platform is read-only or may write assignments, expected discharge information or other fields to the EHR. For each writable field, document the permission, approval workflow, conflict rule and rollback method. If two systems can edit the same value, the interface needs a deterministic rule for which update wins and an audit record explaining the change.
3. Map identifiers and local codes
Build a crosswalk for facility, unit, room, bed, encounter and patient identifiers. Include retired locations, temporary beds, observation areas and beds shared across services. Test name changes and reorganizations, because a display label can change while an internal identifier remains stable. Define how the system handles an unknown code: quarantine the message, route it to an exception queue or reject it with an alert.

4. Configure the selected interface
Use the EHR’s supported interface engine or API gateway. Document the message or resource scope, triggering events, authentication method, encryption in transit, acknowledgement behavior, retry schedule, monitoring endpoint and escalation contacts. Limit access to the minimum fields and actions required for the workflow. If FHIR subscriptions or polling are used, set a reconciliation process so the hospital can detect a missed notification. If ADT is used, retain enough message metadata to trace an update from source through the interface engine to the capacity board.
5. Establish freshness and exception rules
Define what “real time” means operationally. A bed manager may require an update within a few minutes, while a daily census can tolerate more delay. Display the last successful update and source timestamp so users can recognize stale information. Create explicit states for pending, unknown and unavailable rather than presenting an old value as current. Route malformed messages, unmapped locations, duplicate events and authentication failures to named support queues.
6. Test with representative scenarios
Use de-identified or approved test patients and include normal, corrected and failure cases:
- Admission to an available bed.
- Transfer between units, including a transfer that is canceled.
- Discharge followed by a bed-cleaning or blocking period.
- Assignment of a bed before the patient physically arrives.
- Reassignment to a different room or bed.
- Duplicate, delayed, reversed or out-of-order events.
- Unknown location codes and a temporary bed.
- Interface downtime, message backlog and recovery.
For every scenario, reconcile the EHR, interface-engine logs and capacity-board state. Check patient and encounter matching, location display, timestamps, permissions, audit history and the behavior when data is stale. Include bed managers, nursing, registration, information-technology and interface staff in the review; each group sees different failure modes.

7. Pilot, then expand
Start with a defined unit, campus or workflow and set a review period for exceptions and staff feedback. Train users on the meaning of each bed state and on the procedure for reporting a discrepancy. Expand only after the pilot demonstrates that unresolved exceptions are visible, ownership is clear and staff can continue safely during an outage. Bonami Hospital Operations AI describes module-by-module activation, while GE HealthCare describes modular capability; these are vendor deployment options rather than promises of a particular timeline or result.
Security, privacy and governance
Capacity displays can expose protected health information even when they contain fewer clinical details than the EHR. Apply the hospital’s identity, access-management and privacy controls to every interface component. Define role-based access for placement staff, unit leaders, clinicians, registration and technical support. Separate production, test and training data, and prohibit real patient data in unapproved environments.
Require encryption for network connections, secrets management for API credentials, audit logs for reads and writes, and a retention policy that matches hospital and regulatory requirements. Review vendor hosting, subcontractors, incident response, vulnerability-management practices and support access during procurement. Confirm how access is revoked when an employee changes role and how emergency access is logged.

Examples of vendor claims and how to interpret them
GE HealthCare’s Command Center materials describe integrated source systems and real-time capacity views. The company says the product has been used across nearly 500 hospitals and medical facilities representing more than 55,000 beds. That is a vendor-reported deployment footprint, not independent evidence that every installation achieved the same operating results.
The same GE HealthCare page displays customer-reported figures: 19,000 additional patients served annually at Providence Swedish; a 1.07-day reduction in length of stay at The Queen’s Health Systems; $40 million in labor-cost savings at Duke Health; and an 80% reduction in delayed admissions and 90% reduction in deferred admissions through the Transfer Center at Children’s Mercy Kansas City. The page does not establish independent verification, common baselines, comparable measurement periods or causal attribution across hospitals. Treat these numbers as claims to investigate, not forecasts for a new deployment.
Bonami Hospital Operations AI describes connectivity through FHIR APIs and HL7 v2 ADT, integration with existing systems and phased activation. Its outcome figures appear on a vendor marketing page; the available material does not establish independent study methods or broad applicability. Request source data, definitions, denominators, dates and methodology before using any case result in a business case.

Costs, timing and procurement decisions
There is no reliable universal price or implementation duration for this integration. Cost depends on the number of facilities and beds, interface complexity, data-cleansing work, security review, customization, training, support coverage and whether write-back is required. Build a budget by requesting separate line items for software licensing, interface-engine work, API or message fees, implementation services, testing, training, hosting, maintenance and change requests.
Ask vendors to state assumptions in writing: which interfaces are included, how many environments are configured, who supplies mapping, how many test cycles are covered, what happens when the EHR changes, and what support response applies to a stale or unsafe bed board. Do not use a vendor’s customer count or a case-study savings figure as a substitute for a hospital-specific total-cost model.
Troubleshooting common failures
| Symptom | Likely causes | Recovery action |
|---|---|---|
| A patient appears in the wrong unit or bed. | Identifier crosswalk error, late transfer event or local-code mismatch. | Trace the encounter and location identifiers through the interface logs, correct the mapping and reconcile affected records. |
| A discharged bed remains occupied. | Missing or rejected discharge event, duplicate suppression or an interface outage. | Check acknowledgements and backlog, replay the approved event, then verify the source and capacity states agree. |
| Users see old data without warning. | No freshness indicator, stalled subscription or failed polling job. | Expose last-update time, alert on the defined threshold and provide a documented downtime workflow. |
| An assigned bed is treated as available. | Local definitions were not agreed or status ownership is ambiguous. | Clarify the state model, restrict who can change it and test assignment, arrival and cancellation sequences. |
| Write-back overwrites a newer EHR value. | Two systems edit the same field without a conflict rule. | Disable unnecessary writes, establish field ownership and implement version or timestamp checks with audit logging. |
Go-live acceptance checklist
- Every participating facility, unit, room and bed has a verified identifier mapping.
- Admission, transfer, discharge, cancellation and correction scenarios pass end-to-end tests.
- Occupied, assigned, unassigned, blocked and any local states have written definitions.
- Last-update timestamps, stale-data alerts and downtime procedures are visible to users.
- Access roles, audit logs, encryption and support escalation have been reviewed.
- Exception queues have named owners and a target response time.
- Users can explain which system is authoritative and how to report a discrepancy.
- A reconciliation report compares EHR and capacity states after recovery from an outage.
Bottom line for hospital leaders
Choose the integration around a documented capacity workflow, not a marketing promise. Confirm the exact ADT and FHIR capabilities in the installed EHR and software versions; map identities, locations and bed states; limit and govern write-back; test delayed and corrected events with frontline users; and operate the interface with freshness monitoring, reconciliation and clear downtime ownership. Vendor examples can show what is possible, but only a hospital-specific conformance review and pilot can establish whether the live board is safe and useful.
