Compare imported and domestically produced medical devices against the same clinical need, technical specification, operating conditions and analysis period—not by quoted purchase price alone. For each offer, estimate the cost of acquiring, installing, operating, maintaining and eventually disposing of the device, then account for service coverage, spare parts, consumables, downtime consequences and, where relevant, import and currency exposure. Apply the same safety, performance and support tests to every bidder. There is no universal cost winner: the result depends on the device, country, contract, hospital and assumptions used.
Why the quoted price is not the total cost
A purchase quote is only one input to a hospital’s decision. A device may require delivery, site modifications, installation, testing, staff training, utilities, software or service charges, recurring supplies, maintenance and replacement parts. Deinstallation and disposal can add costs at the end of its useful life. A low initial price can therefore coexist with higher costs to keep the device functioning and available for care.
The World Bank’s Medical Diagnostic Imaging Equipment Procurement Guidance defines total cost of ownership (TCO) as “an estimate of the total costs associated with a solution over the whole of the operational life including final disposal.” The guidance is specifically about medical diagnostic imaging equipment; hospitals should adapt its lifecycle approach to the device category they are procuring rather than assume its examples cover every technology.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsTCO makes proposals more comparable by bringing costs beyond acquisition into view. It does not mean every future expense can be known exactly. The hospital should distinguish firm quoted amounts from estimates, document assumptions, and show how uncertain inputs affect the result. The World Bank guidance warns that decisions based only on initial price can miss future costs; it does not establish a universal lifecycle-cost percentage for devices.

First define what is being compared
Before requesting or scoring prices, specify the service the hospital needs and the conditions in which it will deliver that service. A domestic offer and an imported offer are not comparable simply because both are described as the same type of device. Different configurations, included accessories, capacity, software, safety features or intended workloads can make the offers materially different.
Use one analysis period and one operational scenario for all offers. The period should reflect the hospital’s procurement decision and expected use; the available sources do not prescribe a single useful-life period for every device. If a bidder’s warranty or service commitment ends earlier than the hospital’s analysis period, identify how the remaining years will be supported and costed rather than silently treating the quote as covering the entire period.
- Clinical need: Define the service, users, patient or sample volumes, and care setting the device must support.
- Technical and safety requirements: Set minimum performance, configuration, safety, quality and acceptance criteria before seeing bids.
- Operating assumptions: State expected utilization, operating hours, staffing, available utilities and local environmental or infrastructure conditions relevant to the device.
- Time horizon: Use the same start and end dates or operating-life assumption across proposals, and identify any replacement or disposal point within that period.
- Evaluation method: Decide whether costs will be compared as undiscounted totals or converted to present values using a rate approved under applicable hospital or procurement policy. Use the same method and rate for all offers, and disclose it.
If two offers cannot provide the same required service, first determine whether each meets the hospital’s minimum requirements. Do not make an apparently precise TCO ranking between configurations that are clinically or technically unequal; resolve the specification gap or assess the offers in separate service scenarios.
Build a like-for-like lifecycle cost worksheet
Ask every bidder to price the same scope and provide a schedule of what is included, excluded, supplied by the hospital or conditional on a later contract. Then add hospital-borne costs and recurring expenses over the common analysis period. The table below is a practical starting point, not a complete list for every device. The World Bank’s imaging guidance and its resource on establishing private health care facilities support examining delivery, installation and site-related requirements; the applicable items depend on the equipment and location.
| Cost or decision area | What to request or establish for each offer |
|---|---|
| Clinical and technical fit | Confirm that the proposed configuration meets the same service need, performance, safety and setting requirements. Record differences in accessories, software, capacity or other included features. |
| Purchase and delivery | Identify the quoted price and included scope. Ask who pays shipping, insurance, handling, customs, taxes and inland delivery under the contract and local law. Confirm the relevant delivery terms and when ownership and risk transfer. |
| Site preparation and installation | Cost any building or utility changes, installation, commissioning, acceptance testing and validation. Record which tasks the seller includes and which the hospital must arrange. |
| Training and staffing | Specify training for operators and technicians, its language and duration, who provides it, and whether using or servicing the device requires additional staff skills or time. |
| Operation | Estimate applicable electricity, water, gas or other utilities, software or service charges, and routine operating inputs under the shared utilization assumptions. |
| Consumables and supplies | List required supplies, expected use, unit prices, shelf life, supplier dependence and availability. Check whether compatible alternatives are permitted and whether any supply restrictions affect the proposed configuration. |
| Maintenance and service | Compare warranty duration and scope, preventive and corrective maintenance, service-contract coverage, labor and travel charges, response times and access to factory engineering support. |
| Parts and downtime | Document spare-parts locations, backup sources, lead times, stock policies, local repair capability and the operational consequences of unavailability. Request documented availability assumptions; do not invent downtime estimates. |
| Currency and continuity | For costs exposed to foreign currency or overseas supply, identify the currency and payment timing, test exchange-rate scenarios, and model plausible delays or interruptions using stated assumptions. |
| End of life | Include deinstallation, disposal and replacement costs where relevant. Include residual value only if it is supportable, and explain how it was estimated. |
| Uncertainty and evidence | Mark each amount as a binding quote, contract term, hospital estimate or scenario assumption. Record its source, date, unit and responsible party. |
To calculate a comparable total, add the relevant acquisition and delivery costs to site preparation, installation, training, expected operating inputs, maintenance, service, supplies, parts and end-of-life costs over the chosen period. Subtract a residual value only when the estimate is credible and treated consistently across offers. If the hospital uses present-value analysis, convert future cash flows using the same approved method; do not mix discounted and undiscounted totals.

For each recurring item, record the quantity and unit price as well as the total. For example, a consumable estimate should show the assumed use per service unit, the expected number of service units and the quoted price per supply unit. This makes it possible to revise the total if actual demand or pricing changes, instead of burying the assumption in one lump sum. Ask suppliers to identify price-validity dates and any contract terms that allow recurring charges to change.
Request auditable quotes, not just headline prices
A useful request for proposal gives every bidder the same cost template and asks for a complete scope statement. Separate one-time charges from recurring charges, specify the required analysis period, and ask bidders to state what the hospital must provide. A line marked “included” should identify the service or quantity covered; it should not conceal a limit, prerequisite or later charge.

- Clarify delivery responsibility. Ask for the delivery term, delivery destination, party responsible for each logistics step, and the point at which ownership and transit risk pass.
- Request a support plan for the full comparison period. Ask for warranty and service scope, response arrangements, parts sourcing, expected lead times and the process for escalation or factory support.
- Ask for evidence behind estimates. Have bidders disclose quantities, utilization assumptions, unit prices, validity dates and conditions. Label information that is indicative rather than contractually committed.
- Reconcile the bid against hospital costs. Have engineering, clinical, procurement and finance staff identify work, staffing, utilities or supplies the hospital must fund outside the supplier’s quote.
The World Bank’s Project Procurement Framework is relevant where that framework applies. Procurement requirements differ by funding source, jurisdiction and solicitation, so a hospital should confirm the rules governing its own process instead of assuming one framework or contract term applies everywhere.
What to examine in an imported-device offer
For an imported device, begin with a complete delivered-cost statement rather than an equipment-only price. Identify the shipment destination, delivery term and division of responsibility for freight, insurance, handling, customs clearance, applicable duties and taxes, and inland transport. Apply only charges confirmed for the procurement’s jurisdiction and contract. The country and shipment details are not specified here, so no general duty rate or import fee can be stated.
Trace the same supply chain for consumables and replacement parts. A device price may be competitive while recurring inputs depend on imports, a single source or a long replenishment route. Ask where parts are held, how replacements are ordered, what lead times are supported, and whether emergency stock or another documented contingency exists. Do not treat an uncommitted assurance as a guaranteed service level.

Record the currencies of quoted and recurring costs, when payments are due, and whether the contract fixes or adjusts prices. If future expenses are denominated in foreign currency, calculate the base case using a disclosed exchange-rate assumption and test alternative rates. This is a sensitivity analysis, not a prediction of a future exchange rate. Include only exposures that actually apply to the specific bid.
For possible shipping or supply interruption, show the operational effect separately from the price estimate: which service might be interrupted, what alternative arrangements exist, and which assumptions determine the scenario. A hospital should not assign a made-up monetary value to downtime just to complete a table. If it monetizes lost service or recovery costs, it should explain the evidence and method used.
What to examine in a domestic-device offer
A domestic production label does not by itself demonstrate clinical suitability, quality, availability, lower cost or long-term support. Assess the offer against the same technical, safety, quality, service and lifecycle requirements used for an imported device. Ask about manufacturing and quality capacity, the service footprint, spare-parts sourcing, consumables, staff training, warranty and the ability to support the equipment throughout the hospital’s analysis period.
Establish what “domestic” means for the particular offer. The finished device, components, software, service expertise or supplies may have different origins. Where imported inputs remain essential, include their currency and supply exposure just as you would for another proposal. Where production or repair is local, ask for evidence of the relevant capability and commitments rather than assuming proximity guarantees fast repairs or continuous supply.
The World Health Organization’s 2016 report, Towards improving access to medical devices through local production: phase II report of a case study in four sub-Saharan countries, examined policy, infrastructure, capacity and feasibility in Ethiopia, Nigeria, South Africa and the United Republic of Tanzania. Those case studies support assessing local production in context; they do not show that domestic devices are always available, less costly or preferable in other settings.

Compare support and functionality, not only cost totals
A lifecycle cost estimate is most useful when paired with a clear view of whether the device can remain functional and available for the intended service. The World Health Organization Regional Office for South-East Asia wrote on 4 September 2026: “Improving access requires ensuring that devices and diagnostics remain available, appropriate, safe, functional, and fit for purpose throughout their lifecycle.” Its article, From Purchase to Functionality: Life-cycle implementation of medical devices including IVDs, points to maintenance systems, spare-parts planning, service-level agreements, installation, training and functionality monitoring.
Translate those lifecycle needs into bid questions and contract terms. Compare the scope and duration of warranty and service, preventive maintenance schedules, corrective repair arrangements, response targets, travel or labor charges, parts availability and escalation routes. Ask how functionality will be monitored and who is responsible for reporting and acting on problems. A service commitment should be evaluated for the period it actually covers, not treated as a promise for the entire equipment life unless the terms say so.
Hospitals can also use an equipment inventory and maintenance process to track assets, service history, status and parts needs. WHO’s Introduction to medical equipment inventory management is a relevant resource for that lifecycle-management context. The inventory record helps make ongoing maintenance visible, but it does not replace a device-specific cost estimate or a bidder’s contractual service commitments.
Test uncertainty before choosing a preferred offer
Some bid amounts will be firm; others depend on utilization, future prices, exchange rates, maintenance needs, parts delivery or the cost of interruptions. Present a base case and low- and high-cost scenarios for inputs that could materially change the decision. State why each input changes between scenarios and identify whether the scenario is supported by a quote, a contract, historical hospital data or an explicit planning assumption.
Pay particular attention to assumptions that could reverse the ranking. These may include annual consumable use, service-contract uptake, utility demand, currency movements or the period during which the supplier’s support terms apply. Show the effect of changing one important input at a time when useful, then test combined plausible changes if several risks could occur together. Do not disguise a fragile result behind a single total.

- Separate estimates from commitments: a binding quoted price and an indicative future parts price are different kinds of evidence.
- Keep units consistent: label amounts by currency, quantity, time period and whether they are per device, per service unit or per year.
- Make gaps visible: mark costs that are not stated and ask the bidder or responsible hospital team to resolve them before award.
- Show operational consequences: explain how a delay or unavailable part could affect service; do not convert it into money without a defensible method.
- Preserve a decision trail: save the specification, quote versions, assumptions, scenario results and reason for selecting the offer.
A sensitivity result does not forecast what will happen. It shows which assumptions matter enough that the hospital should seek better evidence, negotiate a contract term, hold contingency stock, or reconsider the offer. If the preferred proposal changes under a plausible scenario, decision-makers should see that uncertainty before award.
Make the award decision transparent
Use a two-stage decision: first confirm that each offer meets the minimum clinical, technical, safety and procurement requirements; then compare lifecycle cost and service risks among eligible proposals. This prevents a low cost from compensating for a failure to meet a requirement that the hospital has defined as essential. Local policy criteria, such as any domestic preference or other procurement rule, should be applied only after checking the applicable law and solicitation.
For the cost comparison, report the analysis period, cost method, inclusions, exclusions, base assumptions and scenario results alongside the total. Give decision-makers both the overall figure and the major contributors, so they can see whether the outcome is driven by acquisition cost, operations, service, supplies or another line. Record any non-cost rationale for the decision separately from the TCO calculation rather than blending unlike factors into an unexplained score.

Before tendering or award, confirm the country and its applicable procurement and import rules; the exact device category and required configuration; the hospital’s operating setting and expected utilization; current supplier quotes and delivery terms; local taxes, duties and logistics charges; utility and site requirements; supply and service arrangements; and the analysis period and approved financial method. Without those specifics, a general article can provide a comparison method, but it cannot give a reliable numerical verdict about whether an imported or domestic device costs less.
