Glass ionomer cement (GIC) performance is a formulation-dependent balance, not a contest to maximize fluoride or strength. Conventional and resin-modified GICs can release fluoride for considerable periods, take it up from their surroundings and release it again, while their compressive, flexural and fracture properties vary substantially by composition and intended use. A high fluoride-release value from a laboratory test does not prove that a material is weak, and it does not by itself prove better caries prevention.

To judge a glass ionomer cement restorative dental material, read the fluoride result and the mechanical result together: identify the GIC type, the property being measured, the pH and storage conditions, the test period and the clinical application. The sections below explain how those factors interact and where the evidence stops.

What determines GIC performance?

GICs are acid–base cements made by reacting an ion-leachable glass powder with an aqueous polyacid. Some formulations also contain methacrylate-based resin components. The resulting material can bond to tooth structure, exchange fluoride with its environment and carry load, but each benefit depends on formulation and use.

Performance has at least four separate dimensions:

  • Fluoride behavior: the initial release, later release, fluoride uptake or recharge, solution pH and measurement period.
  • Mechanical behavior: compressive strength, flexural strength, tensile strength, fracture toughness and bond strength are different properties.
  • Material category: conventional, resin-modified, metal-reinforced, high-viscosity and additive-modified GICs should not be treated as interchangeable.
  • Clinical purpose: a luting cement, lining or base, sealant, orthodontic adhesive and restorative cement face different stresses and moisture conditions.

For that reason, one number cannot rank every GIC. A 30-day fluoride result and a compressive-strength value answer different questions and may come from different test protocols.

How fluoride release from glass ionomer cement works

The early wash-out phase

After setting, a GIC commonly shows an early wash-out phase in which relatively more fluoride leaves the cement. The magnitude depends on the glass, polyacid, powder-to-liquid ratio, specimen surface area, storage medium and how soon testing begins. Early measurements therefore should not be interpreted as the material’s permanent release rate.

Long-term release in neutral conditions

In a neutral solution, the initial wash-out is followed by a much slower phase described as diffusion. Fluoride moves through the set cement and out into the surrounding liquid over an extended period. The review by Nicholson, Sidhu and Czarnecka describes both conventional and resin-modified GICs as releasing fluoride for considerable periods after setting: Fluoride exchange by glass-ionomer dental cements and its clinical effects (2023).

What changes in an acidic environment?

Acidic conditions increase early fluoride release. Over the longer term, release in acid is associated with slow dissolution of the cement rather than diffusion alone. A release curve generated at low pH can therefore look substantially different from one generated in a neutral buffer. The pH must be reported beside the number when comparing studies.

Fluoride uptake and recharge

GIC is not only a fluoride source. It can take up fluoride from the surrounding environment and release some of it later when conditions change. A laboratory recharge experiment may expose a specimen to a concentrated fluoride solution and then measure release into a fluoride-free medium. That result demonstrates exchange capacity under the test conditions, not a guaranteed dose delivered in a patient’s mouth.

Does more fluoride release mean lower strength?

No universal inverse relationship has been established. Fluoride release and strength are controlled by overlapping but not identical variables: glass composition, polyacid concentration, powder-to-liquid ratio, porosity, resin content, maturation, water balance and test conditions. Two cements can release different amounts of fluoride while having similar compressive strength, or show similar release but different flexural or fracture behavior.

A 2016 clinical-dentistry review reports that compressive-strength trends improve with higher powder-to-liquid ratios and higher polyacid concentrations: A Review of Glass-Ionomer Cements for Clinical Dentistry. Increasing powder relative to liquid can create a denser cement matrix, while changing polyacid concentration alters the acid–base reaction. Those adjustments can also change working time, viscosity, translucency, water sensitivity and fluoride transport. They are formulation choices, not a simple strength-versus-fluoride dial.

Fluoride values also depend on whether the test captures only dissolved fluoride in the storage solution, how often the solution is replaced and whether the specimen is powdered, polished or left with its original surface. A high value from one protocol cannot be compared directly with a low value from another without matching the methods.

Published fluoride-release and strength values

The American Dental Association’s table for direct-restoration materials gives example 30-day fluoride-release values by category. They are table values, not guarantees for every product in a category; the ADA page is Materials for Direct Restorations (accessed 2026).

GIC category Example fluoride release over 30 days How to read the figure
Type I 700 μg One ADA table value for the stated 30-day period.
Type II 650 μg One ADA table value; it is not a universal product specification.
Type III 1,300–1,600 μg A range in the ADA table, showing variation even within a named type.
Metal-reinforced type IV cement 300 μg One ADA table value; metal reinforcement and lower listed release do not establish a general clinical ranking.

The same ADA table lists example compressive strengths that vary by category:

Material category Example compressive strength Important qualification
Type II 196–251 MPa Category range from the ADA table; test details and product formulation matter.
Metal-reinforced cement 176–212 MPa Not directly comparable with a different specimen or test protocol.
Hybrid restorative 202–306 MPa Range reported for the table’s hybrid category.
High-viscosity material 301 MPa A listed example, not a guarantee for every high-viscosity GIC.

A 2024 clinical-application review reports broad overall GIC ranges of 60–300 MPa for compressive strength and 14–50 MPa for flexural strength: Updates on the clinical application of glass ionomer cement in restorative and preventive dentistry. Those ranges come from a review with its own mix of materials and methods; they should not be merged numerically with the ADA table as though both were one standardized dataset.

How formulation changes strength and fluoride behavior

Conventional GIC

Conventional GIC relies on the acid–base reaction between glass and polyacid. It offers fluoride exchange and chemical adhesion to tooth structure, but its early set is sensitive to moisture contamination and dehydration. Strength continues to mature after placement, so handling and protection during the initial period affect the final result.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Resin-modified GIC

Resin-modified GIC incorporates methacrylate-based monomers. The ADA describes this category as having improved setting and polishability properties compared with conventional formulations. Resin modification changes the setting reaction and can improve early handling, but it does not make every resin-modified cement equivalent in fluoride release, water uptake or load-bearing performance. Compare the specific formulation and test data rather than assuming a category-wide ranking.

Metal-reinforced GIC

The ADA describes metal-reinforced GICs as containing silver alloy or silver sintered to glass, intended to increase stress-load capacity and fracture toughness. Metal reinforcement can alter radiopacity, appearance, finishing and fluoride behavior. The 300 μg 30-day value in the ADA table is an example for a type IV metal-reinforced cement, not evidence that all reinforced materials release less fluoride or are stronger in every mode.

High-viscosity and additive-modified materials

High-viscosity formulations use a different powder–liquid balance and are often evaluated for bulk placement or minimally invasive techniques. Additives can alter ion transport, setting, porosity and strength. A 2026 systematic review of additives in orthodontic GICs included 23 in-vitro studies and found effects varied with additive, concentration, base composition, storage conditions, pH and evaluation period. Because methods and outcomes were heterogeneous, the review did not perform a meta-analysis and called for standardized long-term and clinical studies: Effects of Additive Incorporation on Fluoride Release of Orthodontic Glass Ionomer Cements and Adhesives: A Systematic Review.

Is GIC strong enough for a filling?

That depends on the tooth, cavity design, moisture control, occlusal load and the specific GIC. The 2024 clinical review notes that GIC generally has lower compressive and flexural strength than amalgam and resin composite, which limits broad use in high-stress-bearing areas. This is an application constraint, not a statement that every GIC fails under load.

Match the material to the job:

  • Luting cement: prioritize film thickness, adhesion, solubility and the stresses generated around an indirect restoration; a restorative-strength number alone is insufficient.
  • Liner or base: consider protection of the pulp, handling and compatibility with the overlying restoration rather than maximum bulk strength.
  • Sealant or preventive use: retention, moisture tolerance and fluoride exchange may matter more than the highest compressive-strength value.
  • Restoration: evaluate compressive and flexural data, wear, fracture toughness, water sensitivity and the expected occlusal load together.
  • Orthodontic adhesive: bond strength and durability at the bracket–enamel interface are central; fluoride release alone does not predict bracket survival.

These categories explain why a cement with a lower listed fluoride value can still be appropriate for one indication, while a high-release material may be unsuitable where flexural or fracture demands dominate.

How to compare two GICs without misleading yourself

  1. Identify the exact material type. Record whether each specimen is conventional, resin-modified, metal-reinforced, high-viscosity or additive-modified.
  2. Name the mechanical property. Do not substitute compressive strength for flexural strength, tensile strength, fracture toughness or bond strength.
  3. Align the fluoride protocol. Compare the same solution pH, specimen geometry, storage temperature, solution-refresh schedule and measurement period.
  4. Separate early and late data. Plot the initial wash-out separately from the long-term diffusion or dissolution phase.
  5. Check maturation and storage. Note the time between mixing and testing, whether samples were protected from water loss, and whether they were aged or thermocycled.
  6. Relate the result to the indication. A luting, lining, sealant, orthodontic and load-bearing restorative material has a different failure risk.
  7. Label the evidence level. In-vitro release, bacterial inhibition or remineralization findings establish mechanisms or laboratory effects; they do not automatically establish a patient-level caries-prevention benefit.

What fluoride release means biologically—and what it does not prove

Fluoride concentrations near a GIC can inhibit some bacteria and support remineralization under appropriate laboratory conditions. The 2023 review discusses these mechanisms but also notes that the overall contribution to oral health is uncertain when patients receive fluoride from other sources, including toothpaste. Consequently, a release curve should not be converted into a categorical promise that a restoration prevents recurrent caries.

Clinical effectiveness depends on plaque control, diet, saliva, lesion activity, restoration margins, fluoride exposure from other sources and the longevity of the restoration. Laboratory fluoride release is relevant evidence, but it is only one part of that chain.

Common interpretation errors and how to avoid them

Using one 24-hour value as the lifetime release rate

Early wash-out can dominate short tests. Report the full time course or at least identify whether the number is an initial, cumulative or late-phase result.

Comparing different pH conditions

Acid increases release and can introduce slow dissolution. A neutral-buffer result and an acidic-challenge result answer different questions.

Calling every strength value “strength”

Compressive, flexural, tensile and fracture properties fail differently. State the property, units, specimen preparation and test age.

Assuming category labels predict every product

Type, resin modification, metal reinforcement and viscosity describe broad groups. Powder chemistry, polyacid, additives and manufacturing still create substantial within-category variation.

Turning laboratory biology into a clinical guarantee

Antibacterial inhibition or remineralization in a controlled experiment is mechanistic evidence. It does not replace clinical follow-up or prove that one cement prevents caries better than another.

Bottom line for selecting and evaluating GIC performance

Fluoride release is a dynamic exchange: an early wash-out is followed by diffusion in neutral conditions, while acidic environments increase early release and promote longer-term dissolution. Conventional and resin-modified GICs can release and later recharge fluoride, but their mechanical properties depend on formulation and maturation. Published values range widely, including ADA examples of 650–1,600 μg fluoride over 30 days and compressive-strength examples from 176 to 306 MPa across categories. Those numbers are useful only when their material type and test conditions are preserved.

Choose or compare a GIC by matching the measured property and fluoride protocol to the intended clinical use. Neither the highest fluoride-release figure nor the highest compressive-strength figure, considered alone, establishes superior clinical performance.
