Doctors do not use one universal “after immunotherapy” schedule. For metastatic colorectal cancer, the interval depends on whether treatment is still active, how long immune checkpoint inhibitor (ICI) treatment has continued, the tumour’s mismatch-repair status, and whether the person has had potentially curative surgery.
Current European guidance recommends CT or MRI-based reassessment with carcinoembryonic antigen (CEA) during active treatment, usually every 8–12 weeks. For a specific group—people with deficient mismatch repair or high microsatellite instability (dMMR/MSI-H) tumours who still have a response or stable disease after two years of ICIs and no serious adverse events—the recommended interval is CEA and radiological assessment every three to six months. These are oncology-team plans, not rules that apply to every bowel-cancer survivor.
The monitoring schedules doctors use
“Bowel cancer” is the everyday term; clinical guidelines generally say colorectal cancer, covering cancers of the colon and rectum. The table below separates the settings that are often confused when people ask, “How often will I have scans after immunotherapy?”

|---|---|---|---|
| Metastatic colorectal cancer while treatment is active | Radiological assessment, usually CT or MRI, plus CEA | Every 8–12 weeks | ESMO 2026 guideline |
| dMMR/MSI-H metastatic disease after two years of ICIs, with ongoing response or stable disease and no serious adverse events | Radiological assessment and CEA | Every 3–6 months | ESMO 2026 guideline |
| Non-metastatic colorectal cancer after potentially curative surgery | Serum CEA and CT of the chest, abdomen and pelvis, with colonoscopy in the postoperative pathway | Follow-up for the first three years; CEA at least every six months, at least two CT scans in those three years, and clearance colonoscopy within one year of diagnosis | NICE NG151 and NICE quality standard |
| KEYNOTE-177 clinical trial of pembrolizumab for MSI-H/dMMR metastatic disease | CT imaging of the chest, abdomen and pelvis to measure tumour response | Response assessed at week 9 and every nine weeks afterwards in the trial protocol | KEYNOTE-177 final analysis |
The postoperative NICE schedule is for people treated with potentially curative surgery for non-metastatic disease. It should not be transferred automatically to someone whose metastatic cancer is responding to immunotherapy, and a clinical-trial timetable is not a routine-care prescription.

What doctors check at each follow-up
Scans show where the known cancer stands
CT or MRI lets the oncology team compare current disease with earlier images. They look for shrinkage, stability, new lesions or progression, and for changes at the sites that were involved when treatment began. The exact modality depends on the organs involved, the quality of previous images, kidney function, contrast considerations and local practice. A scan is an assessment of disease status; it does not by itself prove that every cancer cell has gone.
CEA adds a blood-based trend
Carcinoembryonic antigen (CEA) is a tumour marker that ESMO places alongside imaging for active metastatic treatment and for the defined post-two-year ICI group. Doctors interpret a result against earlier values, the scan and the patient’s clinical picture. CEA can be normal even when colorectal cancer is present, and it can rise for reasons unrelated to cancer, so a single result is not a stand-alone detector of recurrence or a definitive measure of response.
Symptoms, examination and treatment effects complete the picture
An appointment also gives the team a chance to ask about pain, bowel or urinary changes, weight, breathing, fatigue, neurological symptoms and general function, and to examine the patient when appropriate. Immunotherapy can cause inflammatory adverse effects in organs such as the bowel, liver, lungs, thyroid and other endocrine glands. The sources cited here do not establish one universal timetable for every symptom review or laboratory test after response; the treating team sets those details around the drugs used and the person’s health.

During active immunotherapy for metastatic disease
When metastatic colorectal cancer is still being treated, ESMO’s 2026 guidance recommends reassessment every 8–12 weeks, usually combining CT or MRI with CEA. The purpose is to decide whether the cancer is responding, remaining stable, progressing or requiring a change in treatment. “Every 8–12 weeks” is a clinical window rather than a promise that every scan will occur on an identical day.

The interval can shift for practical and clinical reasons, such as a recent hospitalisation, a treatment delay, a need to investigate a new symptom or the time required to clarify an indeterminate scan. Patients should ask whether a planned scan is a routine response assessment or an urgent investigation; those are different decisions even when the same imaging machine is used.
After two years of an immune checkpoint inhibitor
For dMMR or MSI-H metastatic tumours, ESMO states: “Patients with dMMR or MSI-H tumours with ongoing response or stable disease after 2 years of ICIs with no serious AEs should undergo CEA and radiological assessments every 3-6 months.” The recommendation applies only when all of those conditions are met: the tumour biology is dMMR/MSI-H, the person has completed two years of ICI therapy, disease is still responding or stable, and there have been no serious adverse events.
This is a less frequent interval than the usual active-treatment reassessment window because it addresses a different treatment moment. It does not mean that immunotherapy has necessarily ended, that monitoring can stop, or that every patient with a good scan qualifies. The oncologist may continue, pause or stop treatment according to the regimen, response, toxicity and the overall treatment plan, then arrange imaging and CEA at an interval within the recommended range.

Stopping an ICI for reasons other than documented progression does not create a single universally specified scan calendar in the material cited here. A pembrolizumab phase III protocol describes continued radiological monitoring after treatment stopped for reasons other than progression and repeat imaging to confirm apparent progression in participants who were clinically stable. Those are trial procedures, not a blanket rule for routine care; the protocol is available through ClinicalTrials.gov.

The practical plan after stopping therefore depends on why treatment stopped, the amount and location of residual disease, the previous scan pattern, CEA results, prior surgery or ablation and the patient’s symptoms. The oncology team should state what the next scan is intended to establish and what finding would change management.
Why surgery changes the follow-up pathway
A person whose non-metastatic colorectal cancer has been removed with potentially curative surgery follows a different surveillance pathway from someone with unresected metastatic disease responding to an ICI. NICE recommends follow-up during the first three years, including serum CEA and CT of the chest, abdomen and pelvis. Its quality statement specifies CEA at least every six months, a minimum of two CT scans during those three years and a clearance colonoscopy within a year of diagnosis.
That schedule is designed to detect local recurrence, distant metastases or a new lesion after curative-intent treatment. It is not evidence that an individual with metastatic cancer should receive exactly two CT scans in three years, nor that a patient who has responded to immunotherapy no longer needs the metastatic-disease assessments described by ESMO.

The trial’s five-year follow-up reported durable responses in its study population, as recorded in the 2025 Annals of Oncology report. Durable response is an important reason to keep follow-up in place, but that report does not establish one surveillance interval for everyone after treatment ends. Trial participants also had eligibility criteria, scheduled assessments and management rules that may not match an individual patient’s situation.
Does a clear scan mean the cancer is cured?
No. A complete radiological response means that currently used imaging does not show measurable cancer in the assessed areas. It does not prove that microscopic disease is absent, and it does not predict the future with certainty. CEA can support the interpretation but cannot certify cure either. Continued appointments are used to detect a change early, monitor delayed treatment effects and reassess health even when scans remain clear.
The meaning of “clear” also depends on the starting point. A person with metastatic disease may have no visible lesions after treatment yet still require metastatic-disease surveillance. Someone who had non-metastatic cancer removed surgically may be following the NICE postoperative pathway. Asking which disease setting the doctor is using prevents a reassuring scan from being interpreted as a promise that follow-up is finished.
How your oncologist chooses the interval
Guideline windows are adapted to the individual. The team considers:

- Whether disease is metastatic, locally advanced or removed with curative intent.
- The sites and volume of cancer before immunotherapy and any residual or treated lesions.
- dMMR/MSI-H status and the specific ICI regimen, duration and reason for stopping or continuing it.
- Changes on previous scans and the direction of CEA results rather than one isolated number.
- Prior surgery, radiation, ablation or other local treatment that can alter how images look.
- New or worsening symptoms, immune-related adverse effects, other illnesses, kidney function and ability to tolerate contrast or a particular scan.
Because these factors can move an appointment earlier or later, a patient should treat the written plan from the oncology team as the operative schedule, not an internet interval detached from the diagnosis.
Questions to take to the next appointment
- What disease setting am I in? Ask whether the plan is for active metastatic disease, surveillance after curative surgery or a defined post-two-year ICI response.
- What is the next test assessing? Clarify whether it is CT, MRI, CEA, a scan to confirm a possible progression or routine surveillance.
- When is the next assessment and why then? Ask how the timing relates to treatment dates, the last scan and any symptoms.
- How will a CEA result be interpreted? Ask what trend would prompt repeat testing, imaging or no change.
- What symptoms should trigger a call before the appointment? Get the team’s preferred contact route and out-of-hours instructions, especially if immunotherapy is ongoing or recently stopped.
- What would change treatment? Knowing in advance whether the team would repeat an equivocal scan, continue treatment, change drugs or consider a local treatment can make the report easier to understand.
When to contact the cancer team sooner
Do not wait for the next scheduled scan to report new, persistent or rapidly worsening symptoms. Contact the oncology service about symptoms such as severe or worsening abdominal pain, persistent diarrhoea, blood in the stool, jaundice, breathlessness, chest pain, confusion, severe weakness, dehydration or a marked change in bowel function. The team can decide whether the symptom is likely to be treatment-related, cancer-related or caused by something else and whether an earlier examination or scan is needed.
Home tumour-marker kits, consumer imaging and supplements are not substitutes for oncology follow-up. The monitoring intervals above come from clinical guidance or a named trial protocol, not from population statistics showing how often every patient is scanned.
