Doctors usually investigate recurring hives by reconstructing the pattern, examining the skin and related symptoms, and ordering tests only when a specific clue justifies them. Hives that recur for more than six weeks fit the chronic urticaria timeframe, but each individual wheal often disappears within 24 hours. Chronic spontaneous urticaria is frequently not an allergy, so a large food or environmental allergy panel is usually less useful than a careful history, photographs and a focused examination.
What “hives that keep coming back” means
Urticaria is the medical term for hives. The overall condition is called acute urticaria when it lasts less than six weeks. Recurrent or persistent hives beyond six weeks are classified as chronic urticaria. This six-week boundary describes the course of the condition, not how long one spot should remain visible.
A typical wheal is raised, itchy and changeable. One spot commonly fades within 24 hours while new spots appear elsewhere, making the episode seem continuous. Deeper swelling, called angioedema, can occur with hives or on its own and may last longer. Swelling without hives can have a different mechanism and should be assessed rather than assumed to be ordinary urticaria.

Acute hives are common: the 2022 EAACI international guideline estimates that about 20% of people experience acute urticaria during their lifetime. That figure does not mean 20% develop chronic hives.
The first step is a detailed timeline
The clinician’s first job is to establish what “recurring” means in your case. Bring the approximate date of the first episode and describe whether attacks happen daily, in clusters, or after symptom-free intervals. The doctor will distinguish the duration of the overall problem from the duration of each individual lesion.
Questions about timing
- When did the first outbreak begin?
- How often do new lesions appear?
- How long does each individual wheal stay in the same place?
- Do old lesions vanish completely, or leave bruising, discoloration or another mark?
- Are there symptom-free periods?
- Did the pattern change after an infection, new medicine, dose change or major exposure?
A photograph taken at the start and again later can help show whether a mark is migrating as expected or remaining fixed. A lesion that behaves unlike ordinary transient wheals may change the diagnostic direction.

What the rash and other symptoms can tell the clinician
Doctors ask you to describe more than “red bumps.” It matters whether the lesions itch, burn or hurt; whether they are pale in the center or uniformly red; how large they become; and which body areas are involved. These descriptions are clues, not proof of a particular cause.
Swelling and systemic symptoms
Tell the clinician about swelling of the lips, eyelids, face, tongue, throat, hands, feet or abdomen. Also report fever, joint pain, unusual fatigue, abdominal pain, vomiting, diarrhea, wheezing or shortness of breath. The combination of hives with a particular symptom may prompt a different evaluation than hives alone.

Features that are less typical of ordinary hives
Fixed lesions that last longer than the usual 24-hour period, painful or burning spots, bruising or residual marks, and prominent systemic symptoms deserve specific attention. Other possibilities listed in professional guidance include urticarial vasculitis, autoinflammatory syndromes, papular urticaria, bradykinin-mediated angioedema, bullous pemphigoid and dermatitis. Atypical findings may lead to specialist review, a skin biopsy or targeted laboratory testing; they do not mean every person with hives needs those procedures.
Reviewing triggers, medicines and exposures
The doctor will look for a repeatable time relationship rather than a one-time coincidence. Be ready to discuss foods, prescription and over-the-counter medicines, supplements, insect exposures, recent infections and physical conditions such as heat, cold, pressure, exercise or sweating, and scratching.

Do not stop a prescribed medicine or begin a restrictive diet on your own while trying to identify a cause. An apparent association can be important, but it is not proof that the exposure caused the hives.
Food, stress and pets
Immediate food allergy usually produces a consistent pattern: the same food is followed by symptoms within a plausible interval, often with other allergic features. In chronic spontaneous urticaria, however, routine food allergy is not the usual explanation. Stress can coincide with flares and may influence symptom perception, but a stressful day does not establish stress as the cause. The same caution applies to pets and other common suspected triggers.
History, review of systems and examination come before broad testing
The current US AAAAI/ACAAI Joint Task Force supplementary material describes history, review of systems and physical examination as the core routine evaluation. It states: “No laboratory testing is required if there are no suspected occult diseases based on history, review of systems, and physical examination.” In other words, a normal, typical presentation does not automatically require a panel of blood tests.

The physical examination looks at active lesions when present, but also checks the rest of the skin and signs of swelling. The clinician reviews general health, recent illness and symptoms that could indicate another disease. If the examination and history are typical for chronic spontaneous urticaria, the evaluation may reasonably stop there initially.

When testing is useful—and when it is not
Testing is most useful when it answers a defined question raised by the history or examination. Untargeted testing can produce false-positive results, unnecessary worry, avoidant diets, expense and additional investigations that do not explain the hives.
| Diagnostic approach | When it fits | What it can and cannot do |
|---|---|---|
| History-led evaluation | The lesions and course are typical, with no signs of another illness. | Can support chronic spontaneous urticaria without naming one external cause; avoids incidental abnormal results. |
| Targeted allergy evaluation | A particular food, medicine, insect or other exposure repeatedly precedes symptoms in a plausible time pattern. | Investigates that exposure; a negative or positive result does not replace the clinical history. |
| Targeted blood or other testing | Symptoms, examination or risk factors suggest a specific disease, infection or inflammatory process. | Addresses the suspected condition rather than screening indiscriminately. |
| Specialist review, possible biopsy or focused tests | Lesions are painful, fixed, bruise, leave marks, or accompany unusual systemic symptoms, or the course does not fit expected urticaria. | Helps distinguish mimics such as urticarial vasculitis or dermatitis; it is not routine for typical hives. |
The 2026 US supplementary guidance notes that testing for parasitic infection may be appropriate when regional prevalence, travel or exposure risk and compatible symptoms support that question. It is not a universal test for chronic hives.
Do you need an allergy test?
Usually, not merely because hives have lasted more than six weeks. Broad panels can detect sensitization—an immune reaction on a test—without proving that the substance causes your episodes. A clinician is more likely to order focused testing when the same exposure repeatedly precedes symptoms and the timing makes biological sense.
Bring the exact product, ingredient list, medicine name or insect context if one exposure appears consistent. Mention whether symptoms occurred after eating a food alone or only when combined with exercise, illness or another factor. Those details help an allergist decide whether testing is likely to clarify the situation.

Do not use a negative panel as permission to ignore a severe reaction, and do not start eliminating numerous foods based solely on an unconfirmed result. Any elimination or reintroduction plan should be discussed with a clinician.
How doctors assess physical or inducible hives
If episodes follow pressure from clothing or straps, cold air or water, heat, sweating, exercise or scratching, the clinician will ask whether the same stimulus reproduces the rash. The timing, body location and consistency of the response matter. A diary can reveal patterns that are difficult to recall during a short visit.
Provocation or other focused assessment may be considered by a specialist when the history strongly suggests an inducible form. The purpose is to answer a specific question about a physical stimulus, not to perform a battery of unrelated tests. Do not deliberately expose yourself to a suspected trigger that has caused breathing problems or extensive swelling.

Sometimes a persistent itchy eruption is not urticaria. Reconsideration is appropriate when individual spots remain in one place, hurt more than they itch, bruise, blister or leave lasting marks; when swelling occurs without hives; when fever, joint symptoms or significant abdominal complaints accompany the rash; or when the course does not respond as expected to the clinician’s management.
Possible alternatives include urticarial vasculitis, autoinflammatory syndromes, papular urticaria, bradykinin-mediated angioedema, bullous pemphigoid and dermatitis. The next step could be a dermatology or allergy/immunology referral, a biopsy, or selected laboratory work. Which option is appropriate depends on the observed features.

What a typical evaluation can—and cannot—establish
A typical history and examination may support chronic spontaneous urticaria while failing to identify one external cause. That is a legitimate clinical conclusion, not evidence that symptoms are imaginary and not proof that a hidden food allergy was missed.
Tests that look for another disease are different from tests intended to predict how someone might respond to a treatment. The 2026 supplementary document treats predictive testing as a separate question; a standard blood panel should not be presented as a guaranteed predictor of treatment response or disease duration.
For general context, AAAAI patient education says that 50% of people with chronic spontaneous urticaria may respond to up to four times the standard dose of a nonsedating antihistamine, and that 80–90% improve within five years. These are treatment-response and disease-course figures, not diagnostic-test yields or promises for an individual. Medication changes should be made with a clinician.
How to prepare for an appointment
A short phone note or calendar is enough; a special notebook is optional. ASCIA’s chronic spontaneous urticaria guidance says, “Keeping a diary of symptoms with a photographic record of hives can be very helpful for this.” Record information while an episode is happening rather than relying on memory weeks later.
- Date each episode. Note the approximate start time, when new lesions appeared and when a particular spot faded.
- Photograph the skin. Take clear, well-lit images from a useful distance and include the date if your phone allows it. Photograph swelling as well as hives.
- Describe symptoms. Record itch, burning, pain, bruising, fever, joint symptoms, breathing changes and stomach symptoms.
- List exposures. Include foods, medicines, supplements, infections, insect contact, exercise, sweating, pressure, heat, cold and scratching.
- Record what you took and what happened. Write down medicine names, dose changes made by your prescriber and whether symptoms improved, without changing treatment solely for the diary.
- Bring product details. Packaging, ingredient lists and a complete medication list can make a suspected exposure easier to evaluate.
If the rash is absent on the appointment day, dated photographs and the timeline may be the most useful evidence available.
When recurring hives are an emergency
Call emergency services or seek immediate medical attention for difficulty breathing or swelling of the tongue or throat. Do not wait for a routine allergy appointment in that situation. Swelling of the face or lips, faintness, repeated vomiting or rapidly worsening symptoms also warrant urgent medical advice.
For persistent or recurring symptoms without emergency signs, arrange a clinician visit and ask whether allergy/immunology or dermatology referral is appropriate. The practical goal is not to order every available test; it is to match each test to a specific clue while recognizing when the pattern is most consistent with chronic spontaneous urticaria.
