Can cold cause fatty lumps under the skin? Sometimes, but doctors first separate two very different problems. Cold panniculitis is an inflammatory injury of the fat layer after cold exposure and usually settles when the exposure stops. Paradoxical adipose hyperplasia (PAH) is an uncommon, delayed enlargement of fat after cryolipolysis (“fat freezing”); it forms a firm, treatment-shaped bulge that may persist and require surgery.
A new, hard, enlarging, painful, or persistent lump—especially one that appeared two to five months after cryolipolysis—needs an in-person assessment. Appearance alone cannot establish the diagnosis, and repeating cold-based treatment over a suspected complication could make evaluation more difficult.
Cold panniculitis and PAH are not the same condition
The phrase “cold-induced fatty growths” is not a standard diagnosis. In practice, it may describe either an injury from cold exposure or a delayed complication of a cosmetic fat-freezing procedure. The history of exposure, timing, skin findings, and shape of the lump guide the work-up.

These conditions can look similar to a patient but have opposite implications for treatment. Cold panniculitis calls for removing the injuring exposure and observing recovery. PAH calls for assessment of a persistent post-procedure mass and discussion of contour-correction options.

How a clinician evaluates a suspected cold-related lump
1. The exposure and procedure history
Expect questions about when the lump or rash began, whether it is changing, and exactly what happened beforehand. For possible cold panniculitis, the relevant details include outdoor or occupational cold, ice or cold packs, cold-water exposure, and repeated whole-body cryotherapy. For possible PAH, the clinician will ask whether cryolipolysis was performed, which body site and applicator were used, the treatment date, and whether the enlargement developed after an initial period of expected swelling or bruising.
Bring the treatment record if you have it, including the treated area, number of cycles, and photographs showing the area before and after treatment. Do not assume that every lump after cryolipolysis is PAH: common temporary effects include redness, bruising, swelling, pain or discomfort, and skin-color changes.
2. Skin and soft-tissue examination
A dermatologist or another qualified clinician examines the color, surface, tenderness, temperature, firmness, mobility, and borders of the lesion. Cold panniculitis may produce tiny hard bumps, raised scaly patches, or deeper tender lumps in areas exposed to cold. The American Academy of Dermatology describes diagnosing its case from the exposure history and skin presentation.

PAH is more suggestive when the enlargement is a clearly demarcated, very firm, raised mass that follows the rectangular outline of the cryolipolysis applicator. FDA consumer guidance describes this delayed treatment-footprint bulge as the characteristic pattern. A clinician also checks for findings that point to another cause of a mass, such as infection, a hernia, a cyst, or a tumor.
3. Imaging or biopsy when the examination is not enough
There is no universally established imaging or biopsy protocol for every suspected case. Testing is selected to answer the individual clinical question and exclude other causes. Published PAH case literature has described a well-demarcated subcutaneous mass, with imaging in the initial report showing adipose tissue in the treated area. Depending on the location and examination, a clinician may choose imaging or, less commonly, tissue sampling; the decision should not be made from an online photograph.
How cold panniculitis is treated
Stop the injuring cold exposure
The essential treatment is to stop the exposure that is damaging the skin and underlying fat. The AAD’s patient guidance puts it plainly: “To get rid of this rash, you must stop injuring your skin.” That means stopping repeated whole-body cryotherapy and avoiding ongoing ice, cold-pack, or other exposure that reproduces the reaction until a clinician confirms it is safe.

Allow the tissue to recover
The AAD says the skin can heal by itself over several weeks after exposure ends. During that period, follow the clinician’s instructions for gentle skin care and monitoring. The rash or lumps may settle gradually rather than disappearing overnight. Darker or lighter patches can remain for months before their color returns toward normal; lingering discoloration does not by itself prove that fat has grown.

When follow-up is important
- Arrange an examination if the lesion is enlarging, unusually painful, very hard, recurrent, or not improving after the cold exposure stops.
- Seek prompt care for spreading redness, drainage, fever, severe pain, blistering, or skin that becomes pale, gray, or black.
- Do not use another cold-based treatment on the area unless a qualified clinician has assessed it.
Cold panniculitis is an inflammatory cold injury, not a general weight-loss condition. Treating the underlying exposure is different from trying to remove a cosmetic fat bulge.
How doctors manage paradoxical adipose hyperplasia after cryolipolysis
Recognize the delayed pattern
FDA describes PAH as “a phenomenon in which fatty tissue grows instead of shrinking in the treated area.” Reported cases develop two to five months after cryolipolysis, rather than during the immediate period of expected swelling or bruising. The bulge is often raised, very firm, and rectangular, corresponding to the applicator area. A clinician confirms the pattern and considers other explanations for a new mass.

Discuss observation versus correction
Unlike cold panniculitis, PAH may not go away on its own. FDA guidance states that the enlarged fatty bulge may require surgery. There is no evidence in the cited guidance that a consumer device, diet, massage, or additional cold treatment reliably reverses it. Until the area is assessed, do not schedule another cryolipolysis session over or near the suspected complication.
Surgical options are individualized
A clinician experienced with PAH and the relevant body area discusses whether liposuction, abdominoplasty, or a combination is appropriate. A small single-center series reported patients treated with liposuction alone and patients treated with liposuction plus abdominoplasty; three patients treated with liposuction required a second procedure. The series reported high satisfaction among its surgically treated patients, but it was a small, retrospective experience and does not establish one universally best operation.

What risks are normal after cryolipolysis, and what is not?
FDA lists redness, bruising, swelling, pain or discomfort, and skin discoloration as common complications that may resolve without intervention. Numbness can last several weeks. Temporary palpable fat nodules, freeze burns, and certain nerve or throat sensations have also been reported, particularly when treatment is performed under the jaw or chin.
PAH and hernia reports are among the more serious complications that can require surgery. FDA advises avoiding treatment in or near a pre-existing hernia or a structurally weak area. A lump that is persistent, enlarging, markedly firm, or shaped like the applicator deserves clinical review rather than reassurance based on the expected short-term side effects.
How common is PAH?
A frequently quoted figure is a 0.0051% incidence estimate reported by the authors of a 2014 JAMA Dermatology case report. That number is historical and study-specific; it is not a current universal risk estimate. The same article cited more than 650,000 cryolipolysis treatment cycles from manufacturer data. That was manufacturer-derived volume reported in the 2014 publication, not an independently measured current total.
The cause of PAH and reliable ways to predict an individual’s risk remain unsettled. The 2014 authors did not identify a single unifying risk factor and described proposed mechanisms as hypotheses. A patient should therefore not be told that a particular body type, sex, age, or treatment setting definitively caused the problem unless stronger evidence becomes available.
When to seek medical care
Contact a clinician soon
- A lump persists or enlarges after a cold exposure or cryolipolysis treatment.
- The mass is hard, clearly outlined, painful, or attached to a treatment footprint.
- Skin discoloration lasts longer than expected or is accompanied by a deeper lump.
- You are unsure whether the original exposure was a cosmetic procedure, cryotherapy, ice injury, or another source of cold.
Seek urgent assessment
- Rapidly increasing swelling, severe or escalating pain, fever, pus, or spreading redness.
- Blistering, loss of sensation that is worsening, or skin turning gray, black, or unusually pale.
- A painful bulge near a known hernia or a structurally weak area, especially with vomiting or inability to pass stool or gas.
These warning signs can indicate a complication other than either cold panniculitis or PAH and should not be triaged from a photograph.
