Doctors do not decide on a targeted cancer therapy from a mutation result alone. They combine the tumour’s exact biomarker and test quality with its origin, stage, previous treatment, current condition, the drug’s evidence and approval status, likely harms, access, and the patient’s preferences. A biomarker match can make a treatment reasonable to consider; it cannot guarantee that the cancer will respond.

The process is a clinical interpretation: obtain an appropriate sample, identify and verify a relevant target, check that the drug has evidence for this cancer and situation, and reassess if the disease changes.

What “targeted therapy” and “biomarker” mean

Targeted therapies are medicines designed to interfere with proteins or molecular pathways that help cancer cells grow, divide, survive or spread. A biomarker is a measurable feature of a tumour that may help guide treatment. It can be a DNA change, a gene fusion, a protein level, a change in how genes are switched on, or another molecular characteristic.

Biomarker testing examines the cancer itself. It is different from a hereditary (germline) test that looks for an inherited variant associated with cancer risk in a person and their relatives. A tumour test can sometimes reveal a finding that also has inherited implications, so the oncology team may recommend genetic counselling or a separate blood or saliva test when appropriate.

Some cancers commonly contain a target for which treatments are established; in others, broad genomic profiling may be considered, particularly when disease is advanced. The usefulness of testing depends on the diagnosis and the treatment decisions that are actually available.

The clinical decision pathway

1. Define the cancer and the immediate clinical problem

The team first confirms where the cancer began, its stage, the sites and amount of spread, how quickly it is progressing, and what treatment has already been given. The same molecular finding can have different significance in two cancer types, or at different stages. Current symptoms, organ function, performance status, other illnesses, medicines and the patient’s goals also affect whether a drug can be used safely.

This step prevents a molecular result from being treated as the whole diagnosis. Tumour type, size and spread remain relevant even when a test identifies an apparently attractive target.

2. Ask whether a target is relevant to this cancer

Clinicians ask whether the detected feature is biologically involved in the tumour and whether a medicine is known to act on it. A report may classify a result as associated with a treatment, potentially resistant to a treatment, benign, or of unknown significance. Only findings with credible clinical and biological support should influence a treatment choice.

A change labelled “variant of unknown significance” is not evidence that a targeted drug will work. Conversely, the absence of a familiar target does not automatically mean that every targeted option is impossible; it means the team must rely on other evidence and approved indications.

3. Select the sample and assay

For a solid tumour, testing may use tissue removed during surgery or obtained by biopsy. The laboratory can examine one marker or many genes and proteins, depending on the clinical question. Tissue may be unsuitable if there is too little tumour, if it has degraded, or if obtaining another biopsy would be unsafe.

A liquid biopsy analyses tumour-derived material in blood. It can be useful when a tumour is difficult or dangerous to reach, when tissue is exhausted, or when a rapid look for certain alterations is needed. A blood test can also be negative simply because the tumour has released too little detectable material; a negative result does not always rule out a target that tissue testing might find.

Testing approach What it can offer Important limitations
Tumour tissue Direct analysis of cancer cells; can support DNA, RNA, protein and other assays, depending on the specimen. Requires an adequate, representative sample; a biopsy may carry risk; different parts of a tumour may not share the same biomarkers.
Blood-based liquid biopsy Less invasive sampling and a way to look for circulating tumour alterations when tissue is unavailable or unsafe to obtain. Some tumours shed little detectable material; a negative blood result may need confirmation with tissue if suspicion remains.

The laboratory report should identify the specimen, assay, quality metrics and the exact alteration or protein finding. An oncologist, pathologist and, in complex cases, a molecular tumour board interpret the result together.

4. Determine whether the test is a companion diagnostic

A companion diagnostic is a test specifically paired with a particular drug and indication to identify patients whose tumours have the relevant marker (or, in some cases, to identify a marker associated with avoiding that drug). Its label is specific to the test, medicine, cancer type and use; a result from a different assay is not automatically interchangeable.

The absence of a companion-diagnostic label does not by itself prove that a finding is useless. In a National Cancer Institute (NCI) report published 15 October 2020, physician and researcher Dr Park said a blood-test finding without such a label “doesn’t mean it couldn’t work.” He added: “It just means [the company] hasn’t gone through the whole process of getting [their test approved] as a companion diagnostic” for that particular drug. Ana Robles, Ph.D., of NCI’s Office of Cancer Clinical Proteomics Research, noted in the same report that “The companion diagnostic labels only cover a few genetic changes that match a patient to a particular therapy,” and that “the regulations haven’t caught up to the science.” Those comments describe the regulatory context at that time, not a guarantee of benefit or current regulatory advice.

5. Match the finding to evidence and legal status

The team then asks what kind of treatment option the result supports:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • On-label treatment: The medicine is authorised for this cancer and the patient’s clinical situation, subject to the product’s requirements.
  • Off-label treatment: The medicine may be used outside its formal indication when a clinician judges that evidence and circumstances justify it. Coverage and supporting data can be uncertain.
  • Clinical trial: A study may offer access to a targeted drug or combination when standard options are unsuitable or when the biomarker is still being evaluated.

Evidence must be relevant, not merely molecularly similar. Doctors consider the cancer type, stage, line of treatment, prior medicines, characteristics of the assay, response data, durability of benefit, known resistance mechanisms and the patient groups represented in the studies. A result found in one cancer does not automatically predict the same benefit in another.

6. Weigh expected benefit, harms and alternatives

Targeted medicines can have substantial side effects, even when they are more selective than conventional chemotherapy. Possible problems depend on the drug and may include effects on the skin, bowel, liver, heart, lungs, blood counts or other organs, as well as interactions with existing medicines. The team balances the chance and duration of benefit against monitoring requirements, treatment burden and the patient’s priorities.

Alternatives can include another targeted medicine, immunotherapy, chemotherapy, radiation, surgery, best supportive care or a clinical trial. “Best” is therefore a shared decision rather than an automatic instruction generated by a laboratory report.

7. Check practical access

Testing availability differs between hospitals and laboratories. Costs vary by assay, and insurance coverage depends on the test, the indication and the evidence for using it. A treatment can be scientifically plausible but inaccessible because the medicine is not approved locally, the insurer will not cover an off-label use, a co-payment is unaffordable, or a suitable trial is not available nearby. The care team can discuss authorisation, financial counselling, referral and trial options.

How to read the result without over-interpreting it

A positive or “actionable” finding

“Actionable” generally means that a treatment, trial or management decision is associated with the finding. It does not state how likely the tumour is to shrink, how long a response might last, or whether the patient can safely receive the treatment. Ask which evidence tier the laboratory uses and whether the recommendation is approved, guideline-supported, investigational or based mainly on laboratory data.

No actionable match

A report may find no target linked to an available treatment, or it may be technically inconclusive. That outcome does not mean that no treatment exists; it means the particular test did not identify a usable match. Reasons include insufficient tumour, poor-quality DNA, a blood sample with too little circulating tumour material, a target for which no drug is available, or a trial that cannot be accessed.

Resistance or an unfavourable finding

Some alterations predict that a drug is unlikely to work or that resistance has developed. The oncologist may use such a result to avoid a harmful or futile treatment and select another approach. The report’s interpretation must be considered with the clinical picture rather than read as an independent prescription.

Why a good biomarker match can still fail

Tumours are mixtures of cells. One group may carry the target while another does not, allowing untreated cells to continue growing. A sample from one region may therefore miss important variation elsewhere. Tumour DNA and protein patterns can also evolve after treatment, at relapse or as the disease spreads.

Laboratory and clinical factors add uncertainty. The specimen may contain too few cancer cells, the assay may not detect every alteration, or a finding may be biologically real but not sufficiently predictive in clinical studies. Patient factors—such as organ function, drug interactions, immune status and the ability to attend monitoring—can also change the risk–benefit balance.

NCI explains that these sources of heterogeneity, molecular change and limited tumour access make accurate response prediction difficult. A test is one input to a treatment decision, not a complete forecast.

When doctors consider retesting

An old specimen may no longer represent the cancer after therapy or at recurrence. If disease returns or progresses, the team may request a new tissue sample, a liquid biopsy or another assay. Retesting is especially relevant when the clinical behaviour has changed, when a previous sample was small or poor quality, or when a new treatment requires a different biomarker.

Retesting is not automatic for every patient. The likely effect on management, biopsy safety, turnaround time, cost and the availability of a suitable treatment determine whether it is worthwhile.

What is known about liquid-biopsy examples

An NCI article dated 15 October 2020 reported FDA approvals of Guardant360 CDx and FoundationOne Liquid CDx for certain cancer uses. In that report, Guardant360 CDx was described as checking changes in more than 60 genes, while FoundationOne Liquid CDx was described as assessing changes in more than 300 genes plus other features. Those were technical descriptions from 2020, not a current comparison or a complete list of indications. Current FDA and manufacturer labelling must be checked for present-day use.

The number of genes on a panel is not a measure of clinical usefulness by itself. A smaller assay may be sufficient for a narrowly defined question, while a broader panel can produce findings whose significance is uncertain. What matters is whether the assay is analytically reliable and whether its result can change a decision for this patient.

A practical checklist for the oncology appointment

Bring the complete laboratory report, not only a summary line in the patient portal. These questions can clarify what the result means:

  • What exact biomarker was found, and was it detected in tissue or blood?
  • Which assay identified it, and was the sample adequate and representative?
  • Is this test validated or labelled as a companion diagnostic for the proposed drug and indication?
  • Is the treatment approved for my cancer type, stage and current line of therapy, or is it off-label or trial-based?
  • What evidence suggests that this finding predicts benefit for someone in my situation?
  • What benefit is realistic, how will it be measured, and how soon would we know whether it is working?
  • What side effects, monitoring, interactions and effects on daily life should I expect?
  • What alternatives are available if I do not take this treatment or if it stops working?
  • Is the specimen current enough, or could the tumour have changed since it was collected?
  • What will the test and treatment cost, what might insurance cover, and are relevant trials accessible?

These questions support a discussion with the treating team; they cannot substitute for an individual medical assessment.
