These seven major epidemics and pandemics spread through very different pathways: infected fleas, respiratory particles, contaminated food or water, or exchange of infected body fluids. They are a curated cross-section—not a definitive ranking of the seven deadliest—because their death estimates vary in scope, reliability and time period, and some describe ongoing epidemics rather than a single outbreak.

Why there is no reliable “deadliest seven” ranking

“Plague” is often used informally for a devastating epidemic, but scientifically it refers to disease caused by the bacterium Yersinia pestis. The seven cases here use the broader everyday meaning. The World Health Organization (WHO) lists several major historical epidemics, but its historical overview does not establish a comparable mortality ranking across all seven cases in this article. Medieval estimates, modern annual counts and cumulative totals from ongoing epidemics measure different things.

The comparison below makes those differences visible. A death toll for one outbreak is not directly comparable with an annual estimate or a running total accumulated over decades.

Smallpox Spread across regions over centuries CDC estimates an average of 3 deaths per 10 people who contracted it.
Cholera pandemics First recorded pandemic in the nineteenth century No pandemic-wide death total established in the cited sources.

How did each epidemic spread?

Justinian’s plague: a historic outbreak, uncertain transmission balance

WHO identifies the plague associated with Justinian as a sixth-century epidemic. Plague can spread in more than one way today: bubonic disease most often follows the bite of an infected flea, while pneumonic plague can pass between people through respiratory particles. Those modern mechanisms help explain the disease, but they do not establish how much each route contributed during Justinian’s outbreak. The available cited sources do not establish a reliable death toll for it. See the WHO report on epidemic-prone infectious diseases and the WHO historical overview.

Black Death: flea-borne plague, with more than one possible route

The fourteenth-century Black Death was caused by Yersinia pestis. Infected flea bites commonly transmit bubonic plague; pneumonic plague can also spread from person to person through respiratory particles. The relative contribution of those routes is not established for every location, so the familiar flea-and-rat explanation should not be treated as the sole route everywhere. WHO reports more than 50 million deaths in Europe during the Black Death; that geographic scope should not be turned into a settled worldwide total. The current WHO plague fact sheet describes the disease and its transmission.

Smallpox: movement of people carried it across trade and colonial routes

Trade, migration, exploration and colonization helped carry smallpox into new populations, according to the CDC’s history of smallpox. The disease killed an average of 3 out of every 10 people who contracted it, CDC estimates. Before vaccination, variolation was used; Edward Jenner’s 1796 observations provided the basis for vaccination. The last naturally acquired case was reported in 1977, and the World Health Assembly declared the world free of smallpox on 8 May 1980.

Cholera: contaminated food and water link disease to infrastructure

Cholera follows a different pathway from flea-borne or respiratory infection: people become infected by consuming food or water contaminated with Vibrio cholerae. WHO dates the first recorded cholera pandemic to the nineteenth century. Because exposure is tied to water and food, safe water and sanitation are central to interrupting transmission. The WHO cholera fact sheet describes the infection and pandemic history; CDC’s review of public-health achievements documents the broader role of clean drinking water and sanitation in controlling infectious disease. The cited sources do not establish one comparable death total for cholera pandemics as a group.

1918 influenza: rapid spread in three waves, origin unresolved

CDC’s review describes three waves that spread more or less simultaneously across Europe, Asia and North America over roughly 12 months in 1918–1919. It concludes that the historical and epidemiological evidence is insufficient to identify a definite geographic origin. Death estimates also differ: a 1999 CDC review recorded an estimate of 20 million deaths worldwide in less than a year, while another CDC history page gives an estimate of 50 million. These are published estimates, not a settled count. The estimates and uncertainty are discussed in the CDC review, the CDC pandemic history and the CDC Emerging Infectious Diseases review.

No vaccine or antibiotics for secondary bacterial infections were available at the time. Public-health responses included isolation, quarantine, hygiene, disinfectants and limits on public gatherings, but measures were applied unevenly. CDC recounts these control measures in its review of infectious-disease control.

HIV/AIDS: transmission through specific body fluids

HIV can be transmitted through exchange of infected blood, breast milk, semen or vaginal secretions. WHO estimated 44.2 million cumulative HIV-related deaths through 2025 and 41.0 million people living with HIV at the end of 2025. These figures, published in the WHO HIV and AIDS fact sheet, describe an ongoing epidemic: the cumulative death estimate is not a toll from one bounded outbreak.

Tuberculosis: a major present-day burden

Tuberculosis is a bacterial infectious disease that most often affects the lungs. WHO estimated 1.23 million deaths in 2024, including 150,000 among people with HIV. These are annual estimates for 2024, not a historical-pandemic total. The figures are reported in the WHO tuberculosis fact sheet.

What the spread routes reveal

These cases show why there is no single “plague” mechanism. Plague can involve infected fleas or respiratory transmission, cholera is tied to contaminated food and water, and HIV transmission involves particular infected body fluids. Smallpox and the 1918 influenza pandemic illustrate how epidemics can travel across regions and populations; tuberculosis remains a substantial contemporary health burden. A route established for a disease today should not automatically be treated as proof of how a particular ancient outbreak spread.

Death figures also need their labels. The Black Death estimate here is for Europe; the 1918 influenza totals are competing worldwide estimates; HIV’s is cumulative through a stated date; and tuberculosis’s is for a single year. Keeping those distinctions intact is essential to understanding both the scale of these events and what the evidence can support.
