COVID-19 variant names and monitoring categories answer different questions. Names such as Pango lineages and Nextstrain clades identify branches of the virus for scientific tracking; WHO’s categories—variant under monitoring (VUM), variant of interest (VOI), and variant of concern (VOC)—signal how public-health authorities are prioritizing assessment. Neither a new name nor a monitoring designation, on its own, proves that a variant causes more severe disease.
Why COVID-19 variants have more than one kind of name
SARS-CoV-2 changes over time. Scientists track groups of related viruses so they can describe which genetic branch a sample belongs to and follow changes in that branch. Public-health organizations also need a way to communicate which lineages merit particular attention. Those are related tasks, but they are not the same task, so the systems use different kinds of labels.
WHO introduced simpler public labels in May 2021. These labels were intended to make discussion easier; they did not replace scientific nomenclatures. In its 31 May 2021 announcement, the World Health Organization said that public labels do not replace existing scientific names assigned by GISAID, Nextstrain, and Pango, which convey scientific information and continue to be used in research.
| Term or system | What it identifies | Main purpose | Is it a risk ranking? |
|---|---|---|---|
| WHO public label | A label used for public communication when WHO assigns one | Make relevant variant names easier to discuss | No. The label itself is not evidence of danger. |
| Pango lineage | A scientific lineage, or branch, of SARS-CoV-2 | Track and describe viral lineages in scientific work | No. A lineage name is nomenclature, not a risk category. |
| Nextstrain clade | A grouping used in scientific tracking | Organize and communicate information about viral groups | No. A clade name is not a WHO severity verdict. |
| VUM, VOI, or VOC | A WHO monitoring and assessment category | Indicate the level and type of public-health attention a variant warrants | These are categories of monitoring and assessment, not interchangeable lineage names. |
WHO’s tracking material presents Pango lineages and Nextstrain clades alongside other information. As a result, a tracking entry may contain more than one identifier for the lineage it describes. These identifiers are not competing names from which a reader must choose the “correct” one: they reflect different nomenclature systems and uses.

What VUM, VOI, and VOC mean
WHO groups monitored variants according to how much attention and assessment they warrant. The categories are useful signals about monitoring priority, but a category should be read with its definition and the evidence in the corresponding risk evaluation. WHO’s Q&A describes the categories; the precise working definitions can change, so use WHO’s current wording rather than relying on an older summary.
| WHO category | What the category signals | How to interpret it |
|---|---|---|
| Variant under monitoring (VUM) | A lineage may warrant prioritized attention and monitoring. | It is being watched; that designation alone does not establish a change in severity or other harm. |
| Variant of interest (VOI) | Changes have known or suspected relevance to viral behavior or human health. | The relevant changes warrant assessment, but the category is not a conclusion that every suspected effect has been demonstrated. |
| Variant of concern (VOC) | The variant meets VOI criteria and additional criteria indicating heightened concern. | It represents a higher level of concern under WHO’s framework; consult the risk evaluation for the evidence and implications. |
The distinctions are about monitoring and public-health assessment, not a simple scale that can be inferred from a lineage’s name. A VUM entry does not mean “harmless,” and a VOI or VOC label does not, by itself, tell an individual how likely they are to become ill. The underlying evidence and the dated assessment matter.
Why the Greek-letter policy changed
When WHO introduced public labels in 2021, Greek letters became familiar labels for some variants. The system later changed. On 15 March 2023, WHO updated its tracking system and working definitions and specified that Greek letters would thereafter be assigned only to VOCs. VOIs are referred to using established scientific nomenclatures, such as Nextstrain and Pango.

That means a reader should not expect every lineage being monitored to receive a Greek-letter name. A lineage may appear in WHO material under a scientific identifier and a monitoring category without having a Greek-letter public label. The category and the name do different jobs: one is a classification for monitoring and assessment, while the other identifies a viral group or, where assigned, supplies a public-facing label.
How WHO monitoring information is organized
WHO’s tracking page brings together identifiers and dated evidence about monitored lineages. Depending on the entry, it can list the lineage, clade, genetic features, earliest documented samples, designation date, and linked risk evaluations. Those fields help readers distinguish what has been observed from what has been concluded about its significance.
- Identify the lineage. Note the scientific lineage and clade shown in the entry. These identify the group being tracked; they do not state how dangerous it is.
- Check the category and designation date. Determine whether WHO lists the lineage as a VUM, VOI, or VOC, and when the designation was made. Do not assume a designation is permanent or that an older label reflects the latest assessment.
- Read the sampling information in context. Earliest documented samples describe when evidence of the lineage was recorded, not necessarily when it first emerged. Keep the date attached to the entry rather than treating it as a definitive origin date.
- Separate genetic features from health effects. A listed feature describes a genetic observation. It does not, by itself, establish a change in transmission, severity, or the performance of countermeasures.
- Open the linked risk evaluation. Use the assessment to understand what evidence WHO considered, what it says about potential implications, and what remains uncertain.
- Keep the date and geography with any finding. Variant proportions and assessments can change. If describing an observation, include the assessment date and the locations or surveillance context WHO specifies.
This is why the tracking page is more useful as an evidence record than as a stand-alone danger list. The designation tells you that WHO is applying a monitoring framework; the accompanying fields and risk evaluation provide the details needed to interpret why.

How to read a variant risk assessment
A risk evaluation is the place to look for the evidence behind a designation. It may describe a lineage’s circulation, genetic features, or possible implications, but those observations should not be collapsed into one claim. A lineage can become more common in available surveillance without that fact alone showing that it causes more severe disease.

One free scan finds every outdated or missing driver and matches the right update for your exact hardware.Free scan · exact hardware matchWhen reading an assessment, distinguish among three kinds of statement:
- Observation: what surveillance or sequence data show, such as an increase in the lineage’s detected proportion in a specified place and period.
- Interpretation: what WHO says those observations may mean for viral behavior or public health, including the limits of the evidence.
- Assessment: WHO’s evaluation of the lineage’s risk or its expected implications for measures such as vaccination.
Pay attention to the comparison used in the assessment. A statement that no increased clinical severity has been indicated compared with other circulating variants is a relative, dated conclusion; it is not the same as saying the virus cannot cause serious illness. Likewise, an expectation that existing vaccines continue to protect against severe disease is not a claim that vaccination prevents every infection.

The source and date are essential parts of a finding. WHO’s assessment reflects the evidence available to it at that time and in the surveillance context described. It should not be treated as a permanent property of the lineage or generalized to locations and periods the evaluation does not address.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.A dated example: PQ.16.1.1
WHO’s PQ.16.1.1 risk evaluation, dated 27 July 2026, described increasing proportions driven largely by detections in the Western Pacific, particularly Singapore. WHO reported that available surveillance did not indicate increased clinical severity compared with other circulating variants and expected existing vaccines to continue protecting against severe disease.

The increase was described in the context of available surveillance and was concentrated largely in the named region and location. The severity statement was a comparison with other variants circulating at the time, not a universal prediction. The vaccine statement concerned expected protection against severe disease, not a guarantee against infection. WHO’s VUM list is also marked as of 27 July 2026; that date describes the list’s status, not a promise that the same lineages or assessments remain current indefinitely.

This example shows why the lineage name cannot substitute for the evaluation. The useful information is the dated observation, its geography, and WHO’s separately stated assessment of severity and vaccine protection—not the fact that PQ.16.1.1 has a distinct name.
Why a new name or mutation does not automatically mean greater danger
Viral changes can affect spread, severity, or how well countermeasures perform. But WHO notes that most changes have little to no impact on virus properties. A mutation list or a new lineage identifier is therefore a prompt to interpret evidence, not a verdict about clinical risk.
Headlines can blur this distinction by treating “new variant” as shorthand for “more dangerous variant.” The terms are not equivalent. A lineage can be newly identified or newly prominent in monitoring without evidence in the available assessment that it causes more severe disease. Conversely, monitoring exists because public-health authorities need to detect and evaluate changes that could matter; the absence of a dramatic conclusion in one assessment does not make continued tracking pointless.

For a claim about a variant’s impact, look for the dated source, the geography and surveillance context, the comparator, and the specific outcome being discussed. “More cases detected,” “increased proportion,” and “increased severity” describe different findings and should not be used as substitutes for one another.
A quick way to interpret a variant report
- If you see a new scientific name, treat it first as an identifier for a lineage or clade, not as a WHO risk category.
- If you see VUM, VOI, or VOC, check WHO’s current definition and the designation date.
- If a report lists mutations, look for evidence about their effect rather than inferring an effect from their presence.
- If a report says a lineage is increasing, check the time period, place, and data context before interpreting what the increase means.
- If you see a claim about severity or vaccines, read the underlying assessment and preserve its comparison, date, and qualification when repeating it.
The practical rule is simple: read the name as an identifier, the category as a monitoring signal, and the risk evaluation as the source for what is known about potential health implications.
