Community TB screening brings testing closer to people who may not otherwise seek care, then directs anyone with a positive screen to diagnostic evaluation. A positive screen is not a TB diagnosis: the program succeeds only when people can complete the next steps and, if TB is confirmed, begin treatment promptly.
What community TB screening is—and what it is not
Systematic TB screening is a proactive process for identifying people who may have TB disease, especially people who face higher risk or barriers to health services. Rather than waiting for someone to recognize symptoms and visit a clinic, a program offers screening in settings where selected groups live, work, receive care, or gather. The World Health Organization (WHO) describes screening as a way to find people who might otherwise be missed by routine, or passive, case detection.
Screening is a triage step, not a final diagnosis. A symptom screen, chest X-ray, or other screening result may indicate that someone needs further evaluation; it does not by itself establish that they have TB. That distinction matters both for individuals—who should not be labelled as having TB on the basis of a screen alone—and for programs planning diagnostic capacity and communicating results.

Who screening programs prioritize
WHO strongly recommends systematic screening for several groups with elevated risk or particular exposure. Programs may also consider groups with other risk factors or substantial barriers to care, based on local circumstances. These are population-level priorities, not a claim that any individual’s identity or circumstances mean they have TB.

Community-wide screening is a distinct, broader approach. WHO says it may be conducted in areas where TB prevalence in the general population is 0.5% or higher, conditional on adequate coverage and rigorous screening and diagnostic tools. That threshold is not a blanket instruction to launch a campaign wherever it is met. A program still has to consider local epidemiology, population access, test choices, available staff, and whether people who screen positive can obtain diagnostic assessment and treatment.
How programs find people who might otherwise be missed
Systematic screening is generally provider-initiated: health workers or program teams offer it instead of waiting for a person to request a TB test. Someone may not recognize symptoms, may not think they need care, or may face practical or other obstacles to reaching a service. Outreach can bring screening to the population rather than relying only on clinic attendance.

- Homes and communities, especially for contact investigations.
- HIV and primary-care clinics, as well as outpatient departments.
- Workplaces and institutions, including penitentiary settings.
- Mobile outreach campaigns that bring services to communities.
The setting is a program choice, not a single prescribed model. A contact investigation may begin with a known person diagnosed with TB and reach household or close contacts; a workplace or mobile campaign may be organized around a broader risk group or access need. Programs should use locally appropriate, respectful language and avoid treating group membership as an individual diagnosis.
What happens from outreach to care
A useful way to understand community screening is as a connected pathway. Each stage depends on the next: finding people has limited value if a positive screen cannot be followed by diagnosis, and diagnosis alone does not meet the individual aim if treatment is delayed or inaccessible.
- Set the target. Use local TB epidemiology, risk-group information, and barriers to access to decide whom the program aims to reach. WHO’s guidance advises tailoring the approach rather than treating one strategy as right for every setting.
- Choose a feasible outreach setting. Match the target population to an accessible place—such as a home or community, clinic, workplace, institution, or mobile campaign—and to services that can support the next steps.
- Offer a screening algorithm. Apply the locally selected screening method or sequence. Depending on population and available services, an algorithm may use one screening option or combine methods in parallel or in sequence.
- Arrange diagnostic evaluation after a positive screen. Explain that the result calls for further assessment, then make the appropriate diagnostic service available at the screening site or through referral. The required investigations depend on the person’s circumstances and clinical assessment.
- Connect people with confirmed TB to clinical care. The pathway should support prompt treatment initiation for people diagnosed with TB, in line with local clinical and public-health guidance.
WHO’s global materials do not establish one standard community-worker workflow, referral form, transport arrangement, or tracing protocol for all countries. Those operational details need to come from the relevant national or local program. Readers seeking screening should ask their local health service or public-health authority where services are available; the WHO global guidance cannot confirm that a specific clinic or campaign operates in a particular place.

WHO lists symptom screening, chest radiography (CXR), and molecular WHO-recommended rapid diagnostic tests on sputum—such as Xpert MTB/RIF and Truenat—as possible screening options. For people living with HIV, C-reactive protein (CRP), a point-of-care blood test, is another option. These methods are not interchangeable in every context. The program’s algorithm should reflect the population, local guidance, available services, and the ability to follow up results.

Some algorithms combine methods. A program might use more than one test in parallel or use tests in sequence, but there is no universal order that should be assumed for every participant. The choice involves both test performance and practical considerations such as staff, equipment, infrastructure, population coverage, and how easily a person can move from screening to diagnostic assessment.
Tests for TB infection, including tuberculin skin tests and interferon-gamma release assays, are not used to screen for TB disease. A test aimed at TB infection should not be confused with an evaluation for active disease.
What a positive screen means
A positive screen means further testing or clinical assessment may be needed; it does not confirm TB disease. WHO notes that confirmation can involve sputum, urine, blood, an X-ray, or other investigations, depending on the case and the clinical assessment. Not every participant will need the same tests or sequence.

In other programs, the person may need a referral to another center. That makes a clear, workable connection to diagnostic services essential: a screening result is only useful if the person can reach the next step and receive an appropriate assessment.

WHO published updated consolidated TB diagnosis guidance in 2025, addressing detection of TB infection, TB disease, and drug resistance, along with updated diagnostic technology classes. Programs should align their implementation with current national guidance and services. A general description of screening cannot determine the right diagnostic work-up for an individual.

Screening is not complete when a campaign counts people reached or tests administered. WHO frames the individual objectives as early detection and prompt treatment initiation, helping reduce poor outcomes and social and economic consequences. At community level, screening aims to reduce TB prevalence and transmission. These objectives depend on a functioning route from an initial screen to diagnosis and treatment.
That route requires capacity beyond the screening event itself. WHO notes that screening increases demand for diagnostic testing and for treatment of additional people identified. Before expanding outreach, a program needs to consider whether staffing, funding, diagnostic services, and treatment capacity can absorb that demand. A campaign that produces more positive screens than the system can assess risks leaving people without the care the screening was meant to make possible.

Access also includes cost. WHO states that people undergoing screening should not pay out-of-pocket for screening tests or any resulting TB diagnosis or treatment, and should not suffer financial hardship as a result of screening. In the WHO 2024 Q&A, the wording is explicit: “People who are undergoing screening should not pay out-of-pocket expenses for screening tests or any resulting diagnosis or treatment of TB, nor should they suffer financial hardship as a result of screening.”

How programs choose an approach
WHO does not identify one universally best outreach model or screening test. Program decisions should weigh the local population and health system together, rather than choosing an approach solely because it is technically available. The 2022 operational handbook warns against indiscriminate mass screening: it can be costly, have low benefit, and produce false positives. Targeting and tailoring screening to context are central safeguards.
- Risk and access: Which population has elevated TB risk, and who is least likely to reach routine care?
- Screening and diagnostic performance: What methods suit the group, and what are the consequences of false-positive results and the follow-up they require?
- Operational capacity: Are trained staff, equipment, infrastructure, diagnostic services, and treatment available?
- Continuity: Can people move quickly and reliably from screening to diagnostic confirmation and, when needed, treatment?
- Coverage and costs: Can the program reach enough of the intended population without shifting screening, diagnosis, or treatment costs onto participants?
The available WHO global sources do not provide a single comparative cost-effectiveness figure for community screening models or a universal rate for completion of linkage to care. Local programs need to assess those questions using their own population, service capacity, and implementation data rather than assuming a global benchmark.
What the figures say—and what they do not
WHO’s 2021 screening guidance materials cite an estimate that, in 2019, 2.9 million of the 10 million people who fell ill with TB were not diagnosed or reported. This is historical context for why case-finding beyond routine presentation matters; it is not a current annual estimate.
