A citation is verified only when the cited source exists, contains the relevant evidence, and actually supports the specific claim being made. A working link, plausible-looking reference, or AI-generated summary is not enough. Clinicians should check answers claim by claim, inspect the original passage, assess whether the evidence fits the patient and setting, and document any uncertainty before using the information in care.

Why a citation needs more than a working link

Generative AI can attach convincing references to an answer while overstating what a paper found, combining results from different studies, citing an outdated recommendation, or inventing a reference that looks authentic. Even when a reference is real, it may support only one clause of a long sentence. The rest may be an inference, an unsupported number, or a recommendation the source never made.

That makes two questions separate:

  • Does the source exist and is the citation accurate? This is a source-verification question.
  • Does the evidence justify this action for this patient? This is a clinical judgment and applicability question.

Verifying references is necessary, but it does not validate a diagnosis, treatment choice, dose, or risk estimate. A qualified clinician remains responsible for interpreting evidence in the patient’s circumstances.

A claim-by-claim verification workflow

Use the following six-step process for any AI answer that could influence diagnosis, treatment, triage, prognosis, consent, or patient advice. It is a practical synthesis of medical education work, professional policy, and citation-evaluation studies, not a regulator-mandated checklist.

1. Decompose the answer into discrete claims

Read the answer sentence by sentence and split compound statements into units that could be checked independently. Include:

  • Numerical claims, such as effect sizes, sensitivity, prevalence, or dosing intervals.
  • Recommendations, contraindications, and statements that a guideline “recommends” or “does not recommend” something.
  • Quoted or paraphrased guidance.
  • Claims about a particular patient’s diagnosis, risk, eligibility, or likely response.
  • Implicit comparisons, such as saying one therapy is safer or more effective than another.

For example, “Drug A reduces recurrence by 30%, is preferred by current guidance, and is safe in renal impairment” is at least three claims. One citation may support the recurrence result while saying nothing about the guideline preference or kidney safety.

2. Identify the evidence each claim requires

Match the claim to an appropriate evidence type before opening the reference. A broad educational explanation may be supported by a reputable review. A current recommendation generally calls for a guideline in the relevant jurisdiction. A numerical treatment effect may require the primary trial or a systematic review. Dosing, contraindications, and interactions should be checked against an authoritative drug label or equivalent clinical reference. A patient-specific assertion requires the patient’s actual history, examination, test results, and applicable clinical standards; a general paper cannot supply those facts.

Claim type Evidence to seek Questions to ask
General mechanism or definition Authoritative review, textbook, or professional reference Is the explanation current and accurately limited?
Effect, risk, or statistic Primary study or systematic review What population, comparator, outcome, and uncertainty were reported?
Recommendation Current guideline or official policy Which organization, version, geography, and recommendation strength apply?
Dose, contraindication, or interaction Current product information or authoritative medicines reference Does the patient’s age, organ function, indication, and co-medication match?
Patient-specific conclusion Complete clinical record plus appropriate evidence What information is missing, and who is accountable for the decision?

3. Confirm that the reference exists and matches

Follow the DOI, PMID, journal record, guideline page, or other authoritative identifier rather than trusting the text supplied by the model. Confirm the title, authors or issuing body, publication date, version, and link. Check that the document is the cited edition, not a similarly titled editorial, protocol, correction, or conference abstract.

A realistic reference can still be nonexistent. A valid URL can also lead to a real but irrelevant document. Record a reference as not verified when you cannot locate an authoritative record, rather than silently accepting it.

4. Inspect the source passage and data

Open the full source and find the language, table, figure, or data behind the claim. Compare the AI’s wording with the source:

  • Unclear: the source is inaccessible, ambiguous, or missing information needed to judge it.

Abstracts and search snippets can help locate a passage, but they may omit eligibility criteria, adverse events, subgroup definitions, statistical uncertainty, or limitations. Do not treat an AI-generated summary of a paper as a substitute for reading the relevant section.

5. Check currency, context, and applicability

Assess the population, intervention, comparator, outcomes, exclusions, setting, and date. A result in adults with preserved kidney function may not apply to a child or to someone receiving dialysis. A recommendation from one country may not reflect local licensing, resistance patterns, or service availability. A superseded guideline should not be presented as current simply because its webpage still loads.

Also check whether the AI has generalized beyond the evidence. A statistically significant average effect does not establish benefit for every patient. Look for absolute as well as relative effects, confidence intervals, follow-up duration, harms, and clinically meaningful thresholds when the source provides them.

6. Document the disposition and escalate uncertainty

Keep a brief audit trail for consequential answers. Record the claim, citation, source version or date, passage checked, disposition, and action taken. Mark unsupported, contradicted, outdated, and uncertain claims explicitly. If a conflict exists between sources, identify which authority governs the decision and ask an appropriate specialist, pharmacist, librarian, guideline group, or evidence-review service to resolve it.

A practical claim log

A simple table makes verification visible to the next clinician and prevents an unverified sentence from becoming part of the record.

For a high-consequence answer, preserve the original AI output, the model and date if available, the sources it supplied, and the corrected version. Do not paste an unverified AI citation into a patient record or manuscript as though it were established evidence.

How to compare competing sources

When an answer cites several documents, compare them on six axes rather than ranking them by domain name, citation count, or the fact that the model displayed them:

  1. Authority and evidence type: Is this an official guideline, systematic review, primary study, label, commentary, or educational page?
  2. Directness: Does it answer the exact claim, or is the connection an inference?
  3. Recency and version: Is it current for the decision date, and has it been corrected or superseded?
  4. Population and clinical context: Do participants, setting, disease severity, and resources resemble the patient and service?
  5. Completeness: Does the source report benefits, harms, uncertainty, and relevant exclusions, or only a favorable subset?
  6. Provenance transparency: Can another person trace the claim to the original document and reproduce the check?

A newer source is not automatically better if it is a commentary with no new evidence, and an older source may remain authoritative for a stable fact. The decision depends on the claim and the quality of the applicable evidence.

Common citation failures and what to do

The link works but the article does not support the sentence

The reference cannot be found

Search the publisher, journal, DOI registry, or bibliographic database using the title and authors. If no authoritative record exists, treat the reference as unverified and do not cite it. Ask the model to provide a different source only as a search aid; independently verify the replacement.

The AI combines several findings into one number

Return to each original study and identify its population, outcome, time frame, and denominator. Separate the numbers or report that no single study supports the combined estimate. A pooled figure requires a genuine meta-analysis or an explicitly justified calculation.

The guidance is old or from the wrong jurisdiction

Check the issuing organization’s current guideline page, update notices, and local policy. State the geography and version in the clinical note. If no current guidance exists, describe the uncertainty and use a documented evidence review rather than presenting an old recommendation as settled.

What published evaluations indicate

Wu and colleagues, in a 2025 Nature Communications evaluation, tested five large language models on 1,200 generated medical questions and more than 40,000 statement-source pairs. Across the tested systems and study conditions, approximately 50% to 90% of evaluated responses were not fully supported by their cited sources. In the reported retrieval-augmented evaluation, GPT-4 achieved 54% response-level support.

“Response-level support” required every claim in an answer to be supported. It is a stricter measure than having a valid URL or finding one supported sentence. The figures are specific to the model versions, source material, questions, and evaluation design; they are not an error rate for every current clinical AI product.

Medical education work titled Teaching Hallucination Literacy in Medical Education describes five competencies that fit this workflow: deconstruct claims, identify the verification target, calibrate the burden of proof, evaluate evidence, and document a defensible decision. That framework is educational; it has not established that one checklist improves patient outcomes.

A 2026 conceptual framework in Frontiers in Artificial Intelligence proposes linking each claim to authoritative references and logging inputs, sources, and reasoning steps. It describes an auditable architecture, not a validated deployed system. Treat such designs as ideas for governance and documentation, not as proof that an AI answer is safe.

Governance and professional responsibilities

American Medical Association policy H-480.931 identifies incorrect or falsified responses, difficulty verifying accuracy or sources, outdated or incomplete information, bias, privacy, cybersecurity, and liability as governance risks. It calls for rigorous validation and verification protocols and education for physicians and staff. Directive D-480.952 calls for disclosure of AI use in clinical care, risk-based governance, useful transparency such as labeling and confidence information, and lifecycle monitoring of safety, clinical effectiveness, accuracy, and reliability.

These policies support organizational controls, but they do not turn a citation check into a universal clinical protocol. Each service should define which answers require second review, how uncertainty is escalated, what may enter the record, and how patient data are protected.

JAMA Network’s author guidance, updated online August 10, 2026, states: “Check all references for completeness, accuracy, and integrity.” It also advises that AI, large language models, and chatbots should not generate, format, or manage references. This is a publication requirement, not automatically a regulation governing every clinical encounter, but the underlying discipline is useful: the human author or clinician remains accountable for every cited claim.

As John Halamka, MD, MS, put it in an AMA report, “Ethical AI requires a lifecycle approach from data curation to model testing, to ongoing monitoring. Only with the right guidelines and guardrails can we ensure our patients benefit from emerging technologies while minimizing bias and unintended consequences.” Citation checking is one control within that lifecycle, not the whole safety system.

When to stop relying on the generated answer

Do not use the answer as a basis for an immediate clinical decision when a citation is missing, cannot be verified, conflicts with an authoritative source, or leaves a material patient-specific question unanswered. Pause and obtain the appropriate guideline, primary evidence, pharmacist review, specialist input, or local protocol. For urgent care, follow established emergency and escalation procedures rather than waiting for an AI response to become verifiable.

The safe endpoint is not “the AI cited something.” It is a documented chain from an atomic claim to a real, relevant, current source, followed by a clinician’s explicit judgment about whether that evidence applies to the individual patient.
