CAR T-cell therapy starts with a patient’s own T cells, which are collected from the blood, genetically changed in a laboratory to recognize a selected cancer-cell target, multiplied, and returned to the hospital for infusion. The full collection-to-infusion process generally takes about 3 to 5 weeks, according to the National Cancer Institute (NCI), although an individual patient’s schedule can differ. Some treatment protocols also use chemotherapy before infusion. Because CAR T cells can trigger serious immune and neurologic side effects, treatment includes close clinical oversight and longer-term safety monitoring.
What CAR T-cell therapy does
T cells are a type of immune cell. In CAR T-cell therapy, a patient’s T cells are changed in a laboratory so they express chimeric antigen receptors, or CARs. A CAR is designed to bind a selected antigen—a target found on cancer cells. Once infused, the engineered cells can recognize cells bearing that target and attack them. NCI describes the treatment as changing a patient’s T cells so they bind to cancer cells and kill them.
The target matters. CAR T products are designed for particular antigens, and some targets can also appear on normal cells. This helps explain why the treatment is not interchangeable across cancers or patients: the relevant cancer, target, specific product, and its approved indication all matter. The fact that a cancer is serious, or that it involves the immune system, does not by itself establish that CAR T is appropriate.

The common pathway described here is autologous: the cells come from the person who will receive them. The general sequence is collection, laboratory engineering, expansion, return to the treatment center, preparation when required, and infusion. The final product is typically given as a single infusion, but that does not mean the entire course of care consists of one appointment.

How the cells are made and returned
1. T cells are collected from the patient
Clinicians collect blood and separate out T cells for the therapy. The collected cells are sent to a manufacturer’s laboratory for processing. This is the starting point for the general autologous process; it is not a collection of tumor tissue, which is used in a different approach called tumor-infiltrating lymphocyte therapy.
2. The cells are engineered to express CARs
In the laboratory, the T cells are genetically engineered so they express CARs. Each receptor is intended to bind a selected antigen on cancer cells. The target is part of the treatment design, not a generic feature of every CAR T product. Because an antigen may also be present on normal cells, the intended target does not necessarily occur only on cancer.
The engineering step gives the T cells a way to recognize the chosen target. It does not mean that the patient’s cancer has been removed or that the modified cells have already been given. At this point the cells are being prepared as a treatment product outside the patient.

The engineered T cells are grown in the laboratory to large numbers. This expansion produces the cell therapy that will be sent back to the hospital. NCI describes the expanded cells as the final CAR T-cell therapy product, returned to the patient as a single infusion.

The laboratory phase is one reason CAR T treatment cannot be understood as an immediate procedure after blood collection. The cells must be engineered and expanded before the infusion can happen. The overall time estimate is a general one; it is not a promise that every patient’s cells will be ready on a particular date.
4. The product returns to the hospital
After manufacturing, the engineered cells are sent back to the hospital for administration. The clinical team coordinates the treatment around the return of the product and the patient’s care plan. The exact arrangements, availability, and schedule depend on the treatment center, product, and individual circumstances; the general sources do not establish a universal logistics plan.
How long the process takes
NCI gives an estimate of about 3 to 5 weeks from the initial blood collection to infusion. This is a general collection-to-infusion timeframe, not the length of a guaranteed schedule for an individual. Product and patient circumstances may affect the actual timing.

The sources do not set out a single universal schedule for those parts of care. Patients should ask their oncology team what dates are planned, what could change them, and how the center will communicate if the expected timing shifts.

What happens before and during infusion
Preparation may include lymphodepleting chemotherapy
Depending on the treatment protocol, a patient may receive lymphodepleting chemotherapy before the CAR T-cell infusion. An NIH Clinical Center patient information sheet explains that this chemotherapy is intended to prepare the body for the CAR T cells. It is not specified as a required step for every protocol in the general description, so whether it is part of a particular patient’s plan should be confirmed with the treating team.
The CAR T cells are infused
The returned cell product is administered to the patient as an infusion. NCI describes the final therapy as a single infusion. The infusion is the point when the engineered cells are returned to the body; it is not the same as the earlier blood collection, laboratory engineering, or cell expansion.
After infusion, the cells can act in the body
Following infusion, CAR T cells may expand inside the patient and attack cells that carry their target antigen. The wording matters: the cells may expand, and their intended recognition depends on the selected target. The general process description does not guarantee a particular response for an individual or establish how quickly a response will occur.
Who CAR T-cell therapy may be for
NCI’s patient overview lists FDA-approved CAR T-cell therapies for blood cancers and describes use for solid tumors as experimental in that overview. This is a broad description, not a current eligibility list. Approved indications and product labels are specific and can change, so a patient’s oncology team and current FDA labeling are the appropriate references for decisions about a particular cancer and treatment.

Suitability depends on more than the cancer name. Relevant considerations include the cancer type and target antigen, the specific approved product and indication, the patient’s treatment history and health, the practical manufacturing and care schedule, and product-specific risks. The available general information does not establish any one patient’s eligibility, local center availability, insurance coverage, or individual treatment plan.
CAR T is not the same as TIL therapy
Both CAR T-cell therapy and tumor-infiltrating lymphocyte (TIL) therapy use immune cells, but the cells are prepared differently. CAR T cells are engineered in a laboratory to express a receptor directed at a selected antigen. TILs are collected from a tumor and expanded. The shared use of immune cells does not make the treatments equivalent or establish that either is suitable in a given case.

CAR T-cell therapy can cause serious side effects. Two important toxicities are cytokine release syndrome (CRS), a systemic inflammatory reaction, and immune effector cell-associated neurotoxicity syndrome (ICANS), a neurologic complication. Infections and depletion of normal B cells are also potential concerns. Their possibility is one reason the treatment is given within a clinical care plan rather than treated as a routine infusion without follow-up.

Cytokine release syndrome
NCI explains that CRS can involve high fever and a sharp fall in blood pressure. Management can include tocilizumab and, when needed, steroids. This is a serious reaction that needs clinical assessment and management; the general description does not provide a patient-specific prediction of whether it will occur or how severe it might be.

Neurologic effects, infection, and B-cell depletion
ICANS is another potentially serious toxicity associated with CAR T-cell therapy. The general source material identifies it as a risk but does not provide a symptom checklist or a product-by-product estimate, so patients should use the specific instructions supplied by their treatment team rather than relying on a generic list. Infections and depletion of normal B cells are also potential side effects to discuss as part of the product-specific care plan.
FDA warning about T-cell malignancies
The FDA required boxed warnings for T-cell malignancies following treatment with specified BCMA-directed or CD19-directed autologous CAR T-cell immunotherapies. The FDA says these malignancies may arise as soon as weeks after infusion and may be fatal, and directs lifelong monitoring for secondary malignancies. This warning identifies an important risk; it does not mean that every person treated will develop a malignancy. Patients should ask their team which warning and follow-up instructions apply to the specific product being considered or administered.
Questions to take to the oncology team
Because products, indications, protocols, and patient circumstances differ, a general explanation cannot substitute for an individual treatment discussion. These questions can help make the plan concrete:
- Which CAR T-cell product and target antigen are relevant to my cancer, and what is its current approved indication?
- What parts of the collection, manufacturing, and infusion schedule are planned for me, and what circumstances could change the timing?
- Does my protocol include lymphodepleting chemotherapy before infusion, and what is its purpose in my plan?
- What symptoms or changes should prompt an immediate call to the treatment center, and which number should I use?
- What monitoring is planned after infusion, including the required long-term monitoring for secondary malignancies?
- How do my treatment history and health affect the risks, logistics, and suitability of this specific therapy?
The answers should come from the team responsible for the patient’s care and from current product-specific information. General descriptions of CAR T-cell therapy explain the process, but cannot confirm a person’s eligibility, predict an outcome, or replace the instructions for a particular treatment.
