Cancer treatment vaccines train immune cells to recognize molecular markers on cancer cells and attack cells carrying those markers. They may deliver tumor material, selected antigens, dendritic cells, or genetic instructions such as mRNA. This is different from preventive HPV and hepatitis B vaccines, which prevent infections that can cause cancer and do not treat an existing tumor. Only a few vaccine-like treatments have specific approved uses; many personalized and mRNA approaches remain experimental.
What a cancer treatment vaccine is designed to do
A treatment vaccine is an immunotherapy for someone who already has cancer. Its objective is not to poison tumor cells directly, but to make the immune system better at identifying them. The vaccine presents an antigen—a recognizable molecular feature associated with cancer—to immune cells. Tumor-associated antigens may be absent from normal tissue or present at much higher levels in cancer cells.
The National Cancer Institute summarizes the goal this way: “Treatment vaccines can help the immune system learn to recognize and react to antigens and destroy cancer cells that contain them.” The approach is selective rather than universal: the antigen, tumor type, disease stage, treatment setting and patient all affect whether a response is possible. A vaccine-induced immune response is not a guarantee that a tumor will shrink or that survival will improve.
For an overview of the medical approach, see the National Cancer Institute explanation of cancer treatment vaccines.

How the immune-training process works
1. Researchers select a tumor antigen
Every vaccine formulation starts with a target. Some targets are shared by many patients with the same cancer; others are found only in one person’s tumor. A shared tumor-associated antigen can support an “off-the-shelf” approach, while a mutation unique to a patient’s cancer may produce a personalized neoantigen.
The target must be displayed by the tumor in a form that immune cells can recognize. If a cancer does not carry enough of the selected antigen, a vaccine aimed at that antigen may have little to attack. This is one reason results cannot automatically be transferred from one cancer type or vaccine platform to another.
2. The vaccine delivers antigen or instructions for making it
Different platforms supply the immune system with different forms of information:
- Whole tumor-cell material can expose immune cells to several antigens at once.
- A shared-antigen vaccine supplies selected proteins or pieces of proteins found in a particular cancer.
- A dendritic-cell vaccine uses immune cells prepared outside the body and returned to the patient.
- An mRNA vaccine carries temporary genetic instructions. Cells that take up the mRNA make the encoded protein, creating antigen material that can be processed and displayed.
- An oncolytic virus is injected into a tumor and can both damage tumor cells and stimulate immunity against material released from those cells.
3. Dendritic cells process and display the target
Dendritic cells are professional antigen-presenting cells. They take up antigen material, process it into recognizable fragments and display those fragments to T cells. Karine Breckpot, Ph.D., of Vrije Universiteit Brussel, described their role in an NCI interview: “Dendritic cells act as teachers, educating T cells so that they can search for and kill cancer cells or virus-infected cells,” depending on the antigen.
This teaching step is why simply placing an antigen in the body is not enough. The material must be presented in a context that activates the appropriate immune response. Vaccine formulations may therefore include an immune-stimulating component, and researchers monitor how effectively antigen-presenting cells activate T cells.

4. T cells learn the target pattern
Activated T cells inspect cells for the selected antigen. When a T-cell receptor recognizes the antigen displayed by a tumor cell, the T cell may release molecules that damage or kill that cell. Other immune signals can recruit and coordinate additional immune activity.
In mRNA approaches, the sequence is chosen so cells make a specific cancer-associated protein or a set of patient-specific neoantigens. The mRNA itself is not intended to permanently alter DNA; it provides instructions that cells use to produce the selected protein before the message is broken down.
5. Recognition still has limits
Recognition is only one part of treatment. The tumor must display the target, immune cells must reach it, and the resulting response must be strong enough to overcome the biology of that cancer. Researchers therefore distinguish an immune marker—such as activation of antigen-specific T cells—from a clinical outcome such as tumor shrinkage, durable control or longer survival.
Treatment vaccines versus cancer-prevention vaccines
The word “cancer vaccine” covers two fundamentally different goals. A therapeutic vaccine is given after cancer exists and targets cancer-cell antigens. A prevention vaccine blocks an infection that can later cause cancer.
| Feature | Treatment vaccine | Prevention vaccine |
|---|---|---|
| When it is used | For a person who already has cancer | Before infection or cancer develops |
| What it targets | Antigens on cancer cells, including shared antigens or patient-specific neoantigens | A cancer-causing infection, such as HPV or hepatitis B |
| What it is intended to do | Help immune cells recognize and attack existing tumor cells | Prevent infection that could lead to cancer later |
| Examples | Sipuleucel-T for a specific prostate-cancer setting; T-VEC for some melanoma cases; investigational personalized vaccines | HPV and hepatitis B vaccines |
| Does it treat an established tumor? | That is the therapeutic aim, although benefit depends on the cancer and treatment | No |
The NCI’s HPV vaccine fact sheet explains the prevention role. An HPV vaccine does not cure an HPV infection or treat an HPV-related tumor that is already present. The same distinction applies to hepatitis B vaccination.

Major cancer-vaccine approaches
| Approach | Antigen source and platform | Immune strategy | Clinical status described by NCI |
|---|---|---|---|
| Patient’s own tumor cells | Material made from an individual’s tumor | Expose the immune system to multiple antigens from that cancer | One treatment-vaccine category; availability and evidence depend on the specific product |
| Shared tumor-associated antigen | A selected antigen found in a cancer type or subgroup | Train T cells against a marker shared by some tumors | Many approaches remain experimental |
| Dendritic-cell vaccine | A patient’s dendritic cells are prepared with tumor antigen and returned | Use antigen-presenting cells to activate T cells | Sipuleucel-T has a limited approved use in advanced prostate cancer |
| Personalized mRNA vaccine | mRNA encoding mutations that may create a patient’s neoantigens | Cells make the selected proteins, which are then presented to T cells | Investigational; early human and animal findings are not proof of broad benefit |
| Oncolytic-virus therapy | A modified herpes simplex virus type 1 injected into a tumor | Replicate in tumor cells, cause them to burst and expose material that can stimulate immunity | T-VEC has a specific use for some unresectable recurrent melanoma |
Examples with established, limited clinical uses
Sipuleucel-T for selected advanced prostate cancer
Sipuleucel-T is a dendritic-cell treatment vaccine. It is used for some men whose prostate cancer has spread, causes few or no symptoms and no longer responds to hormone treatment. The therapy involves preparing the patient’s immune cells so they can present a prostate-cancer-associated target, then infusing the prepared cells back.

This indication is narrow. It does not mean that every person with prostate cancer is a candidate, nor that dendritic-cell vaccines are established for all cancers. The treating oncology team must determine whether the disease setting, prior treatments and overall health fit the approved use.
T-VEC for some recurrent melanoma
Talimogene laherparepvec, commonly called T-VEC or Imlygic, is an oncolytic-virus therapy sometimes described as a treatment vaccine. It is based on herpes simplex virus type 1 and is injected directly into a tumor. The virus replicates in tumor cells, causing them to burst. Viral particles and tumor material released from dying cells can then help alert the immune system to cancer beyond the injected lesion.
The NCI describes T-VEC for some people with melanoma that has returned after surgery and cannot be removed by further surgery. The wording matters: this is not a universal melanoma treatment or a guarantee that untreated tumors elsewhere will respond.
How personalized mRNA cancer vaccines are built
A personalized candidate is designed around molecular features of one patient’s tumor rather than a marker shared by every patient. The general workflow is:

- Collect tumor and comparison tissue. The tumor is analyzed alongside normal tissue so that cancer-specific mutations can be distinguished from inherited or harmless variations.
- Identify possible neoantigens. Researchers look for mutations that could produce abnormal proteins recognizable by T cells and consider whether the tumor is likely to display them.
- Design the mRNA sequence. The selected instructions are packaged for delivery to cells, which then produce the chosen protein fragments.
- Manufacture the individualized product. In the investigational approach discussed by the NCI in 2022, manufacturing took roughly one to two months after tumor tissue was collected. That timetable was specific to the approach described and should not be treated as a universal production time.
- Administer and monitor. Researchers track immune responses and clinical measurements such as imaging and tumor behavior. An immune response alone does not establish that the cancer has been controlled.
In an early head-and-neck cancer study reported by the NCI in 2022, two of the first 10 participants had complete responses and five had tumor shrinkage. The cohort was very small, and the report presented these findings as an early observation—not a general response rate, proof of approval or estimate for all patients. Julie Bauman, M.D., deputy director of the University of Arizona Cancer Center, told NCI that “Personalized cancer vaccines may teach the immune system how cancer cells are different from the rest of the body.”

An NCI report dated April 26, 2023 compared three experimental mRNA vaccines targeting the HPV-16 E7 protein in a mouse model. Each vaccine stimulated antigen-specific CD8-positive T cells, and the report said tumors were eradicated after a single dose in that experimental model.
Those results are animal evidence, not a demonstrated human treatment. The same NCI report noted that approaches successful in mice have not always succeeded in later human trials. The mRNA vaccines discussed there were not FDA-approved at the time, and the article described clinical testing as ongoing or planned. John Schiller, Ph.D., of the NCI Center for Cancer Research, cautioned: “Looking at immune responses to mRNA vaccines did not necessarily predict how well the vaccines worked to get rid of the tumor.”

Why an immune response may not translate into tumor control
Several variables make cancer-vaccine results difficult to generalize:
- Antigen mismatch: A tumor may not carry enough of the selected target, or different tumor cells may display different targets.
- Disease context: Results can differ between newly diagnosed, residual, recurrent and widely metastatic disease.
- Platform differences: A dendritic-cell product, an mRNA formulation and an oncolytic virus present antigen in different ways.
- Evidence type: A mouse response, an increase in antigen-specific T cells and a measurable tumor response are different endpoints.
- Unresolved biology: Researchers do not fully understand every immune component associated with tumor eradication. In the 2023 mouse study, immune measurements did not reliably predict how well a vaccine removed tumors.
For these reasons, it is misleading to rank vaccine platforms based only on an animal comparison or a small early cohort. A credible assessment should specify the cancer, stage, antigen, platform, comparator, follow-up and endpoint.
Side effects and treatment-specific risks
Because these therapies stimulate immunity or use a replicating virus, adverse effects depend on the product. Flu-like symptoms can occur with treatment vaccines. The NCI lists stroke as a risk associated with sipuleucel-T. For T-VEC, reported risks include tumor lysis syndrome and herpesvirus-related complications.
These examples do not predict an individual’s experience. Eligibility, infection risk, immune status, previous treatments and monitoring requirements must be reviewed with the oncology team before treatment. Do not substitute a consumer supplement or a preventive vaccine for a prescribed cancer therapy.
Where the field is heading in 2026
Minutes from the NCI Clinical Trials and Translational Research Advisory Committee meeting on July 15, 2026 describe a proposed National Cancer Vaccine Taskforce Initiative. The planning framework includes evaluating vaccine platforms, discovering antigens, monitoring immune responses, generating evidence through basket trials and building shared research infrastructure.
The minutes also describe planned exploration of adjuvant vaccine approaches in pancreatic, liver, colorectal and fusion-positive pediatric cancers, among other indications. These are proposed research and demonstration activities, not completed efficacy results or proof that those vaccines are available treatments. The document is available as a July 15, 2026 committee-minutes PDF.
